PURPOSE: Early integration of palliative care (PC) is recommended. The determination of predictors for patients' request for PC may guide implementation in clinical practice. Toward this end, we analyzed the symptom burden and distress of cancer patients in outpatient care and examined their need and request for PC. METHODS: Between October 2013 and March 2016, 705 patients receiving outpatient cancer treatment took part in the survey. We used the new MInimal DOcumentation System to detect symptom clusters. Additionally, patients' request for palliative and psychosocial support was assessed. Groups of patients with PC request were compared to patients without PC request regarding their symptom clusters. Logistic regression analysis was applied to discover significant predictors for the requested inclusion of PC. RESULTS: A total of 159 patients (25.5%) requested additional support by PC. Moderate and severe tiredness (40.3%), weakness (37.9%), pain (25.0%), loss of appetite (22.3%), and dyspnea (19.1%) were the most frequent symptoms. The group of patients requesting PC differed significantly in terms of pain, nausea, dyspnea, constipation, weakness, loss of appetite, tiredness, depression, and anxiety from patients without request for PC (p < .01). The perceived need for PC was identified by the significant predictors "depression," "anxiety," and "weakness" with an explained variance of 22%. CONCLUSION: Combining a standardized screening questionnaire and the assessment of patients' request for PC allows systematic monitoring for patients' need for PC in a large Medical Oncology clinic. Depression, anxiety, and weakness are predictors of requesting PC service by patients receiving outpatient cancer treatment.

BACKGROUND: Breathing dry, cold air may cause bronchoconstriction in asthmatics working with self-contained breathing apparatus (SCBA). Air delivered by SCBA is cooler than ambient air. It is unclear whether a bronchial challenge test using mannitol (BCTM) can predict a fall in forced expiratory volume in one second (FEV(1)) during exercise with SCBA. METHODS: A prospective study of army recruits assigned to work with SCBA was carried out. Participants completed self-administered questionnaires on respiratory symptoms, BCTM, and measurement of exhaled nitric oxide. A subgroup of participants with a positive BCTM (BTCM+) and with negative BCTM underwent exercise test on bicycle ergometer (ET) while using SCBA. RESULTS: One hundred and six recruits participated in this study. One hundred and two underwent BCTM and 28 ET. Nineteen out of 97 participants had a positive BCTM and 6 out of 27 had a positive ET. Seventeen out of 19 participants with a positive BCTM had current respiratory symptoms such as wheeze, cough, chest tightness, or dyspnea. Sensitivity, specificity, positive predictive value, and negative predictive value for a positive ET under SCBA were 100%, 64%, 43%, and 100% for the BCTM, respectively. BCTM-positive participants with respiratory symptoms had more physician consultations during MBT compared to BCTM-negative participants. CONCLUSION: Despite medical evaluation and exclusion of asthmatics by a physician during conscription, a high proportion of recruits assigned to work with SCBA smoke, have respiratory symptoms, and have a positive BCTM suggesting current asthma. BCTM has a high sensitivity to predict a significant fall in FEV(1) after exercise with SCBA and participants with a positive BCTM tend to use more health care resources during basic military training. Recruits should be counseled about smoking cessation prior and during basic military training.

BACKGROUND: Deregulation of signal transduction pathways plays a critical role in oncogenesis of colorectal cancer (CRC) and directly affects sensitivity to targeted therapies. Against this background we developed a comprehensive biomarker profiling program including markers of downstream signaling to study their association with clinical outcomes. PATIENTS AND METHODS: A prospectively studied cohort of 160 patients with metastatic CRC was included. Standard diagnostic workup included mutational analyses of Kirsten rat sarcoma viral oncogene homolog (KRAS), neuroblastoma RAS viral oncogene homolog (NRAS), and v-Raf murine sarcoma viral oncogene homolog B (BRAF). In addition, markers of mitogen-activated protein kinase (MAPK), phosphatidylinositol 3-kinase/protein kinase B (PI3K/AKT) and mammalian target of rapamycin pathway activation (phosphorylation of extracellular signal-regulated kinase [ERK], AKT, and p70 ribosomal protein S6 kinase ß-1 [p70S6K]) were studied using standardized immunohistochemistry. RESULTS: There was a significant correlation between markers of ERK and AKT activation in the full cohort. In addition, phosphorylation of p70S6K correlated strongly with ERK and AKT phosphorylation and primary tumor localization in the right colon. Subgroup analyses specified these correlations to patients with all-RAS wild type tumors. In contrast, tumors harboring RAS mutations predominantly exhibited ERK phosphorylation. Interestingly, patients with CRC showing high p70S6K phosphorylation (highest quartile) had a significantly inferior overall survival (hazard ratio [HR], 2.4; P = .002) irrespective of RAS mutational status. This effect remained significant in multivariate analysis (P = .002). A patient subgroup characterized by high p70S6K phosphorylation and right-sided primary tumors had a particularly poor prognosis with a dramatically inferior overall survival (HR, 5.2; P < .001). Patients with right-sided primary tumor and low p70S6K phosphorylation had responses to anti-epidermal growth factor receptor antibody-based therapies and overall survival similar to patients with left-sided primary tumors. CONCLUSION: High phosphorylation of p70S6K is a novel, independent biomarker for poor prognosis, in particular in patients with right-sided primary tumors.
