RESUMEN

The stability of the moving mirror of a Michelson Fourier transform spectrometer (M-FTS) has a non-negligible influence on its spectral quality, which limits its application. We proposed a spectrometer scheme with a pair of rotating parallel mirrors (RPM-FTS), which has advantages of fast response and high stability. The influence of the parallelism error of parallel mirrors on interference was analyzed by establishing a rotation vector model between the parallelism error, rotation angle, and optical path. The modulation depth of the RPM-FTS is more insensitive with the same installation error of the M-FTS; thus, more spectral details can be displayed easily.

OBJECTIVE: Studies have shown that cancer progression of head and neck squamous cell carcinoma (HNSCC) is related with metabolic alterations. The aim of this study is to identify the clinical roles of metabolic alterations in HNSCC. MATERIALS AND METHODS: Metabolism-related genes associated with HNSCC were searched in public databases. A predictive and efficacious LASSO model was fabricated to optimize the diagnosis that was based on these genes. Meantime, ultra-performance liquid chromatography-quadrupole/orbitrap high resolution mass spectrometry (UHPLC-Q-Orbitrap HRMS) was used to compare patients with HNSCC (n = 73) with healthy controls (n = 51) for serum metabolites. Potential biomarkers and alterations in serum metabolites were analysed and evaluated using t test analysis, principal component analysis and orthogonal partial least square-discrimination analysis. RESULTS: Overall, 21 differential metabolites were probed in serum, of which eight metabolites had potential for clinical uses. Transcriptome analysis showed that four genes in the constructed LASSO model were found to be associated with seven differential metabolites. Metabolic pathway analysis by MetaboAnalyst showed that the biomarkers that were related with HNSCC were closely related to four metabolism pathways (p < 0.05). CONCLUSION: To conclude, future research on HNSCC should be directed towards multi-omics to provide treatment, intervention or diagnosis of the disease.

To address the problems of large storage requirements, computational pressure, untimely data supply of off-chip memory, and low computational efficiency during hardware deployment due to the large number of convolutional neural network (CNN) parameters, we developed an innovative hardware-friendly CNN pruning method called KRP, which prunes the convolutional kernel on a row scale. A new retraining method based on LR tracking was used to obtain a CNN model with both a high pruning rate and accuracy. Furthermore, we designed a high-performance convolutional computation module on the FPGA platform to help deploy KRP pruning models. The results of comparative experiments on CNNs such as VGG and ResNet showed that KRP has higher accuracy than most pruning methods. At the same time, the KRP method, together with the GSNQ quantization method developed in our previous study, forms a high-precision hardware-friendly network compression framework that can achieve "lossless" CNN compression with a 27× reduction in network model storage. The results of the comparative experiments on the FPGA showed that the KRP pruning method not only requires much less storage space, but also helps to reduce the on-chip hardware resource consumption by more than half and effectively improves the parallelism of the model in FPGAs with a strong hardware-friendly feature. This study provides more ideas for the application of CNNs in the field of edge computing.

We propose a deep learning method that includes convolution neural network (CNN) and convolutional long short-term memory (ConvLSTM) models to realize atmospheric turbulence compensation and correction of distorted beams. The trained CNN model can automatically obtain the equivalent turbulent compensation phase screen based on the Gaussian beams affected by turbulence and without turbulence. To solve the time delay problem, we use the ConvLSTM model to predict the atmospheric turbulence evolution and acquire a more accurate compensation phase under the Taylor frozen hypothesis. The experimental results show that the distorted Gaussian and vortex beams are effectively and accurately compensated.

