We investigated the influence of biomass burning (BURN), Diwali fireworks, and fog events on the ambient fine particulate matter (PM2.5) oxidative potential (OP) during the postmonsoon (PMON) and winter season in Delhi, India. The real-time hourly averaged OP (based on a dithiothreitol assay) and PM2.5 chemical composition were measured intermittently from October 2019 to January 2020. The peak extrinsic OP (OPv: normalized by the volume of air) was observed during the winter fog (WFOG) (5.23 ± 4.6 nmol·min-1·m-3), whereas the intrinsic OP (OPm; normalized by the PM2.5 mass) was the highest during the Diwali firework-influenced period (29.4 ± 18.48 pmol·min-1·µg-1). Source apportionment analysis using positive matrix factorization revealed that traffic + resuspended dust-related emissions (39%) and secondary sulfate + oxidized organic aerosols (38%) were driving the OPv during the PMON period, whereas BURN aerosols dominated (37%) the OPv during the WFOG period. Firework-related emissions became a significant contributor (∼32%) to the OPv during the Diwali period (4 day period from October 26 to 29), and its contribution peaked (72%) on the night of Diwali. Discerning the influence of seasonal and episodic sources on health-relevant properties of PM2.5, such as OP, could help better understand the causal relationships between PM2.5 and health effects in India.

Polycyclic Aromatic Hydrocarbons (PAHs) and Nitro-PAHs were collected over a year at a traffic dominated site in Agra, to determine the dominant partitioning mechanism. During the entire sampling period, total PAHs and Nitro-PAHs were 3465 ± 3802 and 26.1 ± 25.9 ng m-3 respectively. The gas-particle partitioning behavior of PAHs was studied by applying the Pankow model, Absorption model, and Dual model. Amongst all the partitioning models, the Dual model fits well and indicates that the partitioning of PAHs at the traffic site in Agra depends on both the physical adsorption of PAHs on the Total Suspended Particulate (TSP) surface and absorption of PAHs into the organic layer present on the TSP surface. Pankow model indicates that PAHs are emitted from the source close to the sampling point and due to this PAHs do not get enough time to get partitioned in between both the phases. Incremental lifetime Cancer Risk (ILCR) shows that adults and children are more prone to cancer risk in comparison to infants for both PAHs and Nitro-PAHs. Cancer risk by inhalation was minimum in comparison to both ingestion and dermal exposure. Nitro-PAHs in the particulate phase were high enough to exceed the minimum permissible limit (10-6) of causing cancer by ingestion and dermal exposure.

Many toxicological studies revealed the deleterious effects on human health induced by trace metals in ambient particulate matter (PM). This study reports the season-dependent water-soluble and total metal mass in PM10 collected simultaneously over five microenvironments in a semi-arid urban region, Ahmedabad, located in western India. The mineral dust fraction in PM10 over Bapunagar, Narol, Paldi, Income Tax, and Science City was estimated to be around 39, 45, 47, 44, and 31% during summer (May-June 2017) and 24, 55, 28, 27, and 28% during winter (December 2017-January 2018), respectively, corroborating mineral dust is perennial in the air over Ahmedabad. The PM2.5/PM10 mass ratios over all the sites were higher during winter (40-60%) as compared to those during summer (30-40%), indicating the contribution from the anthropogenic sources to PM mass. Among the metals monitored, the estimated considerable amount of high masses of Zn, Cu, Ni, Cd, and Sb during winter can be ascribed to the anthropogenic inputs based on the estimated enrichment factors (EF). In contrast to the crustal source, these metals might have been possibly emitted from several other man-made sources, which were found to be more water-soluble during both seasons. As per the standards of incremental excess lifetime cancer risk (IELCR), it is estimated that the atmospheric mass concentration of carcinogenic metals such as Cr, Co, and As was higher in all these sites, whereas the metals such as Pb, Ni, and Cd are also found over the industrial site (Narol) in addition to the above-said metals. Notably, people are highly susceptible to these metals, leading to the potential risk of cancer during both seasons.

