Alkynyl radicals and cations are crucial reactive intermediates in chemistry, but often evade direct detection. Herein, we report the direct observation of the phenylethynyl radical (C6H5CC˙) and its cation (C6H5CC+), which are two of the most reactive intermediates in organic chemistry. The radical is generated via pyrolysis of (bromoethynyl)benzene at temperatures above 1500 K and is characterized by photoion mass-selected threshold photoelectron spectroscopy (ms-TPES). Photoionization of the phenylethynyl radical yields the phenylethynyl cation, which has never been synthesized due to its extreme electrophilicity. Vibrationally-resolved ms-TPES assisted by ab initio calculations unveiled the complex electronic structure of the phenylethynyl cation, which appears at an adiabatic ionization energy (AIE) of 8.90 ± 0.05 eV and exhibits an uncommon triplet (3B1) ground state, while the closed-shell singlet (1A1) state lies just 2.8 kcal mol-1 (0.12 eV) higher in energy. The reactive phenylethynyl radical abstracts hydrogen to form ethynylbenzene (C6H5CCH) but also isomerizes via H-shift to the o-, m-, and p-ethynylphenyl isomers (C6H4CCH). These radicals are very reactive and undergo ring-opening followed by H-loss to form a mixture of C8H4 triynes, along with low yields of cyclic 3- and 4-ethynylbenzynes (C6H3CCH). At higher temperatures, dehydrogenation from the unbranched C8H4 triynes forms the linear tetraacetylene (C8H2), an astrochemically relevant polyyne.

More than 8 million older people in Latin America depend on long-term care (LTC), accounting for 12% of people aged ≥ 60 years and almost 27% of those aged ≥ 80. It is crucial to develop sustainable strategies for providing LTC in the area, including institutional care. This special report aims to characterize institutional LTC in four countries (Brazil, Chile, Costa Rica and Mexico), using available information systems, and to identify the strategies adopted to support institutional care in these countries. This narrative review used nationwide, open-access, public data sources to gather demographic estimates and information about institutional LTC coverage and the availability of open-access data for the proportion of people with LTC needs, the number of LTC facilities and the number of residents living in them. These countries have a larger share of older people than the average in Latin America but fewer LTC facilities than required by the demand. National surveys lack standardization in defining disability, LTC and dependency on care. Information about institutional care is mainly fragmented and does not regularly include LTC facilities, their residents and workers. Data are crucial to inform evidence-based decisions to favor prioritization and to support advances in promoting policies around institutional LTC in Latin America. Although information about institutional care in the region is fragmented and insufficient, this paper profiles the four selected countries. It highlights the need for a better structure for data-driven LTC information systems. The lack of information emphasizes the urgency of the need to focus on and encourage research into this topic.


En América Latina, más de 8 millones de personas mayores dependen de los cuidados a largo plazo (CLP), lo que representa el 12% de las personas de 60 años o más y casi el 27% de las de 80 años o más Resulta crucial elaborar estrategias sostenibles para la prestación de CLP en la región, incluida la atención en centros de CLP. Este artículo especial tiene como finalidad determinar las características de la atención prestada en centros de CLP en cuatro países (Brasil, Chile, Costa Rica y México), utilizando los sistemas de información disponibles, así como determinar cuáles son las estrategias adoptadas en estos países para brindar apoyo a la atención en centros de CLP. En esta revisión descriptiva se utilizaron fuentes de datos públicas, de libre acceso y de ámbito nacional para recopilar estimaciones demográficas e información sobre la cobertura de la atención en centros de CLP, así como sobre la disponibilidad de datos de libre acceso acerca de la proporción de personas con necesidades de CLP, el número de centros de CLP y su correspondiente número de residentes. Estos países tienen una proporción de personas mayores superior a la media de América Latina, pero menos centros de CLP de los necesarios para cubrir la demanda. En las encuestas nacionales no hay una definición estandarizada de la discapacidad, los cuidados a largo plazo y la dependencia. La mayor parte de la información sobre la atención en centros de CLP está fragmentada y no incluye datos periódicos sobre los centros de CLP existentes, sus residentes o sus trabajadores. Estos datos son cruciales para fundamentar decisiones basadas en la evidencia destinadas a propiciar la priorización y brindar apoyo a los avances en la promoción de políticas en materia de centros de CLP en América Latina. Aunque la información sobre la atención en centros de CLP en la región es fragmentaria e insuficiente, en este artículo se presenta el perfil de los cuatro países seleccionados. Se resalta la necesidad de mejorar la estructura de los sistemas de información sobre CLP basados en datos. Esta falta de información pone de relieve la necesidad urgente de centrarse en este tema y fomentar la investigación al respecto.