To address the problems of convolutional neural networks (CNNs) consuming more hardware resources (such as DSPs and RAMs on FPGAs) and their accuracy, efficiency, and resources being difficult to balance, meaning they cannot meet the requirements of industrial applications, we proposed an innovative low-bit power-of-two quantization method: the global sign-based network quantization (GSNQ). This method involves designing different quantization ranges according to the sign of the weights, which can provide a larger quantization-value range. Combined with the fine-grained and multi-scale global retraining method proposed in this paper, the accuracy loss of low-bit quantization can be effectively reduced. We also proposed a novel convolutional algorithm using shift operations to replace multiplication to help to deploy the GSNQ quantized models on FPGAs. Quantization comparison experiments performed on LeNet-5, AlexNet, VGG-Net, ResNet, and GoogLeNet showed that GSNQ has higher accuracy than most existing methods and achieves "lossless" quantization (i.e., the accuracy of the quantized CNN model is higher than the baseline) at low-bit quantization in most cases. FPGA comparison experiments showed that our convolutional algorithm does not occupy on-chip DSPs, and it also has a low comprehensive occupancy in terms of on-chip LUTs and FFs, which can effectively improve the computational parallelism, and this proves that GSNQ has good hardware-adaptation capability. This study provides theoretical and experimental support for the industrial application of CNNs.

RESUMEN

Motion blur recovery is a common method in the field of remote sensing image processing that can effectively improve the accuracy of detection and recognition. Among the existing motion blur recovery methods, the algorithms based on deep learning do not rely on a priori knowledge and, thus, have better generalizability. However, the existing deep learning algorithms usually suffer from feature misalignment, resulting in a high probability of missing details or errors in the recovered images. This paper proposes an end-to-end generative adversarial network (SDD-GAN) for single-image motion deblurring to address this problem and to optimize the recovery of blurred remote sensing images. Firstly, this paper applies a feature alignment module (FAFM) in the generator to learn the offset between feature maps to adjust the position of each sample in the convolution kernel and to align the feature maps according to the context; secondly, a feature importance selection module is introduced in the generator to adaptively filter the feature maps in the spatial and channel domains, preserving reliable details in the feature maps and improving the performance of the algorithm. In addition, this paper constructs a self-constructed remote sensing dataset (RSDATA) based on the mechanism of image blurring caused by the high-speed orbital motion of satellites. Comparative experiments are conducted on self-built remote sensing datasets and public datasets as well as on real remote sensing blurred images taken by an in-orbit satellite (CX-6(02)). The results show that the algorithm in this paper outperforms the comparison algorithm in terms of both quantitative evaluation and visual effects.

Osteoporosis is a common disease characterized by reduced bone mineral density and impaired bone strength and is currently one of the leading causes of fracture and morbidity among the elderly worldwide. The pathological generation of osteoclasts is an important event in the development of extensive bone resorption. Thus, the development of a drug that targets osteoclasts may be beneficial in treating osteoporosis. Accordingly, in this study, we investigated the effects of senkyunolide H (SNH), an active component extracted from ligusticum chuanxiong Hort, on osteoporosis through a series of in vivo and in vitro experiments. First, we found that SNH had a therapeutic effect in ovariectomized mice by inhibiting osteoclast formation. Then, the inhibitory effect on osteoclast differentiation was confirmed in vitro. Further western blotting analysis revealed that SNH downregulated receptor activator of nuclear factor (NF)-κΒ ligand-induced NF-κB signaling activation, c-Jun N-terminal kinase (JNK) in the mitogen-activated protein kinase and extracellular signal-regulated kinase (ERK) signaling pathway. These data indicated that SNH may be a potential treatment for postmenopausal osteoporosis.

Purpose: The purpose of this study was to describe an approach to cervical brachytherapy for a patient with a complete bicorporeal uterus and locally advanced cervical cancer (LACC). Materials and methods: The patient was a 53-year-old woman with a complete bicorporeal uterus, diagnosed with stage IIB cervical squamous cell carcinoma due to contact bleeding. The patient underwent concurrent chemoradiotherapy (CCRT), external beam pelvic radiotherapy with 45 Gy/25 fractions, and weekly cisplatin (40 mg/m2). Brachytherapy was administered following the completion of external beam radiotherapy. Results: The brachytherapy, which was CT (Computed Tomography)-guided using two CT-compatible tandems and two CT-compatible ovoids, delivered a prescription dose of HRCTV D90 was 6 Gy*5F, which achieved satisfactory dose coverage. The patient's final HRCTV D90 EQD210 was 84.9 Gy, and IRCTV D90 EQD210 was 63.5 Gy. Rectum D2cc EQD23 was 66.03 Gy, bladder D2cc EQD23 was 75.57 Gy, sigmoid D2cc EQD23 was 63.93 Gy, and intestine D2cc EQD23 was 65.86 Gy. Follow-up at 1 year was CR. Conclusions: For patients with cervical cancer and a complete bicorporeal uterus, using double tandems combined with double ovoids is a feasible treatment method to ensure adequate dose coverage without causing additional damage. This method is also applicable to patients with endometrial cancer.

