RESUMEN

A standard treatment for disseminated intravascular coagulation in neonates has not yet been established. We analyzed the outcomes of 23 neonates who developed disseminated intravascular coagulation due to infection or asphyxia between 2004 and 2017. The overall survival rate was 95.7% on day 28 after anticoagulant therapy. In contrast, the bleeding-free survival rate was 69.6% (95% confidence interval, 53.1%-91.2%). Of the 6 neonates with intracranial bleeding, 2 developed neurological sequelae. The current study showed that intracranial bleeding remained a major problem in the early 2000s, despite the introduction of a new anticoagulant drug, recombinant thrombomodulin, at our institution since 2009.

RESUMEN

Tissue engineering is a highly interdisciplinary research field aiming at repairing, replacing, and regenerating the defective tissues. Over several decades of research, a variety of methods have been developed. The technical methods can be categorized into scaffold-based and scaffold-free strategies. In this mini review, the discussion will be focused on the technical methods of tissue engineering for kidney regeneration and construction.

BACKGROUND: During neonatal resuscitation, careful oxygenation is needed. Pulse oximetry is recommended to evaluate the need for oxygenation, but it is not clear whether peripheral perfusion is adequate for the evaluation of arterial oxygen saturation (SpO2 ). Additionally, there has been no study on the changes in SpO2 immediately after birth in Japan, despite the indispensable need for definitive oxygenation criteria. METHODS: A prospective observational study was performed in neonates at gestational age 35-41 weeks. An SpO2 measurement probe was attached to the neonates immediately after birth at the right palm or wrist, and the perfusion index (PI), pulse rate, and SpO2 were measured until 10 min after birth. RESULTS: Sixty neonates were examined. Stable PI was obtained soon after birth, preceding SpO2 measurement. The median PI (%) was constant at approximately 1.3, and the median SpO2 at 2-10 min was 70%, 81%, 82%, 87%, 89%, 92%, 92%, 94%, and 95%, respectively. The current target value for SpO2 in the Neonatal Cardiopulmonary Resuscitation (NCPR) guideline in Japan is approximately the 25th percentile. CONCLUSION: PI is stable and sufficient in the early postnatal period, meaning that peripheral perfusion is adequate for the measurement of SpO2 . The current target SpO2 used in the NCPR guidelines is at approximately the 25th percentile and is thought to be sufficient for meeting oxygenation criteria.

BACKGROUND: The aim of this study was to investigate residual blood volume in the umbilical cord of extremely premature infants. METHODS: Twenty extremely premature infants were held at or below the placenta while the umbilical cord was clamped and cut at approximately 2-3 cm from the umbilicus within 30 s after birth. The umbilical cord was then clamped near the placenta to obtain a length of approximately 30 cm and cut. The residual blood volume in the segment of cord was drained and measured in milliliters. RESULTS: Mean birthweight was 846 ± 172 g (range, 587-1180 g). The average length of the clamped segment of umbilical cord was 29.8 ± 1.5 cm (range, 27-32 cm). Total residual blood volume and residual blood volume per cm were 15.5 ± 6.7 mL (range, 6-25 mL) and 0.5 ± 0.2 mL/cm (range, 0.2-0.8 mL/cm), respectively. The residual cord blood volume per kilogram of infant weight per 30 cm was 17.7 ± 5.5 mL/kg/30 cm (range, 8.9-29.0 mL/kg/30 cm). CONCLUSION: Infants could receive approximately 18 mL/kg of whole blood by one-time milking of 30 cm umbilical cord. With an average hematocrit of 40%, this volume is equivalent to approximately 13 mL of packed red blood cells (hematocrit 55%).

We have investigated the nanosurface-confined anisotropic growth of ordered-ellipsoidal Fe nanogranules when an Fe plume was deposited at a slanting angle onto an anodized aluminum oxide (AAO) film. Layer-by-layer growth was also investigated. This growth is driven by two critical factors: (1) a new rhombic AAO cell and (2) the slanting deposition of the Fe plume. During slanting deposition, the rhombic AAO cell induces strong restrictions in the nucleation site, growth direction, and granular size; therefore, the degree of freedom during growth is restricted. The magnetic dipoles of the ordered Fe nanogranules are placed along the long axis of the ellipsoid at an angle of 180 degrees (antiparallel) due to the demagnetizing field, shape anisotropy, and magnetic dipole-to-dipole interactions.

A new cyclic diarylheptanoid, acerogenin M (1), has been isolated along with nine known diarylheptanoids, 2-10, and two known phenolic compounds, 11 and 12, from a MeOH extract of the stem bark of Acer nikoense MAXIM. (Aceraceae). The structure of 1 was determined on the basis of a spectroscopic method. Upon evaluation of the inhibitory effects on 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced inflammation (1 microg/ear) in mice of nine of the compounds (2-6, 8, 10-12), six (2, 4-6, 8, 10) showed a marked anti-inflammatory effect with a 50% inhibitory dose (ID(50)) of 0.26-0.81 mg per ear. In addition, upon an evaluation against the Epstein-Barr virus early antigen (EBV-EA) activation induced by TPA for all of the compounds, all exhibited moderate inhibitory effects against EBV-EA induction (IC(50) values of 356-534 mol ratio/32 pmol TPA).

Melanogenesis inhibitory and free radical scavenging activities of nine cyclic (1-9) and one acyclic diarylheptanoids (10), and two phenolic compounds, (+)-rhododendrol (11) and (+)-catechin (12), isolated from the ethyl acetate-soluble fraction of the MeOH extract of the bark of Acer nikoense MAXIM. (Aceraceae) were examined. Upon evaluation of compounds 1-12 on the melanogenesis in the B16 melanoma cells, two compounds, 2 and 8, exhibited marked inhibitory activity with 55.6% and 46.8% reduction, respectively, of melanin content at 25 microg/ml without inhibition of cell proliferation. In addition, upon an evaluation of eleven compounds, 1-7 and 9-12 against the scavenging activities of free radicals (against the 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical), compound 12 (IC(50) 9.0 microgM) followed by compounds 1, 3, 4, and 6 (IC(50) 40.2-44.0 microgM) showed potent scavenging activities.

Sixteen triterpene acids, viz., five of the oleanane-type (1-5), nine of the ursane-type (6-14), and two of the lupane-type (15, 16), were isolated and identified from the ethyl acetate-soluble fraction of the methanol extract of the leaves of loquat, Eriobotrya japonica LINDL. (Rosaceae). Twelve of these compounds, 1-4, 6, 8-13, and 15, were evaluated for their inhibitory effects on 12-O-tetradecanoylphorbol-13-acetate (TPA)-induced inflammation (1 microg/ear) in mice. All the compounds tested showed a marked anti-inflammatory effect, with a 50% inhibitory dose (ID50) of 0.03-0.43 mg per ear. In addition, an evaluation against the Epstein-Barr virus early antigen (EBV-EA) activation induced by TPA for all of the compounds, 12 and 13 showed potent inhibitory effects on EBV-EA induction. Furthermore, euscaphic acid (12) exhibited marked antitumor-promoting activity in an in vivo two-stage carcinogenesis test of mouse tumor by using 7,12-dimethylbenz[a]anthracene (DMBA) as an initiator and TPA as a promoter.