Na América Latina, mais de 8 milhões de pessoas idosas dependem de cuidados de longa duração (CLD), o que representa 12% das pessoas com mais de 60 anos e quase 27% das pessoas com mais de 80 anos. É fundamental criar estratégias sustentáveis para oferecer CLD na região, inclusive cuidados institucionais. O objetivo deste relatório especial é caracterizar CLD institucionais em quatro países (Brasil, Chile, Costa Rica e México), usando os sistemas de informação disponíveis, e identificar as estratégias adotadas para apoiar os cuidados institucionais nesses países. Esta revisão narrativa usou dados públicos de acesso aberto de âmbito nacional para coletar estimativas demográficas e informações sobre a cobertura de CLD institucionais e a disponibilidade de dados de acesso aberto sobre a porcentagem de pessoas com necessidades de CLD, o número de instituições de CLD e o número de residentes nessas instituições. Esses países têm uma parcela maior de pessoas idosas do que a média da América Latina, mas menos instituições de CLD do que a demanda exige. Falta padronização na definição de incapacidade, CLD e dependência de cuidados nas pesquisas nacionais. Em sua maior parte, as informações sobre cuidados institucionais são fragmentadas e não incluem instituições de CLD, seus residentes e trabalhadores de maneira regular. É essencial usar dados para guiar decisões baseadas em evidências a fim de favorecer a priorização e apoiar avanços que promovam políticas para CLD institucionais na América Latina. Embora as informações sobre cuidados institucionais na região sejam fragmentadas e insuficientes, este documento traça o perfil dos quatro países selecionados, destacando a necessidade de uma estrutura melhor para sistemas de informações de CLD orientados por dados. A falta de informações ressalta a urgência de aumentar o foco no tópico e encorajar pesquisas sobre o assunto.

BACKGROUND AND PURPOSE: Non-(acute disseminated encephalomyelitis) (non-ADEM) encephalitis and/or fluid attenuated inversion recovery hyperintense lesions in anti-myelin-oligodendrocyte-glycoprotein-associated encephalitis with seizures (FLAMES) are rarely described in patients with myelin oligodendrocyte glycoprotein (MOG) antibodies (Abs). The aim was (i) to describe the clinical features and disease course of children and adults with non-ADEM encephalitis and/or FLAMES associated with MOG Abs and (ii) to describe their association with other central nervous system autoantibodies. METHODS: This was a systematic review following the PRISMA guidelines. Patients fulfilled criteria for non-ADEM encephalitis and/or FLAMES, and all were MOG Ab positive. RESULTS: In total, 83 (79%) patients with non-ADEM encephalitis (48 also had FLAMES) and 22 (21%) with isolated FLAMES were included. At the first episode, children (n = 45) had more infections (11/45, 24.4%; p = 0.017) and more of the phenotype consisting of non-ADEM encephalitis (42/45, 93.3%; p = 0.014) than adults (n = 38). Children had more episodes consistent with working memory deficits (25/54, 46.3%; p = 0.014) but fewer psychiatric symptoms (16/54, 29.6%; p = 0.002). Twenty-eight (40.6%) of 69 patients had N-methyl-d-aspartate receptor (NMDAR) Abs in cerebrospinal fluid (CSF), being more frequent in adults (19/29, 65.5%; p < 0.001). Compared to negatives, positive CSF NMDAR Abs had more relapses (14/20, 70%; p = 0.050), required ventilatory support more frequently (8/34, 23.5%; p = 0.009) and had more psychiatric episodes (28/34, 82%; p < 0.001) or abnormal movements (14/34, 41.2%; p = 0.008). Apart from an older age in FLAMES, positive and negative CSF NMDAR Ab groups shared similar features. CONCLUSION: Non-ADEM encephalitis patients with MOG Abs show specific clinical and radiological features, depending on the age at first episode. The presence of MOG Abs in non-ADEM encephalitis patients should not rule out to test other autoantibodies, especially concomitant NMDAR Abs in patients with suggestive symptoms such as behavioural or movement alterations.

2-Cyanoindene has recently been identified in the interstellar medium, however current models cannot fully account for its formation pathways. Herein, we identify and characterize 2-naphthylnitrene, which is prone to rearrange to 2- and 3-cyanoindene, in the gas phase using photoion mass-selective threshold photoelectron spectroscopy (ms-TPES). The adiabatic ionization energies (AIE) of triplet nitrene (3A'') to the radical cation in its lowest-energy doublet X̃+(2A') and quartet ã+(4A') electronic states were determined to be 7.72 ± 0.02 and 8.64 ± 0.02 eV, respectively, leading to a doublet-quartet energy splitting (ΔED-Q) of 0.92 eV (88.8 kJ mol-1). A ring-contraction mechanism yields 3-cyanoindene, which is selectively formed under mild pyrolysis conditions (800 K), while the lowest-energy isomer, 2-cyanoindene, is also observed under harsh pyrolysis conditions at 1100 K. The isomer-selective assignment was rationalized by Franck-Condon spectral modeling and by measuring the AIEs at 8.64 ± 0.02 and 8.70 ± 0.02 eV for 2- and 3-cyanoindene, respectively, in good agreement with quantum chemical calculations.

