RESUMEN

OBJECTIVE: The aim of this paper is to report the experiences of applying a model of transpersonal caring in nursing home care according to Favero and Lacerda to adult patients after hematopoietic stem cell transplantation. METHOD: This is a case report on the application of this model to an outpatient monitored by a bone marrow transplant service. In addition to the initial outpatient contact, the patient received home care visits in October 2014. Data were recorded in the field diary and analysed according to the Care Model and Clinical Caritas Process. RESULTS: The provided care served as support to meet basic human needs, and strengthen the belief system. It also promoted the necessary emotional care to cope with the treatment and professional maturity in the caring relationship. CONCLUSION: The experience description revealed that the model can support the application of the Theory of Human Caring in home care and the use of care models in practice, professional training, and research development.

RESUMEN

OBJECTIVE: To evaluate the effectiveness of hyaluronic acid in the healing of partial thickness burns. METHOD: Systematic review of randomized controlled trials on the use of hyaluronic acid for the topical treatment of skin burns, based on recommendations of the Cochrane Handbook for Systematic Reviews of Interventions. RESULTS: Two randomized controlled trials that analyzed 143 patients with partial thickness burns and/or deep partial thickness burns were selected. They compared the application of hyaluronic acid 0.2% associated to silver sulfadiazine 1% 5g/cm2 versus silver sulfadiazine 1% 5g/cm2 alone for the outcome of complete healing. CONCLUSION: This review emphasizes the need for new well-designed randomized controlled trials to establish the therapeutic relevance of hyaluronic acid with respect to the healing of burns of partial thickness or deep partial thickness. OBJETIVO: Avaliar a efetividade do ácido hialurônico na cicatrização de queimaduras de espessura parcial. MÉTODO: Revisão sistemática de ensaios clínicos randomizados sobre a utilização de ácido hialurônico no tratamento tópico de queimaduras de pele, baseada nas recomendações do Cochrane Handbook for Systematic Reviews of Interventions. RESULTADOS: Foram recuperados dois ensaios clínicos randomizados que analisaram 143 pacientes portadores de queimaduras de espessura parcial e/ou espessura parcial profunda, comparando a aplicação de Ácido Hialurônico 0,2% associado à Sulfadiazina de Prata 1% 5g/cm2, versus Sulfadiazina de Prata 1% 5g/cm2 isolada, para o desfecho cicatrização completa. CONCLUSÃO: Esta revisão enfatiza a necessidade de novos ensaios clínicos randomizados bem delineados para estabelecimento da relevância terapêutica do ácido hialurônico no que tange à cicatrização de queimaduras de espessura parcial ou espessura parcial profunda.

RESUMEN

OBJECTIVE: To evaluate the effectiveness of enteral nutritional therapy (ENT) in the healing process of pressure ulcers (PU) in adults and the elderly. METHOD: A systematic review whose studies were identified through the databases of Cochrane, MEDLINE/PubMed, SciELO, LILACS, EMBASE, CINAHL, Web of Science, and manual searches. It included randomized clinical trials (RCTs) without delimiting the period or language of publication, which addressed adults and elderly patients with pressure ulcers in a comparative treatment of enteral nutritional therapy and placebo or between enteral nutritional therapy with different compositions and dosages. RESULTS: We included ten studies that considered different interventions. It resulted in more pressure ulcers healed in the groups that received the intervention. The included studies were heterogeneous with regard to patients, the type of intervention, the sample and the follow-up period, all of which made meta-analysis impossible. CONCLUSION: Although the enteral nutritional therapy demonstrates a promotion of pressure ulcer healing, sufficient evidence to confirm the hypothesis was not found.

RESUMEN

UNLABELLED: This research aimed to analyze the concept of self-management of hypertensive individuals. Theoretical and documentary study based on Walker and Avant's conceptual analysis by means of the Scientific Electronic Library Brazil and the Medical Literature Analysis and Retrieval System Online in the Coordination for Higher Education Personnel Development (CAPES, in Portuguese) and the National Library of Medicine websites. Fourteen (14) articles and one (1) thesis were selected and reviewed in Portuguese and English, in the period January 2007 to September 2012. BACKGROUND: missing doctor's appointments, non-compliance to blood pressure control treatment to recommendations to proper diet standards and stress. Attributer blood pressure control and disease management Consequences home monitoring of blood pressure with control improvement, accomplishment of disease management, compliance and sharing of the creation process of self-management goals and caring activities by the interdiscplinary team through individualized actions. It was concluded that the self-management concept is a dynamic, active process which requires knowledge, attitude, discipline, determination, commitment self-regulation, empowerment and self-efficiency in order to manage the disease and achieve healthy living.