Soot nanoparticles (SNPs) are black carbon prevalent in atmospheric environment with significant impacts on public health, leading to neurodegenerative diseases including development of Parkinson's disease (PD). This study investigated the effects of SNPs exposure on PD symptoms, employing both in vivo and in vitro PD models. In the in vivo experiments, animal behavior assessments showed that SNPs exposure exacerbated motor and cognitive impairments in PD mice. Molecular biology techniques further unveiled that SNPs aggravated degeneration of dopaminergic neurons. In vitro experiments revealed that SNPs exposure intensified ferroptosis of PD cells by increasing reactive oxygen species and iron ion levels, while reducing glutathione levels and mitochondrial membrane potential. Sequencing tests indicated elevated N6-methyladenosine (m6A) alteration of the ferroptosis-related protein, acyl-CoA synthetase long chain family member 4 (ACSL4). This study demonstrates that SNPs may exacerbate the onset and progression of PD by recruiting YTH domain-containing family protein 1 (YTHDF1) protein, enhancing m6A methylation in the ACSL4 5'UTR, amplifying ACSL4 protein expression, and accelerating the ferroptosis process in dopaminergic neurons. These molecular mechanisms underlying SNPs exacerbation of PD development may provide crucial insights for formulating environmental safety regulations and potential therapeutic strategies addressing PD in populations residing in regions with varied air quality.

Reactive oxygen species (ROS)-mediated tumor catalytic therapy is typically hindered by gap junction proteins that form cell-to-cell channels to remove cytotoxic ROS, thereby protecting tumor cells from oxidative damage. In this work, a multifunctional nanozyme, FePGOGA, is designed and prepared by Fe(III)-mediated oxidative polymerization (FeP), followed by glucose oxidase (GOx) and GAP19 peptides co-loading through electrostatic and π-π interactions. The FePGOGA nanozyme exhibits excellent cascade peroxidase- and glutathione-oxidase-like activities that efficiently catalyze hydrogen peroxide conversion to hydroxyl radicals and convert reduced glutathione to oxidized glutathione disulfide. The loaded GOx starves the tumors and aggravates tumor oxidative stress through glucose decomposition, while GAP19 peptides block the hemichannels by inducing degradation of Cx43, thus increasing the accumulation of intracellular ROS, and decreasing the transport of intracellular glucose. Furthermore, the ROS reacts with primary amines of heat shock proteins to destroy their structure and function, enabling tumor photothermal therapy at the widely sought-after mild temperature (mildPTT, ≤45 °C). In vivo experiments demonstrate the significant antitumor effectof FePGOGA on cal27 xenograft tumors under near-infrared light irradiation. This study demonstrates the successful ablation of gap junction proteins to overcome resistance to ROS-mediated therapy, providing a regulator to suppress tumor self-preservation during tumor starvation, catalytic therapy, and mildPTT.

