RESUMEN

A case of severe, oral, not otherwise specified ulcers in a human immunodeficiency virus-infected patient is described. The lesions did not respond to acyclovir, prednisone, pentoxifylline, or foscarnet sodium therapy. Dramatic clinical improvement and progressive ulcer healing were observed after starting oral thalidomide therapy. Clinicians should be aware of the usefulness of thalidomide for the treatment of acquired immunodeficiency syndrome-associated not otherwise specified ulcerations.

RESUMEN

This is a report of a case of a 12-year-old boy with Cushing's syndrome in which high doses of dexamethasone failed to suppress glucocorticoid secretion. Neither CT nor MRI (morphologic studies) revealed any adrenal abnormality. Functional images with NP-59 (I-131 beta-iodomethyl-norcholesterol) clearly revealed intense unilateral uptake in the right adrenal gland. A right adrenalectomy was performed, and the pathologic diagnosis was primary adrenocortical nodular dysplasia. One year later, the cushingoid signs had virtually disappeared and urinary steroid excretion and serum cortisol levels were within the normal range. Nevertheless, primary adrenocortical nodular dysplasia is essentially a bilateral disease. In this case, a remission in the adrenocortical hyperfunction was observed. Although there was confusion in the etiological diagnosis, functional scintigraphy correctly indicated the status of adrenal function.

RESUMEN

We describe a 56 years old male patient with long-term chronic liver disease of unknown etiology presenting with esophageal varices rupture. Prophylaxis of re-bleeding with propranolol and endoscopic sclerotherapy failed to prevent further haemorrhagic events and the placement of a transjugular intrahepatic portosystemic shunt (TIPS) was needed. The portal hemodynamic data revealed sinusoidal portal hypertension and the liver biopsy displayed ductopenic cholestasis. The patient met all criteria of idiopathic ductopenia. Subsequently, the jaundice worsened and the patient required liver transplantation.

RESUMEN

Ultrasound is a useful diagnostic modality to study many structures such as subcutaneous tissue, tendons, muscles, joints, and nerves. It has low cost, wide availability and high resolution. These advantages make ultrasound a good modality in interventional procedures like soft tissue tumors biopsy, aspiration of cysts and other collections, and also in treating symptomatic calcifications like deposition of hidroxiapatite crystals in the rotator cuff, among other indications. Our objetive is to present the experience of the authors in performing musculoskeletal interventional procedures by ultrasound.


El ultrasonido es una modalidad imaginológica útil para el estudio de múltiples estructuras, tales como tejidos subcutáneos, tendones, músculos, articulaciones y nervios. Tiene un bajo costo, amplia disponibilidad y alta resolución. Estas ventajas hacen del ultrasonido una excelente modalidad en procedimientos intervencionales tales como biopsias de tejidos de partes blandas, aspiración de quistes y otras colecciones y también en el tratamiento de calcificaciones sintomáticas, tales como depósitos de cristales de hidroxiapatita de calcio en el manguito rotador. Este artículo desea mostrar la experiencia de los autores en la realización de procedimientos intervencionales musculoesqueléticos guiados por ultrasonido.

Drug-producing countries such as the Andean countries of South America where cocaine is manufactured are confronted by special difficulties associated with the widespread availability of drugs. There have been few detailed reports of patterns of use in relation to the type and severity of cocaine dependence problems within those countries. The present study looks at the patterns of cocaine use in relation to severity of dependence among a clinical sample of South American cocaine users. Information about patterns of cocaine use and severity of dependence was collected from a sample of 68 drug users who were receiving treatment for cocaine problems at treatment centres in Bolivia and Peru. Levels of cocaine consumption were extremely high. The mean daily dose was 16.4 grams. The majority of the users (87 per cent) smoked cocaine in the form of pasta, pitillo or basuco. More than half of the sample reported using cocaine at least 20 times a day. Severity-of-dependence scale scores were high and these are consistent with the frequent and compulsive pattern of use reported within the sample. It is suggested that the more severe cocaine problems reported in South America compared to some western countries may be due to the substantial differences in the amounts of cocaine which are typically ingested. In the Bolivian sample most of the users were taking cocaine in amounts which greatly exceed those usually seen in western countries.

Human colon sialidase has been characterized, and its activity levels in normal mucosa and colonic adenocarcinoma have been determined. Sialidase activity was maximal at pH 5.5, and was unstable with storage at 4 and -20 degrees C. The bulk of activity was pellet-associated, and could not be released with triton X-100 or 3-([3-cholamidopropyl]- dimethylammonio)-1-propanesulfonate. Using 2'-(4-methylumbelliferyl)alpha-D-N-acetylneuraminic acid as substrate, the Km and Vmax values were estimated to be 0.140 mmol/l and 63 mU/g, respectively. Furthermore, an inhibition by substrate concentrations above 1.5 mmol/l was detected. Neuraminic acid caused a competitive inhibition with a Ki of 3.5 mmol/l. A statistically significant increase (p < 0.001) in the sialidase specific activity was found in primary colonic adenocarcinoma (104.20 +/- 8.00 mU/g) compared to that of the normal mucosa (72.50 +/- 7.67 mU/g).

The expression of different sialoglycoconjugates and fucoglycoconjugates in normal mucosa and adenocarcinoma samples from 43 colorectal cancer patients was investigated by using specific lectins and applying a semiquantitative analysis. A pronounced decrease in the intracellular binding of the Maackia amurensis lectin, which recognizes alpha(2,3)-linked sialic acid residues, was found in the tumoral tissue. In contrast, a significant increase in the staining with the Sambucus nigra lectin (SNA I), which binds to alpha(2,6)-linked sialic acid residues, was detected in the epithelial cells as well as in the mucins from tumors. No significant differences in the reactivity with the Aleuria aurantia lectin, which recognizes the sequence Fuc(alpha1,6)GlcNAc, between normal and malignant colorectal tissues were detected. Furthermore, the correlation between lectin-binding profiles and the prognosis of colorectal cancer patients was examined. After an average postoperative follow-up period of 31 months, patients with tumors showing a strong SNA I staining presented a greater probability of disease recurrence. This result suggests that the intensity of staining with SNA I could be a valid parameter for predicting recurrence in colorectal cancer.