OBJECTIVE: Summarizing the evidence from systematic reviews regarding the comparison the effectiveness of interventions to prevent pressure injuries. METHOD: Overview of systematic reviews conducted in accordance with Cochrane guidelines. A search was performed in databases, repositories and systematic review registration sites. RESULTS: 15 reviews were included in this overview. The sensitivity analysis showed a reduction in the incidence of pressure injuries with nutritional supplementation compared to the standard hospital diet (Relative Risk (RR) = 0.83; 95% Confidence Interval (CI): 0.72-0.95). There was evidence of the superiority of constant low-pressure surfaces (RR = 0.38; 95% CI;0.24-0.61), alternating pressure devices (RR = 0.31; 95% CI:0.17-0.58) and alternative foams (RR = 0.40; 95% CI:0.21-0.74) when compared to the standard hospital mattress or standard foam. The use of a silicone cover reduced the incidence of pressure injuries by 75% (RR = 0.25; 95%CI:0.16-0.41) when compared to no cover. CONCLUSION: Although some interventions have been shown to be effective in reducing the incidence of pressure injury, the evidence is limited or very limited and subject to change. RESUMEN

The objective of this reflection was to understand epidemiology as the theoretical-methodological framework for collective health nursing practice. This study is a bibliographic investigation which analyzed articles and books that point out the strengths and weaknesses of classic, social and critical epidemiology, with the purpose to examine their concepts in regards to nursing practice. The connections between the knowledge obtained from the above mentioned views of epidemiology which allow the creation of nursing interventions to change health realities are discussed. Critical epidemiology is supported by the recognition of the protecting processes and wearing determinants of the health-disease process experienced by different social classes. Hence, nurses should plan health interventions aiming beyond the identified illness, proposing interventions committed to changing historical and social processes, taking into account the unique, particular or structural dimensions that eventually determine the health-disease process in individuals or groups.

RESUMEN

OBJECTIVES: to analyze the available evidence on the nursing care provided to patients after hematopoietic stem cell transplantation. METHODS: integrative review with the search for primary studies in four databases and a virtual health library. A broad search strategy was used, including research published in English, Brazilian Portuguese, or Spanish, between 2008 and 2018, totaling a sample of 42 studies. RESULTS: the studies were grouped into three categories: multiple nursing care (n=19), first-line care (n=18), and self-management of care (n=5). CONCLUSIONS: nursing care is critical, comprising patients' physical, psychological and social aspects. It occurs in hospital and home contexts, mainly involving technical actions and health guidance. The evidence identified provide subsidies for decision-making; however, most studies are of the non-experimental type, indicating the need for conducting intervention research.

RESUMEN

The present study aimed at providing a synthesis of clinical studies on pressure ulcer (UP). An integrative review was made by searching the database of MEDLINE, Portal de Evidências, and LILACS for articles, and the Joanna Briggs Institute (JBI) periodical, a reference in clinical trials in Nursing, from 2004-2009. 14 articles were selected and their analysis made it possible to identify four areas of interest involving the characterization of the studies, the sample, the interventions used, as well as the results and conclusions obtained. We found that the studies reviewed have focused on prevention and that there are few studies of this kind in our country. It is important to recognize these limitations and try to overcome them in order to provide advancements and enhance the development of research based on strong clinical evidence to support the professional nursing practice.

OBJECTIVE: to identify, based on the evidence, point-of-care testing in bedbound in critically ill patients. METHOD: integrative review, carried out through search in Pubmed, Virtual Health Library, Joanna Briggs Institute, The British Institute of Radiology, Brazilian Radiology, and Google Scholar databases. We used the PICO research strategy and selected articles published from 2013 onwards, which presented information about point-of-care testing. RESULTS: the different interventions found in the analysis of the 23 selected articles allowed the thematic grouping of care related to safety in communication, patient identification, care with devices, and the prevention and control of infection, which can be used in point-of-care testing. Final considerations: The care described in the evidence provided support for validating a safe care protocol for critically ill patients undergoing imaging studies in bed.

