RESUMEN

Puisque 20 % des Canadiens déclarent avoir une autre langue maternelle que le français ou l'anglais, il n'est pas rare que les professionnels de la santé et les patients maîtrisent des langues différentes. Lorsqu'un médecin et son patient ne peuvent pas communiquer correctement, ils courent le risque de mal se comprendre, ce qui peut se solder par de mauvais résultats cliniques et une réadmission hospitalière. Les services d'interprétation professionnels sont associés à une amélioration de la communication, de l'utilisation des soins, des résultats cliniques et de la satisfaction envers les soins. Il est démontré que le recours à des interprètes non formés ou improvisés, y compris les membres de la famille, accroît les erreurs d'omission, les substitutions, les modifications volontaires et les ajouts. Les enfants et les adolescents n'ont pas acquis une maturité développementale suffisante pour agir en qualité d'interprètes dans le milieu de la santé. Dans un tel contexte, ils sont mis dans une situation inappropriée avec laquelle il peut être difficile de composer et qui peut compromettre durablement à la fois leur propre santé mentale et leur relation avec les autres membres de la famille.

RESUMEN

A qualitative study was carried out with five professionals in a pediatric intensive care unit in 2022. Semi-structured interviews were used to find out how they felt about patients from their own culture, and about the role of their mother tongue in hospital care. Caregivers find it difficult to bring their own culture to the fore when caring for their patients. A description of the obstacles they encounter, as well as the advantages, is included.

The Association des médecins de langue française d'Amérique du Nord (AMLFAN) was founded in Québec at the turn of the twentieth century. The physicians who convened at the Association between 1902 and 1910 shared a concern for the degeneration of the French-Canadian "race" under the effects of alcoholism, tuberculosis, and syphilis. For hygienists such as Arthur Rousseau and Charles-Narcisse Valin, this state of degeneration called for hygienic measures that would help regenerate and improve the French-Canadian race. While their suggestion that marriages be matched scientifically in order to prevent the transmission of hereditary and acquired defects from parent to offspring may be reminiscent of eugenics, French-Canadian physicians seemed to have no knowledge of Sir Francis Galton - eugenics' "founding father" - and his work on the topic. This article compares French-Canadian eugenic discourses with Galtonian eugenics in order to shed light on the particularities of the French-Canadian case.

Clear cells sarcomas (CCS) are exceptionally rare in the tongue, with, to our knowledge, only three previous reports in anglo-saxon literature. Through our case, we will discuss the differential diagnosis of clear cells tumors of the tongue and bring this tumour closer to the newly described entity of the gastrointestinal tract named "clear cells sarcoma-like gastrointestinal (SCCLGI)", recently renamed "gastrointestinal neuroectodermal tumour (GNET)". SCCLGI/GNET share morphological and molecular characteristics with SCC but had until then been observed only in the digestive tract. Our case could be a lingual localization of a SCCLGI/GNET. SCC and SCCLGI/GNET characteristic molecular profil involves EWSR1-ATF1 [t(12; 22) (q13; q12)] and EWSR1-CREB1 [t(2; 22) (q34; q12)] fusion genes, but it is not specific of these tumours.

RESUMEN

The thoracodorsal artery perforator flap is increasingly used in head and neck reconstructions. One of its multiple advantages is the low donor site morbidity compared to the other free flaps usually used for this type of surgery, such as the radial forearm free flap and the anterolateral flap of the thigh. However, the current harvesting technique of the thoracodorsal artery free perforator flap needs a vertical incision rising high in the axillary hollow for the dissection of the pedicle, thus impeding optimal discretion of the donor site, especially for women. We describe an original technique to harvest a pure transversal skin paddle on its own perforator, leaving a horizontal scar thoroughly hidden in the bra and preserving the thoracodorsal pedicle. We detail the requirements for this new type of harvesting.

