INTRODUCTION: Cystine and theanine are amino acids that contribute to the production of glutathione, which is the most potent antioxidant in the body, and it has been reported that these amino acids prevent immunosuppression, has anti- inflammatory effect, and reduce invasiveness. OBJECTIVE: To evaluate the effect of oral administration of amino acids cystine and theanine on stomatitis during chemotherapy. SUBJECTS AND METHODS: For 17 patients with Grade 1(CTCAE v4.0)or worse stomatitis during chemotherapy for gastrointestinal cancer or breast cancer, cystine 700 mg and theanine 280 mg/ day were orally administered for 28 days, and the degree of stomatitis was evaluated objectively and subjectively. RESULTS: As an objective evaluation, changes in Grade showed improvement in 11 cases(64.7%), 5 cases unchanged(29.4%), and 1 case was worse(5.9%). Subjective assessment, pain was in remission(30.8%)or disappeared(61.5%)in 4 of 13 cases. Food intake increased in 5 patients(29.4%). 15 of 17 patients(88.2%)felt it was effective. CONCLUSION: Oral administration of amino acids cystine and theanine during chemotherapy can reduce the symptoms of stomatitis.

OBJECTIVE: In Duchenne muscular dystrophy (DMD) patients cardiac abnormalities are often detected. In adult DMD patients cardiac disease (CD) is a cause of death which increases by age and is related to respiratory dysfunction. Studies have demonstrated that CD in early DMD can be detected by echocardiography (EC) or semi-quantitative gated single photon emission tomography ((201)Tl SQGS), and the accuracy of these two tests is similar. As the disease advances, evaluation of CD by EC becomes difficult due to thoracic deformity and scoliosis. We compared (201)Tl SQGS and EC in the evaluation of cardiac function in late stage DMD, based on the ejection fraction (EF) value calculated by both tests. Twenty-three males with late stage DMD, 12 to 35 years of age (22.2±7.5), were studied by (201)Tl SQGS and EC. The mean EF value by (201)Tl SQGS was 60.8%±14.1%, which differed from that obtained by EC (52.7%±9.8%, P=0.003). Eleven patients less than 20 years old did not demonstrate a significant difference between the two tests (P=0.06), however, 12 patients over 20 years of age had significantly different results between tests (P=0.002). CONCLUSION: Although our patients were few we indicated that in DMD patients, aged older than 20 years, at an advanced stage of the disease, the EF values calculated by EC were lower than those by (201)Tl SQGS possibly due to thoracic deformity.

AIM: Glycemic control is important to improve the prognosis in cirrhotic patients with complications from diabetes. A late evening snack (LES) has been recommended for cirrhotic patients. We investigated the effects of LES on diurnal plasma glucose levels. METHODS: Subjects comprised 47 patients with chronic viral liver disease (chronic hepatitis, n = 11; cirrhosis, n = 36) treated in the Department of Gastroenterology & Hepatology, Dokkyo Medical University Koshigaya Hospital. Diurnal variations in plasma glucose were first investigated with three meals/day, in accordance with the European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines. Starting the next day, patients were given four meals including a LES, without changing meal content. Diurnal variations in plasma glucose were examined on day 7, and urine C-peptide immunoreactivity (CPR), and homeostasis model assessment insulin resistance (HOMA-IR) were investigated. RESULTS: With a LES, plasma glucose levels in patients with chronic hepatitis were significantly lower 2 hours before and 2 hours after dinner. In cirrhotic patients, significant decreases in plasma glucose levels were seen 2 hours after breakfast, before lunch, and before dinner. Significant decreases were noted in average plasma glucose levels and highest plasma glucose levels with four meals including a LES in patients with liver cirrhosis. This decrease was greater when maximum plasma glucose levels were higher on the three-meal regimen. CONCLUSIONS: Improvements in plasma glucose levels were seen with four meals per day, including a LES, in viral chronic liver disease, particularly cirrhosis.
