RESUMEN

Gallbladder cancer is a rare but fatal malignancy. However, the mechanisms underlying gallbladder carcinogenesis and its progression are poorly understood. The function of m6A modification and its regulators was still unclear for gallbladder cancer. The current study seeks to investigate the function of YTH m6A RNA-binding protein 1 (YTHDF1) in gallbladder cancer. Transcriptomic analysis and immunochemical staining of YTHDF1 in gallbladder cancer tissues revealed its upregulation compared to paracancerous tissues. Moreover, YTHDF1 promotes the proliferation assays, Transwell migration assays, and Transwell invasion assays of gallbladder cancer cells in vitro. And it also increased tumour growth in xenograft mouse model and metastases in tail vein injection model in vivo. In vitro, UHRF1 knockdown partly reversed the effects of YTHDF1 overexpression. Mechanistically, dual-luciferase assays proved that YTHDF1 promotes UHRF1 expression via direct binding to the mRNA 3'-UTR in a m6A-dependent manner. Overexpression of YTHDF1 enhanced UHRF1 mRNA stability, as demonstrated by mRNA stability assays, and Co-IP studies confirmed a direct interaction between YTHDF1 and PABPC1. Collectively, these findings provide new insights into the progression of gallbladder cancer as well as a novel post-transcriptional mechanism of YTHDF1 via stabilizing target mRNA.

This Letter reports a new, to the best of our knowledge, high-frequency surface-micromachined optical ultrasound transducer (HF-SMOUT) array for micro photoacoustic computed tomography (µPACT). An 11 × 11 mm2 2D array of 220 × 220 elements (35 µm in diameter) is designed, fabricated, and characterized. The optical resonance wavelength (ORW) of ≥90% of the elements falls within a 6-nm range. The acoustic center frequency and bandwidth of the elements are ∼14 MHz and ∼18 MHz (129%), respectively. The noise equivalent pressure (NEP) is 161 Pa (or 18 m P a/H z) within a measurement bandwidth of 5-75 MHz. The standard deviation of the ORW drift is 0.45 nm and 0.93 nm within 25°C-55°C, respectively, and during a seven-day continuous water immersion. PACT experiments are conducted to evaluate the imaging performances of the HF-SMOUT array. The spatial resolution is estimated as 90 µm (axial) and 250-750 µm (lateral) within a 10 × 10 mm2 field of view (FoV) and the imaging depth of 16 mm. A 3D PA image of a knotted black hair target is also successfully acquired. These results demonstrate the feasibility of using the HF-SMOUT array for µPACT applications.

RESUMEN

Quercetin (QR) is a natural flavonoid with strong anti-inflammatory properties, but it suffers from poor water solubility and bioavailability. Micro/nanomotors (NMs) are tiny devices that convert external energy or chemical fuels into an autonomous motion. They are characterized by their small size, rapid movement, and self-assembly capabilities, which can enhance the delivery of bioactive ingredients. The study synthesized natural polysaccharide-based nanotubes (NTs) using a layer-by-layer self-assembly method and combined with urease (Ure), glucose oxidase (GOx), and Fe3O4 to create three types of NMs. These NMs were well-dispersed and biocompatible. In vitro experiments showed that NMs-Fe3O4 has excellent photothermal conversion properties and potential for use in photothermal therapy. Cellular inflammation model results demonstrated that QR-loaded NMs were not only structurally stable but also improved bioavailability and effectively inhibited the release of inflammatory mediators such as IL-1ß and IL-6, providing a safe and advanced carrier system for the effective use of bioactive components in food.