Evaluation of photobiomodulation therapy (PBMT) in the treatment of pressure ulcers in adults and the elderly. Systematic review, based on the recommendations of the handbook, proposed by Cochrane. The search was carried out in databases, records of randomized clinical trials, list of references cited in the selected articles, as well as a manual search in meetings and specialized journals. A total of 1342 studies were identified, 18 were preselected and 5 were included in this review. Clinical heterogeneity of the participants was observed, in addition to variation in the laser parameters and predominance of studies of low methodological quality. PBMT with the use of laser (658 nm; 4 J cm-2 ;50 mw) showed complete wound healing (P < 0.001) when compared to lasers (990 nm and 808 nm). However, there was no statistically significant difference in relation to time to complete wound healing and in area reduction compared to standard care. PBMT in the infrared wavelength showed efficacy in the healing of the pressure ulcer, similar to the standard care presented in the different studies. PBMT (658 nm) was effective in promoting healing when compared to standard care. Laser can be a therapy of choice in the treatment of pressure ulcers, since no evidence has been found to refute its clinical application.

RESUMEN

This is a methodological research, which aimed at organizing the systematization of nursing assistance in a critical care unit. The following steps were carried out: description of the nursing practice; transcription of nursing diagnoses; elaboration of a protocol for nursing diagnosis based in International Classification for Nursing Practice (ICNP); determination of nursing prescriptions and the elaboration of guidelines for care and procedures. The nursing practice and care complexity in ICU were characterized. Thus, systematization of nursing assistance is understood as a valuable tool for nursing practice.

OBJECTIVE: To identify the tendencies and teaching-learning strategies used for leadership development in the discipline Nursing Administration in higher education institutions in Brazil. METHOD: Non-experimental, type survey, descriptive and exploratory, cross-sectional, with quantitative approach. RESULTS: The dialogued lecture strategy was cited as used by 241 (91%) professors, followed by research, by 237 (89%), and discussion or group work, by 221 (83%). Strategies not used were: spiral learning, 166 (63%), and online programs, 151 (57%) professors. The strategy that presents higher tendency of use is that of online courses, 104 (39%). CONCLUSION: among the strategies mentioned, all are aligned with the directions given in the literature for nursing education, but these strategies and methodologies influence the formation of leader nurses differently.

Objetivo: analisar a literatura científica acerca das tecnologias e cuidados para posicionamento e reposicionamento do cateter central de inserção periférica (PICC) em neonatos. Método: revisão integrativa, com busca realizada em fevereiro de 2022 em quatro bases de dados. Resultados: incluíram-se 32 estudos que abordam uso de tecnologias para verificação da localização do PICC, procedimentos para seu posicionamento e manobras para reposicionamento. Para posicionamento adequado deve-se atentar para seleção do vaso, mensuração correta do dispositivo e manutenção do bem-estar do recém-nascido. Frente ao mal posicionamento sugere-se a movimentação do membro, flush, tração do cateter, e conduta expectante. A verificação da localização da ponta é rotineira, por meio de radiografia, ultrassonografia ou eletrocardiograma. Conclusão: recomenda-se a adoção de tecnologias não invasivas para o posicionamento e reposicionamento do PICC em neonatos. As evidências apontam para competência profissional na tomada de decisão para o cuidado seguro e de qualidade, e prevenção de eventos adversos.


Objective: to analyze the scientific literature on technologies and care for positioning and repositioning of the peripherally inserted central catheter (PICC) in neonates. Method: integrative review, with search conducted in February 2022 in four databases. Results: 32 studies were included that address the use of technologies to verify the location of the PICC, procedures for its positioning and maneuvers for repositioning. For proper positioning should pay attention to the selection of the vessel, correct measurement of the device and maintenance of the well-being of the newborn. In the face of poor positioning, it is suggested limb movement, flush, catheter traction, and expectant management. The verification of the tip location is routine, by radiography, ultrasonography or electrocardiogram. Conclusion: the adoption of non-invasive technologies for the positioning and repositioning of PICC in neonates is recommended. The evidence points to professional competence in decision making for safe and quality care and prevention of adverse events.

La evidencia apunta a la competencia profesional en la toma de decisiones para el cuidado seguro y de calidad, y la prevención de eventos adversos.

