RESUMEN

The differential diagnosis of neoplasms of various localizations is the primary task in clinical practice of all physicians. We present a description of the case of invasion with Dirofilaria repens in the lung of a 68-year-old patient. In 2016 the patient was diagnosed with cancer of the left kidney and underwent a left-sided nephrectomy. During the dynamic observation in 2019, a lump was found in the left lung, which was regarded as a metastasis. An atypical SIX resection of the left lung was performed. Microscopy of the removed lump revealed the presence of a nematode of the genus Dirofilaria, presumably D. repens. The species identity of D. repens was confirmed by polymerase chain reaction using species-specific primers. It is known that the morphological identification of parasites up to the species in the surgical material presents certain difficulties and requires high professional training of the researcher. Therefore, the diagnosis of dirofilariasis in atypical localizations of nematodes in the human body is of great importance in the differentiation of malignant and benign formations, and the use of the polymerase chain reaction method can significantly increase the accuracy in establishing the final diagnosis.

RESUMEN

The paper presents the case of the right-sided inguinal inflammation of a lymph node as a result of invasion of Dirofilaria repens, the parasitic pathogen of subcutaneous dirofilariasis in animals of the canine family. The diagnosis was verified on the basis of the parallel application of morphological studies of cross sections of the nematode in histological samples and the molecular biological method polymerase chain reaction of scrapings of histological material. The localization of this helminth inside the cavities of the human body is extremely rare. Only isolated cases of atypical localization of D. repens are described: in the organs of the chest, cervical lymphatic node in the spermatic cord and epididymis, which led to pseudotumor formations that needed to be differentiated with neoplastic processes. This case is of great interest to experts of various fields (surgeons, oncologists, infectious disease specialists and pathologists), primarily in the differential diagnosis of malignant neoplasms of the lymphatic system.

RESUMEN

The paper presents the results of clinical, epidemiological, and epizootological analyses of local cases of human dirofilariasis in the Nizhny Novgorod Region, which suggest that natural and climatic changes, namely the abnormally hot summer in 2010-2011 and increasing migratory processes among human beings and animals, open up possibilities for forming foci ofdirofilariasis outside habitats of Dirofilaria repens.

RESUMEN

In the Rostov Region, there have been stable foci of dirofilariasis for a long time (since the 1990s). Two helminth species (Dirofilaria repens, D. immitis) parasitize in definitive hosts. There has been recently a species-specific alteration in the foci, by increasing the proportion of canine D. immitis invasion and mixed invasions associated with the change in weather and climatic conditions and in the species-specific composition of carriers. The smaller number of patients with dirofilariasis is a consequence of the reduced affection of dogs and mosquitos.

RESUMEN

Reformation of public health system financing in Russia requires the use and distribution of the most effective and least expensive diagnostic and medical methods. Giardiasis is the most common human anthroponosis. The diagnosis and treatment of this disease call for considerable funds of the budget, medical insurance companies, and the patient himself/herself. The paper gives the results of the clinicoeconomic analysis aimed at detecting the most rational frequency of diagnostic studies. The analysis provides evidence that triple coproprological study has the greatest advantages. The findings make it possible to recommend just this study for wide use in the diagnosis ofgiardiasis.

The study of the based on the A/Leningrad/134/17/57/(H2N2) attenuated adult live influenza vaccine (LIV) investigated features for immunization of the children, aged 3-6 years. During autumn, 1999, out of 256 children, aged 3-6 years, residents of the Leningrad region, who attended the kindergarten, 184 children were immunized with 1 or 2 doses of the live influenza vaccine, and 72 ones were given placebo. There were no any moderate or strong temperature reactions revealed after the inoculation. The LIV was shown to be genetically stable. After a single dose of the vaccine seroconversion to influenza type A virus and to influenza type B virus was observed respectively in 58% and in 39% of seronegative 3-6 year old vaccinees. The twofold LIV administration failed to give any advantages in stimulation of the immune response. During 6 months after immunization the morbidity rate in vaccinees did not exceed the morbidity rate in unvaccinated children. Thus LIV for adults proved safe and immunogenic and can be recommended for single dose immunization both of adults and children.

RESUMEN

Sensitive, emotional, intellectual, motivation, preoperative and postoperative aspects of the disease pattern were studied in 93 patients who had acquired valvular defects and underwent replacement of the valves in conditions of artificial circulation. The findings provided evidence in support of psychic correction for preoperative preparation of the patients and postoperative restoration of their personality and social status.