Recent global models estimate that light absorption by brown carbon (BrC) in several regions of the world is ∼30-70% of that due to black carbon (BC). It is, therefore, important to understand its sources and characteristics on temporal and spatial scales. In this study, we conducted semicontinuous measurements of water-soluble organic carbon (WSOC) and BrC using particle-into-liquid sampler coupled with a liquid waveguide capillary cell and total organic carbon analyzer (PILS-LWCC-TOC) over Kanpur (26.5°N, 80.3°E, 142 m amsl) during a winter season (December 2015 to February 2016). In addition, mass concentrations of organic and inorganic aerosol and BC were also measured. Diurnal variability in the absorption coefficient of BrC at 365 nm (babs_365) showed higher values (35 ± 21 Mm-1) during late evening to early morning hours and was attributed to primary emissions from biomass burning (BB) and fossil fuel burning (FFB). The babs_365 reduced by more than 80% as the day progressed, which was ascribed to photo bleaching/volatilization of BrC and/or due to rising boundary layer height. Further, diurnal variability in the ratios of babs_405/babs_365 and babs_420/babs_365 suggests that the BrC composition was not uniform throughout a day. WSOC exhibited a strong correlation with babs_365 (slope = 1.22 ± 0.007, r2 = 0.70, n = 13 265, intercept = -0.69 ± 0.17), suggesting the presence of a significant but variable fraction of chromophores. Mass absorption efficiency (MAE) values of WSOC ranged from 0.003 to 5.26 m2 g-1 (1.16 ± 0.60) during the study period. Moderate correlation (r2 = 0.50, slope = 1.58 ± 0.019, n = 6471) of babs_365 was observed with the semivolatile oxygenated organic aerosols (SV-OOA) fraction of BB resolved from positive matrix factorization (PMF) analysis of organic mass spectral data obtained from a high-resolution time-of-flight aerosol mass spectrometer (HR-ToF-AMS). The low-volatility OOA (LV-OOA) fraction of BB had a similar correlation to babs_365 (r2 = 0.54, slope = 0.38 ± 0.004, n = 6471) but appears to have a smaller contribution to the absorption.

Genes important to stem cell progression have been involved in the genetics and clinical outcome of cancers. We investigated germ line variants in cancer stem cell (CSC) genes to predict susceptibility and efficacy of chemoradiotherapy treatment in gallbladder cancer (GBC) patients. In this study, we assessed the effect of SNPs in CSC genes (surface markers CD44, ALCAM, EpCAM, CD133) and (molecular markers NANOG, SOX-2, LIN-28A, ALDH1A1, OCT-4) with GBC susceptibility and prognosis. Total 610 GBC patients and 250 controls were genotyped by using PCR-RFLP, ARMS-PCR, and TaqMan allelic discrimination assays. Chemotoxicity graded 2-4 in 200 patients and tumor response was recorded in 140 patients undergoing neoadjuvant chemotherapy (NACT). Differences in genotype and haplotype frequency distributions were calculated by binary logistic regression. Gene-gene interaction model was analyzed by generalized multifactor dimensionality reduction (GMDR). Overall survival was assessed by Kaplan-Meier survival curve and multivariate Cox-proportional methods. ALCAM Ars1157Crs10511244 (P = 0.0035) haplotype was significantly associated with GBC susceptibility. In GMDR analysis, ALCAM rs1157G>A, EpCAM rs1126497T>C emerged as best significant interaction model with GBC susceptibility and ALDH1A1 rs13959T>G with increased risk of grade 3-4 hematological toxicity. SOX-2 rs11915160A>C, OCT-4 rs3130932T>G, and NANOG rs11055786T>C were found best gene-gene interaction model for predicting response to NACT. In both Cox-proportional and recursive partitioning ALCAM rs1157GA+AA genotype showed higher mortality and hazard ratio. ALCAM gene polymorphisms associated with GBC susceptibility and survival while OCT-4, SOX-2, and NANOG variants showed an interactive role with treatment response.