The thermal decomposition of 2- and 4-iodobenzyl iodide at high temperatures was investigated by mass-selective threshold photoelectron spectroscopy (ms-TPES) in the gas phase, as well as by matrix isolation infrared spectroscopy in cryogenic matrices. Scission of the benzylic C-I bond in the precursors at 850 K affords 2- and 4-iodobenzyl radicals (ortho- and para-IC6H4CH2•), respectively, in high yields. The adiabatic ionization energies of ortho-IC6H4CH2• to the X̃+(1A') and ã+(3A') cation states were determined to be 7.31 ± 0.01 and 8.78 ± 0.01 eV, whereas those of para-IC6H4CH2• were measured to be 7.17 ± 0.01 eV for X̃+(1A1) and 8.98 ± 0.01 eV for ã+(3A1). Vibrational frequencies of the ring breathing mode were measured to be 560 ± 80 and 240 ± 80 cm-1 for the X̃+(1A') and ã+(3A') cation states of ortho-IC6H4CH2•, respectively. At higher temperatures, subsequent aryl C-I cleavage takes place to form α,2- and α,4-didehydrotoluene diradicals, which rapidly undergo ring contraction to a stable product, fulvenallene. Nevertheless, the most intense vibrational bands of the elusive α,2- and α,4-didehydrotoluene diradicals were observed in the Ar matrices. In addition, high-energy and astrochemically relevant C7H6 isomers 1-, 2-, and 5-ethynylcyclopentadiene are observed at even higher pyrolysis temperatures along with fulvenallene. Complementary quantum chemical computations on the C7H6 potential energy surface predict a feasible reaction cascade at high temperatures from the diradicals to fulvenallene, supporting the experimental observations in both the gas phase and cryogenic matrices.

The objective of this article was to provide a consensus-based short list of effective indicators to measure health system responsiveness to the needs of older adults which would be relevant to informing public policy. An e-Delphi study was done with no direct interaction between respondents. Virtual surveys were sent to 141 participants with experience in analysis and monitoring of health indicators, management of health systems, and health care of older adults. A baseline list of 24 previously published indicators was used. The criteria for selection as high priority indicators were: usefulness as a tracer of health system responsiveness and usefulness to inform policy. Consensus was defined as: ≥70% agreement among the participants that the indicator was very high or high priority; plus being benchmarked by ≥50% of respondents as having a higher relative weight than other indicators; plus being in the top 10 in the ranking list. The first round of the process included 38 participants with varied professional backgrounds. Consensus was reached for seven indicators after two rounds. Five indicators were related to distal outcomes (mortality, disability, or healthy life expectancy), one to monitoring functional assessments, and one to poverty levels. Health systems professionals should consider these comprehensive priority indicators in their efforts to provide a better health system for older people.


El objetivo de este artículo es proporcionar una lista breve y consensuada de indicadores eficaces para medir la capacidad de respuesta del sistema de salud al atender las necesidades de las personas mayores que sean pertinentes para fundamentar las políticas públicas. Se realizó un estudio con el uso de eDelphi (el software para el método de Delfos), sin interacción directa entre las personas encuestadas. Se enviaron encuestas virtuales a 141 participantes con experiencia en el análisis y el seguimiento de indicadores de salud, la gestión de sistemas de salud y la atención de salud de las personas mayores. Se utilizó una lista de referencia de 24 indicadores publicada con anterioridad. Los criterios para seleccionar los indicadores de alta prioridad fueron: utilidad como elemento de medición de la capacidad de respuesta de los sistemas de salud y utilidad para fundamentar las políticas. El consenso se definió como lo siguiente: un acuerdo ≥70% entre los participantes de que el indicador era de prioridad muy alta o alta; que ≥50% de los encuestados consideraran que tenía un mayor peso relativo que otros indicadores; y que estuviera entre los diez primeros lugares de la lista. En la primera ronda del proceso intervinieron 38 participantes con distintas competencias profesionales. Después de dos rondas, se llegó a un consenso respecto a siete indicadores. Cinco indicadores estaban relacionados con resultados a largo plazo (mortalidad, discapacidad o esperanza de vida sana), uno con el seguimiento de evaluaciones funcionales y uno con los niveles de pobreza. Los profesionales de los sistemas de salud deberían tener en cuenta estos indicadores prioritarios integrales al adoptar medidas tendientes a proporcionar un mejor sistema de salud para las personas mayores.

