RESUMEN

The aim of this triple-blind full-randomized clinical trial was to quantify analgesia in masticatory muscles and temporomandibular joints after occlusal splint therapy associated with the adjuvant administration of nonsteroidal anti-inflammatory drugs (NSAID) isolated or associated with other therapeutic agents. Pain relief was also recorded. Eighteen volunteers who had been suffering from chronic pain in masticatory muscles due to temporomandibular disorders were selected after anamnesis and assessment using RDC/TMD translated to Portuguese. The 3 proposed treatments were NSAID (sodium diclofenac), panacea (sodium diclofenac + carisoprodol + acetaminophen + caffeine), and a placebo. The total treatment duration was 10 days, preceded and succeeded by patients' pain assessment. A washout interval of 11 days was established between each therapy. All participants received all treatments in different moments, in a full randomized crossover methodology. The assessment of drug therapies was performed using visual analogue scale for pain on palpation followed by 11-point numerical scale to quantify pain during treatment. Statistical analysis has shown that, after 10 days of treatment, all therapies were effective for pain relief. NSAID therapy promoted analgesia on the third day, while placebo only promoted analgesia in the eighth day. It has been concluded that sodium diclofenac used as splint adjuvant therapy, promotes significant analgesia in a shorter time.

Baccharis dracunculifolia DC (Asteraceae), popularly known as "alecrim-do-campo," is largely distributed in South America, is shown to exhibit protective actions against gastric ulcers, has anti-inflammatory properties, and is hepatoprotective. Several essential oils obtained from Baccharis species possess biological activities, such as antimicrobial and antivirus activities. This randomized controlled trial evaluated the efficacy of B. dracunculifolia in the reduction of dental biofilm, comparing this natural product with other mouthwashes already known in the dental market. In measuring the time after use of mouthwash (t = 1), there was no difference between products (P = 0.602); that is, subjects in the study had a similar PI after the first use. After one week (t = 2), there was no difference between the four products evaluated (P = 0.674), so, all research individuals completed the study with a similar reduction in dental biofilm between themselves but it was different from initial state (Friedman test). It is possible to conclude that B. dracunculifolia had the same efficiency of the materials used to oral hygiene in reduction of dental plaque and, consequently, prevention of dental caries. Thus, we can consider B. dracunculifolia as a good candidate for new material to be implemented in dental care.

Long-term sample storage can affect the intensity of the hybridization signals provided by molecular diagnostic methods that use chemiluminescent detection. The aim of this study was to evaluate the effect of different storage times on the hybridization signals of 13 bacterial species detected by the Checkerboard DNA-DNA hybridization method using whole-genomic DNA probes. Ninety-six subgingival biofilm samples were collected from 36 healthy subjects, and the intensity of hybridization signals was evaluated at 4 different time periods: (1) immediately after collecting (n = 24) and (2) after storage at -20 °C for 6 months (n = 24), (3) for 12 months (n = 24), and (4) for 24 months (n = 24). The intensity of hybridization signals obtained from groups 1 and 2 were significantly higher than in the other groups (p < 0.001). No differences were found between groups 1 and 2 (p > 0.05). The Checkerboard DNA-DNA hybridization method was suitable to detect hybridization signals from all groups evaluated, and the intensity of signals decreased significantly after long periods of sample storage.

The aim of this trial was to investigate changes occurring in the subgingival microbiological composition of subjects with aggressive periodontitis, treated with antimicrobial photodynamic therapy (aPDT), in a single episode, or scaling and root planing (SRP), in a split-mouth design on -7, 0, and +90 days. Ten patients were randomly assigned to either aPDT using a laser source in conjunction with a photosensitizer or SRP with hand instruments. Subgingival plaque samples were collected and the counts of 40 subgingival species were determined using checkerboard DNA-DNA hybridization. The data were analyzed using the method of generalized estimating equations (GEE) to test the associations between treatments, evaluated parameters, and experimental times (α = .05). The results indicated that aPDT and SRP affects different bacterial species, with aPDT being effective in reducing numbers of A. actinomycetemcomitans than SRP. On the other hand, SRP was more efficient than aPDT in reducing the presence of periodontal pathogens of the Red Complex. Additionally, a recolonization in the sites treated by aPDT was observed, especially for T. forsythia and P. gingivalis. Under our experimental conditions, this trial demonstrates that aPDT and SRP affected different groups of bacteria, suggesting that their association may be beneficial for the non-surgical treatment of aggressive periodontitis.

