BACKGROUND: This meta-analysis aims to investigate the efficacy of early rehabilitation on patients who have undergone surgery for distal radius fractures (DRFs) with palmar plating, focusing on multiple outcome measures including upper limb function, wrist function, back extension mobility, pain levels, and complications. METHODS: A rigorous search strategy adhering to the PRISMA guidelines was employed across four major databases, including PubMed, Embase, Web of Science, and the Cochrane Library. Studies were included based on stringent criteria, and data extraction was performed independently by two reviewers. Meta-analysis was conducted employing both fixed-effect and random-effects models as dictated by heterogeneity, assessed by the I2 statistic and chi-square tests. A total of 7 studies, encompassing diverse demographic groups and timelines, were included for the final analysis. RESULTS: The meta-analysis disclosed that early rehabilitation yielded a statistically significant improvement in upper limb function (SMD -0.27; 95% CI -0.48 to -0.07; P < 0.0001) and back extension mobility (SMD 0.26; 95% CI 0.04 to 0.48; P = 0.021). A notable reduction in pain levels was observed in the early rehabilitation group (SMD -0.28; 95% CI -0.53 to -0.02; P = 0.03). However, there were no significant differences in wrist function (SMD -0.13; 95% CI -0.38 to 0.12; P = 0.36) and complications (OR 0.99; 95% CI 0.61 to 1.61; P = 0.96). CONCLUSIONS: Early rehabilitation post-DRF surgery with palmar plating has been found to be beneficial in enhancing upper limb functionality and back extension mobility, and in reducing pain levels. Nevertheless, no significant impact was observed regarding wrist function and complications.

Firefighters often experience low back pain (LBP), but their back muscle characteristics are not well studied. This study aimed to 1) compare the biomechanical characteristics of back muscles and self-reported back disabilities in frontline firefighters with and without LBP history, and 2) examine the relationships between back disability and biomechanical measurements. We recruited 42 male firefighters and assessed their perceived pain and disabilities, maximum isometric back extension strength, passive stiffness, and fatigability of the longissimus. 54.8% of the participants experienced LBP within the past year. Those indicating higher pain intensity also had greater disability as indicated by the Oswestry Disability Index. There were no significant differences in strength, stiffness or fatigability of the back muscles between firefighters with and without LBP history. Multiple linear regression analysis revealed no significant relationship between the back disability and any biomechanical or demographic measures, likely due to the high functional abilities of the participants.


Approximately 50% of the firefighters who participated in the study experienced low back pain in the past 12 months. Since the pain level was mild to moderate, most of them continued to report to work. Biomechanically, there were no differences in back muscle strength, stiffness, resistance to fatigue, or left-right symmetry between firefighters with and without back pain history. Back disabilities were not related to any biomechanical measures or demographics including age and body mass index. Overall, despite experiencing some back pain, these frontline firefighters are highly functional and did not show diminished physical or neuromuscular responses.

PURPOSE: Redundancy of the musculoskeletal system implies multiple strategies are theoretically available to coordinate back extensor muscles. This study investigated whether coordination between back muscles during a tightly constrained isometric trunk extension task varies within and between individuals, and whether this changes following brief exposure to activation feedback of a muscle. METHODS: Nine healthy participants performed three blocks of two repetitions of ramped isometric trunk extension in side-lying against resistance from 0-30% of maximum voluntary contraction over 30 s (force feedback). Between blocks, participants repeated contractions with visual feedback of electromyography (EMG) from either superficial (SM) or deep multifidus (DM), in two conditions; 'After SM' and 'After DM'. Intramuscular EMG was recorded from SM, DM, and longissimus (LG) simultaneously with shear wave elastography (SWE) from SM or DM. RESULTS: In the 'Natural' condition (force feedback only), group data showed incremental increases in EMG with force, with minor changes in distribution of activation between muscles as force increased. SM was the most active muscle during the 'Natural' condition, but with DM most active in some participants. Individual data showed that coordination between muscles differed substantially between repetitions and individuals. Brief exposure to EMG feedback altered coordination. SWE showed individual variation, but findings differed from EMG. CONCLUSION: This study revealed substantial variation in coordination between back extensor muscles within and between participants, and after exposure to feedback, in a tightly constrained task. Shear modulus revealed similar variation, but with an inconsistent relationship to EMG. These data highlight highly flexible control of back muscles.

PURPOSE: Repeated back extension exercises (RBEEs) have been reported to cause changes in the distribution and intensity of radicular symptoms. Therefore, the objective of this study was to investigate the effects of RBEEs on the neurophysiology of the compromised nerve root and on standing mobility and pain intensity in patients with sub-acute and chronic lumbosacral radiculopathy (LSR). SUBJECTS AND METHODS: A total of 40 patients with unilateral sub-acute/chronic LSR voluntarily participated in the study; the patients performed three sets of 10 RBEEs in the prone position with 1 min of rest between the sets. The soleus H-reflex, standing mobility, and pain intensity were recorded before and after the RBEEs. RESULTS: The results of the study showed that the RBEEs significantly improved the H-reflex, standing mobility, and pain intensity in patients with sub-acute LSR (p < .01); there was not a significant improvement in the patients with chronic LSR (p < .61). CONCLUSION: RBEEs in the prone position are recommended for improving the neurophysiological function of the compromised nerve root and standing mobility in patients with sub-acute LSR.

