RESUMEN

Background: Observational studies have suggested an association between inflammatory markers and low back pain (LBP), but the causal relationship between these factors remains uncertain. Methods: We conducted a bidirectional two-sample Mendelian randomization analysis (MR) study to investigate whether there is a causal relationship between inflammatory markers and low back pain. We obtained genetic data for CRP, along with its upstream inflammatory markers IL-6, IL-8, and IL-10, as well as low back pain from publicly available genome-wide association studies (GWAS). We applied several MR methods, including inverse variance weighting, weighted median, MR-Egger, Wald Ratio, and MR-PRESSO, to test for causal relationships. Sensitivity analyses were also conducted to assess the robustness of the results. Results: Our analyses utilizing the Inverse Variance Weighted (IVW) method, the MR-Egger method, and the weighted median method indicated that IL-6 may be associated with an increased risk of LBP (Effect Size: -0.009, 95% Confidence Interval: -0.013-0.006, p = 9.16e-08); however, in the reverse direction, there was no significant causal effect of LBP on inflammatory markers. Conclusion: Our study used a Mendelian randomization approach and found that elevated IL-6 levels may reduce the risk of LBP.

OBJECTIVE: To investigate the effect of constant compressive stress induced by imitating Tuina stimulation with various durations on the cell cycle, cellular secretion, apoptosis, and expression of myogenic regulatory factors (MRFs), myogenic factor 5(Myf5) and myogenic differentiation (MyoD) of rat skeletal muscle cells (RSkMCs) in vitro. METHODS: Third passage RSkMCs were subjected to constant compressive stresses with various durations at 2000 strain for 15, 30, 60, 90, and 120 min via a four-point bending system. The control group (CG) was cultured in the absence of mechanical loading. Alterations of the cell cycle and apoptosis rate were detected by flow cytometry (FCM). The concentrations of interleukin 6 (IL-6) / prostaglandin E2 (PGE2) and nitric oxide (NO) in supernatants were determined by enzyme-linked immunosorbent assays and the nitrate reductase method, respectively. Expression of Myf5 and MyoD was detected by immunohistochemistry. RESULTS: Compared with the CG, a significant alteration was observed in the synthesis phase fraction (SPF) (P < 0.01). The SPF and proliferation index (PI) were reduced from 15 to 90 min, but reached levels similar to those at 120 min. Apoptosis was increased significantly at 30 min (P < 0.05) and especially at 90 and 120 min (P < 0.01). Expression of MyoD and Myf5 was increased significantly at 15, 30, and 90 min (P < 0.01). Compared with 15 and 30 min, MyoD and Myf5 expression at 60 and 120 min was decreased significantly (P < 0.01). Compared with 60 min, MyoD expression at 90 min was increased significantly (P < 0.05), whereas MyoD and Myf5 expression at 120 min was significantly lower (P < 0.05). The IL-6 concentration was increased at 60 min compared with the CG and 15 min (P < 0.05), whereas the concentrations of PGE2 and NO were the highest at 15 and 30 min, respectively, compared with the CG and other time points (P < 0.05). CONCLUSION: The cell cycle, secretion, apoptosis, and Myf5 and MyoD expression of RSkMCs were regulated by compressive stress in a time-dependent manner. SPF and PI were inhibited at short durations (< 90 min), but NO and PGE2 secretion was the highest at shorter durations (< 30 min). With the prolongation of stimulation time, SPF, PI, and apoptosis were increased, but Myf5 and MyoD expression was decreased gradually at 15-30 min.

OBJECTIVE: To observe the effect of hallux-trampling (HT) combined with Yaotongling suppository (YS) on the microcirculation at waist region and lower limbs in treating patients with lumbar intervertebral disc prolapse (LIDP), and to preliminarily explore its mechanism. METHODS: Adopting randomized, double-blinded medicine and single-blinded manipulation controlled method, 108 patients with LIDP of cold-accumulation and blood-stasis syndrome were assigned equally to 3 groups in random. Group A was treated with HT plus YS, Group B treated with HT plus placebo suppository, and Group C treated with YS plus mock HT. After 2-week treatment, the changes in score of symptoms and signs as well as the microcirculation at various site-points of waist and lower limbs were observed before and after treatment. RESULTS: The total scores of symptoms and signs after treatment in Group A, B and C was 7.340 +/- 4.627, 9.543 +/- 4.745 and 10.086 +/- 4.631 respectively, which was significantly lowered as compared with those before treatment respectively (P < 0.05). Significant difference of microcirculation between pre- and post-treatment was shown at all site-points in Group A and B (P < 0.05), but only shown at the site-points of lower limbs in Group C, so significant difference (P < 0.05) was shown when compared the microcirculation in Group A or B with that in Group C (P < 0.05). CONCLUSION: HT combined YS in treating LIDP could markedly improve the microcirculation at waist and lower limbs to correct the nutritional disturbance of nerve root, and the improvement of microcirculation is correlated with the changes of clinical symptoms and signs.