PURPOSE: Gaps between an abutment and a dental implant are unavoidable, and microleakage may occur, leading to problems such as malodor and peri-implantitis. The aim of the present in vitro study was to investigate leakage of Staphylococcus aureus through the implant/abutment interface by the method of bacterial culture, and to compare the leakage rates of two different types of implant-abutment connections. MATERIALS AND METHODS: Twenty Morse taper implants with abutments were divided into two groups: group A, which were evaluated for microleakage into the inner part of the implants, and group B, which were evaluated for microleakage from the inner part of the implants. Twenty internal-hexagon implants with abutments were also divided into two groups: group C, which were evaluated for microleakage into the inner part of the implants, and group D, which were evaluated for microleakage from the inner part of the implants. For the evaluation of leakage from the implants, the assemblies had the inner parts inoculated with S aureus, and each assembly was incubated in sterile brain heart infusion broth for 1 week. For assessment of leakage into the implants, each assembly was submerged in 4 mL S aureus culture in tubes and incubated for 2 weeks. The microleakage of the two implant connections was compared. RESULTS: Microbial leakage occurred in all groups, and there was no statistically significant difference between groups A and C or between groups B and D. CONCLUSIONS: In vitro, S aureus leakage through the implant/abutment interface occurred with both Morse taper and internal-hexagon implants.

The purpose of this study was to evaluate the effect of a single application of antimicrobial photodynamic therapy (aPDT) on microbiological profile and cytokine pattern in dogs. Periodontal disease was induced by placing 3.0 silk ligatures around the mandibular pre-molars bilaterally during 8 weeks. The dogs were randomly treated with aPDT using a dye/laser system, scaling and root planning (SRP), or with the association of treatments (SRP + aPDT). Plaque samples were collected at baseline, 1, 3, and 4 weeks, and the mean counts of 40 species were determined using DNA-DNA hybridization. Gingival biopsies were removed and the expression of tumor necrosis factor alpha (TNF-α), receptor activator of NF-kB ligand (RANKL), osteoprotegerin (OPG), matrix metalloproteinase (MMP-1), interleukin (IL) 6, IL-10 and total bacterial load by analysis of 16 S rRNA gene were evaluated through real-time PCR. The results shows that the levels of the majority of the species were reduced 1 week post-therapy for all treatments, however, an increase in counts of Prevotella intermedia (p = 0.00), Prevotella. nigrescens (p = 0.00) and Tannerella forsythia (p = 0.00) was observed for aPDT and SRP + aPDT. After 4 weeks, a regrowth of Porphyromonas gingivalis (p = 0.00) and Treponema denticola (p = 0.00), was observed for all treatments. Also, a strikingly reduction of counts on counts of Aggregatibacter actinomycetemcomitans was observed for the aPDT (p = 0.00). For the cytokine pattern, the results were similar for all treatments, and a reduction in the expression of cytokines and bacterial load was observed throughout the study. Our results suggest that SRP, aPDT in a single application, and SRP + aPDT affects different bacterial species and have similar effects on the expression of cytokines evaluated during the treatment of ligature-induced periodontitis.

OBJECTIVE: The aim of this study was to evaluate the efficacy of a spray containing an antimicrobial solution for toothbrush disinfection. METHODS AND MATERIALS: Three different solutions were sprayed on toothbrush bristles among 30 adults after they had brushed: (1) basic formulation (base) plus chlorhexidine; (2) base only, and (3) sterile tap water (control). Each solution was tested for 1 week. After that, the toothbrushes were collected and sonicated in Letheen Broth, diluted in 10-fold series, and plated on selective and nonselective media for detection of anaerobes, aerobes, streptococci, and gram-negative bacilli. After incubation, the colonies of those microorganisms were counted. Presence of mutans streptococci on the bristles was also confirmed. RESULTS: Spray 1 produced a significant reduction in the microbial contamination of toothbrushes for all the microorganisms, spray 2 provided some reduction of contaminants, and spray 3 demonstrated the least antimicrobial effect. CONCLUSION: The antimicrobial spray with chlorhexidine proved to be an effective and practical means for toothbrush disinfection.