INTRODUCTION: Early gastric cancer with current Helicobacter pylori infection (HpC-EGC) is common, but it is still unclear whether H. pylori eradication therapy (Hp-ET) or endoscopic submucosal dissection (ESD) should be performed first. We evaluated Hp-ETs short-term effects on horizontal boundary delineations of HpC-EGC in ESD. METHODS: Prospectively enrolled HpC-EGC patients were randomly assigned to eradication or control groups. Operation scopes of HpC-EGC lesions were delineated with marking dots at 5 mm out of the endoscopic demarcation line by an independent endoscopist, unaware of eradication status, before formal circumferential incision. As representatives, precise delineation rate, the shortest distance of all marking dots to the pathological demarcation line in all slices of one intact resected specimen (Dmin), and negative marking dot specimen rate were examined. RESULTS: Twenty-three HpC-EGC patients (25 lesions) were allocated to eradication group and 26 patients (27 lesions) were allocated to the control group with similar eradication success rates and all were differentiated type. With improving background mucosa inflammation after Hp-ET and similar gastritis-like epithelium rates, 10 lesions (40.0%) in the eradication group were of precise delineation compared to control group with 2 lesions (7.4%) (relative risk = 5.40, 95% CI 1.31-22.28). Dmin of eradication and control groups were 4.17 ± 2.52 mm and 2.67 ± 2.30 mm (p = 0.029), accompanied by 4 (14.8%) and none (0.0%) specimens that exhibited positive marking dots (p = 0.11), respectively. CONCLUSION: For HpC-EGC patients, administrating eradication medication before ESD is beneficial for the precise delineation of lesions and reducing the risk of positive horizontal resection margins.

BACKGROUND: Older adults occasionally receive seizure prophylaxis in an acute ischemic stroke (AIS) setting, despite safety concerns. There are no trial data available about the net impact of early seizure prophylaxis on post-AIS survival. METHODS: Using a stroke registry (American Heart Association's Get With The Guidelines) individually linked to electronic health records, we examined the effect of initiating seizure prophylaxis (ie, epilepsy-specific antiseizure drugs) within 7 days of an AIS admission versus not initiating in patients ≥65 years admitted for a new, nonsevere AIS (National Institutes of Health Stroke Severity score ≤20) between 2014 and 2021 with no recorded use of epilepsy-specific antiseizure drugs in the previous 3 months. We addressed confounding by using inverse-probability weights. We performed standardization accounting for pertinent clinical and health care factors (eg, National Institutes of Health Stroke Severity scale, prescription counts, seizure-like events). RESULTS: The study sample included 151 patients who received antiseizure drugs and 3020 who did not. The crude 30-day mortality risks were 219 deaths per 1000 patients among epilepsy-specific antiseizure drugs initiators and 120 deaths per 1000 among noninitiators. After standardization, the estimated mortality was 251 (95% CI, 190-307) deaths per 1000 among initiators and 120 (95% CI, 86-144) deaths per 1000 among noninitiators, corresponding to a risk difference of 131 (95% CI, 65-200) excess deaths per 1000 patients. In the prespecified subgroup analyses, the risk difference was 52 (95% CI, 11-72) among patients with minor AIS and 138 (95% CI, 52-222) among moderate-to-severe AIS patients. Similarly, the risk differences were 86 (95% CI, 18-118) and 157 (95% CI, 57-219) among patients aged 65 to 74 years and ≥75 years, respectively. CONCLUSIONS: There was a higher risk of 30-day mortality associated with initiating versus not initiating seizure prophylaxis within 7 days post-AIS. This study does not support the role of seizure prophylaxis in reducing 30-day poststroke mortality.

This Letter reports the integration of microlenses (MLs) on a surface-micromachined optical ultrasound transducer (SMOUT) array to enable parallel ultrasound data readout from a multiplicity of elements. The MLs are fabricated by photoresist patterning and reflow, and their focal lengths are optimized with parametric studies. Experiments are conducted to characterize the acoustic responsivity and its uniformity of the SMOUT-ML elements under different conditions. The temporal stability of SMOUT-ML elements immersed in water is assessed by monitoring their acoustic response continuously for 1 week. Parallel ultrasound signal readout is simulated with a small group of SMOUT-ML elements. Experimental results show that high acoustic sensitivity and excellent long-term stability can be achieved by the ML-integrated SMOUT array, which could provide a promising approach for enabling parallel ultrasound data acquisition for improving the imaging speed of 3D acoustic tomography.

