BACKGROUND: One of the most used resources for the treatment of psoriasis is ultraviolet radiation (UV) with psoralens (PUVA) and narrow-band UV (UVB-NB 311-312 nm). Although many researchers have assessed the histopathological effects of this therapy modality, none used a morphological classification system specific to psoriasis. PURPOSE: To assess the clinical and histopathological response in the phototherapy treatment of plaque psoriasis with PUVA and UVB-NB with use of PASI and TROZAK. METHODS: Histopathological changes of plaque psoriasis were quantified with help of the grading system for psoriasis-Trozak in 20 patients of both sexes, before and after 32 PUVA phototherapy sessions (10 patients-GPUVA) and UVB-NB (10 patients-GUVB-NB). The severity and extension of psoriasis was evaluated through PASI. The slides stained with hematoxylin and eosin were scanned in an Aperio CS2 scanner (Leica Biosystems) and evaluated through the software ImageScopeTM (Aperio Technologies). Statistical analysis was performed with the use of the program SPSS 22.0, with application of the Wilcoxon and Mann-Whitney tests. RESULTS: All patients presented improvement in psoriatic plaques with decrease in PASI after treatment (P < 0.01) and significant reduction in histopathological changes in psoriasis from 15.4 ± 1.7 to 3.7 ± 3.2 (P < 0.01) in group GPUVA and from 13.2 ± 1.7 to 4.9 ± 5.2 (P < 0,01) in group GUVB-NB. CONCLUSIONS: Phototherapy, regardless of type, is an effective treatment for moderate and severe psoriasis, with possibility of being quantified clinically by PASI and histopathologically by Trozak.

In Type II lepromatous reaction, there is exacerbation of humoral immunity, classified as Gell & Coombs Type III hypersensitivity reaction. It is more common in lepromatous borderline (LB) and lepromatous lepromatous (LL) patients. Our objective was to study the clinical and laboratorial expressions of lepromatous Type II reactions, establishing concordances between them, and for this the medical records of leprosy patients observed at the Clementino Fraga Filho University Hospital of the Federal University of Rio de Janeiro (HUCFF/UFRJ) were reviewed. There were a total of 358 leprosy cases over a period of 12 years. Demographic, clinical, and laboratory data of 133 patients with Type II reaction were collected. Among the 133 patients, 19 were classified as borderline borderline (BB), 15 (11.3%) as LB, and 97 (72.9%) as LL. Mitsuda intradermal reaction was negative in all the 49 patients who underwent this test. Histopathologic study confirmed the diagnosis. Lepromatous patients (LP) presented positive bacilloscopy more frequently (73.91% of 68 patients) than borderline patients (BP) (26.9% of 24 patients). Among BP, 44% presented erythema nodosum leprosum (ENL), which was seen in 71% of LP. Erythema multiforme (EM) occurred in 32% of BP and 13% of LP. Lucio phenomenon (LPh) was observed in 8 of 34 BP (23.6%), and 15 of 97 LP (15.4%). The understanding of the laboratorial and clinical presentations of reactional episodes are relevant to the development of preventive and therapeutic strategies, in order to avoid potential complications and comorbidities that cause disability, paralysis, deformities, and stigma of leprosy.

BACKGROUND: Among the chronic leg ulcers, venous ulcers are the most common and constitute a major burden to public health. Despite all technology available, some patients do not respond to established treatments. In our study, carboxymethylcellulose was tested in the treatment of refractory chronic venous ulcers. OBJECTIVE: To evaluate the efficacy of carboxymethylcellulose 20% on the healing of chronic venous ulcers refractory to conventional treatments. METHODS: This is an analytical, pre-experimental study. Thirty patients were included with refractory venous ulcers, and applied dressings with carboxymethylcellulose 20% for 20 weeks. The analysis was based on measurement of the area of ulcers, performed at the first visit and after the end of the treatment. RESULTS: There was a reduction of 3.9 cm2 of lesion area (p=0.0001), corresponding to 38.8% (p=0.0001). There was no interruption of treatment and no increase in lesion area in any patient. CONCLUSIONS: Carboxymethylcellulose 20% represents a low cost and effective therapeutic alternative for the treatment of refractory chronic venous ulcers. However, controlled studies are necessary to prove its efficacy.