The purpose of this study was to assess flexural strength and color alteration of acrylic resins immersed in denture cleansers for different periods of time. Rectangular specimens (65 x 10 x 3mm) made from three heat-polymerized acrylic resins (Lucitone 550, QC-20 and Triplex) were assigned to three denture cleansers groups (Bony Plus, Corega Tabs and Efferdent Plus) and a control group (immersion in water). Soaking trials of 15 min and 8 h simulated 30 days of use. Flexural strength testing was carried out with 105 specimens on a universal testing machine. Color alterations were visually assessed by examination of photographs taken from 21 specimens. Flexural strength means (in MPa) were analyzed statistically by analysis of variance and Tukey's test at 5% significance level. There were significant differences (p<0.01) among the resins Lucitone (89.439 +/- 7.962), Triplex (88.024 +/- 5.167) and QC-20 (83.379 +/- 7.153). No significant differences (p>0.05) were found either among the denture cleansers (Bony Plus = 87.693 +/- 6.943; Corega Tabs = 86.955 +/- 7.114; Efferdent Plus = 86.195 +/- 7.865 and control = 86.536 +/- 7.012) or between the soaking periods (15 min = 86.875 +/- 7.625 and 8 h = 87.432 +/- 7.355) throughout the soaking cycles simulating 30 days of use. No color alterations were identified by visual examination. The findings of this study showed that chemical denture cleansers used according to the manufacturers' specifications did not cause flexural strength alterations or color changes in heat-polymerized acrylic resins submitted to soaking cycles that simulated 30 days of use.

BACKGROUND: It is estimated that approximately 85% of all halitosis cases have their origin within the mouth; of these, 50% are caused by tongue residues. Previous studies have established that hydrogen sulfide and mercaptans are the primary components of halitosis. Thus, tongue cleaning gains importance as a means of halitosis management. METHODS: This investigation compared the efficacy of two mechanical methods for tongue cleaning through a handheld sulfide monitor. This crossover trial was carried out with 10 healthy subjects, 20 to 50 years old. Before the baseline measurement of the volatile sulfur compounds (VSCs), the subjects were instructed to refrain from any tongue cleaning method for 48 hours. The 10 participants were then placed in one of two groups (five each): 1) first week: tongue scraper, second week: soft-bristle toothbrush; 2) first week: toothbrush, second week: tongue scraper, with a 48-hour wash-out period between each week. RESULTS: The baseline measurements were compared with those of the end of each week using the Dunn method (alpha = 0.01). The tongue scraper showed a 75% reduction in VSCs, while the toothbrush only achieved a 45% reduction in VSCs. CONCLUSION: Although the tongue coating was removed by both methods, the tongue scraper performed better in reducing the production of volatile sulfur compounds.

CONTEXT: The absence of a protocol for the treatment of halitosis has led us to compare mouthrinses with mechanical oral hygiene procedures for treating morning breath by employing a hand-held sulfide monitor. AIMS: To compare the efficacy of five modalities of treatment for controlling morning halitosis in subjects with no dental or periodontal disease. SETTINGS AND DESIGN: This is a five-period, randomized, crossover clinical trial. MATERIALS AND METHODS: Twenty volunteers were randomly assigned to the trial. Testing involved the use of a conventional tongue scraper, a tongue scraper joined to the back of a toothbrush's head, two mouthrinses (0.05% cetylpyridinium chloride and 0.12% chlorhexidine digluconate) and a soft-bristled toothbrush and fluoride toothpaste for practicing oral hygiene. STATISTICAL ANALYSIS USED: Data analysis was performed using SPSS version 17 for Windows and NCSS 2007 software (P < 0.05). The products and the periods were compared with each other using the Friedman's test. When significant differences (P < 0.05) were determined, the products and periods were compared in pairs by using the Wilcoxon's test and by adjusting the original significance level (0.05) for multiple comparisons by using the Bonferroni's method. RESULTS: The toothbrush's tongue scraper was able to significantly reduce bad breath for up to 2 h. Chlorhexidine reduced bad breath only at the end of the second hour, an effect that lasted for 3 h. CONCLUSIONS: Mechanical tongue cleaning was able to immediately reduce bad breath for a short period, whereas chlorhexidine and mechanical oral hygiene reduced bad breath for longer periods, achieving the best results against morning breath.

