RESUMEN

The frailty of elderly people with diabetes requires various factors to be taken into account including lifestyle, the moment of the diagnosis, the patient's experience, the social context including any financial difficulties. It is thereby apparent that the support requirements differ depending on the situations. Medical telemonitoring can be an appropriate solution.

RESUMEN

Dialysis (haemodialysis, peritoneal dialysis) and renal transplantation are the renal replacement treatments when the personal kidneys have been destroyed. Renal grafting is the desired treatment for most patients who arrive at the terminal stage of their chronic kidney disease. Many patients with chronic kidney disease remain in chronic dialysis until the transplantation or their death. It is important to adapt the conditions of treatment to their social life so that they are the best possible and to optimize clinical and biological monitoring to prevent complications and hospitalizations. This is the objective of telemedicine with its medical practices such as remote monitoring, teleconsultation and teleexpertise, now reimbursed by French health insurance. The medical remote monitoring of dialysed patients helps to carry out the treatment at the nearest home or even at home. The medical remote monitoring of patients grafted for more than 6 months aims to reduce the number of regular visits at the transplantation centre while ensuring the best survival of the graft and a quality of social life. This review of literature presents the various models of medical remote monitoring in patients with renal replacement treatments.