Gallbladder cancer (GBC) is the most common malignancy of the biliary tract with adverse prognosis and poor survival. Wnt signaling plays an important role in embryonic development and regeneration of tissues in all the species. Deregulation of expression and mutations in this pathway may lead to disease state such as cancer. In this study, we assessed the association of common germline variants of Wnt pathway genes (SFRP2, SFRP4, DKK2, DKK3, WISP3, APC, ß-catenin, AXIN-2, GLI-1) to evaluate their contribution in predisposition to GBC and treatment outcomes. The study included 564 GBC patients and 250 controls. Out of 564, 200 patients were followed up for treatment response and survival. Tumor response (RECIST 1.1) was recorded in 116 patients undergoing non-adjuvant chemotherapy (NACT). Survival was assessed by Kaplan-Meier curve and Cox-proportional hazard regression. Single locus analysis showed significant association of SFRP4 rs1802073G > T [p value = 0.0001], DKK2 rs17037102C > T [p value = 0.0001], DKK3 rs3206824C > T [p value = 0.012], APC rs4595552 A/T [p value = 0.021], APC rs11954856G > T [p value = 0.047], AXIN-2 rs4791171C > T [p value = 0.001], ß-catenin rs4135385A > G [p value = 0.031], and GLI-1 rs222826C > G [p value = 0.001] with increased risk of GBC. Gene-gene interaction using GMDR analysis predicted APC rs11954856 and AXIN2 rs4791171 as significant in conferring GBC susceptibility. Cox-proportional hazard model showed GLI-1 rs2228226 CG/GG and AXIN-2 rs4791171 TT genotype higher hazard ratio. In recursive partitioning, AXIN-2 rs4791171 TT genotype showed higher mortality and hazard. Most of studied genetic variants influence GBC susceptibility. APC rs11954856, GLI-1 rs2228226, and AXIN-2 rs4791171 were found to be associated with poor survival in advanced GBC patients.

Organic aerosols (OA) are a significant component of fine particulate matter in the ambient air and are formed through primary and secondary processes. Primary organic aerosols (POA) are directly released from sources, while secondary organic aerosols (SOA) are formed through the oligomerization and/or oxidation of volatile organic compounds (VOCs) in the atmosphere. Recently, there has an increasing attention on the SOA budgets, their formation pathways, and photochemical evolution due to their impacts on climate and human health. Biomass burning (BB) is a significant source of OA, contributing around 5-30 % to the SOA burden globally. Agricultural residue burning (ARB) is a type of BB that contributes ∼10 % of total atmospheric OA mass worldwide, whereas it contributes higher in Asian regions like China and India. ARB emits a significant amount of air pollutants, including VOCs, into the atmosphere. However, there is inadequate information on the transformation of ARB emissions to SOA due to limited laboratory studies. The present review focuses on the formation mechanism of SOA from ARB emissions, summarizing the current state of the art about ARB precursors and their oxidation products from chamber-based studies, including measurement methods and analytical instrumentation. The review also discusses the role of different types of oxidants in OA mass enhancement, factors affecting the overall SOA yield, and the uncertainties involved in the process.

Ammonia (NH3) is the major constituent among all the reactive nitrogen species present in the atmosphere, and the most essential species for secondary inorganic aerosol formation. Recent satellite-based observations have identified the Indo-Gangetic Plain (IGP) as a major hotspot of global NH3 emission; however, the major sources and atmospheric processes affecting its abundance are poorly understood. The present study aims to understand the wintertime sources of NH3 over a semi-urban site (Patiala, 30.3°N, 76.4°E, 249 m amsl) located in the IGP using species specific δ15N in PM2.5. A distinct diurnal variation in the stable isotopic signature of total nitrogen (δ15N-TN) and ammonium (δ15N-NH4+) were observed; although, average day and night time concentrations of TN and NH4+ were similar. Mixing model results using δ15N-NH3 reveal the dominance of non-agricultural emissions (NH3 slip: 47 ± 24%) over agricultural emissions (24 ± 11%), combustion sources (19 ± 14 %), and biomass burning (10 ± 8%) for atmospheric NH3. Diurnal variability in source contributions to NH3 was insignificant. Further, significantly negative correlations of δ15N-NH4+ with ambient relative humidity (RH) and daytime NO3--N concentration were observed, and attributed to the possibility of NH4NO3 volatilization during day-time owing to lower RH and higher temperature, resulting in isotopic enrichment of the remaining NH4+ in aerosol phase. This study, a first of its type from India, highlights the importance of non-agricultural NH3 emissions over the agriculture dominated IGP region, and the role of local meteorology on the isotopic fractionation of δ15N in aerosol NH4+.

