RESUMEN

BACKGROUND: Measles is a contagious viral disease that seriously affects children. The measles vaccine is widely recommended in Brazil and in the world; however, the disease remains relevant for the health authorities. The aim of the present study was to evaluate first and second dose of measles vaccine coverage (VC) in the cities of São Paulo and its spatial dynamics between 2015 and 2020. METHOD: In this mixed-type ecological study, we used secondary, public domain data from 2015 to 2020, extracted from the Digital Information System of the National Immunization Program, Mortality Information System and the National Live Birth Information System. After calculating the VC, the following four categories were created: very low, low, adequate, and high, and the spatial autocorrelation of VC was analyzed using the Global and Local Moran's statistics. RESULTS: A steady decline in adherence to the vaccination was observed, which dynamically worsened until 2020, with a high number of cities fitting the classification of ineffective coverage and being potentially harmful to the effectiveness of the immunization activities of their neighbors. CONCLUSION: A direct neighborhood pattern was observed between the units with low vaccination coverage, which implied that the reduction in measles VC was somehow related to and negatively influenced by the geographic location and social culture of these areas.

RESUMEN

BACKGROUND: Pyogenic granuloma (PG) is a lesion characterized by the proliferation of blood vessels, commonly affecting the skin and the mouth. We aimed to compare clinical, microscopic, and immunohistochemical features of the two types of oral PG: lobular capillary hemangioma (LCH) and non-LCH (NLCH). METHODS: Epidemiological and clinical data from 2000 to 2018 were collected from the archives of our institution, and histopathological sections of PG were reviewed. Immunohistochemical analyses (CD34, D2-40, SMA, mast cell, and Ki-67) were performed in 34 cases. RESULTS: Sixty-two LCH and 107 non-LCH samples were included. The mean (±SD) age of the patients was 38.59 ± 16.96 years; 55.62% were female; 39.64% of cases occurred in the gingiva, 44% of the nodules were pedunculated, and 13.02% of patients reported a history of trauma. NLCH was more prevalent among older patients than LCH. The most prevalent site of LCH was the lips, while NLCH occurred more in the gingiva (P < 0.05). Epithelial atrophy, microvessels, SMA-positive areas, and Ki-67-positive nuclei were more prevalent in LCH (P < 0.05). CONCLUSIONS: PG accounted for 2.25% of lesions archived in the pathology service and most cases were NLCH. LCH and NLCH exhibited clinicopathological differences in terms of age, site, epithelial atrophy, vascularization, and proliferation rate.

RESUMEN

We show that random oblivious transfer protocols that are statistically secure according to a definition based on a list of information-theoretical properties are also statistically universally composable. That is, they are simulatable secure with an unlimited adversary, an unlimited simulator, and an unlimited environment machine. Our result implies that several previous oblivious transfer protocols in the literature that were proven secure under weaker, non-composable definitions of security can actually be used in arbitrary statistically secure applications without lowering the security.

RESUMEN

Concerns about security on Internet of Things (IoT) cover data privacy and integrity, access control, and availability. IoT abuse in distributed denial of service attacks is a major issue, as typical IoT devices' limited computing, communications, and power resources are prioritized in implementing functionality rather than security features. Incidents involving attacks have been reported, but without clear characterization and evaluation of threats and impacts. The main purpose of this work is to methodically assess the possible impacts of a specific class-amplified reflection distributed denial of service attacks (AR-DDoS)-against IoT. The novel approach used to empirically examine the threat represented by running the attack over a controlled environment, with IoT devices, considered the perspective of an attacker. The methodology used in tests includes that perspective, and actively prospects vulnerabilities in computer systems. This methodology defines standardized procedures for tool-independent vulnerability assessment based on strategy, and the decision flows during execution of penetration tests (pentests). After validation in different scenarios, the methodology was applied in amplified reflection distributed denial of service (AR-DDoS) attack threat assessment. Results show that, according to attack intensity, AR-DDoS saturates reflector infrastructure. Therefore, concerns about AR-DDoS are founded, but expected impact on abused IoT infrastructure and devices will be possibly as hard as on final victims.

