BACKGROUND: Iodine deficiency in pregnant women may result in substantial and irreversible impairment in fetal brain development, even from the first few weeks of pregnancy. OBJECTIVE: To assess the nutritional iodine status of pregnant women in our health area and its relationship with dietary factors and thyroid function and to suggest treatment guidelines. PATIENTS AND METHOD: A study in 164 pregnant women in early pregnancy was carried out. Data on urinary iodine concentrations were gathered from 24-hour urine samples. Data on anthropometric parameters and thyroid function were included. Information on dietary habits in the previous week was collected using a nutritional questionnaire. All data were obtained before iodine supplements were administered. RESULTS: The median urinary iodine concentration was 92 [p25-p75 range, 71-139] microg/l. Seventy-eight percent of urinary iodine values were under 150 microg/l. Women who took iodized salt had higher levels of urinary iodine concentrations than women without iodized salt intake. CONCLUSIONS: Seventy-eight percent of pregnant women in our health area were iodine deficient. Iodized salt intake is related to iodine sufficiency and to increased urinary iodine concentrations. Measures to increase intake of iodized salt among the population should be implemented. Iodized salt supplements should be systematically prescribed in women from the beginning of pregnancy.

The aim of this study was to determine the existence of the circadian rhythm (CR) in the onset of acute myocardial infarction (AMI) in different patient subgroups. Information was collected about 41,244 infarctions from the database of the ARIAM (Analysis of Delay in AMI) Spanish multicenter study. CR in AMI were explored in subgroups of cases categorized by age, gender, previous ischemic heart disease (PIHD), outcome in coronary care unit, infarction electrocardiograph (ECG) characteristics (Q wave or non-Q wave), and location of AMI. Cases were classified according to these variables in the different subgroups. To verify the presence of CR, a simple test of equality of time series based on the multiple-sinusoid (24, 12, and 8 h periods) cosinor analysis was developed. For the groups as a whole, the time of pain onset as an indicator of the AMI occurrence showed a CR (p<0.0001), with a morning peak at 10:10 h. All the analyzed subgroups also showed CR. Comparison between subgroups showed significant differences in the PIHD (p<0.01) and infarction ECG characteristics (p<0.01) groups. The CR of the subgroup with Q-wave infarction differed from that of non-Q wave subgroup (p<0.01) when the patients had PIHD (23% in Q wave infarction vs. 39.2% in non-Q wave). AMI onset followed a CR pattern, which is also observed in all analyzed subgroups. Differences in the CR according to the Q/non-Q wave infarction characteristics could be determined by PIHD. The cosinor model fit with three components (24, 12, and 8 h periods) showed a higher sensitivity than the single 24 h period analysis.

The purpose of this study was to determine the best statistical model of cognitive styles, based on the MFFT-20, CEFT and Stroop tests to predict attention deficit hyperactivity disorder (ADHD), analyzing the validity of the model for the diagnosis of the disease. We studied 100 ADHD cases (DSM-IV criteria) and 100 controls, age ranging between 7 and 11 years. Controls were randomly recruited and matched in age, gender and sociodemographic area with ADHD cases. On average, ADHD cases showed more impulsiveness (d: 1.28), less cognitive flexibility (d: 0.91) and more field dependence (d: 1.62) than controls. The logistic regression model that predicts ADHD best is made up of age, CEFT, MFFT-20 and Stroop tests and the formula derived from the model shows 85% sensitivity and 85% specificity for ADHD, regarding the DSM-IV criteria as the standard. The statistical model of cognitive styles presents valid indicators to diagnose ADHD, contributing to an increase in the objectivity of its analysis.