Delhi, among the world's most polluted megacities, is a hotspot of particulate matter emissions, with high contribution from organic aerosol (OA), affecting health and climate in the entire northern India. While the primary organic aerosol (POA) sources can be effectively identified, an incomplete source apportionment of secondary organic aerosol (SOA) causes significant ambiguity in the management of air quality and the assessment of climate change. Present study uses positive matrix factorization analysis on the water-soluble organic aerosol (WSOA) data from the offline-aerosol mass spectrometry (AMS). It revealed POA as the dominant source of WSOA, with biomass-burning OA (31-34 %) and solid fuel combustion OA (∼21 %) being two major contributors. Here we use water-solubility fingerprints to track the SOA precursors, such as oxalates or organic nitrates, instead of identifying them based on their O:C ratio. Non-fossil precursors dominate in more oxidized oxygenated organic carbon (MO-OOC) (∼90 %), a proxy for aged secondary organic carbon (SOC), by coupling offline-AMS with 14C measurements. On the contrary, the oxidation of fossil fuel emissions produces a large quantity of fresh fossil SOC, which accounts for ∼75 % of less oxidized oxygenated organic carbon (LO-OOC). Our study reveals that apart from major POA contributions, large fractions of fossil (10-14 %) and biomass-derived SOA (23-30 %) contribute significantly to the total WSOA load, having impact on climate and air quality of the Delhi megacity. Our study reveals that large-scale unregulated biomass burning was not only found to dominate in POA but was also observed to be a significant contributor to SOA with implications on human health, highlighting the need for effective control strategies.

Salivary gland tumors are relatively rare and can exhibit various clinical behaviors. The study aims to understand the natural history, pathology, diagnostic workup, and treatment strategies for these tumors to improve patient outcomes. The audit included patients with salivary gland tumors detected through radiology or cytology. Patients underwent surgery, with some receiving adjuvant treatment. Demographic information, treatment interventions, and survival outcomes were analyzed using SPSS software. A total 89 as malignant salivart gland tumours were audited Malignant tumors were predominantly found in the parotid gland, with fewer cases in the minor salivary gland and submandibular gland.The median age of presentation was 47 years, and the majority of patients were male. The study examined various pathological and clinical factors, including tumor stage, nodal status, and the presence of facial palsy. Surgical procedures and histological types of tumors were documented. Adverse histological features like positive margins, lymph node positivity, lympho-vascular invasion, extracapsular spread, and perineural invasion were noted. POSTOP RT was administered to high-risk patients. Most malignant salivary gland tumors were found in the parotid gland, while minor salivary gland tumors were underrepresented in the audit. Surgical practices were diverse. Radiotherapy protocols were relatively standardized. The study found that certain histological features, such as lymph node positivity, margin positivity, lympho-vascular invasion, perineural invasion, and extracapsular spread, were associated with adverse effects on DFS and OS. The findings suggest that specific histological features, including LVI and ECE have emerged as independent prognostic factors for DFS and OS.

Most fine ambient particulate matter (PM2.5)-based epidemiological models use globalized concentration-response (CR) functions assuming that the toxicity of PM2.5 is solely mass-dependent without considering its chemical composition. Although oxidative potential (OP) has emerged as an alternate metric of PM2.5 toxicity, the association between PM2.5 mass and OP on a large spatial extent has not been investigated. In this study, we evaluate this relationship using 385 PM2.5 samples collected from 14 different sites across 4 different continents and using 5 different OP (and cytotoxicity) endpoints. Our results show that the relationship between PM2.5 mass vs. OP (and cytotoxicity) is largely non-linear due to significant differences in the intrinsic toxicity, resulting from a spatially heterogeneous chemical composition of PM2.5. These results emphasize the need to develop localized CR functions incorporating other measures of PM2.5 properties (e.g., OP) to better predict the PM2.5-attributed health burdens.