Creating micrometer-resolution high-aspect-ratio three-dimensional (3D) structures remain very challenging despite significant microfabrication methods developed for microelectromechanical systems (MEMS). This is especially the case when such structures are desired to be metallic to support electronic applications. Here, we present a microfabrication process that combines two-photon-polymerization (2PP) printing to create a polymeric high-aspect-ratio three-dimensional structure and electroless metal plating that selectively electroplates only the polymeric structure to create high-aspect-ratio 3D metallic structures having micrometer-resolution. To enable this, the effect of various 2PP processing parameters on SU-8 photoresist microstructures were first systematically studied. These parameters include laser power, slicing/hatching distances, and pre-/post-baking temperature. This optimization resulted in a maximum aspect ratio (height to width) of ~ 12. Following this polymeric structure printing, electroless plating using Tollens' Reagent were utilized to selectively coat silver particles only on the polymeric structure, but not on the silicon substrate. The final 3D metallic structures were evaluated in terms of their resistivity, reproducibly showing resistivity of ~ 10-6 [Ω·m]. The developed 3D metallic structure microfabrication process can be further integrated with conventional 2D lithography to achieve even more complex structures. The developed method overcomes the limitations of current MEMS fabrication processes, allowing a variety of previously impossible metallic microstructures to be created.

BACKGROUND/OBJECTIVES: As the most frequent functional pancreatic neuroendocrine tumor, insulinomas may cause a plethora of symptoms and severe impairment in the living of patients by endogenous hyperinsulinemia and subsequent hypoglycemia. Surgery has been regarded as the first choice although a high risk of complications. Ethanol ablation is a promising non-surgical option that could achieve tumor shrinking in a short-term period. But the impact of symptom control and the long-term efficacy lack sufficient and good-quality evidence. METHODS: A total number of 14 endoscopic ultrasonography-guided ethanol ablations were performed in 9 patients between September 2016 and September 2018 in Peking Union Medical College Hospital. The data were collected and prospectively analyzed. RESULTS: The follow-up duration ranged from 21 to 1567 days in 9 patients, with a median of 994 days. 4 patients were free from relapse during a median follow-up of 1108 days (range: 994-1567 days). In 5 patients who suffered relapses, the median duration with symptom relief after the first ablation was 128 days (range: 13-393 days). If only repeated ablation was taken into consideration, the median duration with symptom relief was 26 days (range: 1-516 days). No complications happened during the procedures. The severe complication rate after the first ablation was 0.0% (0/9), compared to 7.14% (1/14) if each procedure was counted separately. The only severe complication documented was acute pancreatitis which was completely relieved after symptomatic treatment. CONCLUSIONS: For patients who are not suitable for surgical resections, endoscopic ultrasonography-guided ethanol ablation of insulinomas could be an effective and safe alternative to relieve symptoms of hypoglycemia.

RESUMEN

Innate lymphoid cells (ILCs) are a group of innate immune cells that have garnered considerable attention due to their critical roles in regulating immunity and tissue homeostasis. They are particularly abundant in the gastrointestinal tract, where they have been shown to interact with commensal bacteria, pathogens, and other components of the local microenvironment to influence host immune responses to infection and oncogenesis. Their tissue-residency properties enable gastric ILCs a localized and rapid response to alert and stress, which indicates their key potential in regulating immunosurveillance. In this review, we discuss the current understanding of the role of ILCs in the gastric mucosa, with a focus on their interactions with the gastric microbiota and Helicobacter pylori and their contributions to tissue homeostasis and inflammation. We also highlight recent findings on the involvement of ILCs in the pathogenesis of gastric cancer and the implications of targeting ILCs as a therapeutic approach. Overall, this review provides an overview of the diverse functions of ILCs in gastric mucosa and highlights their potential as targets for future therapies for gastric cancer.

