RESUMEN

Ultrafast x-ray photoelectron diffraction (UXPD) for free molecules has a promising potential to probe the local structures of the molecules in an element-specific fashion. Our UXPD scheme consists of three steps: (1) near-infrared laser (NIR) with ns pulse duration aligns sample molecules, (2) ultra-violet laser with fs pulse duration pumps the aligned molecules, and (3) soft x-ray free-electron laser (SXFEL) with fs pulse duration probes the molecules by measuring x-ray photoelectron diffraction (XPD) profiles. Employing steps of (1) and (3), we have measured I 3d XPD profiles from ground state iodobenzene aligned by the NIR laser with the SXFEL. Then, we have intensively calculated I 3d XPD profiles with density functional theory, taking degrees of alignments of the molecules into account, to extract a distance between C and I atoms in iodobenzene from the experimental I 3d XPD profiles. Although we have failed to determine the distance from the comparison between the experimental and theoretical results, we have succeeded in concluding that the degeneracies of the initial state eliminate the sensitivity on molecular structure in the I 3d XPD profiles. Thus, the observation of fine structures in the XPD profiles could be expected, if a nondegenerate molecular orbital is selected for a probe of UXPD. Finally, we have summarized our criteria to perform UXPD successfully: (1) to use SXFEL, (2) to prepare sample molecules with the degree of alignment higher than 0.8, and (3) to select a photoemission process from a nondegenerate inner-shell orbital of sample molecules.

Passage of peripheral blood lymphocytes through a column of hydroxyapatite resulted in a 7-20-fold depletion of immunoglobulin-bearing cells, a 20-fold depletion of monocytes, and a 1.3-fold enrichment of T cells. The effluent population was virtually devoid of B lymphocyte precursors and monocytes, whereas helper cell and suppressor cell populations remained intact. This method will facilitate the rapid preparation of T-enriched cell populations.

RESUMEN

Analysis of the staining of cholera enterotoxin on the surface of cells with specific antibodies against each subunit of cholera enterotoxin, using a fluorescence-activated cell sorter and electron microscopy, showed that not only subunit A but also subunit B penetrates the cell membrane. The detection of subunits inside the cell was facilitated by the use of saponin, an agent that increases membrane permeability.

The purification of the osteoblast-like cells (2-3%) among the bone marrow cells (BMC) of C57BL/6 mice using a specific anti-osteoblast serum and a fluorescence-activated cell sorter is described. The antiserum was raised against osteoblast cells isolated from calvaria from neonatal mice. The majority of the cells of the osteoblast-enriched fraction from bone marrow showed a parathormone-induced increase in cyclic adenine monophosphate but no response to calcitonin. This is similar to the response of osteoblast cells obtained from the calvaria. Electron microscopic studies of the extracellular matrix of cultured osteoblast-like cells purified from bone marrow showed the deposition of apatite crystals within and in close apposition to the vesicles. These findings suggest that the isolated cell population was enriched in osteoblasts. Such a cell system from bone marrow might provide an experimental system for investigating the mechanism of bone formation.

To characterize interleukin (IL)-5-induced eosinophils, we examined the expression of CD44, very late antigen (VLA)-4, and the IL-5 receptor alpha chain, as well as the levels of eosinophil peroxidase and the generation of superoxide. Eosinophils were prepared from IL-5-transgenic mice, then characterized using electron microscopy to determine their responses to stimuli. Whereas CD44 densities remained almost constant, the level of VLA-4 increased in parallel with eosinophil maturation. Although a subset of IL-5-induced eosinophils with high side scatter recovered from bone marrow and rare ones found in blood recognized hyaluronic acid (HA), most did not have this property. Bone marrow eosinophils with high side scatter and lower density contained eosinophil peroxidase, not only in granules, but also in membranous structures for 30% of this population. This population developed HA-binding ability in response to IL-3, IL-4, IL-5, granulocyte-macrophage colony-stimulating factor, macrophage inflammatory protein (MIP)-2, monocyte chemotactic protein (MCP)-1, eotaxin, nerve growth factor (NGF), and opsonized zymosan (OZ). Peripheral blood eosinophils acquired HA-binding ability in response to the same stimuli, but their responses were less than those of bone marrow eosinophils with high levels of side scatter. However, splenic eosinophils did not respond to these stimuli. Although peripheral blood eosinophils did not proliferate when stimulated by IL-5, these were the only cells that released eosinophil peroxidase in response to IL-4, MIP-2, MCP-1, eotaxin, NGF, and OZ. With the exception of a subset of bone marrow eosinophils, the ability to acquire HA binding, but not the ability to generate superoxide, correlated with eosinophil peroxidase activity and major basic protein accumulation in the granules of maturing cells.