RESUMEN

Domain Generation Algorithms (DGAs) are used by malware to generate pseudorandom domain names to establish communication between infected bots and command and control servers. While DGAs can be detected by machine learning (ML) models with great accuracy, offering DGA detection as a service raises privacy concerns when requiring network administrators to disclose their DNS traffic to the service provider. The main scientific contribution of this paper is to propose the first end-to-end framework for privacy-preserving classification as a service of domain names into DGA (malicious) or non-DGA (benign) domains. Our framework achieves these goals by carefully designed protocols that combine two privacy-enhancing technologies (PETs), namely secure multi-party computation (MPC) and differential privacy (DP). Through MPC, our framework enables an enterprise network administrator to outsource the problem of classifying a DNS (Domain Name System) domain as DGA or non-DGA to an external organization without revealing any information about the domain name. Moreover, the service provider's ML model used for DGA detection is never revealed to the network administrator. Furthermore, by using DP, we also ensure that the classification result cannot be used to learn information about individual entries of the training data. Finally, we leverage post-training float16 quantization of deep learning models in MPC to achieve efficient, secure DGA detection. We demonstrate that by using quantization achieves a significant speed-up, resulting in a 23% to 42% reduction in inference runtime without reducing accuracy using a three party secure computation protocol tolerating one corruption. Previous solutions are not end-to-end private, do not provide differential privacy guarantees for the model's outputs, and assume that model embeddings are publicly known. Our best protocol in terms of accuracy runs in about 0.22s.

RESUMEN

INTRODUCTION: Combining mechanical vibration stimulation with resistance training to improve neuromuscular performance is a method that has gained popularity in the field of sports sciences. PURPOSE: To evaluate the immediate and mid-term effects of mechanical vibration stimulation (G-Life NL3000) of the larynx associated to vocal exercises. METHOD: Fourteen professional voice users divided into: Experimental Group - Seven participants who performed a 20-minute exercise protocol with humming applying mechanical vibration stimulation (G-Life NL3000) of the larynx once a day for 30 days; Control Group - Seven participants who performed only the 20-minute exercise protocol with humming. For comparison purposes, the evaluation was carried out pre-stimulation, immediately after simulation (post) and 30 days after stimulation (late-post). RESULTS: The Experimental Group showed a statistically significant progress in the post-immediate evaluation and in the post-treatment evaluation for maximum phonation time (*P = < 0.001), Jitter (*P = 0.008), Shimmer (*P = 0.005), noise-to-harmonics ratio (*P = 0.005). In the GRBASI scale, the Experimental Group showed significant results in the post-treatment evaluation for Grade (*P = 0.005), Roughness (*P = 0.008), Breathiness (*P = 0.018) and Instability (*P = 0.018). A self-perception voice questionnaire showed better voice quality (*P= 0.031) and easier phonation (*P = 0.005) in the Experimental Group. CONCLUSION: Mechanical vibration stimulation of the larynx associated to humming produced positive changes in the vocal pattern and significantly improvement in short and mid-term vocal quality for participants in this study.

RESUMEN

BACKGROUND: In biomedical applications, valuable data is often split between owners who cannot openly share the data because of privacy regulations and concerns. Training machine learning models on the joint data without violating privacy is a major technology challenge that can be addressed by combining techniques from machine learning and cryptography. When collaboratively training machine learning models with the cryptographic technique named secure multi-party computation, the price paid for keeping the data of the owners private is an increase in computational cost and runtime. A careful choice of machine learning techniques, algorithmic and implementation optimizations are a necessity to enable practical secure machine learning over distributed data sets. Such optimizations can be tailored to the kind of data and Machine Learning problem at hand. METHODS: Our setup involves secure two-party computation protocols, along with a trusted initializer that distributes correlated randomness to the two computing parties. We use a gradient descent based algorithm for training a logistic regression like model with a clipped ReLu activation function, and we break down the algorithm into corresponding cryptographic protocols. Our main contributions are a new protocol for computing the activation function that requires neither secure comparison protocols nor Yao's garbled circuits, and a series of cryptographic engineering optimizations to improve the performance. RESULTS: For our largest gene expression data set, we train a model that requires over 7 billion secure multiplications; the training completes in about 26.90 s in a local area network. The implementation in this work is a further optimized version of the implementation with which we won first place in Track 4 of the iDASH 2019 secure genome analysis competition. CONCLUSIONS: In this paper, we present a secure logistic regression training protocol and its implementation, with a new subprotocol to securely compute the activation function. To the best of our knowledge, we present the fastest existing secure multi-party computation implementation for training logistic regression models on high dimensional genome data distributed across a local area network.

