RESUMEN

OBJECTIVE: Empagliflozin, an oral anti-diabetic drug that inhibits the sodium-dependent glucose co-transporter 2 (SGLT2), has pleiotropic effects on the myocardium. The aim of the study is to investigate the effect of empagliflozin on atrial electromechanical delay (AEMD) and the left atrial (LA) mechanical functions in patients with type 2 diabetes mellitus (DM). METHOD: In total 62 patients (40.3% female, mean age 50.5 ± 8.6 years old) with type 2 DM were enrolled to the study. Participants were used a SGLT2 inhibitor (empagliflozin 10-25 mg/daily) for 6 months. Patients were examined initially and after 6 months with echocardiography. LA volume was recorded, atrial conduction times were measured using tissue Doppler imaging (TDI). RESULTS: No significant change was observed in LA volumes (maximal, minimal, and presystolic), total emptying and passive emptying volume at the end of 6 months; however, there was a significant decrease in active emptying volume (8.3 ± 2.9 ml/m2 vs. 7.9 ± 2.9 ml/m2 , p = 0.04). The posteroanterior lateral, septal, and tricuspid conduction times significantly decreased after the empagliflozin treatment. The decrease in right inter-AEMD was statistically significant (13.25 ± 10.21 ms vs. 10.85 ± 9.14 ms, p = 0.011). The changes in inter-AEMD were found to be correlated with the changes in LA active emptying volume (r = 0.408). CONCLUSION: Empagliflozin may enhance the structure and electrical conductions of the atrium and may prevent DM patients from DM-2-related functional disorder and arrhythmia.

RESUMEN

A 37 year-old female patient in whom the transthoracic echocardiography examination revealed dilatation of left heart chambers with left ventricular ejection fraction of 30% and moderate-to-severe mitral valve regurgitation was admitted to our hospital. On 2DTEE examination, mitral valve was normal; however, on 3D images, clefts of both anterior and posterior leaflets were revealed. Isolated cleft mitral valve without any other feature of atrioventricular septal defect is uncommon. 2D echocardiography has limited capability in defining the complex 3D anatomic characteristics of the cleft. 3DTEE allows to visualize the cleft position, morphology, and size, and it is important for surgical planning.

INTRODUCTION: Oxidative stress is known to affect left ventricular functions negatively. There is a strong bidirectional connection between diabetes mellitus (DM) and oxidative stress. In parallel, left ventricular dysfunction is observed more frequently, even in patients with DM without other risk factors. In this context, the objective of this study is to comparatively investigate the potential relationship between oxidative stress and subclinical left ventricular dysfunction (SCLVD) assessed by Myocardial Performance Index (MPI) in patients with and without DM. RESEARCH DESIGN AND METHODS: The sample of this observational cross-sectional single-center study consisted of 151 patients who were evaluated for oxidative stress and SCLVD by tissue Doppler echocardiography. Patients' total oxidant status (TOS), total antioxidant status (TAS), and Oxidative Stress Index (OSI) values were calculated. The effects of oxidative stress and DM on MPI were analyzed. RESULTS: There were 81 patients with DM (mean age: 46.17±10.33 years) and 70 healthy individuals (mean age: 45.72±9.04 years). Mean TOS and OSI values of the DM group were higher than healthy individuals (5.72±0.55 vs 5.31±0.50, p = <0.001; and 4.92±1.93 vs 1.79±0.39, p = <0.001; respectively). The mean TAS value of the DM group was significantly lower than the healthy group (1.21±0.40 vs 3.23±0.51, p = <0.001). There was a significant correlation between OSI and MPI mitral in the DM group (R 0.554, p = <0.001) but not in the healthy group (R -0.069, p=0.249). CONCLUSIONS: Both oxidative stress and myocardial dysfunction were found to be more common in patients with DM. The study's findings indicated the negative effect of oxidative stress on myocardial functions. Accordingly, increased oxidative stress caused more significant deterioration in MPI in patients with DM compared with healthy individuals.

AIM: The aim of our study was to evaluate atrial electromechanical delay (AEMD) and left atrial mechanical functions (LAMF) in the patients with secundum-type atrial septal defect (ASD). METHOD: We included 72 patients with secundum type ASD in the study group and 35 gender and age-matched healthy volunteers for the control group. Maximal, minimal, and presystolic LA volumes were measured by modified Simpson method and indexed to the body surface area (BSA). Inter-AEMD, right and left intra-AEMD were measured from the lateral and septal mitral annulus and tricuspid annulus using tissue Doppler imaging (TDI). RESULTS: The age, gender, systolic and diastolic blood pressure, heart rate (HR), BSA, and body mass index (BMI) parameters were not significantly different between the groups. Left atrial active and total emptying fractions and conduit volumes were significantly lower in the patients with ASD compared with the control group (P = 0.006, P = 0.001, and P = 0.032, respectively). Total emptying volume was increased in patients with ASD (P = 0.021). Passive emptying volume and fraction and active emptying volumes were not different significantly between the groups (P > 0.05). The left intraatrial, right intraatrial, and inter-AEMDs were significantly longer in the ASD group (P = 0.032, P = 0.013, and P = 0.003, respectively). CONCLUSION: The left atrial reservoir and contractile pump functions are reduced; the left intraatrial, right intraatrial, and inter-AEMDs are increased in the patients with ASD.