The undifferentiated carcinoma cell line (HMG) was established from a nude mouse tumor which had been produced by transplantation of a intraperitoneal tumor of 27-year-old woman. The HMG cell line has the following biological properties. 1. The HMG cells are round to oval in shape and grow as floating cell aggregates like a rouleau or a cluster of grapes. 2. 100 passages have been carried out over a year, and the population doubling time is about 17 hrs. 3. In the original tumor, keratin and vimentin were expressed simultaneously, in HMG cells, however, only localization of vimentin was confirmed. 4. By chromosomal analysis, over 90% of the cells revealed 46, XX, with no karyological abnormalities, at passage 82. 5. When heterotransplanted into the subcutis of a nude mouse, HMG cells produced a undifferentiated carcinoma resembling the original tumor.

The cell content of the peritoneal exudate was examined 4 days after an intraperitoneal injection of glycogen in uninfected and Paragonimus westermani-infected guinea pigs. In uninfected animals a reduction in macrophage count and an accumulation of granulocytes in the exudate were observed at 3 and 6 hr after an intraperitoneal injection of purified neutral thiol protease from P. westermani metacercariae. No such effect occurred after the enzyme was injected into infected animals. At 9 hr after enzyme injection, vacuoles were found in the cytoplasm of macrophages in uninfected animals.

Serum levels of six tumor markers (carcinoembryonic antigen (CEA), squamous cell carcinoma antigen (SCCA), immunosuppressive acidic protein (IAP), alpha-fetoprotein (AFP), ferritin (FER), and carbohydrate antigen 19-9 (CA 19-9)) were simultaneously measured in 29 patients with primary squamous cell carcinoma (SCC) of the oral cavity to determine their significance. The positive rates were 34.5% for CEA, 41.4% for SCCA, 51.7% for IAP, 0% for AFP, 10.3% for FER, and 6.9% for CA 19-9 in patients with oral SCC. Therefore, CEA, SCCA, and IAP levels, of which the positive rates were significantly different (P < 0.01) from those of control patients without oral cancer, were considered to be of diagnostic value. The sensitivity (69.0%) and accuracy (90.3%) of the combination assay with these three tumor markers proved to be higher than those obtained with individual markers. A combination assay with CEA, SCCA, and IAP could be useful for the screening of patients with oral cancer.

In order to assist in the analysis of working postures which might cause lumbago in the field of industrial health, a system was developed for the synchronous recording and analyzing of postures, work content and physiological data. The system is composed of a portable unit for recording 3 channels of goniometers, 1 channel of inclinometer and 1 channel of surface electromyogram, a video camera for recording work content, a host computer and some peripheral devices for analyzing the data from the portable unit and video camera. In this system, postures are automatically classified from data on joint angles and upper body inclination angle by using a discriminant function. The joint angles are measured by the goniometers using rubber optical fibers. The angle of upper body inclination is obtained by the inclinometer using a magnetic resistance sensor. In addition to the work content and video images, the postures and the electromyogram can be analyzed and confirmed by simultaneous display of the data on the host computer screen. Based on the trial use of this system in a work model of manual baggage handling, it was proved useful for evaluating in detail the workload caused by working postures.

