BACKGROUND: This study aimed to develop an artificial intelligence-based surgical support model for assessing the acetabular component angle using intraoperative radiographs during total hip arthroplasty and verify its accuracy. METHODS: A total of 268 hips were analyzed. At first, 268 preoperative and intraoperative anteroposterior pelvic radiographs were amplified to 536. These radiographs were used to create a learning model to estimate the acetabular component angle from the radiographs intraoperatively. The ground truth was the anteversion and inclination angles obtained from the computed tomography-based navigation system intraoperatively. Bone landmarks on the preoperative and intraoperative radiographs were manually annotated. The distances and angles between each landmark were used as predictor variables. The estimation accuracy was assessed for internal and external test datasets. Mean absolute error (MAE) and R2 values were used as accuracy measures. RESULTS: The MAE and R2 for the internal test set showed 2.19 and 0.850 for anteversion, and 1.18 and 0.805 for inclination, respectively. The MAE and R2 for the external test set showed 2.78 and 0.789 for anteversion, and 1.56 and 0.744 for inclination, respectively. CONCLUSIONS: We developed an artificial intelligence-based surgical support model for accurately assessing the acetabular component angle using intraoperative radiographs. Excellent estimation accuracy was confirmed for the external test set. In the future, the model may help to reduce the risk of adverse postoperative events.

BACKGROUND: Monoblock ceramic cups are designed to accommodate large-diameter femoral heads. This has the potential to offer the advantages of an increased range of motion and enhanced joint stability. These features could benefit younger and high-demand patients in need of total hip arthroplasty. The aim of this study was to assess the survival rate and the reasons for revision of the DeltaMotion cup. METHODS: Data from the AOANJRR were analyzed for all patients who had undergone a primary conventional THA performed between January 1, 2001 and December 31, 2021. Only prostheses with ceramic/ceramic, ceramic/XLPE, metal/XLPE, or CM/XLPE bearing surfaces were included. The primary outcome measure was the cumulative percent revision for all causes. Secondary outcome measures were revision for dislocation/instability, ceramic breakage, or noise. A subanalysis for cup size was also performed. RESULTS: There were 486,946 primary conventional THA procedures undertaken for any reason. Of these, 4,033 used the DeltaMotion cup and 482,913 were modular designs. The DeltaMotion cup had the lowest CPR for all diagnoses compared to the modular bearings at all time points, had a significantly lower revision rate for prosthesis dislocation and no revisions for squeaking compared to other modular bearings. There were 175 ceramic breakages recorded in the modular bearing group and 1 ceramic breakage in the DeltaMotion group. CONCLUSIONS: The DeltaMotion cup had a low rate of all-cause revision, and for dislocation, ceramic breakage, and noise. Although this cup is no longer manufactured, ongoing follow-up of newer monoblock ceramic cups will determine their suitability for younger and more active patients.

Using finite element (FE) models of intact and implanted hemipelvises, the study aimed to investigate the influences of musculoskeletal loading and implant-bone interface conditions on preclinical analysis of an uncemented acetabular component after total hip arthroplasty (THA). A new musculoskeletal loading dataset, corresponding to daily activities of sitting up-down, stairs up-down and normal walking, for a pelvic bone was generated based on previously validated Gait2392 model. Three implant-bone interface conditions, fully bonded and debonded having two rim press-fits (1 mm and 2 mm), were analyzed. High tensile (2000-2415 µÏµ) and compressive strains (900-1035 µÏµ) were predicted for 2 mm press-fit, which might evoke microdamage in pelvic cortex. Strain shielding in periprosthetic cancellous bone was higher for bonded condition during sitting up activity, compared to other combinations of interface and loading conditions. Only the nodes around acetabular rim (less than 6%) were susceptible to interfacial debonding. Although maximum micromotion increased with increase in press-fit, postoperatively for all load cases, these were within a favorable range (52-143 µm) for bone ingrowth. Micromotions reduced (39-105 µm) with bone remodeling, indicating lesser chances of implant migration. Bone apposition was predominant around acetabular rim, compared to dome, for all interface conditions. Periprosthetic bone resorption of 10-20% and bone apposition of 10-15% were predicted for bonded condition. Whereas for press-fit (1 mm and 2 mm), predominant bone apposition of 200-300% was observed. This study highlights the importance of variations in loading and interface conditions on in silico evaluations of an uncemented acetabular component.