Objective: The cases of edentulous treatment are increasing in Turkey. The patients are less satisfied with the health care facilities of the public sector health institutes. The government of Turkey is facing critical challenges for medical reforms in the public sector to improve the living standards of the public. The objective of current research is to determine the influence of health behavior on immediate loading implant success and patient satisfaction in complete edentulous treatment. Method: A Likert scale questionnaire is employed and the response rate of this research is 71%. The population of this research is the patients visiting different clinics for edentulous treatment and loading implant success. Results: The research findings demonstrate the satisfaction of the patients and implant success is influenced by the health behavior of patients. Conclusion: The model of this research is enrichment in the knowledge of the edentulous treatment of the patients. The practical implications of this research are outstanding to influence the satisfaction of the edentulous treatment patients who are getting the treatment of loading implant success. The research has certain limitations that are straightforwardly presented with future directions to enrich the knowledge of edentulous treatment by the research work of scholars in health care.

The myocardial performance index (MPI) is an index that shows both systolic and diastolic functions of the ventricle. Presystolic wave (PSW) is a late diastolic wave encountered in the left ventricular outflow tract (LVOT) and is associated with increased left ventricular stiffness and decreased left ventricular compliance. In our study, MPI was compared between patients with metabolic syndrome and normal patients, and we also investigated whether PSW could predict subclinical left ventricular dysfunction (SCLVD) in patients with metabolic syndrome. A total of 119 patients, 59 with metabolic syndrome and 60 healthy volunteers, were included in our study. Our study is a two-center prospective study. The patient groups were compared in terms of demographic, laboratory and echocardiographic parameters. Univariate and multivariate regression analyzes were performed to detect predictors of SCLVD. Higher MPI and PSW were found in patients with metabolic syndrome compared to the normal population (0.56 ± 0.11 vs. 0.46 ± 0.07, p < 0.001, 34 (57.6%) vs. 19 (31.7%), p = 0.004, respectively). MPI was found to be higher in patients with metabolic syndrome with PSW ( +) (0.59 ± 0.13 vs. 0.52 ± 0.05, p = 0.005). Smoking and PSW were found as Independent Predictors of Subclinical Left Ventricular Dysfunction in the Multivariate Logistic Regression Analysis Model (OR 0.146, 95%CI 0.028-0.767, p = 0.023 and OR 10.689, 95%CI 2.176-52.515, p = 0.004, respectively). Higher MPI and SCLVD were detected in patients with metabolic syndrome compared to the normal population. In addition, PSW positivity was associated with SCLVD in this patient group.

Introduction: Coronary slow flow (CSF) is a condition commonly encountered during angiography. Recent studies have shown the adverse effects of CSF on left ventricular diastolic functions. CSF reportedly increases the novel ventricular repolarization parameters. Ranolazine is a preparation with a prominent anti-anginal activity that has positive effects on anti-arrhythmic and diastolic parameters. In this context, this study was carried out to investigate the effects of ranolazine on left ventricular diastolic functions and repolarization in patients with CSF. Material and methods: Forty-six patients with CSF and 29 control subjects were included in the patient and control groups, respectively. Both groups received ranolazine for one month and were evaluated using 12-lead electrocardiography, conventional echocardiography, and tissue Doppler imaging at the baseline and after one month of ranolazine treatment. Results: Corrected P, QT dispersion, and Tp-e interval values were significantly higher in the patient group than in the control group. There was a significant decrease in isovolumic relaxation time (IVRT) and deceleration time (DT) values after the ranolazine treatment compared to the baseline values in the patient group but not the control group. A significant increase was observed in the mean E and A velocities and the mean E/A ratio after the ranolazine treatment compared to the baseline values in the patient group. Additionally, there was a significant difference between the Tp-e interval and corrected P dispersion values measured after the ranolazine treatment compared to the baseline values in the patient group but not in the control group. Conclusion: This study's findings demonstrated that ranolazine positively affected impaired diastolic functions and repolarization parameters, particularly in patients with CSF.