Défices sensoriais persistentes após a reconstrução do ligamento cruzado anterior (LCA) podem causar alterações como fraqueza muscular e comprometimento do equilíbrio e do desempenho funcional. Diversos estudos discutem se essas alterações também estão presentes no membro inferior não acometido e se esse membro pode ser utilizado como referência nas avaliações desses pacientes. O objetivo do presente estudo foi comparar o torque extensor e flexor do joelho, o equilíbrio unipodal e o desempenho funcional sobre o membro não-acometido de pacientes submetidos à reconstrução do LCA, com membros inferiores de sujeitos saudáveis. Participaram do estudo 23 indivíduos submetidos à reconstrução do LCA e 22 sujeitos saudáveis, que foram avaliados quanto ao torque extensor e flexor do joelho durante contração isocinética concêntrica a 60 graus/s no dinamômetro isocinético, à velocidade média de oscilação do centro de pressão em apoio unipodal no baropodômetro, e submetidos a dois testes de salto para avaliar o desempenho funcional. Os resultados não revelaram diferenças entre o membro não-acometido e os membros inferiores do grupo controle em todas as variáveis estudadas (p>0,05). Esses resultados sugerem que o membro não-acometido de pacientes submetidos à reconstrução do LCA pode ser utilizado como referência nos estudos de avaliação desses pacientes.


Persistent sensorial deficits after anterior cruciate ligament (ACL) reconstruction might lead to muscle weakness, balance and functional performance disorders. Several studies inquire whether these alterations also appear in the uninjured limb and whether the latter could be used as reference for patient assessment. The aim of the present study was to compare knee extensor and flexor torque, balance and functional performance of non-injured lower limb, after contralateral ACL reconstruction, with lower limbs of healthy subjects. Twenty three male patients who had had ACL reconstruction, and 22 control subjects were assessed as to knee flexor and extensor torque during concentric isokinetic contraction at 60o/s with an isokinetic dynamometer, mean speed of centre of pressure swing during single-limb stance, and submitted to two hop tests to assess functional performance. Results showed no significant differences between uninjured side and healthy subjects lower limbs at all assessed variables (p>0.05). These findings suggest that the uninjured limb may be safely used as reference in studies to assess post-ACL patients.

Introdução: Exercícios de alongamento são comumente usados precedendo a realização de atividades físicas, no entanto, pesquisas recentes têm sugerido uma redução da força muscular após o alongamento. Objetivo: O objetivo deste estudo foi observar o efeito agudo de três técnicas de alongamento muscular sobre o torque flexor do joelho, produzido pelos músculos ísquiotibiais. Método: Sessenta mulheres jovens, saudáveis, (idade média de 22,S ± 3 anos), com encurtamento clínico dos ísquiostibiais, foram alocadas aleatoriamente em 4 grupos: controle, alongamento estático, manter-relaxar, e agonista-manter-relaxar. Todas foram submetidas a um protocolo de avaliação no qual realizaram 5 contrações concêntricas máximas de flexão/extensão do joelho a 600/s antes e após a aplicação do respectivo alongamento, o qual teve duração média de um minuto. O grupo controle permaneceu 5 minutos em repouso. Os dados foram analisados estatisticamente e o nível de significância foi atribuído em 5%. Resultados: Não foi identificada diferença entre os grupos antes nem depois da intervenção. Também não houve diferença intragrupo após a intervenção. Conclusão: Os resultados sugerem que o alongamento da musculatura posterior da coxa, com duração média de um minuto, não provoca modificações imediatas no torque flexor do joelho.


Introduction: Stretching exercises are commonly used prior to physical activities, however, recent researchs identified reduction of muscle strength post-stretching. Objective: The aim of this study was to observe the acute effect of three techniques of muscle stretching on torque produced by hamstrings on the knee. Method: Sixty women, healthy, mean age 22.55 ± 3.04 years, with shortening hamstrings, were randomly distributed into 4 groups (control, static stretching, hold-relax, and agonist-hold-relax). They were submitted to evaluation protocol in which performed 5 maximal concentric contractions of knee flexion/extension at 60°/s before and after the application of stretching technique, which had total duration about 48-60s, depending on the group. The control group remained at rest 5 minutes. The data were statistically analyzed and the levei of significance set at 5%. Results: No difference was detected between groups before or after the intervention. There was no intragroup difference after the intervention in any of the groups. Conclusion: The results suggest that the muscle stretching, lasting up to 60 seconds does not cause acute changes in the knee flexion torque.