Microglia-mediated neuroinflammation plays a crucial role in the pathogenesis of Parkinson's disease (PD). Triggering receptor expressed on myeloid cells 2 (TREM2) confers strong neuroprotective effects in PD by regulating the phenotype of microglia. Recent studies suggest that TREM2 regulates high glucose-induced microglial inflammation through the NLRP3 signaling pathway. This study aimed to investigate the effect of TREM2 on NLRP3 inflammasome activation and neuroinflammation in PD. Mice were injected with AAV-TREM2-shRNA into both sides of the substantia nigra using a stereotactic injection method, followed by intraperitoneal injection of MPTP to establish chronic PD mouse model. Behavioral assessments including the pole test and rotarod test were conducted to evaluate the effects of TREM2 deficiency on MPTP-induced motor dysfunction. Immunohistochemistry of TREM2 and tyrosine hydroxylase (TH), immunohistochemistry and immunofluorescence Iba1, Western blot of NLRP3 inflammasome and its downstream inflammatory factors IL-1ß and IL-18, and the key pyroptosis factors GSDMD and GSDMD-N were performed to explore the effect of TREM2 on NLRP3 inflammasome and neuroinflammation. In an in vitro experiment, lentivirus was used to interfere with the expression of TREM2 in BV2 microglia, and then lipopolysaccharide (LPS) and adenopterin nucleoside triphosphate (ATP) were used to stimulate inflammation to construct a cellular inflammation model. The expression differences of NLRP3 inflammasome and its components were detected by qPCR and Western blot. In vivo, TREM2 knockdown aggravated the loss of dopaminergic neuron and the decline of motor function. After TREM2 knockdown, the number of activated microglia was significantly increased, and the expression of cleaved caspase-1, NLRP3 inflammasome, IL-1ß, GSDMD, and GSDMD-N was increased. In vitro, TREM2 knockdown aggravated the inflammatory response of BV2 cells stimulated by LPS and promoted the activation of NLRP3 inflammasome through the NF-κB pathway. In addition, TREM2 knockdown also promoted the expression of TLR4/MyD88, an upstream factor of the NF-κB pathway. Our vivo and vitro data showed that TREM2 knockdown promoted NLRP3 inflammasome activation and downstream inflammatory response, promoted pyroptosis, and aggravated dopaminergic neuron loss. TREM2 acts as an anti-inflammatory in PD through the TLR4/MyD88/NF-κB pathway, which extends previous findings and supports the notion that TREM2 ameliorates neuroinflammation in PD.

OBJECTIVE: To analyze the various factors affecting the economic burden of disease in patients with epilepsy. METHODS: This research used a multi-stage random sampling method, comprehensive intervention and without intervention of the epilepsy project group conducted a field survey of health examination among patients with epilepsy of 874 (422 cases in intervention group and 452 cases in control group) in Mengzhou, Xiuwu, Yancheng, Yuanhui, Wuyang, Linying, Shaoling, Xinye and Fangcheng. DALY indicators combined with human capital approach was used to measure the economic burden of disease in patients from two aspects of the direct economic burden and indirect economic burden. RESULTS: The age of comprehensive intervention group was (43.7 ± 8.5) years old, and it was (44.4 ± 7.2) years old in no intervention control group. The age difference was not statistically significant (P = 0.365). The direct economic burden per capita of the survey was 1019.6 yuan per year, while epilepsy drugs 434.2 yuan per year, 485.2 and 66.4 yuan per year in the intervention group, while 1518.5 and 777.5 yuan per year in the control group. Each object in this study lost (16.67 ± 10.01) DALY, with 8.58 DALY in intervention group and 24.23 DALY in no intervention control group. The per capita indirect economic burden of the intervention and control groups were 68 115.2, 401 036.5 yuan, the per capita total economic burden of disease were 68 600.5, 408 755.0 yuan. Multivariate logistic regression analysis showed that disease duration year of ≥ 37, 30 - 36, 20 - 29, 10 - 19, OR (95%CI) were 4.44 (2.37 - 8.33), 3.32 (1.85 - 5.94), 3.25 (1.90 - 5.56), 2.53 (1.46 - 4.41). If patients with longer duration, the more serious the patient's disease burden. Medication compliance (in order to comply with a good group control) OR (95%CI) was 3.37 (2.34 - 4.83), medication in the single form (serving multi-drug groups for control) OR (95%CI) 0.41 (0.29 - 0.59). Serving a single agent and good compliance of economic burden of disease were lighter. CONCLUSION: Epilepsy caused serious economic damage. Longer duration, medication adherence poor and serving more than medicine by heavier economic burden of disease; comprehensive intervention could reduce the economic burden of disease of patients with epilepsy.