This paper reports a new 2D surface-micromachined optical ultrasound transducer (SMOUT) array consisting of 350 × 350 elements with highly uniform optical and acoustic performances. Each SMOUT element consists of a vacuum-sealed Fabry-Perot (F-P) interferometric cavity formed by two parallel partially reflective distributed Bragg reflectors (DBRs). Optical mapping in the 4 cm × 4 cm center region of the SMOUT array shows that the optical resonance wavelength (ORW) of > 94% of the elements falls within a narrow range of ≤ 10 nm. The center frequency, acoustic bandwidth and noise equivalent pressure (NEP) of the elements are determined to be 5 MHz, 5 MHz, and 20.7 Pa (with 16 times of signal averaging) or 172.5 Pa (without averaging) over a bandwidth of 10 MHz, respectively. The temperature and temporal stability of the SMOUT elements is also tested, which shows there is little variation in their ORW under large ambient temperature fluctuation and during continuous water immersion. To demonstrate its imaging capability, 2D and 3D PACT based on the SMOUT array is also conducted within a 3 cm × 3 cm field of view (FOV) at a depth of 3cm with no interrogation wavelength tuning. These results show that the SMOUT array could overcome some of the major limitations in existing ultrasound transducer arrays for PACT and provide a promising solution for achieving high-speed 3D imaging.

OBJECTIVE: Administrative databases seldom include detailed clinical variables and vital status, limiting the scope of population-based studies. We demonstrate a comprehensive process for integrating 3 databases (all-payor inpatient hospitalizations, clinical acute stroke registry and vital statistics) into a single statewide ischemic stroke database. MATERIALS AND METHODS: The 3 Massachusetts databases spanned 2007-2017. Our integration process was composed of 3 phases: 1) hospitalizations-registry linkage, 2) hospitalizations-vital linkage, and 3) final integration of all 3 databases. Following data uniqueness assessment, rule-based deterministic linkage on indirect identifiers were applied in the first two phases. We validated the linkages by comparing additional patient variables not used in the linkage process in the absence of a gold standard database crosswalk. RESULTS: During the overlapping period from 1/1/2008 to 9/30/2015, there were 47,713 stroke admissions in the hospitalizations database and 43,487 admissions in the registry. We linked 38,493 (80.7%) of cases, 95% of which were validated. There were 391,176 deaths reported in Massachusetts between 1/1/2010 and 3/6/2017 in the vital database. Of the 38,493 encounters in the hospitalizations-registry linked data, 10,660 (27.7%) were linked to deaths, reflecting the cumulative mortality over the 7-year period among all registry-linked ischemic stroke hospitalization records. CONCLUSION: We demonstrate that a high-quality integration of the statewide hospitalizations, clinical registry, and vital statistics databases is achievable leveraging indirect identifiers. This data integration framework takes advantage of rich clinical data in registries and long term outcomes from hospitalizations and vital records and may have value for larger scale outcomes research.

BACKGROUND: Trehalose, an intracellular protective agent reported to mediate defense against many stresses, can alleviate high-temperature-induced damage in Pleurotus ostreatus. In this study, the mechanism by which trehalose relieves heat stress was explored by the addition of exogenous trehalose and the use of trehalose-6-phosphate synthase 1 (tps1) overexpression transformants. RESULTS: The results suggested that treatment with exogenous trehalose or overexpression of tps1 alleviated the accumulation of lactic acid under heat stress and downregulated the expression of the phosphofructokinase (pfk) and pyruvate kinase (pk) genes, suggesting an ameliorative effect of trehalose on the enhanced glycolysis in P. ostreatus under heat stress. However, the upregulation of hexokinase (hk) gene expression by trehalose indicated the involvement of the pentose phosphate pathway (PPP) in heat stress resistance. Moreover, treatment with exogenous trehalose or overexpression of tps1 increased the gene expression level and enzymatic activity of glucose-6-phosphate dehydrogenase (g6pdh) and increased the production of both the reduced form of nicotinamide adenine dinucleotide phosphate (NADPH) and glutathione (GSH), confirming the effect of trehalose on alleviating oxidative damage by enhancing PPP in P. ostreatus under heat stress. Furthermore, treatment with exogenous trehalose or overexpression of tps1 ameliorated the decrease in the oxygen consumption rate (OCR) caused by heat stress, suggesting a relationship between trehalose and mitochondrial function under heat stress. CONCLUSIONS: Trehalose alleviates high-temperature stress in P. ostreatus by inhibiting glycolysis and stimulating PPP activity. This study may provide further insights into the heat stress defense mechanism of trehalose in edible fungi from the perspective of intracellular metabolism.