INTRODUCTION: In total hip arthroplasty (THA), the correct position of the acetabular component directly determines the outcome of the surgery, or the success of the surgery. Therefore, how to accurately locate the position of the acetabular component has become a very critical step in THA. As an important anatomical structure of the hip joint, the transverse acetabular ligament (TAL) is helpful for acetabular component orientation in THA. The aim of this systematic review was to investigate application of TAL in THA. MATERIALS AND METHODS: A systematic literature search of PUBMED, EMBASE, and Cochrane Library was performed (January and February 2023) using keywords "total hip arthroplasty," "total hip replacement," "total hip replacements," "total hip arthroplasties," "total hip prosthesis," and "transverse acetabular ligament" in all possible combinations. Reference lists of included articles were reviewed. Study design, surgical approach, patient demographics, TAL identification rate, appearance of the TAL, anteversion and inclination angle and rate of dislocations were recorded. RESULTS: In total, 19 studies met the screening criteria. Study designs were prospective cohorts (42%), retrospective cohorts (32%), Case series (21%), and randomized controlled trial (5%). Twelve of the 19 (63.2%) studies investigated the application of TAL as an anatomical landmark for locating acetabular component position in THA. Analysis revealed that TAL is a reliable anatomical landmark for acetabular component orientation within the safe zone in THA. CONCLUSIONS: TAL can reliably be used to align the acetabular component in the safe zone for anteversion and inclination in THA. However, TAL has individual variation influenced by some risk factors. More randomized controlled studies with larger numbers of patients are needed to investigate the precision and accuracy of TAL as an intraoperative landmark in THA. LEVEL OF EVIDENCE: IV.

BACKGROUND: The incidence of dislocation after revision total hip arthroplasty (rTHA) is reported to be up to 25% and remains a common source of failure. Constrained acetabular components and dual mobility implants are two implant classes being utilized to alleviate this burden in patients who have recurrent instability or major intraoperative instability. This meta-analysis evaluated the incidence and temporal trends of dislocation after implantation with constrained acetabular components and dual mobility implants in rTHA. METHODS: Longitudinal studies reporting dislocation after the use of constrained acetabular components or dual mobility implants in rTHA were sought from Medline and Embase to October 2022. Secondary outcomes included re-revision surgery for dislocation and all causes. A total of 75 relevant citations were identified comprising 36 datasets of 3,784 constrained acetabular components and 47 datasets of 10,216 dual mobility implants. RESULTS: For constrained acetabular components, the pooled incidence of dislocation was 9% (95% confidence interval: 7.2, 11.7) (range 0.0%-35.3%) over a weighted mean follow-up of 6 years, in contrast to 3% (95% confidence interval: 2.2, 4.4) (range 0.0%-21.4%) over 5 years for dual mobility implants. Re-revision rates for dislocation after using constrained acetabular components were around 9%, in contrast to 2% for dual mobility implants. Re-revision rates for all causes after using constrained acetabular components were around 19%, in contrast to 8% for dual mobility implants. CONCLUSION: Dual mobility implants in the context of rTHA demonstrate lower incidences of dislocation (3% versus 9%), re-revision for dislocation (2% versus 9%), and rer-evision for any cause (8% versus 19%) in contrast to constrained acetabular components. This must be considered by surgeons when implanting such devices, often selected to treat instability.

BACKGROUND: Measuring cup orientation is time consuming and inaccurate, but orientation influences the risk of impingement and dislocation following total hip arthroplasty (THA). This study designed an artificial intelligence (AI) program to autonomously determine cup orientation, correct for pelvis orientation, and identify cup retroversion from an antero-posterior pelvic radiographs. METHODS: There were 2,945 patients between 2012 and 2019 identified to have 504 computed tomographic (CT) scans of their THA. A 3-dimensional (3D) reconstruction was performed on all CTs, where cup orientation was measured relative to the anterior pelvic plane. Patients were randomly allocated to training (4,000 x-rays), validation (511 x-rays), and testing (690 x-rays) groups. Data augmentation was applied to the training set (n = 4,000,000) to increase model robustness. Statistical analyses were performed only on the test group in their accuracy with CT measurements. RESULTS: AI predictions averaged 0.22 ± 0.03 seconds to run on a given radiograph. Pearson correlation coefficient was 0.976 and 0.984 for AI measurements with CT, while hand measurements were 0.650 and 0.687 for anteversion and inclination, respectively. The AI measurements more closely represented CT scans when compared to hand measurements (P < .001). Measurements averaged 0.04 ± 2.21°, 0.14 ± 1.66°, -0.31 ± 8.35°, and 6.48° ± 7.43° from CT measurements for AI anteversion, AI inclination, hand anteversion, and hand inclination, respectively. AI predictions identified 17 radiographs as retroverted with 100.0% accuracy (total retroverted, n = 45). CONCLUSION: The AI algorithms may correct for pelvis orientation when measuring cup orientation on radiographs, outperform hand measurements, and may be implemented in a timely fashion. This is the first method to identify a retroverted cup from a single AP radiograph.