Foi realizado um estudo eDelphi sem interação direta entre os entrevistados. Questionários virtuais foram enviados a 141 participantes com experiência em análise e monitoramento de indicadores de saúde, gestão de sistemas de saúde e atenção à saúde de pessoas idosas, tendo como base uma lista de 24 indicadores publicados anteriormente. Os critérios para seleção como indicadores de alta prioridade foram sua utilidade como marcador da capacidade de resposta do sistema de saúde e utilidade para informar políticas públicas. O consenso foi definido como: ≥70% de concordância entre os participantes de que o indicador tinha prioridade muito alta ou alta; avaliação por ≥50% dos entrevistados de que tinha um peso relativo maior do que outros indicadores; e posicionamento entre os 10 primeiros na lista de classificação. A primeira rodada do processo incluiu 38 participantes com diversos perfis profissionais. Após duas rodadas, chegou-se a um consenso sobre sete indicadores. Cinco estavam relacionados a resultados distais (mortalidade, incapacidade ou expectativa de vida saudável), um ao monitoramento de avaliações funcionais e o último aos níveis de pobreza. Os profissionais de sistemas de saúde devem considerar esses indicadores prioritários abrangentes em seus esforços para oferecer um sistema de saúde melhor para as pessoas idosas.

RESUMO

The objective of this article is to summarize the evolution of the regional commitments of the Pan American Health Organization (PAHO) on health promotion and strategies to improve the health and well-being of women, children, adolescents, and older persons. PAHO regional strategies approved by Member States in the last 20 years are used as the main source of information. The article presents the challenges of making health promotion a public health strategy widely applied in the Region of the Americas and the efforts to renew Member States' collective actions. The article also describes current PAHO efforts to include the positive aspects of health (i.e., well-being, optimal development, and functional ability) and the life course approach as opportunities to advance equity. The article reflects on immunization as a public good and the urgency to address the current challenges as a core element of the regional efforts to transform health systems after more than two years of the COVID-19 pandemic.


El objetivo de este artículo es resumir la evolución de los compromisos regionales de la Organización Panamericana de la Salud (OPS) en materia de promoción de la salud y estrategias para mejorar la salud y el bienestar de mujeres, niños y niñas, adolescentes y personas mayores. Se han empleado como principal fuente de información las estrategias regionales de la OPS aprobadas por los Estados Miembros en los últimos 20 años. En el artículo se presentan los desafíos de convertir la promoción de la salud en una estrategia de salud pública de amplia ejecución en la Región de las Américas y los esfuerzos para renovar las medidas colectivas de los Estados Miembros. Asimismo, se describe la labor actual de la OPS para incluir los aspectos positivos de la salud (como el bienestar, el desarrollo óptimo y la capacidad funcional) y el enfoque del curso de vida como oportunidades para fomentar la equidad. Finalmente, se reflexiona sobre la inmunización como bien público y la urgencia de abordar los desafíos actuales como elemento central de los esfuerzos regionales para transformar los sistemas de salud tras más de dos años de pandemia de COVID-19.


O objetivo deste artigo é resumir a evolução dos compromissos regionais da Organização Pan-Americana da Saúde (OPAS) relativos à promoção da saúde e estratégias para melhorar a saúde e o bem-estar de mulheres, crianças, adolescentes e pessoas idosas. As estratégias regionais da OPAS aprovadas pelos Estados Membros nos últimos 20 anos são a principal fonte de informação. O artigo apresenta os desafios enfrentados para fazer da promoção da saúde uma estratégia de saúde pública amplamente aplicada na Região das Américas e os esforços para renovar as ações coletivas dos Estados Membros. O artigo também descreve os atuais esforços da OPAS para incluir os aspectos positivos da saúde (isto é, bem-estar, desenvolvimento ideal e habilidade funcional) e a abordagem de curso da vida como oportunidades para promover a equidade. O artigo faz reflexões sobre a imunização como um bem público e a urgência de abordar os desafios atuais como um elemento central dos esforços regionais para transformar os sistemas de saúde após mais de dois anos da pandemia de COVID-19.

