RESUMO

Subclinical bioprosthetic valve thrombosis (BPVT) is a relatively common finding in asymptomatic patients during follow-up imaging. However, its clinical significance is unclear. Data from registries associate BPVT with elevated valve gradients, thromboembolic complications, recurrence, and valve degeneration. Given the dynamic nature of the disease process, management is challenging. The duration of anticoagulation is unpredictable, and the need for frequent monitoring of BPVT, even in subclinical scenarios, is unclear. Our report is shedding the light on the clinical implications BPVT.

RESUMO

Cardiac atrial myxoma is a primary cardiac tumor with a prevalence of 0.0017-0.19% at autopsy. Anomalous origin of right coronary artery (ARCA) from opposite sinus of Valsalva is considered the second most common cause of sudden cardiac death among young athletes with an incidence of 0.09%-0.92% and mortality risk of 0-57%. We present a case of symptomatic left atrial myxoma in the presence of ARCA from the opposite sinus of valsalva. The myxoma was excised and the anomalous right coronary artery was re-implanted during the same operative session.

RESUMO

Thyroid storm is an extreme form of hyperthyroidism associated with a high mortality rate. Heart failure is considered the leading cause of mortality in patients with thyroid storm, though the underlying cardiac pathology is unclear. Approximately 6% of patients with thyroid storm have heart failure symptoms as the initial presenting complaint. Roughly, one-third of these patients develop dilated cardiomyopathy (DCM). In this report, we present a case of cardiogenic pulmonary edema and sustained ventricular tachycardia in a patient with hyperthyroidism presenting with thyroid storm.
