RESUMO

The effect of 30-min vector potential variation on the phagocytosis of macrophages from laboratory mice was investigated. It was found that the effect changes the characteristics of mice immunity: the quantity and percentage of neutrophils and lymphocytes in the blood, the activity of capture, degranulation, the splitting of bacterial agents, and the dynamics of formation of reactive oxygen species.

RESUMO

The possibility of using fluoresceine isothiocyanate to label plasmatic membranes of lymphocytes was explored. Membrane labeling can be done under physiological conditions. The label interacted with the surface located membrane component, and the binding centers became saturated with the label concentration which left the membrane undamaged. Modification of the membrane surface resulting from fluoresceine isothiocyanate treatment influenced significantly intercellular interactions mediating mitogen-induced blast transformation.

A total of 25 patients suffering from infective allergic bronchial asthma underwent extracorporeal connection of the donor's spleen (ECCDS). Before ECCDS was made the health status was unstable in all the patients, with asphyxia attacks occurring fairly often, namely up to 12 times a day. Nine patients received the maintenance therapy with steroid hormones. After ECCDS 23 persons (92%) manifested complete disappearance of asphyxia attacks or considerable abatement of their severity and rate. In all the steroid-dependent patients, the daily dosage of steroids could be lowered, while in 3 patients, hormonal drugs were afterwards discontinued. Examination of external respiratory function demonstrated the improvement of bronchial patency at the level of large, medium-size and small bronchi. There was a decrease of the concentration of the circulating immune complexes and IgE in the blood plasma. During 2 years of the follow up, 16 patients did not show any disease relapses, whereas the remainder demonstrated less gravity of attacks as compared to the period preceding the use of ECCDS. It is concluded that the use of ECCDS is desirable in the multimodality treatment of infective allergic bronchial asthma.

Twenty patients suffering from severe systemic lupus erythematosus (SLE) underwent 52 sessions of splenoperfusion (SP) by means of extracorporeal perfusion of the patient's blood through isolated porcine spleen. It got prepared directly before the perfusion. The organ was obtained from a healthy animal in sterile conditions, washed from the blood after cannulation of the artery and vein. The patient's blood was pumped from the peripheral vein to the spleen artery. After passing through the splenic vascular bed the blood outflew from the spleen vein into the opposite peripheral vein of the patient. SP was followed by positive clinical changes: normalization of body temperature, regression of skin symptoms and articular syndrome, reduction of lymphadenopathy, hepato- and splenomegaly. The tests indicated inhibition of SLE activity. Sp-related immunocorrective effect was also present as shown by depression of B-cell hyperreactivity and T-cell immunity stimulation. 1-4-year follow-up of 19 patients showed them to be in remission. The data obtained favour SP inclusion into intensive therapy of severe SLE.

It was shown in experiments on a model of fatal staphylococcal sepsis in 200 mice and 15 dogs that the perfusate of a pig's spleen obtained by perfusion of the isolated organ with a dextran solution (rheopolyglucin, polyglucin) is capable of preventing the animals' death. Intravenous infusion of 400-500 ml of a xenospleen perfusate at an interval of 1-3 days, which was included in the complex of postoperative treatment of 45 patients with destructive diseases of the abdominal organs complicated by peritonitis and sepsis, contributed to reduction of lethality and the terms of recovery. It is shown that the main effect, besides detoxification, is based on immunomodulation at the cost of biologically active substances produced by the cells of a xenospleen and contained in the perfusate. The best results were produced with the use of the perfusate of a xenospleen in the early periods after radical operation and thorough cleansing of the source of the infection.

To exert an effect on the autoimmune process in 54 New Zealand mice aged 6 and 10 months, 0.2 ml of a solution obtained in perfusion of isolated pig's spleen (perfusate) was injected intravenously at intervals of 2--3 days (a total of 8 injections). The control group (43 mice) received injections of an isotonic buffer solution in the same regimen. After the end of perfusate administration the number of antibodies to DNA and the level of circulating immune complexes (IC) reduced markedly in the blood of the experimental mice as compared to these values in the control group. Kidney immunofluorescence demonstrated reduced number of IC deposits in the glomeruli of 6- and 10-month old mice of the experimental group. The perfusate contributes, probably, to intensified excretion of IC from the blood flow and kidneys of the experimental mice.

A relationship of the functional status of the viscera with the immune system was studied in 38 cardiac surgical patients with septic complications and multi-organ failure in the postoperative period. In deceased patients (n = 13), the total functional organ compensation index was lower whereas the total functional decompensation index was higher than those in survivors (n = 25). In the deceased patients, each vital organ was more commonly decompensated than that in the survivors, but a definite discrepancy was ascertained only in the immune system (92% versus 16%, p < 0.0001). The average number of decompensated organs was found to increase with the severity of immunological disorders. Therefore, there is a rather high correlation between the functional status of vital organs and the degree of immunological imbalance. The authors show it necessary to use immunity-corrective drugs (splenopid) to enhance the efficiency of treatment organ failure.

RESUMO

The article presents results of using the extracorporal connection with the donor (porcine) spleen (ECCDS) for the treatment of 14 patients with critical mechanical traumas and their complications. An experience with 30 ECCDS-s has shown them to possess a pronounced detoxicating and immunomodulatory effect followed by a positive clinical result. In 6 cases of ECCDS a temporary chill was noted. There were no other complications.

The authors claim that the specific clinical symptoms of multiple organ failure (MOF) are explained by impaired interactions of two functionally related systems: the immune system (IS) responsible for regeneration and the reticuloendothelial system (RES) responsible for inflammation. Organ regeneration during MOF is suppressed as a result of suppressed morphogenetic function of lymphocytes, imbalanced macrophage-lymphocyte interactions, and RES hyperactivation. For prevention and treatment of MOF, the authors recommend stimulation of reparative processes in the organs by splenic therapy: assisted xenospleen, infusion of splenic perfusate or xenosplenic peptides. Xenospleen therapy is preferable to other methods of immunocorrection due to more expressed morphogenetic activity of splenic T lymphocytes and their cytokines.

Experiments on 25 intact dogs, 2 calves and clinical tests in 45 patients have shown that in the blood of the aorta or pulmonary vein leaving the lungs there is a rise in generation of active oxygen forms by leukocytes, their phagocytic activity, the activity of myeloperoxidase, NADPN-oxidase, complement, immunoglobulins content. As to the liver, kidneys, spleen, no such increased generation of active oxygen forms by leukocytes was registered. The results suggest an important role of the lungs in regulation of free radical activity of leukocytes, their phagocytic ability and thus in anti-infection defense of the body.

The article presents results of using hemoperfusion through the swine spleen and infusions of the xenosplenic perfusate in treatment of purulent peritonitis and various suppurative complications in 109 patients. A positive effect was noted (20% reduction of lethality, 1.5-3.5 times shorter period of treatment).

Studies conducted in the clinic (in patients with cardiac diseases) and experiments (performed on intact dogs) by means of the hemiluminescent method and the nitroblue tetrazolium test showed that the lungs, in distinction to other organs (heart and others), have a stimulating effect on the generation of active oxygen forms (AOF) by the leukocytes. In this way the lungs may probably play a double role in the organism: potentiate its defence (by intensifying the microbicidal activity of the phagocytes) and facilitate damage (by secretion of AOF by the phagocytes beyond them--into the tissues); the resultant effect depends on the balance of these two types of action. In carcinoma of the lung the stimulating effect of its involved lobe (part) on the leukocytes diminishes.