OBJECTIVE: The aim: The purpose of the article is to analyze the ways of solving the problem of revision hip joint replacement. The article discusses the methods of treatment using a custom triflange acetabular component. PATIENTS AND METHODS: Materials and methods: The analysis of 37 literary sources includes a discussion of the features of the use of individual triflange acetabular components and errors in revision hip arthroplasty, which are associated with various factors. CONCLUSION: Conclusions: A review of studies devoted to the use of custom triflange acetabular components confirms the effectiveness in the early postoperative period in the treatment of critical acetabular defects and pelvic ring discontinuity. The CTAC use is particularly relevant in case of the pelvic ring disintegration, as it provides for the appropriate endoprosthesis adaptation with the healthy bone, as well as for the bone defects plastics and recovery of the hip joint biome¬chanics. So, the use of individual constructions is indicated for the patients with significant bone mass loss, where augment adaptation and adjustment is impossible. This method is used more often when there is no other alternative. Research results showed a trend that special three-flange components of the acetabulum have better long-term results compared to traditional standard components for large bone defects. Improving production and increasing the number of CTACs should reduce their cost. In summary, the custom triflange acetabulum components provide a personalized secure fit that can reduce the risk of complications and improve patient outcomes. BACKGROUND: Ceramic-on-ceramic (CoC) bearings for total hip arthroplasty (THA) have been offering very favorable results and survivorship since their introduction. In order to increase range of movement (ROM) and decrease dislocation rates, some manufacturers have introduced larger diameter head (LDH) CoC bearings. This has been achieved with the use of preassembled cup designs, in which the ceramic liner is already fitted into the metal backing and implanted as a monoblock component by the surgeon. In this report we present data from a series of 5 patients with ceramic liner dissociation from a monoblock cup. CASE PRESENTATION: All cases were overweight men with acetabular components of 56 or 58 mm. After a mean of 5.5 (range, 3.5-6.7) years, all patients reported sudden pain and audible noise when performing activities of daily living. Liner displacement was suspected on plain radiographs and confirmed by Ct-scan. Pneumarthrosis was present in all cases. Taper modular junction wear and corrosion signs were observed in the four revised patients. CONCLUSION: Although one of our case is still treated conservatively, implant revision is probably inevitable. Further LDH CoC implant design should take in consideration this potential complication by avoiding bearing diameters over 40mm and/or improving locking mechanism or by providing a real monoblock acetabular implant.

BACKGROUND: Hip instability following total hip arthroplasty (THA) can be a major cause of revision surgery. Physiological patient position impacts acetabular anteversion and abduction, and influences the functional component positioning. Osteoarthritis of the spine leads to abnormal spinopelvic biomechanics and motion, but there is no consensus on the degree of component variability for THAs performed by anterior approach. Therefore, we sought to present guidelines for changes in acetabular component positioning between supine and standing positions for patients undergoing primary THA by a uniform anterior approach. METHODS: Perioperative patient radiographs of the pelvis and lumbar spine were collected. Images were used to determine acetabular component positioning and degree of coexisting spinal pathology, categorized as a Lane Grade (LG). Final analysis of variance was performed on a sample size of 643 anterior primary THAs. RESULTS: From supine to standing position, as the severity of lumbar pathology increased the change in anteversion also increased (LG:0 = -0.11° ± 4.65°, LG:1 = 2.02° ± 4.09°, LG:2-3 = 5.78° ± 5.72°, P < .001). The mean supine anteversion in patients with absent lumbar pathology was 19.72° ± 5.05° and was lower in patients with worsening lumbar pathology (LG:1 = 18.25° ± 4.81°, LG:2-3 = 16.73° ± 5.28°, P < .001). CONCLUSION: Patients undergoing primary THA by anterior approach with worsening spinal pathology have larger increases in component anteversion when transitioning from supine to standing positions. Consideration should be given to this expected variability when placing the patient's acetabular component.

