RESUMEN

OBJECTIVE: COVID-19 disrupted health service delivery and weakened global and national health systems. The objective of this study was to describe the changes in health service utilization in three local government areas (LGAs) in Nigeria and examine factors involved. METHODS: A cross-sectional mixed-methods approach was used. A total of 315 pregnant women seen for antenatal care in 80 health facilities in three LGAs between October 1 and November 30, 2020, participated in exit interviews; 93 women participated in focus group discussions (FGDs). Descriptive analyses and a multivariable logistic analysis were conducted to examine associations between characteristics and decreased service utilization. Content analysis was used to identify the emerging themes related to health service utilization during the pandemic. RESULTS: One quarter of women reported that they reduced or ceased health service. The biggest reported changes were in immunization (47 to 30%, p < 0.001) and a small but statistically significant decline in antenatal care (98.7 to 93.8%, p < 0.001) was observed. Qualitative findings show that lockdowns, transportation issues, increased costs and fear of contracting COVID-19 or being labeled as COVID-positive were the most common reasons for not seeking care during this period of the pandemic. CONCLUSIONS: The pandemic negatively impacted health service utilization amongst pregnant women in Nigeria. A better understanding of differences in state response could help inform future actions. The findings highlight the need for health systems to consider how to facilitate service utilization during a pandemic, such as providing safe transport or increasing outreach, and to minimize stigma for those seeking care.

Implementing Internet of things (IoT) technology in the context of intelligent buildings and infrastructure development has garnered significant attention within the construction sector. Nonetheless, the implementation of IoT could be improved by assessing various barriers. The purpose of this study was to examine the obstacles related to the adaptation of IoT techniques within the construction sector, as well as the effects on the advancement of intelligent building and infrastructure systems. The study employed a mixed-method approach involving exploratory factor analysis (EFA) and structural equation modeling (SEM) to determine six types of barriers: knowledge, technical, standardization, creativity, complexity, and economics. The study revealed that the implementation of IoT for developing smart construction and infrastructure in the construction sector was significantly influenced by all six constructs. The results of this study offer significant ramifications for the field. The study underscores the necessity for heightened consciousness and instruction regarding the advantages of implementing IoT. The study posits that the technical barriers, including interoperability, modernization of legacy infrastructure, and coordination and collaboration difficulties, require attention from the industry. The study highlights the significance of establishing industry-wide standards and protocols for implementing IoT and regulatory and legal frameworks. Finally, the study underscores the necessity for augmented funding and financing options for IoT endeavors. Subsequent study endeavors may expand upon the present findings by delving into the barriers encountered by alternative sectors and nations and assessing the efficacy of the suggested measures in this investigation.

INTRODUCTION: Plantar fasciitis (PFS) is described by an intense pain over medial tubercle of calcaneus, increased with the first step after waking up, after rest and during weightbearing activity. It is the most common cause of plantar heel pain in adults with the prevalence estimated 10% of the general population. Ultrasound imaging is commonly being used to measure the PF thickness, evaluate the efficacy of different treatments and a guide therapeutic technique in patients with PFS. The objective of this study was to systematically review the studies that were previously published to evaluate the role of ultrasound in the assessment of PF in patients with PFS. METHODS: A systematic search was carried out over the last 5 years from 2017 to 2022 on basis the following electronic databases: Science Direct, Scopus, Web of Science, Springer and PubMed. The keywords that used in the searching were: ultrasound, sonography, ultrasonography, plantar fasciitis, imaging of plantar fascia, physiotherapy of plantar fasciitis, interventional treatment of plantar fasciitis, randomized controlled trial of plantar fasciitis and interventional ultrasound. The review focused on the assessment of PF in patients with PFS underwent different interventions using B-mode, shear wave elastography (SWE) and color Doppler ultrasound. RESULTS: During the search process, 1661 were recorded using the proper keywords from 2017 to 2022 in which 666 original articles were found after removing the review and duplicated articles. Of these, thirty articles met the inclusion criteria and included in this review. The articles have assessed the PF in patients with PFS under different conditions using different ultrasound modes. Twenty-six articles evaluated the effectiveness of different treatment on PF in patients with PFS using different ultrasound modes. In 8 of 26 articles, the ultrasound was used as both an assessment tool of PF and guide therapeutic technique in patients with PFS. In 18 articles, the ultrasound was used as only assessment tool to identify the PF thickness and its observation changes in patients with PFS. Four articles compared the PF thickness and its intrafascial changes between patients with PFS and healthy subjects. CONCLUSION: The ultrasound can be a reliable tool in assessment the effect of different interventions on PF by evaluating its thickness, echogenicity and stiffness changes in patients with PFS. There were different methods and treatments were used among the studies.

