RESUMO
Redo bariatric procedures are common. However, redo sleeve gastrectomy is not a frequent case of repeated bariatric surgery and can be performed rather as a necessary measure in difficult intraoperative conditions. We report a patient who underwent laparoscopic adjustable gastric band placement, its blockage and surgical removal, sleeve gastrectomy and redo sleeve gastrectomy. After that, staple-line suture failure developed that required endoscopic clipping.

The article presents a clinical observation of a patient with congenital adrenal dysfunction (CHD), a salt-losing form of 21-hydroxylase enzyme deficiency (homozygous mutation I 172N), and also with morbid obesity, due to long-term use of high doses of glucocorticosteroids, who underwent bariatric surgery - laparoscopic sleeve resection of the stomach. A feature of the presented case is the elimination of one of the causes of decompensation of the disease, namely, overweight, as well as insulin resistance, which requires the intake of large doses of glucocorticoids, which in turn leads to a worsening of the course of obesity, thereby causing a vicious circle. 7 months after surgical treatment, the goal was achieved - a reduction in the dose of Prednisolone by 25%, with a decrease in body weight by 72.1% of overweight. The presented case clearly demonstrates the possibility of performing bariatric surgery for the treatment of morbid obesity in patients with CAH with the participation and control of a specialized multidisciplinary team. If there are indications for bariatric intervention, VDKN should not be an absolute contraindication to such operations, and the ratio of the safety profile and the efficacy profile testifies in favor of the need for their implementation.

The role of P2X7 receptors in modulation of the functional activity of macrophages in mice with model peritonitis has been studied. It is established that the functional activity of murine macrophages under such conditions is increased, which is accompanied by the growth of P2X7(+) macrophages and a bidirectional change of the their functional activity under the action of P2X7 modulators. The role of P2X7-associated mechanisms in regulation of the macrophage activity during inflammation is discussed.

The morbidity was studied of 3000 preschool children born and inhabiting the territory of six Rovno Province districts belonging to the "Western trail" of the Chernobyl accident. It was found that the children born in 1986-1987 showed a 1.5-3 times increase of the morbidity as compared with the pre-accident period.

Introduction of novel information technologies in the health system of Ukraine is a requisite condition of enhancing the effectiveness of work of medical bodies, ensuring high-quality medical care. Our objective in this study was to devise a mechanism of information support of the process of state accreditation of the public health institutions. A structural scheme is developed of a uniform information-analytical system for information support of the process of accreditation of medical institutions. An automated information system has been set up at the level of medico-prophylactic institutions, which consists of information-reference and information-analytical subsystems and provides information support for the process of accreditation. Ways are discussed of setting up an information-control system of accreditation using potentialities of the sectoral corporative computer network "Ukrmednet".

The investigation has been performed in 24 male patients with posttraumatic strictures of the urethra operated under peridural anesthesia. The immunological status was studied at different steps of the surgical treatment. It has been established that all the patients examined had an initial disturbance of both cellular and humoral immunity. The peridural anesthesia fails to exercise an immunosuppressive effect upon the patient's organism.