The oxidative potential (OP) of particulate matter (PM) is a major driver of PM-associated health effects. In India, the emission sources defining PM-OP, and their local/regional nature, are yet to be established. Here, to address this gap we determine the geographical origin, sources of PM, and its OP at five Indo-Gangetic Plain sites inside and outside Delhi. Our findings reveal that although uniformly high PM concentrations are recorded across the entire region, local emission sources and formation processes dominate PM pollution. Specifically, ammonium chloride, and organic aerosols (OA) from traffic exhaust, residential heating, and oxidation of unsaturated vapors from fossil fuels are the dominant PM sources inside Delhi. Ammonium sulfate and nitrate, and secondary OA from biomass burning vapors, are produced outside Delhi. Nevertheless, PM-OP is overwhelmingly driven by OA from incomplete combustion of biomass and fossil fuels, including traffic. These findings suggest that addressing local inefficient combustion processes can effectively mitigate PM health exposure in northern India.

The first protocol for the synthesis of perspicamide A and related diverse analogues has been developed from economical and readily available starting materials. Furthermore, a few synthesized analogues, 24a, 24b, 24c, 24d, and 24l, exhibited potent activity against Leishmania donovani with IC(50) values ranging from 3.75 to 10.37 µM and a selectivity index (SI) ranging from 9.58 to 53.12, which is improved compared to the standard drug Miltefosine (IC(50) 12.4 µM and SI 4.1).

A 69-year-old man with history of end-stage-renal disease (ESRD) underwent successful kidney transplantation from a cadaveric donor in November 2011. However, posttransplant recovery was complicated by delayed graft function and recurrent gross hematuria. Serial Doppler ultrasound (US) of the renal allograft demonstrated a pseudoaneurysm with interval increase in size. However, it could not be visualized with other modalities, including an initial angiogram (postoperative day 49) and a second angiogram (postoperative day 68), followed by surgical exploration (postoperative day 71), which demonstrated complete intra-aneurysmal thrombosis on intraoperative Doppler US. Unfortunately, the patient's hematuria continued and a repeat Doppler US 48 hours later demonstrated a persistent pseudoaneurysm. Therefore, on postoperative day 75, we performed targeted percutaneous intra-aneurysmal thrombin injection under dual image guidance, which showed complete intra-aneurysmal thrombosis on intraprocedural Doppler US. Hematuria recurred the next day. A third angiogram (postoperative day 77) finally illuminated the hidden pseudoaneurysm occult on the first and second angiographic studies (sensitivity [index case] 33%) and surgery. This allowed for successful coil embolization of a subsegmental feeding branch with an excellent outcome. We support a more aggressive management with serial angiography and embolization of the intrarenal symptomatic pseudoaneurysm rather than surgery in renal allograft recipients, with the benefits outweighing the risks.

Indo-Gangetic Plain (IGP) experiences a heavy load of particulate pollution impacting the 9 % of the global population living in this region. The present study examines the dithiothreitol (DTT) assay-based oxidative potential (OP) of PM2.5 and the major sources responsible for the observed OP over the central IGP (Kanpur) during winter. The volume normalized OP (OPV) of PM2.5 varied from 2.7 to 10 nmol DTT min-1 m-3 (5.5 ± 1.5) and mass normalized OP (OPM) of PM2.5 varied from 19 to 58 pmol DTT min-1 µg-1 (34 ± 8.0), respectively. Major sources of PM2.5 were identified using the positive matrix factorization (PMF) and the contribution of these sources to observed OP was estimated through multivariate linear regression of OPv with PMF-resolved factors. Although the PM2.5 mass was dominated by secondary aerosols (SA, 28 %), followed by crustal dust (CD, 24 %), resuspended fine dust (RFD, 14 %), traffic emissions (TE, 8 %), industrial emissions (IE, 17 %), and trash burning (TB, 9 %), their proportionate contribution to OP (except SA) was different likely due to differences in redox properties of chemical species coming from these sources. The SA showed the highest contribution (23 %) to observed OP, followed by RFD (19 %), IE (8 %), TE & TB (5 %), CD (4 %), and others (36 %). Our results highlight the significance of determining the chemical composition of particulates along with their mass concentrations for a better understanding of the relationship between PM and health impacts. Such studies are still lacking in the literature, and these results have direct implications for making better mitigation strategies for healthier air quality.