Fifteen Japanese patients with Wegener's granulomatosis (WG) were evaluated according to their lymphocyte subset abnormalities. Two colour immunofluorescent flow cytometry was used to distinguish the lymphocyte subset alterations. WG group showed a decrease in the percentage of CD4+ cells and the increase of CD8+ cells. Within the NK cell family, the functionally unidentified CD8+57+ cells were markedly elevated. The disproportion of the lymphocyte subsets (CD4+ decreases CD8+57+ increases) were similar to those of Acquired Immunodeficiency Syndrome (AIDS) and AIDS relating complexes (ARC). To assess silent infection of Human immunodeficiency virus (HIV), the polymerase chain reaction (PCR) was done for all patients to selectively amplify specific HIV proviral DNA sequences in peripheral blood mononuclear cells, HIV-1 proviral DNA was not found in any patients but these changes of WG might suggest a possible adaptive response to unknown viral infection.

Cordyceps ophioglossoides is one of the Japanese old 'Kanpoh' drugs used for metrorrhagia as a decoction. We found that CO-N, a galactosaminoglycan from Cordyceps ophioglossoides, reacted with sera from patients with some collagen diseases. By using CO-N, we made a new serological activity index (CO-N reaction). In the present study, we investigated CO-N reaction on patients with Wegener's granulomatosis (WG). The aggregation titres of CO-N (CO-N numbers) displayed a possible correlation with their clinical activity. CO-N reaction might also serve an important role in supporting the diagnosis of active WG and in helping to assess the degree of disease activity. The purpose of this report is to introduce this new serological activity index for Wegener's granulomatosis.

RESUMEN

Most patients have no response to injectable collagen or silicone, but some cases may have positive or 'undersea' (= clinically negative but immunologically positive) response to collagen. From the results of the Macrophage migration inhibition test, the relative immunogenicity was augmented most when we used implants with the following combination. The first immunization was collagen and the second one was collagen with silicone. The augmented antigenicity might be enough to cause an allergic reaction to the patients who had no response to each implant alone.

The adsorption and chromatographic properties of hydroxyapatite sorbents for application to different viruses have been investigated. The strong adsorption of viruses was observed on macroporous hydroxyapatite with hydrophilic properties of the sorbent surface. The viruses were purified on this sorbent without loss of biological activity. The column can be used for virus vaccine production.

RESUMEN

Methods of allocating nursing staff in hospitals should be established that improve the "sufficiency rate" of existing care needs for nursing in patients. The conception of a support system for the optimisation of organising nursing staff was proposed so that the degree of meeting care needs could be maximised. In this study, the structure of care needs was defined by sampling information related to eight factors. A basic framework was developed for a support system for the optimisation of organising nursing staff by using data based on (1) rate of satisfying eight selected criteria, (2) condition factors that can affect this rate, and (3) organising nursing. The possible usage of this support system is also discussed in this paper.

We proposed a prediction methodology for the incidence of infectious diseases using incidence data on measles and influenza for forty years in Japan. We also proposed a diagram that makes it possible to convey information on infectious disease incidence more attractively to a wider audience. This can be a useful tool for health promotion in the community. The obtained results are as follows: 1. It was advantageous to use data transformed by logarithm in statistical analysis of infectious disease incidence. 2. The incidences of measles and influenza exhibited strong seasonality. Measles was most frequent in June and influenza in February. 3. Long-term trends were extracted from the derived data obtained by eliminating seasonal effects from the original data. For measles, a decline was accelerated by the introduction of vaccination program in 1978. Influenza also showed a decline for these thirty years. 4. The observed incidence data were quite well predicted by only the trend and the seasonality. The squares of multiple regression coefficients of measles and influenza were 0.84 and 0.58, respectively. The analysis of the residuals suggested there was a possibility of improvement in prediction. 5. The improvement in prediction was attained by incorporating an autoregressive component of the residuals. As a result, the squares of multiple correlation coefficients of measles and influenza increased to 0.97 and to 0.79, respectively. 6. We finally proposed the TS-decomposition diagram to facilitate practical use of incidence data. In this diagram, current incidence data and predicted values for the near future are plotted on the plane where the trend and the seasonality are superimposed.(ABSTRACT TRUNCATED AT 250 WORDS)