RESUMEN Objetivo: Resumir las pruebas de las revisiones sistemáticas sobre la comparación de la efectividad de las intervenciones para prevenir las lesiones por presión. Método: Resumen de las revisiones sistemáticas realizadas de acuerdo con las recomendaciones Cochrane. Se realizó una búsqueda en bases de datos, repositorios y sitios de registro de revisiones sistemáticas. Resultados: Se incluyeron 15 revisiones en esta revisión. El análisis de sensibilidad mostró una reducción de la incidencia de lesiones por presión con la suplementación nutricional en comparación con la dieta hospitalaria estándar (riesgo relativo [RR] = 0,83; intervalo de confianza [IC] del 95%: 0,72-0,95). Hubo pruebas de la superioridad de las superficies de baja presión constante (RR=0,38; IC del 95%:0,24-0,61), los dispositivos de presión alternante (RR = 0,31; IC del 95%:0,17-0,58) y las espumas alternativas (RR = 0,40; IC del 95%:0,21-0,74) en comparación con el colchón hospitalario estándar o la espuma estándar. El uso de una funda de silicona redujo la incidencia de lesiones por presión en un 75% (RR = 0,25; IC del 95%:0,16-0,41) en comparación con la ausencia de funda. Método: Overview de revisões sistemáticas conduzida de acordo com as recomendações Cochrane. Realizou-se busca em bases de dados, repositórios e site de registro de revisões sistemáticas. Resultados: Foram incluídas 15 revisões nesta overview. A análise de sensibilidade demonstrou redução na incidência de lesão por pressão com a suplementação nutricional comparada a dieta hospitalar padrão (Risco Relativo (RR) = 0,83; Intervalo de Confiança(IC) 95%:0,72-0,95). Evidenciaram-se superioridade das superfícies de baixa pressão constante (RR = 0,38; IC 95%;0,24-0,61), dos dispositivos de pressão alternada (RR = 0,31; IC95%:0,17-0,58) e das espumas alternativas (RR = 0,40; IC95%:0,21-0,74) quando comparadas ao colchão hospitalar padrão ou de espuma padrão. O uso de cobertura de silicone reduziu em 75% a incidência de lesão por pressão (RR = 0,25; IC95%:0,16-0,41) quando comparada a nenhuma cobertura. Conclusão: Embora algumas intervenções demonstrem-se efetivas na redução da incidência da lesão por pressão, as evidências são limitadas ou muito limitadas e sujeitas a alteração. RESUMEN

This study is aimed at discussing the conception of permanent, continuous, in service education among nurses at a school hospital. It is a research of qualitative nature, for which the data was gathered using the technique of focal group in six sessions recorded in audio and video. Nine nurses of a school hospital took part in it. For the analysis of the data was used the technique of collective subject speech, which consists of the organization and computation of the qualitative data of oral nature recorded, from which resulted a single speech. The research showed that the nurses differentiated the terms permanent education, continuous education and in service education, building their own concepts, highlighting the importance of differentiation in order to define the kind of approach to be taken to address the educative need presented.