OBJECTIVE: To measure and assess the levels of occupational exposure to power frequency electromagnetic fields in workers of power grid. METHODS: PMM8053 electromagnetic fields measuring system with EHP-50 probe was used to measure the levels of electromagnetic fields at working place. Personal dosimeters (EMDEX LITE) were utilized to measure the individual exposure levels of power frequency magnetic field. The results were evaluated with the limitation criteria of GBZ2.2 and ICNIRP. RESULTS: In the 500 kV ultra high voltage substation, the intensity at 90% measure points of power electric field was more than 5 kV/m. The magnetic field intensity in the areas nearby reactors and capacitors was often higher than 100 µT, even several hundreds µT. The mean daily exposure levels of workers in power grid were between 0.04 and 5.0 µT, and the exposure levels of 70% workers were higher than 0.4 µT. CONCLUSION: In the areas of ultra high voltage and nearby the reactors and capacitors are the key control points for occupational health in power grid. There is acute health risk of workers exposed to high accumulative exposure levels.

Oral squamous cell carcinoma (OSCC) is a common cancer with high recurrence, metastasis rates and poor prognosis. Circular RNAs (circRNAs) take part in regulating OSCC. Herein, we examined the role of circ_0008068 in OSCC. The circ_0008068, Katanin p60 ATPase-containing subunit A-like 1 (KATNAL1) mRNA, microRNA-153-3p (miR-153-3p) and acylgycerol kinase (AGK) contents were indicated by quantitative real-time polymerase chain reaction (qRT-PCR) and western blot. Moreover, in vitro and in vivo assays were conducted to scrutinize the effects of circ_0008068 on OSCC. Additionally, the contact between miR-153-3p and circ_0008068 or AGK was assessed by dual-luciferase reporter assay and RNA immunoprecipitation (RIP) assay. Thereafter, we found that the appearance of circ_0008068 and AGK was increased, and miR-153-3p content was diminished in OSCC. Circ_0008068 lack subdued cell proliferation, migration, invasion, tube formation and glycolysis metabolism, but stimulated cell apoptosis in OSCC. In addition, circ_0008068 bound to miR-153-3p to modulate the expression of its target AGK. Besides, miR-153-3p was validated to act as a tumor suppressor in OSCC tumorigenesis by suppressing AGK. Additionally, circ_0008068 knockdown also attenuated tumor growth in nude mice. RESUMEN

Constructed wetland (CW) is an environment-friendly and low-cost technology for nutrients removal from domestic wastewater. For a well-tuned CW, hydraulic loading rate (HLR) is one of the critical factors, particularly under the challenging circumstance of more frequent heavy rainfall events brought by global warming. In this study, a comprehensive investigation was conducted to explore the influence of different HLRs on the CW's bulk substrate and root-associated microbiota aiming to yield new insight for CW management from a hybrid perspective of environmental microbiology and engineering science. The response of the microbial community and associated nutrients removal performance under different HLR settings were analyzed after a one-year operation. Results showed that the bulk substrate and rhizosphere genera involved in desulfurization and denitrification, such as Ferritrophicum, Sulfurimonas, and Sulfurisoma, were enriched in the higher HLR condition and associated with the higher total nitrogen (TN) and nitrate nitrogen (NO3--N) removal compared to the lower HLR condition. Co-occurrence network analysis demonstrated a more complex network under the higher HLR condition. Besides, it was observed that more stochastic in microbial assembly under the higher HLR condition. Surprisingly, zoonotic pathogens were observed and showed a greater prevalence under the higher HLR condition, indicating the potential correlation between HLR and pathogen intrusion. Collectively, this study revealed that the microbiota could be significantly altered under different HLR conditions, thereby resulting in differences in nutrients removal performance.

