This study was designed to analyze the effect of class II malocclusion as a factor in the development of obstructive sleep apnea syndrome. Although mandibular retrusion has been reported coincidentally with obstructive sleep apnea syndrome many times, no causal relationship has been established. No previous study has analyzed the occurrence of obstructive sleep apnea syndrome in patients with class II malocclusion without sleep complaints. In this study, we selected 12 patients with class II malocclusion who required surgical mandibular-lengthening or repositioning procedures. These patients were surveyed for sleep habits or sleep complaints and then studied with overnight polysomnography for sleeping or breathing abnormalities. None of these patients had obstructive sleep apnea syndrome. From this sample population, an incidence of obstructive sleep apnea syndrome of no more than 26.5% in the surgical population of patients with class II malocclusion can be extrapolated.

Interest in the anatomy of the buccal fat pad was aroused by two clinical cases where the fat pad was involved by pathology. In one, there was an extensive lipoma of the buccal fat pad which had been unsuccessfully operated on on two previous occasions. The reason for the lack of success was incomplete removal of the fat pad. In the second case, there was an arteriovenous malformation involving the fat pad which necessitated complete removal. Fresh cadaver dissections were carried out to accurately determine the anatomy. Particular attention was paid to the temporal extension, since this area is frequently disregarded.

A patient with an oroantral fistula of seventeen years' duration presented clinically complaining of symptoms suggesting recurring episodes of acute maxillary sinusitis. After control of the acute sinusitis, the defect was repaired using a buccal flap procedure and the sinus was enucleated via a Caldwell-Luc approach. The sinus contents included what appeared to be a chicken bone. The patient healed uneventfully. A discussion of the management of chronic oroantral fistulas and a case report are presented.

A series of 160 consecutive iliac bone graft cases involving harvesting of either a superior medial, lateral, or full-thickness graft was reviewed to study the associated morbidity. The histories were examined for indication of infection, wound breakdown, pain, blood loss, and length of hospital stay. Although there were few problems associated with any procedure, those patients with superior medial grafts had less blood loss and ambulated earlier. All patients were asked to complete a mail-in questionnaire that reported postoperative pain, neurologic problems, ambulation difficulties, and subjective appearance of the surgical scar. Overall, the patients' acceptance of the procedure and their perception of the outcome was favorable.

Preoperative donation of autologous blood (PDAB) is the technique of choice for the transfusion of autologous blood for the maxillofacial surgeon. The use of PDAB is increasing, but lack of preoperative planning and lack of available utilization protocols are obstacles to its more widespread use. A technique for PDAB, a review of our experience, and guidelines for community applications are presented.

An unusual case involving an adult patient with marked facial and mandibular asymmetry secondary to congenital muscular torticollis is presented. The etiology and pathology of congenital muscular torticollis are reviewed. A team approach was used to correct this deformity. Procedures included a transcranial total orbital osteotomy, open distal myectomy of the sternocleidomastoid muscle, and a mandibular horizontal osteotomy.