RESUMEN

Clear cell sarcomas (SCC), also called "soft-tissue melanoma", are rare and aggressive tumors that preferentially affect the lower limbs (tendons and fasciae) and which have also been described in head and neck localizations. Their clinical and immunohistochemical mimicry with melanoma makes it difficult to diagnose sarcomas. SCC treatment is mainly focused on large-scale resection surgery with adjuvant radiotherapy because of their low chemo-sensitivity and extreme lymphophilia. In case of head and neck localization, these treatments may lead to function and aesthetic sequelae thus requiring the use of modern techniques of reconstructive surgery. The authors describe the diagnosis, treatment and follow-up of large lingual SCC case using a DIEP free flap reconstruction according to an original technique developed in the department. Given the characteristics of patients with SCC (a high proportion of women between 20 and 40 years old) and its inherent qualities (low morbidity of the donor site, volume delivered and excellent plasticity), the fascio-cutaneous free flap type "DIEP" "taken according to the design of the" Cathedral triptych seems to be a viable choice among the range of reconstruction solutions.

BACKGROUND: Regular cannabis use may be associated with several oral changes not usually identified by dermatologists: xerostomia, increased risk of caries, periodontitis, leukoedema, gingival hyperplasia, and higher prevalence and density of Candida albicans, leukoplakia or gingivitis. PATIENTS AND METHODS: We report herein the appearance of a characteristic green tongue in a patient following intensive marijuana inhalation. DISCUSSION: This complication has rarely been reported in the medical literature. Paradoxically, it is clearly described in different Internet search engines, particularly Google.

BACKGROUND: Little information is available on the prevalence and clinical aspects of tongue involvement in children with psoriasis. The aim was to evaluate the prevalence, clinical aspects and risk factors concerning tongue involvement in children with psoriasis. PATIENTS AND METHODS: This study was carried out in two stages. We performed a multicentre, cross-sectional study in 23 French dermatology centers. All children seen for psoriasis during the one-year study were systematically included. The clinical features of the tongue and of psoriasis were recorded. Association with clinical aspects of psoriasis and comorbidities was evaluated. We then carried out a literature review to evaluate the prevalence of tongue involvement in children with psoriasis and its positive predictive value for psoriasis. A search was conducted in the PUBMED database using the following keywords: "child" and "psoriasis" and ("tongue" or "glossitis" or "migratory glossitis" or "benign migratory glossitis" or "geographic tongue" or "fissured tongue"). RESULTS: 7.7% of the 313 children with psoriasis had tongue involvement. The clinical aspects were geographic tongue (4.2%), fissured tongue (2.8%) and both (0.64%). There was no association between tongue involvement and the clinical characteristics of the children. Two hundred and ninety-five articles were referenced and 3 were analysed. Psoriasis is very rare in cases of tongue abnormalities. CONCLUSION: The prevalence of tongue involvement was 7.7% in children with psoriasis. No clinical or epidemiological association was shown. Tongue involvement does not modify the management of psoriasis. In the literature review it was not possible to evaluate either the prevalence of tongue involvement in psoriasis or the positive predictive value thereof.

RESUMEN

The native language is a vehicle for transmission and inscription into a culture. In children of migrants who grow up between two languages and two cultures, language develops in a context of bilingualism. This specific context requires bilingual assessments to ensure that a child's complete skills are assessed. The language assessment of non-French-speaking children and first-time immigrants, has been set up at Avicenne hospital in Bobigny. It is the first transcultural tool for the assessment of native languages.

A high number of children consulting a speech therapist for a language disorder have in common the direct or indirect experience of migration and therefore find themselves in a context of plurilingualism. A dedicated group, called Mélimémo, has been set up for children in such situations. It enables their linguistic and cultural diversity to be integrated into the speech therapy.

Many children supported by professionals from the mother and infant welfare protection centre grow up in a plurilingual environment. Taking into account this plurilingualism provides better understanding of the relationships which the children establish with their environment. This can also help them to engage in a transgenerational transmission as well as in multiple cultural affiliations.