PURPOSE: To examine indications for, duration of use, and rate of adverse drug events (ADE) attributable to anticonvulsant initiation, as adjudicated by expert review of electronic health records (EHR) of older adults. METHODS: We identified a cohort of community dwelling Medicare beneficiaries with linked EHR (aged 65+, continuously enrolled with a large health system/until death between 2012 and 2014, n = 20 945) and drew a stratified EHR review sample (n = 1534). An expert reviewed all records to adjudicate anticonvulsant use, years of use, indication for use, and evidence of ADEs attributable to anticonvulsant initiation. After excluding patients with insufficient EHR data (n = 37; 2%), we reconstructed the cohort using inverse probability weights to resemble the original cohort of eligible beneficiaries (n = 20 380). Among incident users of a single anticonvulsant, we estimated the rate of ADEs and described the type and severity of ADEs. RESULTS: Overall, 12% (n = 2469) of eligible beneficiaries used at least one anticonvulsant in the 2012 to 2014 period (4% [n = 757] incident users, 8% [n = 1712] prevalent users). Incident users were most frequently prescribed gabapentin (n = 461/757, 61%), benzodiazepines (n = 122/757, 16%), and levetiracetam (n = 74/757, 10%); the most common indication was pain relief (n = 214; 28%) followed by epilepsy (n = 53; 7%). Among incident users, the overall ADE rate was 10/100 person-years (95% CI 4-20/100 person-years), of which 29% (n = 28/97) were life threatening (eg, somnolence). Most ADEs among incident monotherapy users were nervous system related (68%, n = 66/97). CONCLUSION: Many older adult community dwelling traditional Medicare beneficiaries had clinically significant ADEs likely attributable to the initiation of anticonvulsant therapy, which was begun for a range of indications.

High temperature is a major threat to Pleurotus ostreatus cultivation. In this study, a potential mechanism by which P. ostreatus mycelia growth is inhibited under heat stress was explored. Lactate, as a microbial fermentation product, was found unexpectedly in the mycelia of P. ostreatus under heat stress, and the time-dependent accumulation and corresponding inhibitory effect of lactate on mycelial growth was further confirmed. The addition of a glycolysis inhibitor, 2-deoxy-D-glucose (2DG), reduced the lactate content in mycelia and slightly restored mycelial growth under high-temperature conditions, which indicated the accumulation of lactate can be inhibited by glycolysis inhibition. Further data revealed mitochondrial dysfunction under high-temperature conditions, with evidence of decreased oxygen consumption and adenosine triphosphate (ATP) synthesis and increased reactive oxygen species (ROS). The removal of ROS with ascorbic acid decreased the lactate content, and mycelial growth recovered to a certain extent, indicating lactate accumulation could be affected by the mitochondrial ROS. Moreover, metabolic data showed that glycolysis and the tricarboxylic acid cycle were enhanced. This study reported the accumulation of lactate in P. ostreatus mycelia under heat stress and the inhibitory effect of lactate on the growth of mycelia, which might provide further insights into the stress response mechanism of edible fungi. Key Points • Lactate can accumulate in Pleurotus ostreatus mycelia under heat stress and inhibit its growth. • The accumulation of lactate may be due to the acceleration of glycolysis and the dysfunction of mitochondria of P. ostreatus mycelia under high-temperature stress. • The glycolysis and tricarboxylic acid cycle of P. ostreatus mycelia were accelerated under high-temperature stress.

RESUMEN

The coal pulverizing system is an important auxiliary system in thermal power generation systems. The working condition of a coal pulverizing system may directly affect the safety and economy of power generation. Prognostics and health management is an effective approach to ensure the reliability of coal pulverizing systems. As the coal pulverizing system is a typical dynamic and nonlinear high-dimensional system, it is difficult to construct accurate mathematical models used for anomaly detection. In this paper, a novel data-driven integrated framework for anomaly detection of the coal pulverizing system is proposed. A neural network model based on gated recurrent unit (GRU) networks, a type of recurrent neural network (RNN), is constructed to describe the temporal characteristics of high-dimensional data and predict the system condition value. Then, aiming at the prediction error, a novel unsupervised clustering algorithm for anomaly detection is proposed. The proposed framework is validated by a real case study from an industrial coal pulverizing system. The results show that the proposed framework can detect the anomaly successfully.