Understanding the structure and problems of communication between physicians and patients should be achieved before introducing a medical information network system for community health. To this end a questionnaire survey was performed on 2,362 patients who saw physicians in medical facilities in a certain district of Hiroshima city. As a result, it was revealed that 'explanations given by doctors' and 'explanatory ability of patients' were mutually closely related and formed a basis for other aspects of communication and patient behavior. Satisfaction with the explanation given by physicians declined with examination time of less than 5 minutes and examination in hospitals rather than in clinics. Explanatory ability of patients decreased as age of patients increased and showed a remarkable decrease in patients of more than 80 years old. This suggested the existence of risk that information necessary for safe examinations may not be transmitted to physicians. These results pointed out the importance of a supporting system of communication especially for the elderly patient.

Recent developments in computer and communication technology were studied in relation to medical information network systems, using computers and IC cards, to solve problems in community health. Trial use of personal computer network systems among physicians and IC card systems for personal health data management are already in existence in some parts of Japan. These trials were studied and analyzed based on a questionnaire survey of physicians and patients. Results of the study produced some useful points that should be considered when introducing these systems. These included: 1) details on expectations and specifications for these systems by physicians and patients, 2) easy access to valuable information is a key point for active network systems among physicians, 3) plausibility of improvement of communication between physicians and patients by using these systems, 4) recognition that an important problem concerns patient information privacy and must be considered before introducing these systems. A study of practical merits of these systems and methodology for realization indicates that participation by active and attractive providers of information can be expected to stimulate frequent use of the network system. The cost of introducing these systems can be partially borne by eliminating the large investment now allocated for processing requests for reimbursement of medical services. Investigation into the introduction of medical information systems provides a good opportunity to elucidate problems in the present medical systems.

Participation rates of atomic bomb survivors in health examinations were analyzed with a multiple logistic regression model. Rates were obtained for each stratum by sex, age group and residential ward. The effect of residential wards was considered in relation to the social indices of the wards. The effects of sex, age group and residential ward on participation rates were as follows: (1) Females had higher participation rates than males in the working age group (40-64). No sex differences were seen after age 65. (2) The participation rates had a peak around age 70 and were exceptionally high also in the youngest age group (40-44). (3) Participation rates were higher in suburban wards than in central wards. The participation rates had a negative correlation with urban characteristics while they had a positive correlation with suburban characteristics. In particular they had significant negative correlation to the number of clinics and positive correlation with voter participation rates in elections. These results suggest the need for a new type of urban health examination and reflection of the general attitude of the residents to the government.

General practitioners (GPs) play an important part in community health care. Their opinion regarding introduction of a medical information network system using a personal computer and factors influencing the decision to introduce such a system were surveyed among 977 randomly sampled GPs. A response from 727 GPs was obtained. Based on a two stage selection method, 20 factors were selected as effective factors among the 73 factors surveyed, with the following results: (1) Responses to the question concerning the approval of system introduction were classified into three groups: "not necessary + too early to make a decision (DA)" (DA group), "approve (A)" (A group) and "undecided (UD)" (UD group). Each group had nearly equal numbers of respondees. (2) Significant effective factors were "experience with personal computers", "a desire to use a personal computer", "a desire to use a computer-diagnosis-system", "cost", and "a volition to operate a computer by himself". (3) The "A group" had high experience and affirmative opinions, while the "DA group" had low experience and negative opinions. The "UD group" also tended to respond with "do not know" regarding the factors. (4) Approval of system introduction requires providing basic knowledge on computers, opportunities for positive experience with computers, and practical examples of problem solutions in a general practitioner's work.