The question of mother tongues in the socio-educational context of school was the subject of a memorandum of 2016. Moreover, research in psychology and linguistics shows the extent to which the family language and culture are important elements in the construction of the child. These must be taken into account at school as well as in any support and care environment.

OBJECTIVE: To assess the association between the prevalence of tongue cyst-positive and antigen-positive pigs across different settings in Africa, to evaluate whether examining pigs for cysts could be used as a rapid surveillance tool for identifying geographical areas with a higher probability of high transmission of cysticercosis. METHODS: Published data were collated from 26 study sites across Africa that reported the prevalence of porcine cysticercosis by both lingual and serological examinations. The study sites were located in 10 countries across Africa. RESULTS: Seroprevalence rates ranged from 4% to 41%. Despite the varied study sites, the relationship between the two variables was highly consistent and suggests identification of tongue cysts may be useful for cysticercosis surveillance. We found that all areas with more than 10% of pigs having cysts in their tongues had at least 30% seroprevalence (PPV of 100%), although this cut-off is less reliable at predicting that an area is of low transmission (NPV of 84%). CONCLUSION: Assessing the prevalence of tongue cyst-positive pigs is a potential rapid epidemiological tool for identifying areas at high risk of cysticercosis, although further refinement and validation is required using standardised data sets.

The purpose of this scoping review was to map key themes in the Canadian literature on the professional integration of internationally educated nurses (IENs), identify the types and sources of and gaps in evidence, and offer recommendations for research. The work was guided by a 6-step methodological framework for scoping reviews. The search and selection of academic and grey literature for the period 2000-13 resulted in a sample of 157 papers for full-text screening. Themes derived from the literature reflect stages of IENs' professional integration: pre-immigration and early arrival, professional recertification, workforce integration, and workplace integration. Data were extracted, coded, and collated using electronic charts. Numerical and qualitative thematic summaries were used to analyze the data. Recommendations for research are as follows: create data systems to track IEN immigration and integration; determine the effectiveness of programs and policies for IENs; and examine the influence of language proficiency on professional recertification, workplace integration, and patient safety.


La présente analyse exhaustive a pour objectif de recenser les thèmes clés traités dans la documentation canadienne portant sur l'intégration professionnelle du personnel infirmier formé à l'étranger (PIFÉ), de déterminer les types et les sources des données probantes ainsi que les lacunes, et d'émettre des recommandations en matière de recherche. Dans le cadre de cette analyse, les auteures ont utilisé un cadre méthodologique comportant six étapes. Le recensement et la sélection d'une littérature didactique et d'une documentation parallèle couvrant la période de 2000 et 2013 a mené à un examen des textes intégraux de 157 documents de recherche. Les thèmes extraits de la littérature reflètent les étapes de l'intégration professionnelle des PIFÉ: la pré-immigration et l'arrivée au Canada; le recertification professionnelle; l'intégration au marché du travail; et l'intégration au travail. Des données ont été extraites, codées et rassemblées en utilisant des tableaux électroniques. Des résumés thématiques numériques et qualitatifs ont été utilisés pour analyser les données. Les recommandations en matière de recherche sont les suivantes: créer des systèmes de données pour suivre la démarche d'immigration et d'intégration du PIFÉ; déterminer l'efficacité des programmes et des politiques à l'intention des PIFÉ; et examiner l'influence de la maîtrise de la langue sur la recertification professionnelle, l'intégration au travail et la sécurité des patients.

RESUMEN

Cribriform adenocarcinoma of salivary gland origin is a rare and recently described lesion. In spite of the high incidence of metastatic spread, the prognosis remains very good. We report a case of a 64-year-old man with cribriform adenocarcinoma of salivary gland origin of the ventral tongue without locoregional or distant metastasis. The patient is currently 43-month post treatment without any local or regional recurrence of the disease. This entity should be kept in mind regarding its good prognosis and its resemblances with papillary carcinoma of the thyroid and adenoid cystic carcinoma with which it should not be confused.

