RESUMEN

Peptoniphilus asaccharolyticus are gram-positive anaerobic cocci (GPAC) usually found as commensals of the skin or in the setting of polymicrobial colonisation of chronic wounds and ulcers. However, its pathogenic potential in more severe, invasive infections such as bone, joint or blood stream infections remains unclear, with studies on underlying virulence factors still pending. In this case report we present two cases of P. asaccharolyticus-associated infections of the bone and joint as well as a review of the literature. The cases cast a new light on possible synergistic interactions between P. asaccharolyticus and more virulent aerobic bacteria as well as on its role as pathogen in severe mono-infection.

RESUMEN

The lifestyle recommendations of the World Cancer Research Fund (WCRF)/American Institute for Cancer Research (AICR) are primarily intended for cancer prevention. In the absence of specific recommendations for cancer survivors, we investigated adherence of colorectal cancer (CRC) survivors to the WCRF/AICR lifestyle recommendations and associations with health-related quality of life (HRQoL). The cross-sectional part of the Energy for life after ColoRectal cancer (EnCoRe) study was conducted in 155 CRC survivors (stage I-III), 2-10 years post diagnosis. Dietary intake, physical activity and general body fatness were measured by 7-d food diaries, by questionnaires and accelerometers and BMI, respectively. Adherence to each of the ten WCRF/AICR recommendations was scored as 0 (no/low adherence), 0·5 (moderate adherence) or 1 point (complete adherence), and summed into an overall adherence score (range: 0-10). HRQoL, disability and distress were assessed by validated questionnaires. Associations of the overall WCRF/AICR adherence score with HRQoL outcomes were analysed by confounder-adjusted linear regression. The mean adherence score was 5·1 (sd 1·4, range: 1·5-8·5). In confounder-adjusted models, a higher adherence score was significantly associated with the HRQoL dimension better physical functioning (ß per 1 point difference in score: 2·6; 95 % CI 0·2, 5·1) and with less fatigue (ß: -3·3; 95 % CI -6·4, -0·1). In conclusion, higher adherence of CRC survivors to WCRF/AICR lifestyle recommendations for cancer prevention was associated with better physical functioning and with less fatigue. This study adds to the limited knowledge on adherence to lifestyle behaviours in CRC survivors and relationships with quality of life. Prospective studies are needed to investigate longitudinal associations.

OBJECTIVES: Infections caused by multidrug-resistant Enterobacterales pose a significant challenge to clinical patient care, particularly in resource-constrained settings where epidemiological data on antimicrobial resistance are scarce. The aim of this study was to determine the prevalence of extended spectrum beta-lactamase-(ESBL)-producing Klebsiella pneumoniae among clinical samples from a teaching hospital in Bouaké, central Côte d'Ivoire. METHODS: Clinical specimens were collected from sterile and non-sterile body sites and were subjected to microbiological diagnostics (April 2016-June 2017). The antimicrobial susceptibility patterns of K. pneumoniae were analysed using automated resistance testing and double-disk diffusion to test for ESBL production. Multiplex PCR was carried out to determine the presence of the resistance-conferring genes blaCTX-M, blaSHV and blaTEM. RESULTS: A total of 107 isolates were included, most of which were obtained from bloodstream (39%; n=42) and urinary tract infections (39%; n=42). Among all K. pneumoniae isolates, 84% (n=90) were ESBL producers, many of which were also not susceptible to sulfonamides (99%), quinolones (81%) and aminoglycosides (79%). The majority of ESBL-producing strains harboured all three investigated bla genes. CONCLUSION: The high prevalence of ESBL-producing K. pneumoniae in clinical isolates from Côte d'Ivoire calls for revised empirical treatment regimens in critically ill patients with suspected Gram-negative infections, and the establishment of antimicrobial resistance surveillance systems.

RESUMEN

Safety and efficacy of vaccinations during pregnancy have been a matter of debate. In the aftermath of the 2009 H1N1 influenza pandemic, a growing body of research has emerged, which points toward the importance of adhering to influenza vaccination recommendations for pregnant women. The same applies for vaccination against pertussis. Some vaccines (e.g., live attenuated) are still contraindicated during pregnancy. However, data indicate that these vaccines do not result in fetal impairment when administered accidentally during pregnancy. In the following, we provide a review on vaccination-related safety and efficacy aspects in pregnant women, shedding some light on potential barriers that stymie vaccination uptake among pregnant women and introducing strategies to overcome these barriers.

OBJECTIVE: To determine whether prenatal and perinatal maternal consumption of alcohol, tobacco and/or illicit drugs is associated with risk of neuroblastoma. DATA SOURCES: Medline and Embase (both from inception to February 2017), and reference lists of included studies. STUDY SELECTION: To be eligible, a study had to be an original report including data on intake of alcohol, tobacco smoking and/or consumption of illicit drugs during pregnancy and risk of neuroblastoma in the child. DATA EXTRACTION: From eligible studies, data study characteristics as well as effect measures and confounders were extracted. We assessed unadjusted and confounder-adjusted estimates, performed risk of bias analysis, constructed random-effects models and assessed heterogeneity. RESULTS: We identified 14 case-control studies (1987-2016) involving a total of 3114 children with neuroblastoma. Meta-analysis of unadjusted estimates showed an association between alcohol (OR 1.26; 95% CI 1.07 to 1.49), tobacco (OR 1.22; 95% CI 1.04 to 1.44) and illicit drug consumption during pregnancy and risk of neuroblastoma during childhood, with illicit drug consumption showing the strongest association (OR 3.26; 95% CI 1.36 to 7.86). However, adjusted estimates were highly heterogeneous. LIMITATIONS: All studies were at high risk of bias. CONCLUSIONS: Smoking, alcohol or illicit drugs during pregnancy might play a role in the development of neuroblastoma. However, well-designed studies are needed to assess whether these exposures are causal and whether time period during pregnancy, dose or co-consumption of substances is critical. RESUMEN

We report on a 68-year-old male with a cerebellopontine angle tumor manifesting at the 8th cranial nerve and presenting histopathological features of a rhabdomyoma. A literature review revealed four reports of intracranial nerve rhabdomyoma, all of adult type and including one manifestation at the vestibular nerve. We present the first case of a fetal type extracardiac rhabdomyoma manifesting at a cranial nerve. Although rare, rhabdomyomas must be considered in the differential diagnosis of vestibular schwannomas.