The manipulation characteristic and experience in the treatment of tendon bi syndrome, by LUO Caigui, the veteran TCM physician, were introduced in the article. The tendon bi syndrome is treated on the base of syndrome differentiation for the acupoint selection in the guidance of the theory on the tendon region of meridian in Huangdi Neijing (Yellow Emperor's Inner Canon) and in accordance with the etiology and clinical characteristics of tendon bi syndrome. Focusing on the palpation and the responses under the fingers, as well as the application of warming method, the acupuncture (rubbing and twisting) and tuina (relaxing, separating, warming and regulating) manipulations are adopted in combination on the positive points and the intermuscular spaces. This compound manipulation results in the rapid deqi, prolonged needling sensation and obvious warming effects. With this therapeuticmethods, the clinical effects are achieved significantly, the symptoms and physical signs improved and the duration of sickness shortened.

RESUMEN

The meridian diseases, abnormal psoas and abdominal muscles and abdominal pressure are key causes of low back pain. We treat low back pain mainly with lumbar back area, neglecting abdomen. The anterior-posterior acupoint combination, regulating yin to treat yang, is effective. Based on the Chinese meridian-collateral and contemporary anatomy related to low back pain, the combination method, tuina and its mechanism are introduced, so as to develop the thought and method for low back pain. A typical case is added.

Long-term unregulated mining of ion-adsorption clays (IAC) in China has resulted in severe ecological destruction and created large areas of wasteland in dire need of rehabilitation. Soil amendment and revegetation are two important means of rehabilitation of IAC mining wasteland. In this study, we used sludge biochar prepared by pyrolysis of municipal sewage sludge as a soil ameliorant, selected alfalfa as a revegetation plant, and conducted pot trials in a climate-controlled chamber. We investigated the effects of alfalfa revegetation, sludge biochar amendment, and their combined amendment on soil physicochemical properties in soil from an IAC mining wasteland as well as the impact of sludge biochar on plant growth. At the same time, we also assessed the impacts of these amendments on the soil microbial community by means of the Illumina Miseq sequences method. Results showed that alfalfa revegetation and sludge biochar both improved soil physicochemical properties and microbial community structure. When alfalfa revegetation and sludge biochar amendment were combined, we detected additive effects on the improvement of soil physicochemical properties as well as increases in the richness and diversity of bacterial and fungal communities. Redundancy analyses suggested that alfalfa revegetation and sludge biochar amendment significantly affected soil microbial community structure. Critical environmental factors consisted of soil available K, pH, organic matter, carbon⁻nitrogen ratio, bulk density, and total porosity. Sludge biochar amendment significantly promoted the growth of alfalfa and changed its root morphology. Combining alfalfa the revegetation with sludge biochar amendment may serve to not only achieve the revegetation of IAC mining wasteland, but also address the challenge of municipal sludge disposal by making the waste profitable.

Ammonia nitrogen wastewater (about 100 mg/L) was treated by two-stage ozone oxidation method. The effects of ozone flow rate and initial pH on ammonia removal were studied, and the mechanism of ammonia nitrogen removal by ozone oxidation was discussed. After the primary stage of ozone oxidation, the ammonia removal efficiency reached 59.32% and pH decreased to 6.63 under conditions of 1 L/min ozone flow rate and initial pH 11. Then, the removal efficiency could be over 85% (the left ammonia concentration was lower than 15 mg/L) after the second stage, which means the wastewater could have met the national discharge standards of China. Besides, the mechanism of ammonia removal by ozone oxidation was proposed by detecting the products of the oxidation: ozone oxidation directly and ·OH oxidation; ammonia was mainly transformed into NO3(-)-N, less into NO2(-)-N, not into N2.

This method has significant efficacy on improving low back pain and numbness of lower extremities, which is characterized with short-time manipulation, quick de-qi and long effective time. The acupuncture methods, manipulations, precautions, etc. are elaborated in details. A typical case is added.