PURPOSE: To study the diagnostic and therapeutic aspects of hemangioma of the tongue. OBSERVATION: A 65 year old man admitted to ENT for a swelling of the tongue that appeared two years ago and progressively increased in volume leading to permanent protrusion. The swelling took up the entire anterior third of the tongue. It had a reddish appearance. On palpation, it was a rounded, firm, well-limited, slightly sensitive mass, measuring 5 cm in diameter. The rest of the ENT examination was unremarkable. Lingual CT scan revealed a very limited mass with hyperechogenic content that did not increase in size after injection of the contrast agent. Surgical excision was performed and the postoperative follow-up was simple. Histology concluded that there was a hemangioma of the tongue. CONCLUSION: Hemangioma of the tongue is a rare pathology. It must be considered in front of any lingual mass in adults. Its positive diagnosis is clinical and histological.

La tuméfaction prenait toute le tiers antérieur de la langue. Elle était d'aspect rougeâtre. A la palpation, il s'agissait d'une masse arrondie, ferme, bien limitée, légèrement sensible, mesurant 5 cm de grand diamètre. Le reste de l'examen ORL était sans particularité. La TDM linguale a objectivé une masse bien limitée à contenu hyperechogène ne se rehaussant pas après injection du produit de contraste. L'exérèse chirurgicale a été effectuée et les suites opératoires ont été simples. L'histologie a conclu à un hémangiome de la langue. CONCLUSION: L'hémangiome de la langue est une pathologie rare. Il faut y penser devant toute masse linguale chez l'adulte. Son diagnostic positif est clinique et histologique.

Recently, we validated a simple method for estimating peritoneal dialysis (PD) peritonitis rate. Despite good agreement between estimates and gold-standard measurements in two large dialysis registries, the International Society of Peritoneal Dialysis (ISPD) was hesitant to recommend adoption of the estimating equation. Their perception is that inaccuracies, as small as they are, might still be detrimental to clinical decision-making. In this study, we apply new analyses to the original validation data sets. We quantify agreement using standards from the International Organization for Standardization (ISO). We also identify a subset of centres with poorest performance of the estimating equation and qualitatively assess the potential for compromised clinical decision-making associated with its use. Inter-assay % coefficient of variation between estimates and measurements was 4.2% in the Australia and New Zealand Dialysis and Transplant Registry and 4.6% in Le Registre de Dialyse Péritonéale de Langue Française, easily meeting ISO requirements. Mandel's h values and Grubb's tests confirmed more outlying estimates compared to the measurements, while Mandel's k values and Cochran's C tests showed that identical precision by the two methods. Misclassification of centres as being above versus below the ISPD standard of 0.4 episodes/patient-year occurred only with rates close to the threshold, affecting approximately 3% of patient-years. In the 26 (out of 268) centres with poorest performance of the estimating equation, examination of the time series of their annual PD peritonitis rate estimates/measurements showed that using estimates would not detrimental to clinical decision-making. In conclusion, the estimating equation is sufficiently accurate for routine clinical use.

Tongue Lacerations in Children. The Zurich Tongue Scheme Abstract. Treating injured children is part of everyday life at a pediatrician's office or in an emergency department. Among the most common injuries are lacerations, mainly of the head, but the tongue can also be affected. Lacerations of the tongue mostly occur in early childhood due to fall when the child bites its tongue. No evidence-based treatment recommendations exist, even though such injuries are common. It makes a huge difference to the parents and children affected whether the treatment consists of a primary wound closure in general anesthesia or spontaneous healing can be awaited. In this article, the development and implementation of the Zurich Tongue Scheme will be discussed, the first evidence-based treatment recommendations for uncomplicated tongue lacerations in children.