OBJECTIVE: This study aimed to evaluate the efficacy of manual physiotherapy on clinical outcomes, morphology of plantar fascia (PF), thicknesses of calcaneal fat pad (CFP) and Kager's fat pad (KFP) with ultrasound imaging in plantar fasciitis (PFS) patients. Also, to evaluate the PF thickness, pain and foot functional outcomes among PFS phases. METHODS: A randomized controlled trial was conducted on 122 subjects divided into three groups: group A (40 patients with PFS) underwent manual physiotherapy, group B (42 patients with PFS) without any intervention and group C (40 healthy subjects) were matched by age, gender and BMI with each patient in group A and B. The following outcomes were evaluated at baseline and one-month of follow-ups: morphology of PF and thicknesses of CFP and KFP, pain, foot functional limitation. RESULTS: PF thickness was significantly thickened in group A and B compared to group C (P < 0.001). A significant decrease in incidence of PF echogenicity and CFP thickness were found in group A and B compared to group C. Moreover, significant improvement was observed in PF thickness (P < 0.001), PF echogenicity (P < 0.001) and CFP thickness (P = 0.002) in group A at one-month after the treatment. Furthermore, pain intensity and foot functional limitation was significantly improved within group A after receiving the treatment. Significant improvement was noted in PF thickness, pain intensity and foot functional limitation among patients with acute phase. CONCLUSION: The manual physiotherapy is effective in treatment of PF thickening, hypoechogenicity, pain intensity and activity limitations, particularly in patients associated with acute PFS.

Interfacial solar water evaporation has attracted tremendous attention for sunlight harvesting for water purification. However, salt formation and stability of the photothermal materials (PTMs) remain a challenge that need addressing before bringing this technology to real-world applications. In this work, a nanoscale thin film of gold (Au) on a polytetrafluoroethylene (PTFE) membrane has been prepared using a magnetic sputtering technique. The fabricated membrane displays a robust mechanical strength and chemical stability arising from the adhesiveness of the thin film Au nanolayer on the PTFE membrane as well as the chemical inertness of the noble metal PTM. The Au nanolayer/PTFE membrane with cellulose sponge substrate resulted in an evaporation rate of 0.88 kg m-2 h-1 under 1 sun intensity. Remarkable salt ion rejection of 99.9% has been obtained, meeting the required standard for drinking water. Moreover, the membrane exhibited excellent stability and reusability in natural seawater and high salinity brine (150 g/L) and even in severe conditions (acidic, basic, and oxidized). No noticeable salt formation was observed on the evaporator surface after the tests. These findings reveal promising prospects for using a magnetron sputtering technique to fabricate a stable photothermal membrane for seawater and high salinity brine desalination.

INTRODUCTION: Adverse Events Following Immunization (AEFI) are one of the main reasons for inadequate immunization coverage in Kaduna State, and AEFI underreporting serves as a barrier to achieving goals of global pharmaco-vigilance for vaccine. The purpose of this study is to estimate the completeness of variables in the AEFI line-listing forms, calculate AEFI reporting rates by Local Government Areas & vaccine type and profile the reported cases according to their reactions. METHODS: we conducted a descriptive, cross-sectional, retrospective study of primary surveillance records. We calculated AEFI reporting rates in the State and Local Government areas and AEFI Vaccine reaction rates to the various antigens. We used Binary logistic regression to determine the association between gender and vaccine reactions. RESULTS: seven thousand eight hundred and twenty-four (7,824) AEFI cases were reported. The completeness of variables on the filled AEFI line-list varied from 21% to 100%. The State had a high AEFI reporting rate of 9.09 per 10,000 administered doses. Fever (<38oC) was the main AEFI reaction. Severe AEFI cases accounted for only 0.89% of the total reported cases. Pentavalent vaccine was the suspect antigen responsible for the highest number of AEFI cases, with a vaccine reaction rate of 44.77 per 10,000 doses. The Zaria Local Government area had the highest AEFI reporting rate, while the Sanga Local Government area had the lowest AEFI reporting rate in the State. The difference between genders in the number of reported AEFI cases was not statistically significant (p>0.05). There were 35% higher odds of occurrence of bleeding among males than among females (aOR: 1.354; P-value: p=.012; 95% CI: 1.070-1.715; Nagelkerke-R2-: 0.003). The other reactions were not significantly related to gender. CONCLUSION: our study shows a higher occurrence of severe AEFI in subjects undergoing pentavalent vaccine. Thiscaused the highest incidence of AEFI. There was no significant association between gender and AEFI reactions.

Effective site supervision plays an important role in construction project delivery. This situates site supervisors in taking the responsibilities of coordinating and controlling all physical aspects of the day-to-day task in construction projects. A cross-sectional design approach was adopted by administering a well-structured questionnaire to selected built environment professionals. Descriptive statistics was performed on the data obtained and are presented on figures and tables. The data was also subjected to inferential statistics using Kruskal Wallis test to analyze the perceptions of respondents on effects of site supervision of construction works on completion time and quality respectively. The significance of the analyzed data is on identifying the effects of site supervision on project completion time and quality delivery. The analyzed data will also guide project stakeholders in selecting competent personnel in executing construction projects.
