RESUMEN

Results of treatment of 72 patients, suffering choledocholithiasis, using transpapillary endoscopic interventions, were analyzed. In patients of the first group a complete endoscopic papillosphincterotomy was performed, and in the second group ­ a partial endoscopic papillosphincterotomy in combination with balloon dilatation of duodenal papilla magna. The rate of occurrence of late and immediate complications in the groups was compared.

RESUMEN

Results of treatment was studied in 2008 - 2015 yrs in 57 patients, suffering "difficult stones" (choledocholithiasis), in whom a dosed papillotomy in combination with the balloon dilatation. The advantages of application of combined dosed endoscopic papillosphincterotomy and balloon dilatation, comparing with complete endoscopic papillosphincterotomy, while treating "difficult stones" of common biliary duct, were established. Application of the procedure have guaranteed a good access through the duodenal papilla magna and have permitted to perform the calculi extraction in a less traumatic way, what have promoted its function preservation, and reduction of a postoperative complications rate, the patients' stationary treatment duration, and the remote complications rate. Using questionnaire SF-36, the quality of life was analyzed in patients, in whom in remote period the duodenal papilla magna function was preserved.

RESUMEN

Losses of health from myocardial infarction in the city of Kemerovo were estimated accordingly to the disability-adjusted life year (DALY) index, as well as its compartments: YLL (years lost due to premature death) and YLD (years lost disability). When ranking districts of the city of Kemerovo both in terms of chemicals exposure and the value of the DALY index there was revealed the opposite direction of these indices. The performance of the correlation analysis of health losses with concentrations of chemical contaminants allowed to reveal a feedback between them among the elderly patients (60 years and older) and direct--in young (up to 45 years old) cases. This fact probably may attest to the significant contribution of the environmental pollution in the process of development and progression of atherosclerotic changes in the young population.

The aim of the study was to examine the immunomorphological features and expression of immunohistochemical markers that indicate the presence of immune inflammation in the placenta in normal pregnancy and in pregnancy complicated by bronchial asthma (BA) of varying degrees of severity. The of 80 samples of placenta from patients with BA were studied immunohistochemically and histologically to demonstrate the fixation of C3 fraction of complement, fibrinogen, IL-4, IL-6, IgA, IgM, IgG, and the expression of CD31, CD35, CD57 and CD83 molecules. The largest number of immune deposits fixed in the placenta and damaging the structures of the placental barrier (vascular endothelium and syncytiotrophoblast membranes) was found in women with moderate to severe BA. Their precipitation lead to the development of immunopathological process in the placenta and could result in placental insufficiency, adversely affecting the course of pregnancy and childbirth. The presence of pathogenic immune complexes in the placenta and the detection of antigen-presenting cells in areas of immunological inflammation in the placenta of women with BA are associated with the unfavorable course of pregnancy, parturition and could be a predictor of the possible development of allergic diseases in children.

RESUMEN

Tendency to growth of prevalence of ischemic heart disease (IHD) occurring in Russian Federation despite application of preventive measures designates necessity of search for novel nontraditional factors of risk. Among other studied factors of genesis of cardiovascular diseases in general and of IHD in particular is the role of xenobiotics - chemical pollutants, substances foreign to the body. In this paper we present results of a number of epidemiological studies on the problem of xenobiotics and IHD. Special attention is given to the difficulty of isolation of the leading chemical pollutant and as a consequence of pathogenetic link what leads to underestimation of pathological states caused by ecological factors especially in such urbanized region with developed chemical industry as Kusbass.

RESUMEN

The analysis of age features of prevalence of an arterial hypertensia in the basic professional groups of the coal-mining enterprises was carried out. In total 1575 workers of collieries and cuts of Kuzbas united in 9 professional groups participated in the research. The results of research demonstrated the distinctions of age structure in the professional groups, testifying about expressed professional senescence among workers of mines, which reveals itself in the decrease in relative density of persons of 50+ years. In these groups with the expressed professional senescence the decrease in relative density of persons with an arterial hypertensia--so-called effect of the "healthy worker", in turn, is observed.

RESUMEN

The literature review discusses the debatable problems on terms of separation of different anlages of human maxillo-facial apparatus, chronology of histo- and organogenetic remodeling of hard and soft tissues during the period of their formation in the first trimester of pregnancy. It is suggested that these controversies are most likely determined by imperfection of current embryogenesis periodization systems and of criteria of human embryos and fetuses age definition; therefore further research in this direction is required.

Several chiral thiols, i.e. traditionally used enantiomerically pure SH reagents and novel N-R-mandelyl-L-cysteine (R-NMC) containing additional chiral center, have been applied as co-reagents in precolumn derivatization with o-phthalaldehyde for enantiomeric HPLC analysis of individual alpha-amino acids and their mixtures. The R-NMC-derived isoindoles as well as adducts with other thiols have a characteristic absorption maximum at 340 nm, and are highly fluorescent allowing detection of 10 microg/l of an amino acid. Investigated 19 amino acids were analyzed separately and in a mixture by a gradient HPLC after precolumn derivatization. The chromatographic behavior of formed isoindoles substantially differs for each of the thiols used for modification. In contrast to traditional enantiomeric thiols application of diastereomeric R-NMC provides higher resolution for alpha-amino acid enantiomers, with L,D-elution order (except for Arg). Combined use of R-NMC and other thiol enlarges the possibilities of this method, allowing accurate chiral analysis of complex amino acid mixtures.

