RESUMO

This new guideline covers the rapidly advancing field of interventional bronchoscopy using flexible bronchoscopy. It includes the use of more complex diagnostic procedures such as endobronchial ultrasound, interventions for the relief of central airway obstruction due to malignancy and the recent development of endobronchial therapies for chronic obstructive pulmonary disease and asthma. The guideline aims to help all those who undertake flexible bronchoscopy to understand more about this important area. It also aims to inform respiratory physicians and other specialists dealing with lung cancer of the procedures possible in the management and palliation of central airway obstruction. The guideline covers transbronchial needle aspiration and endobronchial ultrasound-guided transbronchial needle aspiration, electrocautery/diathermy, argon plasma coagulation and thermal laser, cryotherapy, cryoextraction, photodynamic therapy, brachytherapy, tracheobronchial stenting, electromagnetic navigation bronchoscopy, endobronchial valves for emphysema and bronchial thermoplasty for asthma.

Can the detection rate of flexible bronchoscopy for lung cancer be increased by a series of simple quality improvement measures? Bronchoscopy-associated clinical parameters were prospectively recorded between 2001 and 2007 in patients with suspected lung malignancy. The detection rate of bronchoscopy, diagnostic yield of each biopsy modality and the possible impact of different service-improvement measures were assessed. 746 bronchoscopies were performed in 704 patients. The detection rate of bronchoscopy for malignancy was 83.6%, and increased over time (67.3% detection rate in 2001 (95% CI 52.9-79.7), 89.7% detection rate in 2007 (95% CI 81.3-95.2); p<0.001). Detection rate increased for bronchoscopically visible (75.0% in 2001 to 94.5% in 2007) and non-visible tumours (41.7% in 2001 to 81.2% in 2007; p<0.001 for both analyses). Prior computed tomography availability was associated with a higher diagnostic yield that did not reach statistical significance. Logistic regression analysis identified tumour visibility, year of study, use of transbronchial needle aspiration and pathologist identity as independent predictors of a positive diagnosis. A significant increase in bronchoscopic detection rate for malignancy occurred in association with a number of simple improvement measures.

Community-acquired pneumonia (CAP) is the most common reason for acute admission to hospital and the fourth most common cause of death in the UK. It is important to identify patients with severe pneumonia and the worst prognosis. We conducted this study to validate a rule designed to do this devised in New Zealand. CAP was defined by evidence of new shadowing on the chest X-ray and clinical features of pneumonia. A standardised proforma was completed documenting clinical features and investigation results. Severe pneumonia was identified by two or more of the following: confusion; respiratory rate > or = 30/min; diastolic blood pressure < or = 60 mmHg; urea >7 mmol/l. One hundred patients (mean age 58.8 years) were included; 32 satisfied the rule. Seven died, all of whom satisfied the rule. Ten patients received intensive care, including six who died. Of 11 patients who died and/or received intensive care, nine satisfied the rule. The sensitivity and specificity of the rule for predicting death and/or intensive care were 82% and 73% respectively.