BACKGROUND: We suggested a standing back extension exercise 'One Stretch' based on the McKenzie method, to examine the ability to improve or prevent low back pain (LBP) in Japanese care workers. METHODS: We conducted a single-center, non-randomized, controlled study in Japan. Care workers in an intervention group received an exercise manual and a 30-minute seminar on LBP and were encouraged with a group approach, while care workers in a control group were given only the manual. All care workers answered questionnaires at the baseline and end of a 1-year study period. The subjective improvement of LBP and compliance with the exercise were evaluated. RESULTS: In all, 64 workers in the intervention group and 72 in the control group participated in this study. More care workers in the intervention group exercised regularly and improved or prevented LBP than in the control group (P = 0·003 and P<0·0001, respectively). In the intervention group, none had a first medical consultation or were absent from disability for LBP by the end of the study period. CONCLUSION: The exercise 'One Stretch' would be effective to improve or prevent LBP in care workers. Our group approach would lead to better compliance with the exercise.

The objective of this study was to determine the effects of a 10-week dynamic back extension training programme and its effects on back muscle strength, back muscle endurance and spinal range of motion (ROM) for healthy young females. Seventy-three young females (age: 19.32±1.80 years, height: 158.89±4.71 cm, body weight: 55.67±6.30 kg) volunteered for the study. Prior to the training period, all participants completed anthropometric measurements, back muscle strength and endurance test, lateral bending and spinal ROM measurements. After measurements, the participants were divided into two groups. The exercise group (N:35) performed the dynamic back extension exercise 3 days per week for 10 weeks. The control group (N:38) did not participate in any type of exercise. The mixed design ANOVA (group x time) was used to determine the difference in pre- and post-training values. The present findings show that there were significant differences between pre-training and post-training values for back muscle strength and spinal ROM in the exercise group. Following the dynamic strength training programme, back muscle strength and spine ROM values except flexion of the lumbar 5-sacrum 1 (L5-S1) segment of the exercise group showed a significant increase when compared with the pre test values. The control group did not show any significant changes when compared with the pre-training values. The results demonstrate that the 10-week dynamic strength training programme was effective for spinal extension ROM and back muscle strength, but there was no change in back muscle endurance. In this context, this programme could potentially be used to prevent the decrease of spinal ROM as well as provide an increase in the fitness parameters of healthy individuals.

Background: Despite preliminary evidence demonstrating the relevance of trunk muscle strength for physical function in older adults, it is not clear which muscle-related trunk parameter is the best predictor for physical functions. Therefore, this study aimed to compare trunk muscle morphology or strength parameters regarding their predictive ability for physical functions. Methods: Seventy-four older adults (38 men, 36 women, mean age 76.85 years) were tested for maximum absolute and relative isokinetic trunk flexion and extension strength, trunk lean mass, and trunk muscle quality. Functional assessment included normal and fast walking speed, repeated sit-to-stand transfer, timed up and go, and postural sway during a closed-feet and a semi-tandem stance adjusted for body height. Pearson's correlations were used to compare relationship between trunk strength adjusted and unadjusted for body weight to physical functions. Linear regression analysis including sex and age as co-variables was performed between trunk muscle and functional test parameters. Results: Relative back extension strength was the most consistent significant predictor for all physical function tests (p = 0.004-0.04) except for postural sway. Relative trunk flexion strength was related to normal walking speed (p = 0.024). Trunk lean mass was related to timed up and go performance (p = 0.024). Conclusion: Relative back extension strength is associated with better performance in nearly all standard tests for physical function in older adults, while trunk flexion strength and lean mass seem to play a minor role. Our findings emphasize the importance of trunk muscle strength, especially the back extensor muscles, for physical function in older adults.

BACKGROUND: The optimum repetition number of standing back-extension exercise (SBEE) effective for the prevention and improvement of low back pain (LBP) is unknown. OBJECTIVE: To determine the effect of physiotherapy on LBP by investigating the optimum repetition number of SBEE via optical analysis and electromyographic (EMG) examination of the multifidus muscles. METHODS: Hemodynamics and multifidus muscle activity were examined in 16 healthy adult men using near-infrared spectroscopy and surface EMG after performing repetitive SBEE. RESULTS: Oxidized hemoglobin (Oxy-Hb) levels significantly increased in the second extension phase but decreased in the third and subsequent extension phases; deoxidized hemoglobin (deOxy-Hb) levels increased in the third and subsequent extension phases. In the standing phase, no significant difference was observed; in the third and subsequent phases, Oxy-Hb levels decreased and deOxy-Hb levels increased. Muscular activity significantly decreased in the second standing phase but increased in the third and subsequent phases. No significant difference was observed in the extension phase with respect to the number of SBEE repetitions performed. In healthy individuals, hemodynamics improved up to second repetition of SBEE; subsequent repetitions may decrease hemodynamics because of increased activity of the multifidus muscles. CONCLUSIONS: In healthy individuals, hemodynamics improved up to second repetition of SBEE.