This study demonstrates that an amino-beta-cyclodextrin-bonded phase column exhibits enantioselectivity for various amino acid derivatives. Mixtures of methanol, acetonitrile, tetrahydrofuran or dioxan and triethylamine buffers (pH 4.0-7.0) were used as mobile phases. The effect of the mobile phase on the resolution process was studied by varying the mobile phase composition (type and percentage of organic modifiers, pH, and ionic strength of the buffer solution). The 1-octanol-water partition coefficients are calculated and tabulated for 16 derivatized amino acids. The chromatographic data for 42 pairs of derivatized amino acids resolved on the amino-beta-cyclodextrin-bonded phase are summarised. The separation of adrenaline, noradrenaline and amphetamine on a novel vancomycin stationary phase is demonstrated.

RESUMEN

Immunologic vigor was studied in 108 patients with skin melanoma and 20 healthy subjects by time-delayed hypersensitivity skin and leukocyte adherence inhibition tests. A correlation between changes in immune response and the clinical picture of melanoma was established. During remission immunologic indexes became stabilized in most patients. As tumor progressed, immune response deteriorated markedly, blood serum developed a blocking effect and promoted leukocyte adherence. Healthy subjects who were in close contact with melanoma, showed positive reaction in leukocyte adherence inhibition test with melanoma tissue extract.

RESUMEN

A clinical case of the development of undesirable pharmacological phenomena in a woman of childbearing age with epilepsy is described. The development of undesirable pharmacological phenomena at reception of average therapeutic doses of valproic acid has been caused by the primary (idiopathic) and secondary (valproate-induced) infringement of a folic cycle against a combination of CYP2C9*3 gene polymorphism and a mutation in the MTHFR gene.

Experiments on 200 rats showed that inhibitors of arachidonic acid conversion capable of reducing coagulation activity of platelets decrease the content of markers for thrombin--fibrinogen interaction in the plasma and, therefore, decelerate intravascular blood coagulation and increase thrombin tolerance.

In 50 human embryos, distributed according to the age scale from the 21st day up to 12 weeks of the intrauterine development, which includes stages X-XXIII and beginning of the fetal period (classification of Carnegie Institute), the diameter and volume of the cell nuclei have been followed in the process of formation of epithelial and mesenchymal derivatives of the eye accessory apparatus. In early histogenesis of the eye accessory apparatus the dimensions of the cell nuclei of all epithelial and mesenchymal anlages decrease linearly. In the cells of the epithelial derivatives decrease of the nuclear dimensions in expressed more significantly than the mesenchymal ones. The dynamics of the nuclear dimensions is compared with the form of the same nuclei in each anlage. The nuclear form changes. The differentiation in all the anlages occurs similarly: cells with nuclei having various size appear. Each tissue by the moment, when it appears from the tissue source, possesses cells with nuclei, significantly differing from the nuclei in the latter. In accordance with the modern cytological data, problems and matters making the base of the changes in the nuclei in the course of embryonal histogenesis are discussed.

In 50 human embryos at the age of 21 days--12 weeks of development, including the stages X-XXIII and beginning of the fetal period according to Carnegie's classification, spatial-temporal regularities on rearrangement of cellular material in the mesenchymal derivatives of the ocular auxiliary apparatus have been investigated. The main attention has been paid to ascertain the moments, when the first signs of differences appear in the previously homogenous cellular material of the differentiating mesenchyme around the ocular cup and in the eyelids, taking into account appearance of derivates in the ocular auxiliary apparatus: sclera, oculomotor muscles, stroma of the lacrimal gland, cartilagenous laminae in the eyelids and in the orbicular muscle. Regularities of cytochemical differentiation of the mesenchyme and its derivatives have been studied, taking into account the organ's changing topography. Morphometrical investigations of cells in the anlages mentioned are expanded and correlated with the time of their cytochemical differentiation. Correlation of the morphometrical and cytochemical data with the histological investigations has been performed.

Dose-dependence of DNA synthesis inhibition in cells of HeLa suspension culture, bone marrow and thymus of mice was investigated. A contribution of inhibition of replicon synthesis and DNA chain elongation to the total effect of DNA synthesis inhibition by gamma-radiation was estimated by mathematical simulation. The method is proposed for evaluation of Di that characterizes cell radiosensitivity. Radiosensitivity of HeLa cells after preirradiation or exposure to 5-fluorodeoxyuridine was assessed.

It was shown that the dose--effect curves describing the radiosensitivity of CFUc of the bone marrow irradiated in vitro (0.04-3.7 Gy) and treated with normal rabbit serum (NRS) and anti-mouse-brain serum (AMBS) has two differently sloping portions indicating that two CFUc populations differing in radiosensitivity are present in the bone marrow. D0 was 0.93 Gy after irradiation with doses of 0.04-0.75 Gy and treatment with NRS, and 0.33 Gy after incubation of the bone marrow with AMBS. The addition of thymus cells "straightened" the dose--effect curve for the bone marrow treated with AMBS: in this case D0 was 1.81 Gy exceeding considerably the values of D0 for intact bone marrow. The CFUc population is suggested to be heterogeneous in radiosensitivity.