BACKGROUND: Ceramic-on-ceramic bearings permit the use of large femoral head size while maintaining a favorable effect on wear rates. However, because of increased device rigidity, periprosthetic bone quality could be negatively affected due to stress shielding. The purpose of this study is to assess pelvic periprosthetic bone remodeling around a monoblock ceramic-on-ceramic acetabular component compared to that around a conventional modular metal-on-polyethylene device. METHODS: Participants were randomized to receive hip replacement using either a porous-coated, modular metal-on-polyethylene acetabular component (n = 46) or a hydroxyapatite and titanium-coated monoblock shell with an integrated ceramic-on-ceramic bearing (n = 40). Radiographic assessments were completed preoperatively and postoperatively, and measurements of bone mineral density (BMD) using dual-energy X-ray absorptiometry with region free analysis were performed postoperatively and over 2-years of follow-up. RESULTS: There was no significant difference in BMD between the 2 groups at baseline or over the following 2 years. At follow-up, complete shell-to-bone contact without a radiolucent line was observed in 26 (67%) of the modular devices and in 37 (93%) of monoblock (P < .001). The modular device was an independent predictor of radiolucent lines (odds ratio 19.1, P = .007). No cases underwent revision surgery for acetabular loosening. CONCLUSION: Both the porous-coated modular and hydroxyapatite-coated monoblock acetabular components showed successful clinical results at short-term follow-up with no difference in pixel-level BMD. Using a large head monoblock device does not appear to be associated with an adverse effect on the local bone environment when compared to a modular device. NCT: NCT01558752.

BACKGROUND: Dislocation after a primary total hip replacement (pTHR) remains a common cause of treatment failure. Constrained acetabular components (CACs) and dual mobility implants (DMIs) may mitigate this in patients at high risk of dislocation or with significant intraoperative instability. This meta-analysis evaluated the incidence and temporal trends of dislocation after implantation with CACs and DMIs in pTHR. METHODS: Longitudinal studies reporting dislocation after the use of CACs or DMIs in pTHR were sought from Medline and Embase to September 2020. Secondary outcomes included revision surgery for dislocation and for all causes. RESULTS: A total of 46 studies (3 CAC and 43 DMI) comprising 582 CACs and 18,748 DMIs were included. The pooled incidence of dislocation was 1.08% (95% confidence interval [CI]: 0.00-3.72; range 0.27%-2.60%) over a weighted mean follow-up of 4.1 years for CACs, compared with 0.25% (95% CI: 0.08-0.46; range 0.00%-4.72%) over 6.2 years for DMIs. For DMIs, there was a temporal decline in dislocations from the 1980s onward, and dislocation rates remained low (<1%) until 15 years postoperatively. There were insufficient data for similar analysis of CACs. All studies were at high risk of bias. The incidence of revision for dislocation after CACs was 0.3% vs 0.1% for DMIs, and the incidence of revision for all causes after CACs was 4.8% vs 2.7% for DMIs. CONCLUSION: DMIs demonstrated a lower incidence of dislocation compared with CACs; however, there was a relative absence of CACs used in the context of pTHR in the literature. Temporal trends in dislocation have improved over time for DMIs.