Purpose: Image guided radiotherapy (IGRT) is one of the most commonly used treatment in LAPC. Dose escalation >74 Gy has shown to improve the biochemical control and freedom from failure rate in LAPC.We started treating LAPC patients with dose escalated IGRT in our institute since 2008. We did a retrospective analysis to see the biochemical relapse-free survival, cancer-specific survival, and bladder and rectal toxicity. Methods: A total of 50 consecutive prostate cancer patients were treated with dose escalated IGRT between January 2008 to Dec 2013. Out of these, 37 patients of LAPC were analyzed and their medical records were retrieved. All were biopsy proven adenocarcinoma of prostate with D'Amico high risk category (PSA >20 ng/mL or Gleason score (GS) >7 or T2c-T4). Three gold fiducial markers were placed in the prostate. Patients were immobilized in supine position with either ankle or knee rest. Partial bladder filling and rectum emptying protocol was followed. Clinical target volume (CTV) segmentation was done according to EORTC recommendation. Population based PTV expansion from CTV of 10 mm (cranio-caudal), 10 mm (medio-lateral), 10 mm (anterior) and 5 mm (posterior) was given. In patients with radiologically enlarged pelvic lymph node, whole pelvis intensity modulated radiation therapy (IMRT) to a dose of 50.4 Gy/28# followed by prostatic boost 26Gy/13# by IMRT using image guidance. Rest of the patients received prostate only RT to a dose of 76Gy/38# by IGRT. Daily On board KV images were taken and 2D-2D fiducial marker matching was done and shifts were applied on machine before treatment. Biochemical relapse was defined as per Phoenix definition (nadir + 2 ng/mL). Radiation Therapy Oncology Group (RTOG) toxicity grading system was used to document acute and late toxicity. Results: Median age of patients was 66 years. Median pre-treatment PSA was 22 ng/mL. Thirty patients (81%) had T3/T4 lesions and nodal metastasis was seen in 11 (30%). Median GS was 8. Median radiotherapy dose was 76 Gy. Imaging before radiation delivery was done in 19(51%) patients and 100% in 14 (38%) patients. With a median follow up of 6.5 years, 5-year biochemical relapse-free survival (bRFS) and cancer-specific survival (CSS) was 66% and 79% respectively. Mean bRFS and CSS were 71 months and 83 months however Median bRFS and CSS were not reached. Distant metastasis was seen in 8 (22%). RTOG grade III bladder and rectal toxicity was seen in 2 (6%) and 2 (6%) patients respectively. Conclusion: Dose escalated IGRT with fiducial marker positional verification for LAPC is doable in Indian setup provided more emphasis given on daily on-board imaging with rigorous bladder filling and rectal emptying protocol. Long term follow up is needed to assess the effect on distant disease-free survival and CSS.

This study reports day-night and seasonal variations of aqueous brown carbon (BrCaq) and constituent humic-like substances (HULIS) (neutral and acidic HULIS: HULIS-n and HULIS-a) from the eastern Indo-Gangetic Plain (IGP) of India during 2019-2020. This is followed by the application of the receptor model positive matrix factorization (PMF) for optical source apportionment of BrCaq and the use of stable isotopic ratios (δ13C and δ15N) to understand atmospheric processing. Nighttime BrCaq absorption and mass absorption efficiencies (MAE) were enhanced by 40-150 % and 50-190 %, respectively, compared to the daytime across seasons, possibly as a combined effect from daytime photobleaching, dark-phase secondary formation, and increased nighttime emissions. MAE250 nm/MAE365 nm (i.e., E2/E3) ratios and Angstrom Exponents revealed that BrCaq and HULIS-n were relatively more aromatic and conjugated during the biomass burning-dominated periods while BrCaq and HULIS-a were comprised mostly of non-conjugated aliphatic structures from secondary processes during the photochemistry-dominated summer. The relative radiative forcing of BrCaq with respect to elemental carbon (EC) was 10-12 % in the post-monsoon and winter in the 300-400 nm range. Optical source apportionment using PMF revealed that BrCaq absorption at 300, 365 and 420 nm wavelengths in the eastern IGP is mostly from biomass burning (60-75 %), followed by combined marine and fossil fuel-derived sources (24-31 %), and secondary processes (up to 10 %). Source-specific MAEs at 365 nm were estimated to be the highest for the combined marine and fossil fuel source (1.34 m2 g-1) followed by biomass burning (0.78 m2 g-1) and secondary processing (0.13 m2 g-1). Finally, δ13C and δ15N isotopic analysis confirmed the importance of summertime photochemistry and wintertime NO3--dominated chemistry in constraining BrC characteristics. Overall, the quantitative apportionment of BrCaq sources and processing reported here can be expected to lead to targeted source-specific measurements and a better understanding of BrC climate forcing in the future.