OBJECTIVE: to evaluate the effectiveness and safety in the use of second-generation central venous catheters impregnated in clorhexidine and silver sulfadiazine when compared with other catheters, being them impregnated or not, in order to prevent the bloodstream infection prevention. METHOD: systematic review with meta-analysis. Databases searched: MEDLINE, EMBASE, CINAHL, LILACS/SciELO, Cochrane CENTRAL; search in Congress Proceedings and records from Clinical Trials. RESULTS: 1.235 studies were identified, 97 were pre-selected and 4 were included. In catheter-related bloodstream infection, there was no statistical significance between second-generation impregnated catheter compared with the non-impregnated ones, absolute relative risk 1,5% confidence interval 95% (3%-1%), relative risk 0,68 (confidence interval 95%, 0,40-1,15) and number needed to treat 66. In the sensitivity analysis, there was less bloodstream infection in impregnated catheters (relative risk 0,50, confidence interval 95%, 0,26-0,96). Lower colonization, absolute relative risk 9,6% (confidence interval 95%, 10% to 4%), relative risk 0,51 (confidence interval 95% from 0,38-0,85) and number needed to treat 5. CONCLUSION: the use of second-generation catheters was effective in reducing the catheter colonization and infection when a sensitivity analysis is performed. Future clinical trials are suggested to evaluate sepsis rates, mortality and adverse effects. OBJETIVO: evaluar la efectividad y seguridad del uso de catéteres venosos centrales de segunda generación, impregnados en clorhexidina y sulfadiazina de plata, comparados con otros catéteres impregnados o no impregnados, para prevención de infección de la corriente sanguínea. MÉTODO: revisión sistemática con metaanálisis. La búsqueda fue realizada en las bases: MEDLINE, EMBASE, CINAHL, LILACS/SciELO, Cochrane CENTRAL; fueron consultados anales de congresos y registros de ensayos clínicos. RESULTADOS: fueron identificados 1.235 estudios, 97 preseleccionados y cuatro incluidos. En la infección de la corriente sanguínea, relacionada al catéter, no hubo significación estadística entre catéter de segunda generación impregnado en comparación a los no impregnados, riesgo relativo absoluto 1,5%, intervalo de confianza 95% (3%-1%), riesgo relativo 0,68 (intervalo de confianza 95%, 0,40-1,15) y número necesario para tratar 66. En el análisis de sensibilidad, hubo disminución de la infección de la corriente sanguínea en los catéteres impregnados (riesgo relativo 0,50, intervalo de confianza 95%, 0,26-0,96). Reducción de la colonización, riesgo relativo absoluto de 9,6% (intervalo de confianza 95%, 10% a 4%), riesgo relativo 0,51 (intervalo de confianza 95% de 0,38-0,85) y número necesario para tratar 5. CONCLUSIÓN: el uso de los catéteres de segunda generación fue efectivo en la reducción de la colonización del catéter y de infección cuando realizado análisis de sensibilidad. Se sugirieron ensayos clínicos futuros que evalúen tasas de sepsis, mortalidad y efectos adversos. OBJETIVO: avaliar a efetividade e segurança do uso de cateteres venosos centrais de segunda geração, impregnados em clorexidina e sulfadiazina de prata, comparados com outros cateteres impregnados ou não, na prevenção de infecção de corrente sanguínea. MÉTODO: revisão sistemática com metanálise. Busca realizada nas bases: MEDLINE, EMBASE, CINAHL, LILACS/SciELO, Cochrane CENTRAL; consulta em anais de congresso e registro de ensaios clínicos. RESULTADOS: foram identificados 1.235 estudos, 97 pré-selecionados e quatro incluídos. Na infecção de corrente sanguínea, relacionada ao cateter, não houve significância estatística entre cateter de segunda geração impregnado em comparação aos não impregnados risco relativo absoluto 1,5%, intervalo de confiança 95% (3%-1%), risco relativo 0,68 (intervalo de confiança 95%, 0,40-1,15) e número necessário para tratar 66. Na análise de sensibilidade, houve diminuição da infecção de corrente sanguínea nos cateteres impregnados (risco relativo 0,50, intervalo de confiança 95%, 0,26-0,96). Redução da colonização, risco relativo absoluto de 9,6% (intervalo de confiança 95%, 10% a 4%), risco relativo 0,51 (intervalo de confiança 95% de 0,38-0,85) e número necessário para tratar 5. CONCLUSÃO: o uso dos cateteres de segunda geração foi efetivo na redução de colonização do cateter e de infecção quando realizada análise de sensibilidade. Sugerem-se ensaios clínicos futuros que avaliem taxas de sepse, mortalidade e efeitos adversos.

ABSTRACT Objectives: to analyze the available evidence on the nursing care provided to patients after hematopoietic stem cell transplantation. Methods: integrative review with the search for primary studies in four databases and a virtual health library. A broad search strategy was used, including research published in English, Brazilian Portuguese, or Spanish, between 2008 and 2018, totaling a sample of 42 studies. Results: the studies were grouped into three categories: multiple nursing care (n=19), first-line care (n=18), and self-management of care (n=5). Conclusions: nursing care is critical, comprising patients' physical, psychological and social aspects. It occurs in hospital and home contexts, mainly involving technical actions and health guidance. The evidence identified provide subsidies for decision-making; however, most studies are of the non-experimental type, indicating the need for conducting intervention research.

Métodos: revisión integradora con búsqueda de estudios primarios en cuatro bases de datos y biblioteca virtual en salud. Se utilizó una amplia estrategia de búsqueda, incluyendo investigaciones publicadas en inglés, portugués o español, entre 2008 y 2018, totalizando una muestra de 42 estudios. Resultados: los estudios se agruparon en tres categorías: cuidados múltiples de enfermería (n=19), cuidados de primera línea (n=18) y cuidados autogestionados (n=5). Conclusiones: el cuidado de enfermería es crítico, entendiendo los aspectos físicos, psicológicos y sociales del paciente. Ocurre en contextos hospitalarios y domiciliarios, involucrando principalmente acciones técnicas y de orientación sanitaria. Las evidencias identificadas brindan subsidios para la toma de decisiones, sin embargo, la mayoría de los estudios son del tipo no experimental, lo que indica la necesidad de realizar investigación de intervención.

RESUMEN

The correct and safe medication administration, independently of the form, is under the responsibility of the nursing team. To this team it is necessary specific knowledge and abilities, as well as continuous update of the processes that involve such care. This article has as objective to make some considerations about this subject, adjusting them to the common nursing practice. For this, we present a literature review that approaches the main application methods used for subcutaneous medication. After that, we argue some aspects for professional improvement, so that the medication administration be satisfactory and safe in the caring process.