Background and Objectives: Atraumatic intrapelvic protrusion of the acetabular component following excessive reaming of the acetabulum with a far medial positioning of the cup is a rare, but serious complication of a total hip arthroplasty (THA). This study analyzes the factors contributing to this uncommon complication and presents the outcome after the revision surgery using the Ganz reinforcement ring combined with a bone graft and plating of the posterior column and/or screws for the anterior column. Materials and Methods: A retrospective case series study with seven patients (four males, mean age 76 ± 10 years (60−86)) that underwent a revision THA within 24 ± 17 days (5−60) after an atraumatic periprosthetic acetabular fracture with a medial cup protrusion was performed. All fractures were reconstructed with a Ganz reinforcement ring and bone graft with a mean follow-up of 1.7 ± 1.7 years (0.5−5). Radiographs were evaluated for the following: (i) cup positioning immediately after the primary THA and the revision surgery, (ii) cup migration in the follow-up, and (iii) fracture healing. Results: The position of the acetabular component as assessed on the postoperative radiographs after the index surgery and before the complete medial cup protrusion showed a cup placement beyond the ilioischial line indicative of a fracture of the medial wall. The revision surgery with the reconstruction of the medial wall with a Ganz reinforcement ring combined with a bone graft restored in the presented cases the center of rotation in the horizontal direction with a statistical significance (p < 0.05). During the follow-up, there was no aseptic loosening with the relevant cup migration or significant change in the position of the acetabular cup at the final follow-up (p > 0.05) after the revision. All seven fractures and bone grafts realized a bone union until the latest follow-up. Conclusions: Following excessive reaming, the acetabular component was placed too far medially and resulted in an intrapelvic cup protrusion. An unstable cup following a fracture of the medial wall was evident on the immediate postoperative radiographs. In the case of the medial wall perforation with an intrapelvic cup protrusion after the primary THA, the reconstruction with a Ganz reinforcement ring was a successful treatment option resulting in the fracture healing and a stable cup positioning. Surgeons should be aware of that rare and probably underreported complication and restore the anatomic center of rotation by treating the defect intraoperatively.

BACKGROUND: Inappropriate acetabular component angular position is believed to increase the risk of hip dislocation after total hip arthroplasty. However, manual measurement of these angles is time consuming and prone to interobserver variability. The purpose of this study was to develop a deep learning tool to automate the measurement of acetabular component angles on postoperative radiographs. METHODS: Two cohorts of 600 anteroposterior (AP) pelvis and 600 cross-table lateral hip postoperative radiographs were used to develop deep learning models to segment the acetabular component and the ischial tuberosities. Cohorts were manually annotated, augmented, and randomly split to train-validation-test data sets on an 8:1:1 basis. Two U-Net convolutional neural network models (one for AP and one for cross-table lateral radiographs) were trained for 50 epochs. Image processing was then deployed to measure the acetabular component angles on the predicted masks for anatomical landmarks. Performance of the tool was tested on 80 AP and 80 cross-table lateral radiographs. RESULTS: The convolutional neural network models achieved a mean Dice similarity coefficient of 0.878 and 0.903 on AP and cross-table lateral test data sets, respectively. The mean difference between human-level and machine-level measurements was 1.35° (σ = 1.07°) and 1.39° (σ = 1.27°) for the inclination and anteversion angles, respectively. Differences of 5° or more between human-level and machine-level measurements were observed in less than 2.5% of cases. CONCLUSION: We developed a highly accurate deep learning tool to automate the measurement of angular position of acetabular components for use in both clinical and research settings. INTRODUCTION: Correct acetabular component positioning improves hip biomechanics, impingement free range of motion and may lead to a reduced risk of postoperative dislocation. The C-arm is a simple and cost-effective tool for THA in a supine position, however, evaluation of the images can be challenging due to parallax. The current study aimed to investigate whether a software app-based measurement technique can control acetabular component position and leg length in anterior THA. MATERIALS AND METHODS: Ninety-three patients with end-stage osteoarthritis aged 65.2 ± 9.6 years (range 30-86 years) were included in this prospective study. All patients underwent direct anterior THA using a Hana orthopedic surgery table. C-arm imaging and an iPad software app were used to determine intraoperative acetabular anteversion, inclination and leg length discrepancy. The app provides a measuring tool that is applied to intraoperative c-arm images. The intraoperative measurements were compared to the measurements of standard postoperative AP-pelvis radiographs. RESULTS: Intraoperative software app-based anteversion measurements averaged 20.5° (range 16.0° to 24.0°) compared to 20.9° (range 14.7° to 25.6°) postoperatively. Mean intraoperative inclination was 40.5° (range 35° to 48°) compared to postoperative 40.7° (range 35° to 49°). Mean intraoperative leg length discrepancy was 0.9 mm (range - 4-5 mm) compared to postoperative 0.6 mm (range - 5-6 mm). A strong Pearson's correlation was observed between the intraoperative and postoperative measurements for anteversion (r = .701; P < .0001), inclination (r = .816; P < .0001) and leg length discrepancy (r = .542; P < .0001). CONCLUSIONS: The software app used in the current study allowed for a simple and accurate measurement of intraoperative cup position and leg length in direct anterior THA.