OBJECTIVE: To develop N-(levodopa) chitosan derivatives through click chemistry to study their effect in brain cells.Significance: This study presents a proof-of-concept that macromolecules such as N-(Levodopa) chitosan derivatives traverse brain cell membranes and induce biomedical functionalities. METHODS: Through click chemistry, we developed N-(levodopa) chitosan derivatives. They were physically and chemically characterized by FT-IR, 1H-NMR, TGA and Dynamic Light Scattering analyses. Solution and nanoparticles of N-(levodopa) chitosan derivatives were tested in primary cell cultures from the postnatal rat olfactory bulb, substantia nigra and corpus callosum. Ca2+ imaging and UPLC experiments were used to investigate if the biomaterial modulated the brain cell physiology. RESULTS: N-(levodopa) chitosan derivatives induced intracellular Ca2+ responses in primary cell cultures of the rat brain. UPLC experiments indicated that levodopa attached to chitosan was converted into dopamine by brain cells. CONCLUSION: The present study shows that N-(levodopa) chitosan may be useful to develop new treatment strategies, which could serve as molecular reservoirs of biomedical drugs to treat degenerative disorders of the nervous system.

RESUMO

The life course approach effectively responds to pressing health needs and fills critical gaps to improve health outcomes in the era of COVID-19 and beyond. This article outlines four main reasons to adopt and implement the life course approach in public health at national and local levels: (i) the approach effectively responds to new health trends and evidence, (ii) it fills longstanding gaps in care, (iii) it best addresses health inequities, and (iv) it can help achieve more with less.


El enfoque del curso de vida da respuestas eficaces a las urgentes necesidades de salud y salva brechas críticas para mejorar los resultados de salud en la era de la COVID-19 y los años posteriores. En este artículo se describen las cuatro razones principales para adoptar y poner en práctica el enfoque del curso de vida en la salud pública a nivel local y nacional: este enfoque (i) da respuestas eficaces a las tendencias y la evidencia nuevas en el ámbito de la salud, (ii) salva brechas de larga data en la atención, (iii) aborda de la mejor manera posible las inequidades de salud, y (iv) puede contribuir a lograr más con menos recursos.


A abordagem de curso de vida responde de forma efetiva a necessidades urgentes de saúde e preenche lacunas críticas para melhorar os resultados de saúde na era da COVID-19 e mais além. Este artigo descreve quatro motivos principais para adotar e implementar a abordagem de curso de vida na saúde pública em nível nacional e local: (i) a abordagem responde de forma efetiva a novas evidências e tendências em saúde, (ii) preenche lacunas antigas nos cuidados de saúde, (iii) lida melhor com iniquidades em saúde e (iv) pode ajudar a conquistar mais com menos recursos.

Hearing loss is one of the leading causes of disability worldwide and its prevalence increases with age, though in most cases, this disability is potentially preventable and treatable. Thus, its prevention, early identification, and comprehensive rehabilitation should be points of action at the local level, since hearing loss has been linked to numerous adverse health outcomes affecting the physical, mental, social, and economic conditions of older people. The objective of this article is to highlight the importance of hearing loss prevention, promote rehabilitation throughout the life course, especially in older people, and describe strategies put forth by the World Health Organization and Pan American Health Organization.


A perda auditiva representa uma das principais causas de incapacidade em todo o mundo e sua prevalência aumenta com a idade. Representa um tipo de deficiência que, na maioria dos casos, é potencialmente prevenível e tratável e, por essa razão, sua prevenção, identificação precoce e reabilitação integral devem ser alguns dos pontos de ação no âmbito local, considerando que está relacionada a inúmeros desfechos adversos de saúde, incluindo os domínios físico, mental, social e econômico das pessoas idosas. O objetivo deste artigo é ressaltar a importância da prevenção da perda auditiva e de sua reabilitação ao longo da vida, especialmente nas pessoas idosas, e descrever estratégias propostas pela Organização Mundial da Saúde e pela Organização Pan-Americana da Saúde.

Objective: Describe health outcomes for men based on analysis of their interrelationship with masculinities, which will make it possible to identify actions to improve health outcomes of men, women, and children. Methods: Update and expansion of the findings stated in the report on Masculinities and Health in the Region of the Americas, published by the Pan American Health Organization in 2019, which was based on a targeted, up-to-date search for secondary data on men's health and other research addressing the topic. Results: Men's and women's health is different, not only because of biological factors, but also because of gender constructs and inequalities, and the intersection of social determinants. Considerable differences are seen in mortality and morbidity patterns by sex over the life course, including men's over-mortality from preventable causes such as violent deaths, road accidents, and use of alcohol and other drugs (cocaine, cannabis, and amphetamines, among others). Several causes of mortality and morbidity are linked to expressions of the hegemonic model of masculinity, which endangers the overall health of men and people close to them. Conclusions: We propose the adoption of coordinated and intersectoral policies with a relational and intersectional gender perspective that includes men, in order to generate health actions in all policies aimed at men in all their diversity, with positive consequences as well for children, adolescents, and women.