OBJECTIVE: The aim of our study was to compare the traditional radial artery, distal radial artery, and transfemoral artery, which are vascular access sites for coronary angiography, in terms of pain level using the visual analog scale. METHODS: Between April 2021 and May 2022, consecutive patients from three centers were included in our study. A total of 540 patients, 180 from each of the traditional radial artery, distal radial artery , and transfemoral artery groups, were included. The visual analog scale was applied to the patients as soon as they were taken to bed. RESULTS: When the visual analog scale was compared between the groups, it was found to be significantly different (transfemoral artery: 2.7±1.6, traditional radial artery: 3.9±1.9, and distal radial artery: 4.9±2.1, respectively, p<0.001). When the patients were classified as mild, moderate, and severe based on the visual analog scale score, a significant difference was found between the groups in terms of body mass index, process time, access time, and number of punctures (p<0.001). Based on the receiver operating characteristic analysis, body mass index>29.8 kg/m2 predicted severe pain with 72.5% sensitivity and 73.2% specificity [(area under the curve: 0.770, 95%CI: 0.724-0.815, p<0.0001)]. CONCLUSION: In our study, we found that the femoral approach caused less access site pain and a high body mass index predicts severe pain.

We report an unusual case of moderate mitral regurgitation caused by abnormal insertion of chordae tendinea to the interatrial septum and tethering the middle scallop of the anterior mitral leaflet. This is an extremely rare congenital abnormality causing mitral regurgitation.

COVID-19 has emerged as a global pandemic affecting the world, and its adverse effects on society still continue. So far, about 243.57 million people have been diagnosed with COVID-19, of which about 4.94 million have died. In this study, a new model, called COVIDetNet, is proposed for automated COVID-19 detection. A lightweight CNN architecture trained instead of the popular and pretrained convolution neural network (CNN) models such as VGG16, VGG19, AlexNet, ResNet50, ResNet100, and MobileNetV2 from scratch with chest x-ray (CXR) images was designed. A new feature set was created by concatenating the features of all layers of the designed CNN architecture. Then, the most efficient features chosen among the features concatenating with the Relief feature selection algorithm were classified using the support vector machine (SVM) method. The experimental works were carried out on a public COVID-19 CXR database. Experimental results demonstrated 99.24% accuracy, 99.60% specificity, 99.39% sensitivity, 99.04% precision, and an F1 score of 99.21%. Also, in comparison to AlexNet and VGG16 models, the deep feature extraction durations were reduced by approximately 6-fold and 38-fold, respectively. The COVIDetNet model provided a higher accuracy score than state-of-the-art models when compared to multi-class research studies. Overall, the proposed model will be beneficial for specialist medical staff to detect COVID-19 cases, as it provides faster and higher accuracy than existing CXR-based approaches.

Ticagrelor is a potent P2Y12 inhibitor that is increasingly used in acute coronary syndrome. Although drug-related intrahepatic hemorrhages have been reported with anticoagulant drugs, no case of intrahepatic hemorrhage due to ticagrelor has been encountered in the literature. With this case, we tried to explain that potent antiaggregant drugs may cause this rare bleeding complication. Although ticagrelor is used for rapid antiaggregation effect in acute coronary syndrome, de-escalation to clopidogrel treatment in elderly patients during the hospitalization period is a more reasonable option.

BACKGROUND: Plants have an important place in the life of all living things. Today, there is a risk of extinction for many plant species due to climate change and its environmental impact. Therefore, researchers have conducted various studies with the aim of protecting the diversity of the planet's plant life. Generally, research in this area is aimed at determining plant species and diseases, with works predominantly based on plant images. Advances in deep learning techniques have provided very successful results in this field, and have become widely used in research studies to identify plant species. METHODS: In this paper, a Multi-Division Convolutional Neural Network (MD-CNN)-based plant recognition system was developed in order to address an agricultural problem related to the classification of plant species. In the proposed system, we divide plant images into equal nxn-sized pieces, and then deep features are extracted for each piece using a Convolutional Neural Network (CNN). For each part of the obtained deep features, effective features are selected using the Principal Component Analysis (PCA) algorithm. Finally, the obtained effective features are combined and classification conducted using the Support Vector Machine (SVM) method. RESULTS: In order to test the performance of the proposed deep-based system, eight different plant datasets were used: Flavia, Swedish, ICL, Foliage, Folio, Flower17, Flower102, and LeafSnap. According to the results of these experimental studies, 100% accuracy scores were achieved for the Flavia, Swedish, and Folio datasets, whilst the ICL, Foliage, Flower17, Flower102, and LeafSnap datasets achieved results of 99.77%, 99.93%, 97.87%, 98.03%, and 94.38%, respectively.

RESUMEN

Traumatic injuries of teeth are the main cause of emergency treatment in dental practice. Horizontal root fractures are more frequently observed in the maxillary anterior region and young male patients. The most common type of root fracture is in the middle third, followed by apical, and coronal part. This case report describes two horizontal root fractures of the maxillary right central and lateral incisors due to a sport injury. The lateral incisor was healed satisfactorily without treatment. The central incisor was endodontically treated.