OBJECTIVE: Oral squamous cell carcinoma (OSCC) is one of the most common cancers, accounting for over 90% of malignant lesions in the oral cavity. Long non-coding RNAs play an important role in the development of OSCC. This study aimed to investigate the effects of lncRNA XIST on the malignant behaviors of OSCC cells and its possible molecular mechanisms. METHODS: Real-time quantitative PCR and Western blot were used to detect the RNA and protein level, respectively. CAL27 and SCC25 cells with the lowest expression level of XIST were used for further study. MTT, transwell assay, colony formation, and xenograft model were applied to examine the effect of XIST on the progression of OSCC. FISH assay was performed to investigate the co-location of XIST and miR-455-3p in OSCC cells. The bioinformatics analysis, luciferase, and RNA pull down assay were utilized to predict and verify the target genes of miR-455-3p. RESULTS: XIST was downregulated in OSCC tissues and cell lines. Overexpression of XIST inhibited the proliferation, migration, and invasion ability of OSCC cells. Bioinformatics analysis and luciferase reporter assay confirmed XIST could bind to miR-455-3p. Besides, miR-455-3p directly targeted BTG2 in OSCC cells. Rescue experiments further confirmed the positive interaction between miR-455-3p and XIST as well as between miR-455-3p and BTG2. CONCLUSION: XIST was down-regulated in OSCC. XIST regulated the expression of BTG2 via sponging miR-455-3p. XIST/miR-455-3p/BTG2 signal axis inhibited the malignant progression of OSCC.

High-voltage electricity lines are known to generate extremely low-frequency electromagnetic fields (ELF-EMFs). With the process of urbanization, increasing concerns has been focused on the potentially hazardous impacts of ELF-EMF on human health, and the conclusions are controversial. Little is known about the method of prevention against ELF-EMF induced healthy problems. A total of 186 male workers with occupational exposure to high-voltage electricity lines, and 154 male subjects with insignificant exposure as reference control were enrolled in this study. Resveratrol or placebo was given as dietary supplements (500 mg twice daily), and several inflammatory biomarkers and biomarkers of oxidative stress were assessed. Workers who had long-term exposure to high-voltage electricity lines exhibited elevated urinary levels of 8-hydroxy-2-deoxy-guanosine (8-OHdG) and F2-isoprostane, compared to the reference group. Lower plasma nuclear factor kappa B (NF-κB) and interleukin (IL)-6 were observed in exposed workers compared to the reference group. Resveratrol significantly reversed the adverse impacts of ELF-EMF. Stimulated cytokine production by resveratrol was found in exposed workers but not in the reference group. This study supported that occupational and long-term exposure to high-voltage electricity lines has an adverse effect on homeostasis of human body, and resveratrol supplement could be an effective protection strategy against the adverse effects induced by ELF-EMFs.

Although it has been several decades since the focus on the effect of extremely low frequency electromagnetic fields (ELF-EMF) of high-voltage power lines on human health, no consistent conclusion has been drawn. The present study aimed to investigate the change in oxidative stress after exposure to ELF-EMFs, and potential protective effects of green tea polyphenol supplementation (GTPS) on ELF-EMFs induced oxidative stress. A total of 867 subjects, including workers with or without exposure to ELF-EMFs of 110-420kV power lines, participated and were randomized into GTPS and placebo treatment groups. Oxidative stress and oxidative damage to DNA were assessed by urinary tests of 8-isoprostane and 8-OHdG. Significant increased urinary 8-isoprostane and 8-OHdG were observed in workers with ELF-EMFs exposure, which were diminished after 12 months of GTPS. No protective effects of GTPS on oxidative stress and oxidative damage to DNA were observed after three months of GTPS withdraw. We found a negative impact of high-voltage power lines on the health of workers. Long-term GTPS could be an efficient protection against the health issues induced by high-voltage power lines.