Objetivo: Descrever os desfechos da saúde dos homens a partir da análise de sua inter-relação com as masculinidades, que permitirá identificar ações para melhorar os desfechos de saúde de homens, mulheres e crianças. Métodos: Atualização e ampliação dos achados do relatório Masculinidades e Saúde nas Américas, publicado pela Organização Pan-Americana da Saúde em 2019, que foi realizado a partir de uma busca intencional e atualizada de dados secundários sobre a saúde do homem e outras pesquisas que abordam o tema. Resultados: A saúde dos homens e das mulheres é distinta não apenas por fatores biológicos, mas também pela construção e pelas desigualdades de gênero, e pela intersecção dos determinantes sociais. Existem diferenças importantes nos padrões de morbimortalidade por sexo ao longo do curso de vida, incluindo a sobremortalidade dos homens por causas evitáveis como mortes violentas, acidentes de trânsito, e consumo de álcool e outras drogas (cocaína, cannabis e anfetaminas, entre outros). Várias causas de mortalidade e morbidade estão vinculadas à expressão do modelo hegemônico de masculinidade, que representa um risco à saúde integral dos homens e das pessoas próximas a eles. Conclusões: Propomos o desenvolvimento de políticas coordenadas e intersetoriais, com perspectiva de gênero relacional e interseccional que inclua homens, a fim de gerar ações de saúde em todas as políticas para a diversidade dos homens, com consequências positivas também para crianças, adolescentes e mulheres.

This paper analyzes the field performance of two cup anemometers installed in Zaragoza (Spain). Data acquired over almost three years, from January 2015 to December 2017, were analyzed. The effect of the different variables (wind speed, temperature, harmonics, wind speed variations, etc.) on two cup anemometers was studied. Data analysis was performed with ROOT, an open-source scientific software toolkit developed by CERN (Conseil Européen pour la Recherche Nucléaire) for the study of particle physics. The effects of temperature, wind speed, and wind dispersion (as a first approximation to atmospheric turbulence) on the first and third harmonics of the anemometers' rotation speed (i.e., the anemometers' output signature) were studied together with their evolution throughout the measurement period. The results are consistent with previous studies on the influence of velocity, turbulence, and temperature on the anemometer performance. Although more research is needed to assess the effect of the anemometer wear and tear degradation on the harmonic response of the rotor's angular speed, the results show the impact of a recalibration on the performance of an anemometer by comparing this performance with that of a second anemometer.

Solvation is a complex phenomenon involving electrostatic and van der Waals forces as well as chemically more specific effects such as hydrogen bonding. To disentangle global solvent effects (macrosolvation) from local solvent effects (microsolvation), we studied the UV-vis and IR spectra of a solvatochromic pyridinium-N-phenolate dye (a derivative of Reichardt's dye) in rare gas matrices, in mixtures of argon and water, and in water ice. The π-π* transition of the betaine dye in the visible region and its C-O stretching vibration in the IR region are highly sensitive to solvent effects. By annealing argon matrices of the betaine dye doped with low concentrations of water, we were able to synthesize 1:1 water-dye complexes. Formation of hydrogen-bonded complexes leads to small shifts of the π-π* transition only, as long as the global polarity of the matrix environment does not change. In contrast, changes of the global polarity result in large spectral band shifts. Hydrogen-bonded complexes of the betaine dye are more sensitive to global polarity changes than the dye itself, explaining why ET values determined with Reichardt's dyes are very different for protic and nonprotic solvents, even if the relative permittivities of these solvents are similar.

Among all C-, N-, and O-centered polyradicals, high-spin nitrenes possess the largest magnetic anisotropy and are of considerable interest as multi-level molecular spin systems for exploration of organic molecular magnetism and quantum information processing. Although the first representatives of quintet and septet nitrenes were obtained almost 50 years ago, the experimental and theoretical studies of these highly reactive species became possible only recently, owing to new achievements in molecular spectroscopy and computational chemistry. Meanwhile, dozens of various quintet dinitrenes and septet trinitrenes were successfully characterized by IR, UV/Vis, and EPR spectroscopy, thus providing important information about the electronic structure, magnetic properties and reactivity of these compounds.

