RESUMEN

The purpose of this study was to examine the propulsion asymmetries of wheelchair athletes while sprinting on an instrumented, dual-roller ergometer system. Eighteen experienced wheelchair rugby players (8 low point (LP) (class ≤1.5) and 10 high point (HP) (class ≥2.0)) performed a 15-second sprint in their sports wheelchair on the instrumented ergometer. Asymmetry was defined as the difference in distance and power output (PO) between left and right sides when the best side reached 28 m. Propulsion techniques were quantified based on torque and velocity data. HP players covered an average 3 m further than the LP players (P = .002) and achieved faster sprint times than LP players (6.95 ± 0.89 vs 8.03 ± 0.68 seconds, P = .005) and at the time the best player finished (5.96 seconds). Higher peak POs (667 ± 108 vs 357 ± 78 W, P = .0001) and greater peak speeds that were also evident were for HP players (4.80 ± 0.71 vs 4.09 ± 0.45 m/s, P = .011). Greater asymmetries were found in HP players for distance (1.86 ± 1.43 vs 0.70 ± 0.65 m, P = .016), absolute peak PO (P = .049), and speed (0.35 ± 0.25 vs 0.11 ± 0.10 m/s, P = .009). Although HP players had faster sprint times over 28 m (achieved by a higher PO), high standard deviations show the heterogeneity within the two groups (eg, some LP players were better than HP players). Quantification of asymmetries is important not only for classifiers but also for sports practitioners wishing to improve performance as they could be addressed through training and/or wheelchair configuration.

RESUMEN

A derivative of the pEX3 expression vector was constructed that codes for the first 407 amino acids of the 1051 amino acids of the pEX3 fusion protein. The amount of truncated fusion protein (40 mg/g cells), obtained by expression in E. coli, was similar to that produced by the original pEX3 vector. The truncated fusion protein was purified more easily from E. coli contaminants than the original fusion protein by washing with 2 M urea and 0.5% Triton X-100.

To determine whether Hb criteria should be adjusted for obesity, the ranges and medians of Hb concentrations for 354 obese and 1894 nonobese, white children, aged 6 to 14 yr from HANES I were compared. Although median Hb concentrations average 0.125 g/dl higher in obese individuals with or without exclusion of iron-deficient individuals this slight difference is not significant (t tests p less than 0.1). It is suggested that while age, sex, altitude, and potentially, race are valid considerations when screening for anemia, a further adjustment for obesity appears unwarranted and would complicate efforts in optimizing screening for anemia.

RESUMEN

An evaluation of 212 24-hour dietary intakes showed than an assessment using a nutrient adequacy score and comparing actual nutrient intakes to Recommended Dietary Allowances was similar to an assessment with a dietary score based on food groupings. This suggests that the simple dietary score can be substituted for the more complete and time-consuming dietary analysis when program effectiveness is evaluated.

RESUMEN

Robert Murphy's career, culminating in the study of disability, gives evidence of deep commitment to understanding the human condition in adversity. His life-long interest in the psychological, sociological and cultural significance of behavior; his work on alienation, developed through fieldwork in small scale societies in South America and Africa; his fascination with the estrangement that arises from the conflict between our need to integrate and to be autonomous; and his own experience of a progressive debilitating disease provided him with rich insights for his interpretation of disability. This paper examines Murphy's contributions to our understanding of the meanings and implications of disability, for those who personally experience it, and for the 'others' in their lives.

RESUMEN

Sociological research on the disabled has for the past 25 years made extensive use of a social deviance model to characterize the status of the physically impaired. The present article, which is based on a three-year anthropological investigation of the social relations of paraplegics and quadriplegics in the New York metropolitan area, argues that there are shortcomings in the deviance model and offers, instead, a model taken from the anthropological study of ritual. The disabled are viewed as being in a 'liminal' state, as in the liminal phases of rites of passages. They are persons having an undefined status: they are neither ill nor well, neither socially alive and active nor socially expunged and removed. The status of the disabled in American society and the symbolism of disability in American culture are reexamined within this framework. This perspective is extended to other types of deep adversity, such as acute loss of income and status or catastrophic illness.