A fluorometric method is described for the determination of ofloxacin (OFL). It is based on the use of the fluorescent intercalator SYBR Green I (SG-I). The OFL-aptamer has G-quadruplex structures and can be recognized by SG-I. It results in strong fluorescence of SG-I. If OFL is present, OFL will bind to its aptamer to form stable complexes. This induces the despiralization of partial dsDNA regions, leads to changes in the structure of the aptamer. Thus, SG-I is released from the OFL-aptamer into solution. Hence, the fluorescence of SG-I drops. Fluorescence decreases linearly in the 1.1 to 200 nM OFL concentration range, and the limit of detection is 0.34 nM. The method shows good selectivity to much interference including analogues, hormones, pesticides. It is also effortless and fast with the times of measurement of <40 min. In addition, good recoveries of 91.3-119.0% were found for tap water, river water and artificial urine spiked with OFL with relative standard deviation (RSD) of ≤11.6%. RESUMEN

We propose and demonstrate a broadly wavelength tunable mode-locked thulium-doped all-fiber laser, delivering sub-400 femtosecond (fs) pulses and tuning from 1867 nm to 2010 nm. The tunable range of 143 nm, to our best knowledge, is the widest in the femtosecond mode-locked thulium-doped fiber lasers (TDFLs). The broadly tunable range and femtosecond pulse oscillation attribute to the large free spectral range and bandwidth of the fiber-based Lyot filter, benefiting from the hybrid device based compact configuration. The hybrid device, integrating wavelength-division-multiplexer, polarization-sensitive isolator and output coupler, effectively diminishes the birefringence and net dispersion.

RESUMEN

We present a mid-infrared (mid-IR) supercontinuum (SC) light source pumped by femtosecond pulses from a thulium doped fiber amplifier (TDFA) at 2 µm. An octave-spanning spectrum from 1.1 to 3.7 µm with an average power of 253 mW has been obtained from a single mode ZBLAN fiber. Spectral flatness of 10 dB over a 1390 nm range has been obtained in the mid-IR region from 1940 - 3330 nm. It is resulted from the enhanced self phase modulation process in femtosecond regime. The all-fiber configuration makes such broadband coherent source a compact candidate for various applications.

Hydrogen is regarded as a kind of clean energy with high caloricity and non-pollution, which has been studied by many experts and scholars home and abroad. Microwave discharge plasma shows light future in the area of hydrogen production from ethanol solution, providing a new way to produce hydrogen. In order to further improve the technology and analyze the mechanism of hydrogen production with microwave discharge in liquid, emission spectrum of hydrogen production by microwave discharge plasma in ethanol solution was being studied. In this paper, plasma was generated on the top of electrode by 2.45 GHz microwave, and the spectral characteristics of hydrogen production from ethanol by microwave discharge in liquid were being studied using emission spectrometer. The results showed that a large number of H, O, OH, CH, C2 and other active particles could be produced in the process of hydrogen production from ethanol by microwave discharge in liquid. The emission spectrum intensity of OH, H, O radicals generated from ethanol is far more than that generated from pure water. Bond of O-H split by more high-energy particles from water molecule was more difficult than that from ethanol molecule, so in the process of hydrogen production by microwave discharge plasma in ethanol solution; the main source of hydrogen was the dehydrogenation and restructuring of ethanol molecules instead of water decomposition. Under the definite external pressure and temperature, the emission spectrum intensity of OH, H, O radicals increased with the increase of microwave power markedly, but the emission spectrum intensity of CH, C2 active particles had the tendency to decrease with the increase of microwave power. It indicated that the number of high energy electrons and active particles high energy electron energy increased as the increase of microwave power, so more CH, C2 active particles were split more thoroughly.