BACKGROUND: Cross-table lateral (CL) radiography is a convenient and feasible method to assess cup version angle (VA) after total hip arthroplasty; However, pelvic tilt (PT) may contribute to its measurement inaccuracy. How PT affects CL radiographic measurements have not been well studied. We sought (1) to determine the effect of the PT on cup version measurement on CL radiography and (2) to develop a method for reducing measurement errors caused by the PT. METHODS: We used 3D technique to construct standard model and capture CL radiography simulation. A linear regression model was created to analyze the relationship between PT and VA. CL radiography and computed tomography (CT) were performed for the enrolled patients after surgery. The consistency between CL and CT measurements were verified by intra-class correlation coefficient (ICC). RESULTS: There was a high correlation between the VA and PT. For each 1-degree increased in the PT, the VA decreased by 0.76° (R2 = 0.995, p < 0.001). Based on the data, we created a corrective formula to convert the radiographic measurements into values approximating the actual VA under a natural pelvic position. The VA measurements corrected by our equation was in high agreement with the CT-measured values with reference to the corresponding PT (ICC = 0.988, p < 0.001), which was in sharp contrast to that without PT control (ICC = 0.454, p = 0.203). CONCLUSIONS: The PT may contribute to cup version measurement inaccuracies on CL radiography. Our mathematical algorithm can serve as a reliable method to improve the accuracy of CL radiography.

BACKGROUND: Severe acetabular defects require special treatment with either impaction bone grafting, metal augmented cups or cup-cage constructs. Even these options are often not adequate, especially in hips with Paprosky type 3 defects with loss of anterior and posterior columns. This study investigates the clinical and radiological outcomes of custom-made acetabular components (© Materialise NV, Leuven, Belgium) for Paprosky type 3 defects. METHODS: Sixteen patients were eligible for this trial, nine of whom agreed to be included. All of them completed one year of follow-up. The Harris hip score and the Oxford hip score were used to compare pre- and postoperative functional outcomes. Radiological follow-up comprised anteversion and inclination of the implanted cup and offset measurements in both hips (femoral, medial, ischial offset and center of rotation). Statistical analyses were performed with IBM SPSS Statistics. RESULTS: The mean follow-up time of the nine patients was 12.2 months (range: 10-18). The Oxford hip score and Harris hip score improved from 19.8 and 50.1 to 29.4 and 68.8, respectively (p = 0.009 and 0.01). There were complications in three cases (33.3%), which led to one re-revision (11.1%). Radiologic follow-up showed restoration of the height of the center of rotation and of the global offset. Significant difference was detected in the femoral offset. CONCLUSIONS: The functional and radiological outcomes are promising. However, long-term outcomes still need to be examined. BACKGROUND: Malposition of the acetabular component of a hip prosthesis can lead to poor outcomes. Traditional placement with fluoroscopic guidance results in a 35% malpositioning rate. We compared the (1) accuracy and precision of component placement, (2) procedure time, (3) radiation dose, and (4) usability of a novel 3-dimensional augmented reality (AR) guidance system vs standard fluoroscopic guidance for acetabular component placement. METHODS: We simulated component placement using a radiopaque foam pelvis. Cone-beam computed tomographic data and optical data from a red-green-blue-depth camera were coregistered to create the AR environment. Eight orthopedic surgery trainees completed component placement using both methods. We measured component position (inclination, anteversion), procedure time, radiation dose, and usability (System Usability Scale score, Surgical Task Load Index value). Alpha = .05. RESULTS: Compared with fluoroscopic technique, AR technique was significantly more accurate for achieving target inclination (P = .01) and anteversion (P = .02) and more precise for achieving target anteversion (P < .01). AR technique was faster (mean ± standard deviation, 1.8 ± 0.25 vs 3.9 ± 1.6 minute; P < .01), and participants rated it as significantly easier to use according to both scales (P < .05). Radiation dose was not significantly different between techniques (P = .48). CONCLUSION: A novel 3-dimensional AR guidance system produced more accurate inclination and anteversion and more precise anteversion in the placement of the acetabular component of a hip prosthesis. AR guidance was faster and easier to use than standard fluoroscopic guidance and did not involve greater radiation dose.