The objective of this study was to evaluate bone formation after application of different doses of recombinant human bone morphogenetic protein-2 (rhBMP-2) combined with monoolein or poloxamer gels, in critical bone defects of rats. Forty-five Wistar rats were divided into nine treatment groups with five animals each: I: application of 1 µg rhBMP-2 + monoolein; II: 3 µg rhBMP-2 + monoolein; III: 7 µg rhBMP-2 + monoolein; IV: 1 µg rhBMP-2 + poloxamer; V: 3 µg rhBMP-2 + poloxamer; VI: 7 µg rhBMP-2 + poloxamer; VII: monoolein only; VIII: poloxamer only; and IX: critical bone defect only. A critical-sized defect of 6 mm diameter was produced in the left parietal bone and it was filled with gels of the above mentioned treatments. After 2 weeks, the calvarial bones were removed for histological processing. Bone formation in the groups that received poloxamer gel and rhBMP-2 was not significantly different from the control group (IX). Groups receiving monoolein and rhBMP-2 (1 and 3 µg) and those that received only the carriers (VII and VIII) had less bone formation in relation to the control. The association of rhBMP-2 to both poloxamer and monoolein did not exhibit any significant differentiation in bone formation in comparison with the control group.

The aim of this investigation was to monitor metronidazole concentrations in the gingival crevicular fluid (GCF) collected from periodontal pockets of dogs after treatment with an experimental 15% metronidazole gel. Five dogs had periodontitis induced by cotton ligatures placed subgingivally and maintained for a 30-day period. After the induction period, only pockets with 4 mm or deeper received the gel. Each pocket was filled up to the gingival margin by means of a syringe with a blunt-end needle. GCF was collected in paper strips and quantified in an electronic device before and after 15 minutes, 1 h, 6 h, 24 h and 48 h of gel administration. The GCF samples were assayed for metronidazole content by means of a high performance liquid chromatography method. Concentrations of metronidazole in the GCF of the 5 dogs (mean +/- SD, in microg/mL) were 0 +/- 0 before gel application and 47,185.75 +/- 24,874.35 after 15 minutes, 26,457.34 +/- 25,516.91 after 1 h, 24.18 +/- 23.11 after 6 h, 3.78 +/- 3.45 after 24 h and 3.34 +/- 5.54 after 48 h. A single administration of the 15% metronidazole gel released the drug in the GCF of dogs in levels several-fold higher than the minimum inhibitory concentration for some periodontopathogens grown in subgingival biofilms for up to one hour, but metronidazole could be detected in the GCF at least 48 hours after the gel application.

This study evaluate spontaneous pain after and before administration of sodium diclofenac, isolated or associated to carisoprodol, acetaminophen and caffeine, in chronic temporomandibular disorders (TMD) patients. Were selected eighteen volunteers, both men and women, between 35-70 years of age (mean age 50 years). The inclusion criteria was masticatory muscle pain, and the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was used on the diagnose. The selection of treatment for each individual was done by a triple-blind full-randomized crossover methodology. Thus, all patients were submitted to all treatment at different moments, in a non standardized sequence, avoiding tendentious results. The treatments were: A (sodium diclofenac + carisoprodol + acetaminophen + caffeine), B (sodium diclofenac) and C (placebo), all associated with an occlusal splint. Each treatment period was followed by an eleven-day washout. There weren't observed differences between initial and final values of treatments. However, there were statistically significant differences in evaluative and miscellaneous sensorial groups after B treatment; and in sensorial, affective, and total score groups after B and C treatments. Within the limitations of this investigation, we conclude that treatment of muscular TMD patients with sodium diclofenac isolated promoted higher analgesia than treatment with sodium diclofenac more associations or placebo, when associated to an occlusal splint.