The effects of multivalent metal ions (Cu(2+)/Zn(2+)/Al(3+)) on the aggregation of salmon calcitonin (sCT)--a therapeutic peptide used worldwide in the treatment of osteoporosis and Paget's disease--have been studied in vitro using NMR (both solution state and solid state), TEM, ThT-fluorescence, and FT-IR spectroscopy. Overall, the various results indicated that the metal-ions-induced conformational transitions in the peptide--mostly toward the ß-sheet--facilitate the aggregation of sCT in solution. First, the solution NMR has been used to check the interaction between the peptide and the metal ions. Following this, the formation and characterization of calcitonin aggregates has been performed using TEM, solid state NMR, and FT-IR spectroscopy. The TEM and ThT-fluorescence results revealed that the sCT peptide incubated with Cu(2+) and Zn(2+) metal ions (in aqueous environment) forms globular aggregates, while that with Al(3+) ions forms fibrils. The solid state NMR and FT-IR studies revealed the presence of a substantial amount of ß-sheet content in sCT aggregates (formed in the presence of these metal ions) compared to the monomeric sCT, indicating that the metal binding is concomitant with conformational changes. The present study becomes crucial while prescribing this drug peptide under physio-pathological conditions associated with an abnormal accumulation of metal ions (Cu(2+)/Zn(2+)/Al(3+)) in the body (i.e., abnormal metal ion homeostasis).

BACKGROUND: The aim of this article is to report a rare case of unresolved incapacitating pelvic congestion syndrome (PCS) in a patient with a history of May-Thurner syndrome previously treated with stenting and left ovarian vein embolization. Additionally, this article highlights the role of pelvic venography in patients with PCS and reviews the coexistence. METHODS: A 32-year-old woman was referred to us for the evaluation of recurrent pelvic pain and dyspareunia requiring analgesics. Initially, she developed left lower-extremity deep vein thrombosis a few months after her first pregnancy. On further workup, she was diagnosed with May-Thurner syndrome and underwent left common iliac and left external iliac vein stenting. Furthermore, left ovarian vein coil embolization was performed for symptoms suggesting PCS at the same outside facility. The patient was referred to us for persistent pelvic pain approximately 1 year after she underwent left ovarian vein coil embolization. A diagnosis of incompletely resolved PCS was considered. RESULTS: Iliocaval venogram demonstrated patent left common iliac and external iliac venous stents in situ. Subsequent right ovarian venogram revealed a patent, but grossly dilated, right ovarian vein with retrograde flow and cross-pelvic collaterals confirming grade III PCS. Right ovarian vein coil embolization was performed, with excellent patient outcome. CONCLUSION: In the setting of a combined diagnosis of PCS and May-Thurner syndrome, persistent incapacitating PCS after initial iliac stenting should be followed with a complete pelvic venous evaluation including ovarian and left renal venography to rule out residual pelvic congestion secondary to any coexisting ovarian vein incompetencies or nutcracker syndrome.

The aim of this article is to report a case of symptomatic fibromuscular dysplasia (FMD) involving the external iliac arteries (EIAs). An 88-year-old woman was admitted to the vascular service, with a painful right posterior ankle ulcer that had progressively worsened during the course of a month. Her medical history included diabetes and hypertension. Bilateral lower-extremity pulses were absent, and femoral and tibial Doppler waveforms were monophasic. Pelvic and bilateral lower-extremity angiograms were obtained, which revealed findings in both EIAs consistent with a diagnosis of extrarenal FMD. Percutaneous transluminal angioplasty with subsequent stenting of the right EIA was performed, using a self-expanding stent. Completion digital subtraction angiography demonstrated a widely patent right EIA with brisk flow of contrast across the stent. Postprocedural arterial duplex scan showed a biphasic waveform pattern in the common femoral artery. FMD can involve the EIA and be associated with critical limb ischemia. FMD of the EIA responds well to endovascular management.