BACKGROUND: Recurrent instability after total hip arthroplasty is a difficult complication. In certain cases, a constrained acetabular device is needed to address these issues. The purpose of this study is to report the midterm outcomes and survivorship of a single novel constrained liner device. METHODS: A retrospective review as performed on all procedures (except first stage exchange for infection) in which a Freedom Constrained (Zimmer Biomet, Warsaw, IN) liner was used between December 2003 and November 2016. Patients with 2-year minimum follow-up or failure were included, yielding a cohort of 177 patients. Procedures were 130 aseptic revisions, 40 reimplantations following infection eradication, and 7 complex primaries. The constrained mechanism was implanted in 46 hips (26%) to treat active instability and 131 hips (74%) for increased risk of instability and intraoperative instability. Patients had on average 3.4 previous surgeries. RESULTS: With an average 7.1-year follow-up, 11 hips dislocated (6.2%), and 13 hips (7.3%) were revised for acetabular aseptic loosening, resulting in an overall constrained aseptic or mechanical failure rate of 13.6%. Nineteen hips (10.7%) failed from infection with 58% of these having had a previous infection. Patients with active instability had significantly higher failure for dislocation than patients who were at risk (15.2% vs 3%, P = .01). All-cause survival rate at 7 years was 74.8%, aseptic survival was 83.6%, and survival for instability was 91.8%. CONCLUSION: Revision for instability remains challenging as many patients have had numerous previous surgeries and at-risk anatomy. Constrained inserts are one option to manage instability, but a high rate of recurrence can still occur.

BACKGROUND: The anatomic dual mobility (ADM) acetabular component was introduced because of previously described low dislocation rates for this type of construct. The shape of the anatomic cup and the motion of polyethylene liner may have implications for acetabular cup fixation and polyethylene liner wear; therefore, the purpose of this study was to assess the migration patterns and wear rates of the ADM component using radiostereometric analysis. METHODS: Uncemented ADM acetabular components were implanted in 27 patients. Radiostereometric analysis exams were taken at 6 follow-up visits over 3 years. Proximal translation and sagittal rotation of the cup and polyethylene total wear and wear rates were calculated. Oxford 12 Hip scores and satisfaction were recorded. RESULTS: Mean proximal translation was below the 0.2 mm threshold at 2 years associated with acceptable long-term survivorship (0.16 mm [standard deviation {SD} 0.31] at 3 years). Mean sagittal rotation was 0.29 degrees (SD 1.03) and was greater in female subjects (P < .001). Following bedding-in, the annual wear rate was 0.02 mm/y, below the 0.1 mm/y threshold. There was no association between cup migration and polyethylene wear. Patient satisfaction at 3 years was 96%. Mean Oxford 12 Hip scores improved from 21 (SD 7) preoperatively to 43 (SD 7) 3 years postoperatively. CONCLUSIONS: The ADM cup demonstrated stable migration at 3 years indicating low risk for aseptic loosening. Bedding-in in the first year was followed by low annual wear rates. These finding suggest no increased risk of the dual mobility and anatomic design on fixation or wear.

BACKGROUND: It is proposed that highly porous coatings on acetabular components, such as a porous tantalum coating, provide adequate fixation without ancillary screw fixation in primary total hip arthroplasty (THA). However, tantalum acetabular components have been associated with higher rates of revision than other uncemented components in national registries. The aim of this randomized controlled trial is to determine whether the early migration of a solid-backed tantalum acetabular component was no greater than that of a titanium acetabular component with ancillary screw fixation that has proven good clinical results. METHODS: Sixty-six patients aged 40 to 64 years, with osteoarthritis and Charnley grade A or B activity grade and who underwent primary THA, were recruited into the trial. Patients were randomized intraoperatively to receive either the tantalum or titanium acetabular component. All patients received the same cemented polished tapered femoral stem, 28-mm cobalt-chromium femoral head, and highly cross-linked polyethylene liner. Acetabular component migration was measured using radiostereometric analysis at 4-6 days postoperatively and at 6 weeks, 3 months, 1 and 2 years following THA. RESULTS: The mean proximal migration at 2 years for the tantalum cohort was 0.17 mm (95% confidence interval, 0.09-0.24) which was no greater than that of the titanium cohort which was 0.19 mm (0.07-0.32). Harris hip scores and functional activity scores were similar between groups. CONCLUSION: These results demonstrate that early stability can be achieved without ancillary screw fixation through the use of a highly porous high friction coating on a solid-backed modular acetabular component.
