RESUMEN

BACKGROUND: In the background of an aging population, the risk of cognitive impairment in the older population is prominent. Exposure to complex neighborhood built environments may be beneficial to the cognitive health of older adults, and the purpose of this study was to systematically review the scientific evidence on the effects of neighborhood built environments on cognitive function in older adults. METHODS: Keywords and references were searched in Web of Science, Pubmed, PsycINFO, and MEDLINE. Studies examining the relationship between the built environment and cognitive function in older adults were included. The neighborhood built environment as an independent variable was classified according to seven aspects: density, design, diversity, destination accessibility, public transportation distance, blue/green space, and built environment quality. The cognitive function as the dependent variable was classified according to overall cognitive function, domain-specific cognitive function, and incidence of dementia. The quality of the included literature was assessed using the National Institutes of Health's Observational Cohort and Cross-Sectional Study Quality Assessment Tool. RESULTS: A total of 56 studies were included that met the inclusion criteria, including 31 cross-sectional studies, 23 longitudinal studies, 1 cross-sectional study design combined with a case-control design, and 1 longitudinal study design combined with a case-control design. Most of the studies reviewed indicate that the built environment factors that were positively associated with cognitive function in older adults were population density, street connectivity, walkability, number of public transportation stops around the residence, land use mix, neighborhood resources, green space, and quality of the neighborhood built environment. Built environment factors that were negatively associated with cognitive function in older adults were street integration, distance from residence to main road. The relationship between residential density, destination accessibility, and blue space with cognitive function in older adults needs to be further explored. CONCLUSION: Preliminary evidence suggests an association between the neighborhood built environment and cognitive function in older adults. The causal relationship between the built environment and cognitive function can be further explored in the future using standardized and combined subjective and objective assessment methods, and longitudinal or quasi-experimental study designs. For public health interventions on the cognitive health of older adults, it is recommended that relevant authorities include the neighborhood built environment in their intervention programs.

Objective: To explore the characteristics of Non-Negative Matrix Factorization (NNMF) in analyzing the mechanical characteristics of foot functional units during walking and running. Methods: Eighteen subjects (9 males and 9 females) were recruited, and the ground reaction force curves of each foot region during walking and running were collected using a plantar pressure measurement system. NNMF was used to extract the mechanical features of different foot regions and to determine the number of foot functional units. The differences between the base matrices of walking and running were compared by traditional t-tests, and the differences in coefficient matrices were compared by one-dimensional statistical parameter mapping. Results: 1) When the number of foot functional units for walking and running were both 2, the Variability Accounted For (VAF) by the matrix exceeded 0.90 (VAF walk = 0.96 ± 0.02, VAF run = 0.95 ± 0.04); 2) In foot functional unit 1, both walking and running exhibited buffering function, with the heel region being the main force-bearing area and the forefoot also participating in partial buffering; 3) In foot functional unit 2, both walking and running exhibited push-off function, with the middle part of the forefoot having a higher contribution weight; 4) In foot functional unit 1, compared to walking, the overall force characteristics of the running foot were greater during the support phase of the 0%-20% stage, with the third and fourth metatarsal areas having higher contribution weights and the lateral heel area having lower weights; 5) In foot functional unit 2, compared to walking, the overall force was higher during the beginning and 11%-69% stages of running, and lower during the 4%-5% and 73%-92% stages. During running, the thumb area, the first metatarsal area and the midfoot area had higher contribution weights than during walking; in the third and fourth metatarsal areas, the contribution weights were lower during running than during walking. Conclusion: Based on the mechanical characteristics of the foot, walking and running can both be decomposed into two foot functional units: buffering and push-off. The forefoot occupies a certain weight in both buffering and push-off functions, indicating that there may be a complex foot function transformation mechanism in the transverse arch of foot. Compared to walking, running completes push-off earlier, and the force region is more inclined towards the inner side of the foot, with the hallux area having a greater weight during push-off. This study suggests that NNMF is feasible for analyzing foot mechanical characteristics.

Topological data analysis is becoming more and more popular in recent years. It has found various applications in many different fields, for its convenience in analyzing and understanding the structure and dynamic of complex systems. We used topological data analysis to analyze the firings of a network of stochastic spiking neurons, which can be in a sub-critical, critical, or super-critical state depending on the value of the control parameter. We calculated several topological features regarding Betti curves and then analyzed the behaviors of these features, using them as inputs for machine learning to discriminate the three states of the network.

BACKGROUND: Diabetic kidney disease (DKD) is one of the common complications of diabetes, the most crucial cause of end-stage renal disease, and a significant cause of death in diabetes patients. In recent years, Xuefu Zhuyu decoction has also been widely used in treating diabetic nephropathy. However, there need to be more systematic reviews that comprehensively describe the effectiveness and safety of Xuefu Zhuyu decoction in treating DKD. Therefore, we will conduct a systematic review and meta-analysis to evaluate the indicators of Xuefu Zhuyu decoction in the treatment of DKD to provide evidence-based support. METHODS: Eligible published randomized controlled trials from January 2005 to October 2022 will be obtained by searching PubMed, Cochrane Library, EMBASE, Web of Science, CNKI, Wanfang, and VIP in Chinese and English. The Cochrane Collaboration Risk of bias tool will be used for methodological quality assessment and risk of bias. The meta-analysis will be performed using Cochrane RevMan 5.4 software. RESULTS: This study will compare the following indicators: the primary outcomes: urinary albumin excretion rate and urea nitrogen. Secondary outcomes: blood creatinine; 24 hours urine protein quantification; glycosylated hemoglobin; fasting blood glucose; 2-hour postprandial blood glucose; total cholesterol; triglycerides; total effective rate; incidence of adverse events. CONCLUSION: The results of this systematic review will provide an objective, evidence-based basis for judging the efficacy and safety of Xuefu Zhuyu decoction in treating DKD.