Este estudio evaluó el dolor espontáneo antes y después de la administración de diclofenaco sódico, aislado o asociado a carisoprodol, paracetamol y cafeína, en pacientes con trastornos temporomandibulares crónicos (TTM). Se seleccionaron dieciocho voluntarios, hombres y mujeres, entre 35-70 años de edad (edad media 50 años). Los criterios de inclusión fueron dolor muscular masticatorio, y los criterios diagnósticos para trastornos temporomandibulares (RDC / TMD) como diagnóstico. La selección del tratamiento para cada individuo se llevó a cabo mediante una metodología de cruce triple ciego completo al azar. Por lo tanto, todos los pacientes fueron sometidos a todos los tratamientos en diferentes momentos, en una secuencia no estandarizada, evitando los resultados tendenciosos. Los tratamientos fueron: A (diclofenaco sódico + carisoprodol + acetaminofen + cafeína), B (diclofenaco sódico) y C (placebo), todos asociados a una férula oclusal. Cada período de tratamiento fue seguido por once días. No se encontraron diferencias entre los valores inicial y final de los tratamientos. Sin embargo, hubo diferencias estadísticamente significativas en los grupos de evaluación sensorial y después del tratamiento B, y en los grupos de calificación sensorial, afectivo, y el total después de los tratamientos B y C. Dentro de las limitaciones de esta investigación, se concluye que el tratamiento con diclofenaco sódico aislado en pacientes con TTM musculares promueve una mayor analgesia que el tratamiento con diclofenaco sódico más asociaciones o placebo, cuando se asocia a una férula oclusal.

The purpose of this study was to evaluate bacterial survival rate on toothbrushes after brushing and the efficacy of their decontamination by spraying antimicrobial solutions. Thirty subjects were instructed to spray the solutions on toothbrush bristles after brushing. Each volunteer tested three sprays, one solution per week; the sprays were labeled spray 1 (cetylpyridinium chloride - CPC - and basic formulation), 2 (basic formulation only) and 3 (control - sterile tap water). At the end of each week, the brushes were collected and sonicated in Letheen Broth®; the suspensions were ten-fold diluted and the dilutions were plated on various culture media. Anaerobic bacteria, evaluated by colony count of black pigment producing organisms on Ask medium, were recovered from 83.3% of the samples, Streptococci from 80% and aerobic Gram-negative bacilli from 46.7% of them in the control tests. There was a significant decrease in toothbrush contamination with antimicrobial sprays 1 and 2, the first showing the greatest decrease on bacterial counts.

RESUMEN

The aim of this investigation was to monitor metronidazole concentrations in the gingival crevicular fluid (GCF) collected from periodontal pockets of dogs after treatment with an experimental 15 percent metronidazole gel. Five dogs had periodontitis induced by cotton ligatures placed subgingivally and maintained for a 30-day period. After the induction period, only pockets with 4 mm or deeper received the gel. Each pocket was filled up to the gingival margin by means of a syringe with a blunt-end needle. GCF was collected in paper strips and quantified in an electronic device before and after 15 minutes, 1 h, 6 h, 24 h and 48 h of gel administration. The GCF samples were assayed for metronidazole content by means of a high performance liquid chromatography method. Concentrations of metronidazole in the GCF of the 5 dogs (mean ± SD, in µg/mL) were 0 ± 0 before gel application and 47,185.75 ± 24,874.35 after 15 minutes, 26,457.34 ± 25,516.91 after 1 h, 24.18 ± 23.11 after 6 h, 3.78 ± 3.45 after 24 h and 3.34 ± 5.54 after 48 h. A single administration of the 15 percent metronidazole gel released the drug in the GCF of dogs in levels several-fold higher than the minimum inhibitory concentration for some periodontopathogens grown in subgingival biofilms for up to one hour, but metronidazole could be detected in the GCF at least 48 hours after the gel application.