RESUMEN

Objective To determine the anatomical parameters of the atlas (C1) in a sample of the Brazilian population and compare them with the results already presented in the literature for other populations and, thus, to identify and change the indications of implants used in the treatment of the upper cervical spine. Methods We performed a retrospective observational study of a prospective database, including 100 patients seen between January 2012 and December 2013. We used axial and sagittal sections of computed tomography (CT) scans. The parameters studied were; axial: distance from posterior tubercle to the screw entry point (DPT_EP), safety angle of the axial screw (SA_AS), ideal axial screw length (_ASL) and distance of the spinal canal from the transverse foramen (DSC_TF); Sagittal: ideal sagittal screw length (_SSL), safety angle of the sagittal screw (SA_SS) and posterior arch thickness (_PAT). All of the parameters were divided according to age, gender, and left and right sides. Results The minimum mean point of entry ranged from 20 mm to 22.7 mm. The average security zone ranged from 18.09° to 23.68°. The mean posterior arch thickness ranged from 7.21 mm to 8.95 mm. The lowest averages were found in females. Using an original and reproducible technique of CT evaluation. Conclusion The anatomical parameters of the atlas found in the sample of the studied population were similar to those previously presented in the literature. However, differences between the sexes were observed.

RESUMEN

BACKGROUND: For the past few months, HMOs have faced crowded emergency rooms and insufficient hospital and intensive-care-unit beds, all from the worst pandemic of this century, COVID-19. METHODS: In a large HMO in Brazil, our approach was to allow treating physicians to prescribe antiviral medications immediately at presentation, and prednisone starting on day-6 of symptoms to treat pulmonary inflammation. We implemented this COVID-19 protocol for outpatients and studied 717 consecutive SARS-CoV-2-positive patients age 40 years or older presenting at our emergency rooms. RESULTS: Use of hydroxychloroquine (HCQ), prednisone or both significantly reduced hospitalization risk by 50-60%. Ivermectin, azithromycin and oseltamivir did not substantially reduce risk further. Hospitalization risk was doubled for people with type-2 diabetes or obesity, increased by two-thirds for people with heart disease, and by 75% for each decade of age over age 40. Similar magnitudes of reduced risk with HCQ and prednisone use were seen for mortality risk, though were not significant because of only 11 deaths among the 717 patients. No cardiac arrhythmias requiring medication termination were observed for any of the medications. CONCLUSIONS: This work adds to the growing literature of studies that have found substantial benefit for use of HCQ combined with other agents in the early outpatient treatment of COVID-19, and adds the possibility of steroid use to enhance treatment efficacy.

RESUMEN

Machine learning (ML) is revolutionizing research and industry. Many ML applications rely on the use of large amounts of personal data for training and inference. Among the most intimate exploited data sources is electroencephalogram (EEG) data, a kind of data that is so rich with information that application developers can easily gain knowledge beyond the professed scope from unprotected EEG signals, including passwords, ATM PINs, and other intimate data. The challenge we address is how to engage in meaningful ML with EEG data while protecting the privacy of users. Hence, we propose cryptographic protocols based on secure multiparty computation (SMC) to perform linear regression over EEG signals from many users in a fully privacy-preserving (PP) fashion, i.e., such that each individual's EEG signals are not revealed to anyone else. To illustrate the potential of our secure framework, we show how it allows estimating the drowsiness of drivers from their EEG signals as would be possible in the unencrypted case, and at a very reasonable computational cost. Our solution is the first application of commodity-based SMC to EEG data, as well as the largest documented experiment of secret sharing-based SMC in general, namely, with 15 players involved in all the computations.

We used the Single-Item Narcissism Scale, the Multidimensional Interpersonal Reactivity Scale, the Pro-Sociability Scale, the Aggression Questionnaire, and a sociodemographic questionnaire. The data were processed using ibmspssand amos through descriptive and inferential analyses (correlations and mediation). The main results pointed to negative direct effects of narcissism on empathy and pro-social behavior and to direct positive effects on ag-gression. Furthermore, it was confirmed that empathy has a mediating role in the relation between narcissism and pro-sociability, which was not found for the relation between narcissism and aggression. These findings corro-borate previous studies and contribute to the comprehension of the interrelationships of the studied constructs.