RESUMEN

A study was undertaken to examine nutrition surveillance activities and their usefulness in managing programs of nutrition intervention. Questionnaires were returned by 24 of 26 directors of nutrition units in State or metropolitan health departments participating in 1981 in the coordinated nutrition surveillance system of the Centers for Disease Control, which monitors high risk pediatric patients and pregnant women. The mean years of experience in surveillance activities among the agencies was 4. Only 25 percent of the responding departments reported a self-sufficient computerized surveillance system. Personnel most involved in the coordinating, analyzing, and interpreting of the data were nutritionists who spent an average of 17 hours per month. Major uses of surveillance data reported for purposes of the nutrition programs were to (a) identify collection sites with problems such as errors in measuring heights and weights and hematocrits warranting checks for quality control, (b) define the extent of nutrition-related disorders in the target populations, (c) provide objective local data to assist in decision-making and program planning, (d) enhance followup of specific clients, and (e) provide feedback to clinic staffs about the quality and relative impact of their services. The survey results yielded evidence that nutrition surveillance activities have important consequences for the planning, implementation, and evaluation of programs of nutritional intervention.

Guided by the assumption that the initial symptomatology of patients with acute viral hepatitis may allow to predict the further course of the disease, bodily complaints, mood and laboratory findings of 50 patients were assessed. For single parameters such as liver complaints, transaminases or anxious depression, no differences were found between patients with rapid, medium and slow recovery. As far as the pattern of symptoms were concerned, however, the three groups differed from each other. This finding implies the use of different illness coping strategies in the various groups.

This study was designed to determine the effect of cholesterol screening on college students' knowledge retention and self-reported behavior change. Students (N = 180, age M = 21.4 +/- 4.1 years) enrolled in a self-paced personal fitness course participated in a screening accompanied by two educational handouts and oral information from the technician. Six weeks later, subjects completed a questionnaire that revealed that 76% remembered their exact cholesterol level. Only 36%, however, remembered specifically why their cholesterol level was important, 22% remembered the three National Cholesterol Education Program (NCEP) categories and values, and only 36% could fully identify their correct NCEP follow-up. Chi-square analyses revealed no significant differences between expected and observed frequencies for gender or cholesterol category by cognitive information retention. Analyses did reveal more self-reported dietary and exercise changes for those testing above 200 mg/dL than for those below 200. Although college students may remember their cholesterol level after 6 weeks and may take some action if they are above the desirable level, the knowledge base established in a typical screening situation appears inadequate. Educational efforts accompanying most screenings should be improved, and some attention should be given to young adults who test at the high end of the desirable category.

Cultural contexts influence the ways individuals interpret and experience functional losses associated with post-polio sequelae. Using in-depth multiple interview case studies from two National Institute on Aging projects, the concept of "biographies" is presented to place the individuals' polio-related experiences within the context of their lives. Two major cultural contexts shape the construction of polio biographies: normative life course expectations and developmental tasks; and traditions associated with polio recovery and rehabilitation. The authors identify key dimensions of personal concern among polio survivors that can be used as entrance points for effective clinical intervention and to promote treatment compliance.

Injury to the triangular fibrocartilage complex associated with distal radius fracture may cause symptoms of ulnar instability. Assessed by a radioulnar stress test, increased laxity of the distal radioulnar joint has in two previous studies been depicted to be associated with poorer outcome. This prospective study of 40 adults investigates the correlation of this test with functional outcome as measured by DASH. No clinically significant difference was found in relation to this test at two and five years after injury. Therefore using this test alone to decide whether or not to perform an acute repair of the TFCC cannot be recommended.

RESUMEN

Lesions of the triangular fibrocartilage complex (TFCC) are commonly associated with distal radius fractures and may adversely affect the functional outcome. This prospective study evaluated computed tomography, using the radioulnar ratio (RUR), to detect laxity of the distal radioulnar joint in 48 consecutive patients with acute distal radius fractures and compared the results with a radioulnar stress test. We found the clinical stress test to be reliable in chronic cases, but the RUR method of questionable value in both acute and chronic cases.