RESUMEN

A novel method to detect human falls in depth videos is presented in this paper. A fast and robust shape sequence descriptor, namely the Silhouette Orientation Volume (SOV), is used to represent actions and classify falls. The SOV descriptor provides high classification accuracy even with a combination of simple associated models, such as Bag-of-Words and the Naïve Bayes classifier. Experiments on the public SDU-Fall dataset show that this new approach achieves up to 91.89% fall detection accuracy with a single-view depth camera. The classification rate is about 5% higher than the results reported in the literature. An overall accuracy of 89.63% was obtained for the six-class action recognition, which is about 25% higher than the state of the art. Moreover, a perfect silhouette-based action recognition rate of 100% is achieved on the Weizmann action dataset.

Hyaluronic acid (HA) is one of the essential components of extracellular matrix, which plays a predominant role in tissue morphogenesis, cell migration, differentiation, and adhesion. Bone allografts are frequently used to repair and reconstruct bone defects. In this study, two cavities of 3 mm diameter and depth have been created in the right tibia of 30 mature rabbits in accordance with the principles of general surgery. One of the cavities in the tibia is filled with HA and bone graft and the other is filled with only spongiosal bone graft, for the purpose of control. On the 20th, 30th, and 40th days, rabbits have been sacrificed in equal numbers and defective regions have been extracted. The Kruskal-Wallis test was applied to the data obtained in the result of histopathologic survey of specimens. In conclusion, the cavities that have been filled with HA and bone graft have shown higher scores than the control group during every period of the study.

Treatments with bismuth-containing quadruple therapy (QT), sequential therapy (ST), or concomitant therapy (CT) have been proposed as empirical first-line regimens for Helicobacter pylori. We compared the efficacy and tolerability of 10 days bismuth-containing quadruple QT, 10 days ST, and 10 days CT with as first-line treatments for H. pylori in a randomized crossover study. The subjects were randomly divided into three groups. The first 130 patients were treated with rabeprazole, bismuth potassium citrate, metronidazole, and tetracycline for 10 days. The second 130 patients in the sequential group were treated with rabeprazole and amoxicillin for 5 days, and then rabeprazole, clarithromycin, and metronidazole for an additional 5 days. The last 130 patients in the concomitant group were treated with rabeprazole, amoxicillin, clarithromycin, and metronidazole for 10 days. H. pylori eradication was confirmed by urea breath test at 6 weeks. The primary outcome was eradication rates of first-line treatment by intention to treat and per protocol (PP) analyzes. There was no difference between the average ages and the male/female ratio of the groups. The PP analysis was performed on 121, 119, and 118 patients in the QT, ST, and CT groups, respectively. In the PP analysis, the successful eradication 94.2% (114/121), 95.0% (113/119), and 95.8% (113/118) the QT, ST, and CT groups, respectively. There was no significant difference among the three groups (p = 0.86). 10 days QT, ST, and CT are highly effective as empirical first-line therapies for H. pylori in the region with high clarithromycin resistance.

RESUMEN

In this study, the effects of guided bone regeneration (GBR) on the healing of bone defects were evaluated. Resorbable membranes were placed in experimentally formed cavities in the right posterior tibia of 30 rabbits. Decalcified histological sections were evaluated using optical microscopy at 10, 20, and 30 days after GBR. Osteocondrial bone union, active bone formation and spongiosal bone formation values of the GBR group are higher than the control group. It was found that GBR technique had a positive and accelerating influence in all phases of bone healing.

AIMS: Atrial septal defect (ASD) is one of the most common congenital heart diseases in adults. We prospectively evaluated early and mid-term effects of the percutaneous closure of secundum ASD on atrial electromechanical delay (AEMD) and left atrial (LA) mechanical functions at the first day and sixth month in patients undergoing percutaneous closure. METHODS AND RESULTS: Forty-one patients were included in this study. Twenty-six (63.4%) of the 41 patients were female and the mean age was 41 ± 13 years. All the patients had echocardiographic examination before the procedure and at the first day and sixth month after the procedure. LA volumes (maximal, minimal, and presystolic) and EMD (lateral, septal, and tricuspid) were measured. Left and right intra- and inter-AEMD were not changed at the first day but both were significantly shorter at the sixth month. There was no change in the total emptying volume and fraction before and after the procedure. LA maximal, minimal, and pre-systolic volumes, active emptying volume, and fractions were decreased at the first day and at the sixth month compared with pre-procedural volumes. LA passive emptying volume, passive emptying fraction, and conduit volume were increased at the first day and at the sixth month compared with pre-procedural volumes. CONCLUSION: Our results revealed that there was no change in the LA mechanical reservoir functions, but improved conduit function and impaired contractility functions early and in the mid-term after percutaneous closure of ASD and decreased AEMD only in the mid-term.