RESUMEN

The goal of this study was to conduct a detailed computed tomography (CT) assessment in the Brazilian population of the screw starting point, trajectory, and dimensions of pedicle in the cervical spine. Two hundred consecutive patients were retrospectively evaluated using cervical spine CT, with imaging reconstruction of each cervical vertebrae in the axial plane with 2 mm, and in sagittal reconstructions with 3 mm. Parameters in axial plane included the pedicle width (PW), pedicle axis length (PAL), pedicle transverse angle (PTA), and the distance from the entry point to the point between the lamina and spinous process (DEP). Measurements in the sagittal plane involved the pedicle height (PH) and the pedicle sagittal angle (PSA).The mean PW and PH were smaller in females than in males in all cervical vertebrae, but there were no significant differences of PTA among genders. PSA ranged from 15.2° to 23.7°. Mean values of PAL and DEP had a tendency to decrease from the proximal to distal cervical vertebrae. PW was <4 mm in 7.5% of men (C3) and 25% of women (C3), and <4.5 mm in 20% (C3 male) and 66% (C3 female). The intra- and inter-observer reliability were very good for the tomographic measurement of PW, and good for PH. For PAL, the intraobserver reliability was good, but the interobserver reliability varied from moderate to good. Considering PTA and PSA, the intraobserver reliability was good, but the interobserver reliability moderate for PTA and poor or fair for PSA. DEP measurements showed poor intraobserver reliability, and poor or moderate interobserver reliability. Our results presented similar trend of previous studies, but the frequency of patients with PW <4.5 mm in our population is higher, suggesting an increased risk during the attempting of transpedicular screw technique.

RESUMEN

The expansion of agriculture in the southeast Amazon-Savanah transitional area has greatly decreased forest cover and influenced stream structure and functioning. We assessed the effects of forest cover loss on the integrity of streams by assessing stream physical conditions and the taxonomic and functional diversity of fish assemblages in this transitional area. We hypothesize that low forest cover, especially adjacent to streams, results in poor physical conditions (e.g., warmer temperatures, less physical structure, etc.), which in turn will decrease the taxonomic and functional diversity of fish assemblages. We detected that loss of forest cover negatively affects natural stream conditions and reduces the functional diversity of fish assemblages, but we did not find a strong effect on taxonomic diversity. Ambush and stalking predators, diurnal surface pickers (groups that exhibit opportunistic life history strategies), grazers, pickers, and browsers (groups that exhibit equilibrium life history strategies) were the functional groups with the strongest relation to altered environments. These groups can explore different niches, both with natural characteristics or altered by human activities. Our results suggest that the preservation of riparian zones can minimize the loss of specialized fish species in assemblages of Amazon-Savannah stream systems.(AU)

OBJECTIVE: To identify the impacts generated by the pandemic on the mental health of professional nurses. METHOD: Systematic review with meta-synthesis, where a search was performed in the Google academic databases and the Virtual Health Library (VHL/SCIELO). Descriptors in Health Sciences Burnout, professional exhaustion, pandemic and COVID-19 were used, making use of the Boolean operator AND. RESULTS: After conducting the search in the databases mentioned with the listed descriptors, 29 studies were found. A posteriori, the inclusion and exclusion criteria were applied, resulting in a final sample of 10 studies. CONCLUSION: Exhaustive workloads, poor working conditions, deficiency in staffing, few material resources, devaluation and low remuneration. These factors, associated with the fear of transmitting the disease to family and friends, social isolation and others increase the risk of developing psychological problems causing deleterious effects on mental health.(AU)

RESUMEN

Introduction: Syphilis is a very old disease involved by many mysteries. From its origins to the reason why even having simple and effective diagnosis and treatment for more than half a century, it remains, in 2021, as a public health problem practically all over the world. Be it in high-developing countries as well as in developing ones. Objective: To relate and describe, with dating, by day, month and year, in a calendar model, events and important characters that involve studies and quotes about syphilis. Methods: We used the prior personal knowledge of health professionals and researchers, who have worked in the field of sexually transmitted diseases for decades, combined with the search on the Internet, often on Wikipedia and Google, about events and characters involving subjects and publications about syphilis. Results: After several analyses and researches, we listed 68 facts and characters from around the world, including several Brazilians. Conclusion: At a time (2020 and 2021) where the subject, worldwide, on a daily basis, is SARS-CoV-2/COVID-19, this unprecedented publication on facts and characters involving syphilis can be a milestone so that, in addition to technical and scientific knowledge, the academic community realizes the anthropological, sociological, and cultural wealth that involves an entity that is much more than an infectious disease, such as syphilis.