Importance: Diabetes is a known risk factor for cardiovascular disease (CVD). Substantial uncertainty remains, however, about the relevance to CVD risk for blood glucose levels below the diabetes threshold. Objective: To examine the association of random plasma glucose (RPG) levels with the risk for major CVD in Chinese adults without known diabetes. Design, Setting, and Participants: This prospective cohort study included 467 508 men and women aged 30 to 79 years with no history of diabetes, ischemic heart disease (IHD), stroke, or transient ischemic attack. Participants were recruited from 5 urban and 5 rural diverse locations across China from June 25, 2004, to July 15, 2008, and followed up to January 1, 2014. Exposures: Baseline and usual (longer-term average) RPG level. Main Outcomes and Measures: Cardiovascular deaths, major coronary events (MCE) (including fatal IHD and nonfatal myocardial infarction), ischemic stroke (IS), major occlusive vascular disease (MOVD) (including MCE or IS), and intracerebral hemorrhage. Preliminary validation of stroke and IHD events demonstrated positive predictive values of approximately 90% and 85%, respectively. Cox regression yielded adjusted hazard ratios (aHRs) for CVD associated with RPG levels. Results: Among the 467 508 participants (41.0% men; 59.0% women; mean [SD] age, 51 [11] years), a significant positive association of baseline RPG levels with CVD risks continued to 4.0 mmol/L (72 mg/dL). After adjusting for regression dilution bias, each 1-mmol/L (18-mg/dL) higher usual RPG level above 5.9 mmol/L (106 mg/dL) was associated with an 11% higher risk for cardiovascular death (6645 deaths; aHR, 1.11; 95% CI, 1.10-1.13). Similarly strong positive associations were seen for MCE (3270 events; aHR, 1.10; 95% CI, 1.08-1.13), IS (19 153 events; aHR, 1.08; 95% CI, 1.07-1.09), and MOVD (22 023 events; aHR, 1.08; 95% CI, 1.07-1.09). For intracerebral hemorrhage, the association was weaker, but also significant (4326 events; aHR, 1.05; 95% CI, 1.02-1.07). These associations persisted after excluding participants who developed diabetes during follow-up. Conclusions and Relevance: Among adult Chinese without diabetes, lower RPG levels are associated with lower risks for major CVDs, even within a normal range of blood glucose levels.

OBJECTIVE: To explore the optimal doses of inhaled corticosteroids (ICS) for the management of Chinese asthmatics in order to enhance cost-effectiveness. METHODS: A randomized, paralleled and controlled multi-center study was carried out in 11 provincial hospitals. After one week run-in period when only inhaled salbutamol as needed was given, the asthmatic patients (n = 86) were divided into two groups: patients in group G (n = 42) received inhaled ICS with the doses recommended by Global Initiative for Asthma (GINA), ie, fluticasone propionate (FP) 250 microg bid for moderate asthma and 375 microg bid for severe asthma. Patients in group H (n = 44) received half of the above doses of FP (125 microg bid for moderate asthma and 125 microg in the morning, 250 microg in the evening for severe asthma). RESULTS: There were no significant differences between group G and group H in the 32 measurements after management, including day symptom score (severe asthma in group G 0.7 +/- 0.8, group H 0.4 +/- 0.6 and moderate asthma in group G 0.4 +/- 0.5, group H 0.3 +/- 0.5), FEV(1) [severe asthma in group G (1.5 +/- 0.5) L, group H (1.8 +/- 0.6) L/min and moderate asthma in group G (2.3 +/- 0.6) L, group H (2.3 +/- 0.8) L/min, FEV(1)% of predicted [severe asthma in group G (54 +/- 17)%, group H (59 +/- 19)%, and moderate asthma in group G (79 +/- 14)%, group H (79 +/- 15)%], the increased morning PEF value (45, 67 L/min in moderate and severe asthma in group G and 56, 65 L/min in moderate and severe asthma in group H respectively), frequency of nights awakened (81, 69 in severe asthma in groups G and H respectively), numbers of well controlled cases (88 and 98 d in severe asthma in groups G and H respectively) and the use of extra bronchodilators (salbutamol, puff/day, 5.0 and 3.4 d in severe asthma in groups G and H respectively, P > 0.05). On the other hand, in the subgroups of moderate asthma, night symptom score (0.30 +/- 0.22 in group G, 0.13 +/- 0.33, t = -2.06, P < 0.05), number of exacerbations (3/22 in group G, 11/24 in group H, chi(2) = 4.74, P < 0.05), number of total controlled cases (18/22 cases in group G, 12/24 cases in group H, chi(2) = 4.97, P < 0.05) were significantly different between group G and group H. In the subgroups of severe asthma, the total days of exacerbations were different between group G and group H (11 days in group G, 6 days in group H, U = 31, P < 0.05). CONCLUSION: As a whole, similar therapeutic effect was achieved using half of the GINA recommended dose of ICS with/without salbutamol inhalation as needed compared with those receiving the GINA recommended full dose of ICS in most Chinese asthmatics.