OBJECTIVE: To describe the life expectancy, healthy life expectancy, disease burden, and leading causes of mortality and disability in adults aged 65 years and older in the Region of the Americas from 1990 to 2019. METHODS: We used estimates from the Global Burden of Disease Study 2019 to examine the level and trends of life expectancy, healthy life expectancy, years of life lost, years lived with disability, and disability-adjusted life years (DALYs). RESULTS: Across the Region, life expectancy at 65 years increased from 17.1 years (95% uncertainty intervals (UI): 17.0-17.1) in 1990 to 19.2 years (95% UI: 18.9-19.4) in 2019 while healthy life expectancy increased from 12.2 years (95% UI: 10.9-12.4) to 13.6 years (95% UI: 12.2-14.9). All-cause DALY rates decreased in each older persons' age group; however, absolute proportional DALYs increased from 22% to 32%. Ischemic heart disease, stroke, and chronic obstructive pulmonary disease were the leading causes of premature mortality. Diabetes mellitus, age-related and other hearing loss, and lower back pain were the leading causes of disability. CONCLUSION: The increase in life expectancy and decrease of DALYs indicate the positive effect of improvements in social conditions and health policies. However, the smaller increase in healthy life expectancy suggests that, despite living longer, people spend a substantial amount of time in their old age with disability and illness. Preventable and controllable diseases account for most of the disease burden in older adults in the Americas. Society-wide and life-course approaches, and adequate health services are needed to respond to the health needs of older people in the Region.

In the current context of the aging of populations and the increase in multiple chronic conditions and dependence, it is important that health systems provide opportunities to improve capacities of older adults to enable healthy aging. Opportunities to enhance older adults' abilities, including self-management, can be offered through evidence-based programs. Such programs have been proven effective in improving individuals' symptoms and quality of life, often lowering health-care costs. Self-management evidence-based programs can foster the development of personal skills, increase confidence and motivation on self-care, and help individuals to make better decisions about their own health. This special report describes the implementation history of a self-management program in the Region of the Americas, and the barriers to and facilitators of implementation that can serve as examples for evidence-based program dissemination in the Region.


En el contexto actual de envejecimiento poblacional y aumento de la dependencia y de diversas enfermedades crónicas, es importante que los sistemas de salud brinden oportunidades para mejorar las capacidades de las personas mayores para propiciar un envejecimiento saludable. Las oportunidades de mejorar las capacidades de las personas mayores, incluido el autocuidado, se pueden ofrecer mediante programas basados en la evidencia. Estos programas han resultado eficaces para mejorar y la calidad de vida y los síntomas de las personas y, a menudo, para reducir los costos de salud. Los programas de autocuidado basados en la evidencia pueden fomentar el desarrollo de aptitudes personales, aumentar la confianza y la motivación sobre el autocuidado y ayudar a las personas a tomar mejores decisiones sobre su propia salud. En este informe especial se describe el proceso de ejecución de un programa de autocuidado en la Región de las Américas, y los factores facilitadores y los obstáculos para la ejecución que pueden servir de ejemplo para la difusión de los programas basados en la evidencia en la Región.


No contexto atual de envelhecimento populacional e o consequente aumento de diversas doenças crônicas e da dependência, é importante que os sistemas de saúde criem oportunidades para a melhora da capacidade funcional da pessoa idosa visando ao envelhecimento saudável. Programas desenvolvidos com base em evidências científicas podem ser oferecidos porque comprovadamente ajudam a melhorar os sintomas e a qualidade de vida da pessoa idosa, reduzindo os custos em saúde. Os programas com enfoque no autocuidado estimulam o desenvolvimento de habilidades pessoais, aumentam a confiança e a motivação das pessoas idosas no próprio cuidado e contribuem para que elas tomem melhores decisões sobre a própria saúde. Este informe especial apresenta um programa de autocuidado implementado na Região das Américas, com a descrição do processo de implementação, das barreiras e dos facilitadores ­ uma experiência que pode servir de exemplo para difundir programas com base em evidências científicas na região.

OBJECTIVE: To identify key indicators that will allow empirical measurement of a health system's responsiveness to older people. METHODS: We conducted a series of consultations with experts to develop a relevant list of indicators. Concept mapping was used to devise the list, including the steps of preparation, brainstorming and structuring. Additionally, four countries were used as national case studies to test the feasibility of measuring health system responsiveness with readily available national-level data (Barbados, Brazil, Chile, and Mexico). RESULTS: Our study resulted in a list of 25 indicators scored with high usefulness for informing public policy, 10 of which were also categorized as being of high availability. National case studies were useful to assess the feasibility of measuring health system responsiveness in different settings. CONCLUSIONS: Responsiveness can be comprehensively assessed by (i) approaching the intrinsic features of the system via its inputs, outputs, and outcomes, and (ii) measuring the impact of the system on meeting the needs of older people in terms of their health, financial protection, and expectations. Further consensus is needed to develop a list of core indicators that could be used as a baseline for measuring a health system's responsiveness to the needs of older people.