RESUMEN

The purpose of this study was to evaluate the efficacy of monoolein gel as a carrier for rhBMP-2 in the healing bone process of critical bone defects created in Wistar rats mandibles using digitalized radiographic method to analyze this process. In the group 1, the rhBMP-2 was dissolved in aqueous solution and in the group 2, the rhBMP-2 was combined with monoolein gel as a carrier. The results showed that in both of groups it was found efficient bone repair, with a great optical density in the group that the rhBMP-2 was combined with the monoolein gel, but without statistical difference between them.


El objetivo de este estudio fue evaluar la eficiencia del gel de monoolein como portador de la rhBMP-2 en el proceso de recuperación de la osificación en defectos osteos creados en mandíbulas de ratos Wistar, por medio de radiografias digitalizadas como método de análisis. En el grupo 1, la rhBMP-2 fue disuelta en solución acuosa y en el grupo 2, la rhBMP-2 fue combinada al gel de monoolein como portador. Los resultados mostraron que los dos grupos presentaron un eficiente reparo osteo, con mayor densidad óptica en el grupo en que la rhBMP-2 fue combinada con el gel de monoolein, pero sin diferencia estadística significante entre ellos.

RESUMEN

O objetivo deste estudo foi avaliar a resistência à flexão e alterações de cor de resinas acrílicas termopolimerizáveis imersas em higienizadores de prótese por diferentes períodos de tempo. Corpos-de-prova retangulares (65x10x3mm) de três resinas acrílicas (Lucitone 550, QC-20 e Triplex) foram divididos em três grupos de higienizadores de prótese (Bony Plus, Corega Tabs e Efferdent Plus) e um grupo controle (imersão em água). Foram realizadas imersões por 15 min e 8 horas, simulando 30 dias de uso. O teste de resistência à flexão foi realizado com 105 corpos-de-prova em máquina de ensaios universais. Alterações de cor foram avaliadas por meio de fotografias tiradas de 21 corpos-de-prova. Os resultados de resistência à flexão (em MPa) foram submetidos à análise de variância e teste de Tukey, com nível de significância de 5%. Houve diferenças significantes (p<0.01) entre as resinas Lucitone (89,439 ± 7,962); Triplex (88,024 ± 5,167) and QC-20 (83,379 ± 7,153). Não foram encontradas diferenças significantes (p>0.05) entre os higienizadores de prótese (Bony Plus = 87,693 ± 6,943; Corega Tabs = 86,955 ± 7,114; Efferdent Plus = 86,195 ± 7,865; e controle = 86,536 ± 7.012) nem entre os períodos de imersão (15 min = 86,875 ± 7,625 e 8 h = 87,432 ± 7,355) durante os ciclos de imersão que simularam 30 dias de uso. A inspeção visual não detectou qualquer alteração de cor. Os achados desse estudo revelaram que os higienizadores de prótese, quando usados de acordo com as instruções dos fabricantes, não afetaram a resistência à flexão nem causaram alterações de cor nas resinas acrílicas termopolimerizáveis após um período de uso simulado de 30 dias.

O propósito deste estudo foi avaliar a taxa de sobrevivência bacteriana em escovas dentais após a escovação e a eficácia na sua descontaminação pelo borrifamento de soluções antimicrobianas. Trinta indivíduos foram instruídos a borrifar as soluções nas cerdas das escovas após a escovação. Cada voluntário testou três sprays, uma solução por semana; os sprays foram rotulados spray 1 (cloreto de cetilpiridínio - CCP - e formulação básica), 2 (formulação básica apenas) e 3 (controle - água de torneira esterilizada). Ao final de cada semana, as escovas eram recolhidas e introduzidas no caldo Letheen®, submetidas a ultra-som, à diluição decimal seriada e as suspensões semeadas em vários meios de cultura. As bactérias anaeróbias, avaliadas pela contagem de colônias de microrganismos produtores de pigmento negro no meio Ask, foram recuperadas em 83,3% das amostras, estreptococos em 80% e bacilos aeróbios Gram-negativos em 46,7% das amostras nos testes controle. Houve uma significante redução na contaminação das escovas dentais com os sprays antimicrobianos 1 e 2, o primeiro mostrando maior redução nas contagens de bactérias.
