RESUMEN
The activity of acid phosphotase in the blood serum and punctates of the liver was studied in 49 patients with acute cyclic form of viral hepatitis of slight and medium-grave course with the use of histological and histochemical methods. The degree of manifestation of necrotic changes in the punctate tissue was determined with the help of the stereometric method. As a result of the studies conducted, it was established that the degree of hyperfermentemia was the highest at the period when the disease was in full swing, and that it did not depend on the form of its course and extent of necrotic changes observed in the liver punctates. The authors consider a morphological evidence of high activity of acid phosphotase in the blood serum aggregations of large hepatocytes, cytoplasm of which was loaded with mature lipofuscin, the topographic identity of the latter with acid phosphotase at present being proved.

S. flexneri 2a outer membrane proteins of 38, 43, 62 and 78 kD, determined by the 140 mD invasiveness plasmid, serving as antigens and specific rabbit sera serving as antibodies were used for diagnosing S. flexneri infection in the enzyme immunoassay. The examination of 96 patients and 20 healthy donors showed the possibility of the detection of S. flexneri 2a protein invasins and antibodies to them at different periods of the disease. During the severe course of shigellosis a higher level of protein invasins in the blood serum of patients was established in comparison with that observed during the medium severe course. An elevated level of protein invasins in the blood serum was accompanied by a lower content of specific antibodies to them, which was probably due to the immunosuppressive action of the invasiveness plasmid, established in earlier experiments.

Microcirculation was studied over time in 51 patients with acute viral hepatitides types A and B using bulbar microphotographic vasography. The results were compared with some immunological indices (the blood level of circulating immune complexes, the platelet aggregation test). The expression of microcirculatory changes correlated both with a type of virus hepatitis and the severity of disease. The relationship of disorders in the system of microcirculation with immune factors, in particular with immunopathological reactions developing during HBV infections, was discussed.

Flexner's dysentery is often accompanied with intestinal dysbacteriosis. Disbiotic changes in the intestine contribute to specific shigella endotoxins entering blood flow thus prolonging clinical symptoms of the underlying disease. Administration of bacterial biological preparations (bifidumbacterin forte, lactobacterin) relieves specific endotoxemia, reduces the duration of the disease and has an immunomodulating action.

The paper presents analysis of clinical efficacy of etiotropic drugs in severe Flexner's dysentery chosen by sensitivity of the causing agent, schemes of antibacterial drugs of choice in the treatment of this disease, principles of detoxication therapy. Use of bacterial biological drugs in combined treatment of dysentery is validated.

Patients with acute viral hepatitis B, A and mixed hepatitis B + C were treated in two independent clinics with phosphogliv--a new hepatoprotective drug based on polyunsaturated phosphatidylcholine and glycyrrhizic acid salt. Phosphogliv removed some symptoms of intoxication (nausea, weakness, jaundice, etc.) quicker than basic therapy. Among biochemical hepatitis markers, serum bilirubin level was most responsive to phosphogliv. Standard therapy decreases bilirubin by 30% on the average for 5 days, phosphogliv reduces bilirubin for one more week to half those values observed in control patients. At that point low aminotransferase activities were seen in phosphogliv treated patients. No side effects were seen. The new hepatoprotector phosphogliv which repairs biomembranes represents drugs of new generation compared to phospholipid drug essential.

Dirofilaria repens Railliet et Henry, 1911 was for the first time studied in the Moscow Region where 11 patients (6 males and 5 females) aged 4-72 years were recorded in 2000-2002. The geographical distribution (the southern and south-eastern parts of the region) of local cases of dirofiliriasis, was observed. There was a relatively high rate of D. repens in the male genitalia of the infected patients as compared with that reported by other authors. In accordance with the current terminology, D. repens in the Moscow Region is proposed to be regarded as emerging infection. The authors show it necessary to obligatorily record all autochtonous cases of D. repens infection, to make epidemiological surveillance and analysis in order to early detect and prevent dirofilariasis-associated complications.