The Region of the Americas is facing accelerated demographic and epidemiological changes. As these trends will continue in future years, long-term care needs are expected to rise. How can countries respond to these challenges? We propose that countries in the Region should invest in the implementation of long-term care systems. Considering the heterogeneity in the Region, we propose a strategy based on three components: (i) understanding the problem; (ii) thinking about solutions; and (iii) building support and consensus. Depending on each country's needs and capacities, these three elements suggest short-term and long-term actions and goals, from generating better information on long-term care needs to the implementation of long-term care systems. Long-term care is a relevant issue for the Region today. The task is challenging, but countries need to embrace it and move forward before it is too late.


La Región de las Américas está haciendo frente a acelerados cambios demográficos y epidemiológicos. Considerando que estas tendencias se mantendrán en los años venideros, se prevé que la necesidad de cuidados a largo plazo se incrementará. ¿Cómo pueden responder los países a estos retos?Proponemos que los países de la Región inviertan en el establecimiento de sistemas de cuidados a largo plazo. Considerando la heterogeneidad de la Región, proponemos una estrategia fundamentada en tres componentes: a) comprender el problema; b) pensar en soluciones; y c) generar apoyo y consenso. De acuerdo con las necesidades y las capacidades de cada país, estos tres elementos determinan los objetivos y las medidas a corto y largo plazo, desde la producción de mejor información sobre las necesidades de cuidados a largo plazo hasta el establecimiento de sistemas de cuidados a largo plazo.Hoy en día, los cuidados a largo plazo son una cuestión pertinente en la Región. Aunque esta tarea represente un reto, los países deben aceptarlo y progresar antes de que sea demasiado tarde.


A Região das Américas encontra-se em transição demográfica e epidemiológica acelerada. Esta tendência deve persistir nos próximos anos e antecipa-se um crescimento da demanda por assistência a longo prazo. Como os países podem enfrentar estes desafios?A nossa proposta é que os países das Américas invistam na implementação de sistemas de assistência a longo prazo. Como a Região se caracteriza pela heterogeneidade entre os países, propomos uma estratégia alicerçada em três componentes: (i) entender o problema, (ii) buscar soluções e (iii) granjear apoio e consenso. Dependendo das necessidades e da capacidade de cada país, esses três componentes devem servir para estabelecer metas e ações a curto ou longo prazo, que vão desde aprofundar o conhecimento sobre as necessidades existentes a implementar os sistemas em si. A assistência a longo prazo é uma questão atual de interesse para a Região. A tarefa envolve muitos desafios, mas os países precisam encampá-la e avançar antes que seja tarde demais.

The photoionization of phenylnitrene was investigated by photoion mass-selected threshold photoelectron spectroscopy in the gas phase. Flash vacuum pyrolysis of phenyl azide at 480 °C produces the nitrene, which subsequently rearranges at higher temperatures affording three isomeric cyanocyclopentadienes, in contrast to low-temperature trapping experiments. Temperature control of the reactor and threshold photoelectron spectra allows for optimizing the generation of phenylnitrene or its thermal rearrangement products, as well as obtaining vibrational information for the corresponding ions. The adiabatic ionization energies (AIE) of the triplet nitrene (3A2) to the radical cation in its lowest-energy doublet (2B2) and quartet (4A1) spin states were determined to 8.29 ± 0.01 and 9.73 ± 0.01 eV, respectively. Vibrational frequencies of ring breathing modes were measured at 500 ± 80 and 484 ± 80 cm-1 for both the [Formula: see text](2B2) and [Formula: see text](4A1) cationic states, respectively. The AIE differ from the values previously reported; hence, we revise the doublet-quartet energy splitting of the phenylnitrene radical cation to 1.44 eV, in excellent agreement with composite methods and coupled cluster calculations, but considerably higher than the literature reference (1.1 eV).

In bivalve mollusk aquaculture, massive disease outbreaks with high mortality and large economic losses can occur, as in northwest Mexico in the 1990s. A range of pathogens can affect bivalves; one of great concern is ostreid herpesvirus 1 (OsHV-1), of which there are several strains. This virus has been detected in the Gulf of California in occasional or sporadic samplings, but to date, there have been few systematic studies. Monthly samples of Crassostrea gigas, water, and sediment were taken in the La Cruz coastal lagoon and analyzed by PCR. The native mollusk, Dosinia ponderosa, which lives outside the lagoon, was sampled as a control. The virus was found throughout the year only in C. gigas, with prevalence up to 60%. In total, 9 genotype variants were detected, and genetic analysis suggests that linear genotypic evolution has occurred from strain JF894308, present in La Cruz in 2011. There has been no evidence of the entry of new viral genotypes in the recent past, thus confinement of the virus within the lagoons of the Gulf of California could promote a native genotypic diversity in the short term.
