RESUMEN

Abstract Polymyxins are antibiotics developed in the 1950s. Polymyxin-induced neurotoxicity has been often described in medical literature. The same cannot be said of nephrotoxicity or tubulopathy in particular. This report describes the case of a patient prescribed polymyxin B to treat a surgical wound infection, which led to significant increases in fractional excretion of calcium, magnesium, and potassium and subsequent persistent decreases in the levels of these ions, with serious consequences for the patient. Severe hypocalcemia, hypomagnesemia, and hypokalemia may occur during treatment with polymyxin. Calcium, magnesium and potassium serum levels must be monitored during treatment to prevent life-threatening conditions.


Resumo Polimixinas são um grupo de antibióticos desenvolvidos na década de 1950. Seus efeitos neurotóxicos são comumente descritos na literatura, porém há menos relatos sobre seus efeitos nefrotóxicos, especialmente tubulopatias. O objetivo deste relato é descrever o uso de polimixina B em uma paciente para tratamento de infecção de ferida operatória, promovendo grande aumento das frações de excreção de cálcio, magnésio e potássio e acarretando reduções graves e persistentes desses íons, com sérias consequências para a paciente. Hipocalcemia, hipomagnesemia e hipocalemia severas podem ocorrer durante terapia com polimixina e é sugerido que sejam monitorizadas as concentrações séricas desses eletrólitos durante o tratamento como forma de evitar condições de risco à vida.

A Artrite reumatoide (AR) é uma doença associada com aumento de risco cardiovascular em relação à população em geral. A leflunomida, utilizada para seu tratamento, tem um efeito hipouricemiante mas não se sabe se este fato influi na redução do risco cardiovascular. Objetivo: Estudar a influência do uso da leflunomida em níveis de ácido úrico sérico e na espessura da camada médio-intimal da carótida. Métodos: Sessenta pacientes com AR foram estudados: 30 usando leflunomida e 30 sem o seu uso. Dados acerca de fatores de risco cardiovascular, ácido úrico sérico e espessura da camada médio-intimal da carótida por ecografia foram obtidos. Resultado: A leflunomida mostrou-se capaz de reduzir os níveis de ácido úrico sérico (p=0.03) mas não influenciou os valores da espessura médio-intimal da carótida (p=0.51) Conclusão: Não foi possível demonstrar redução dos valores de espessura da média-íntima da carótida em pacientes usuários de leflunomida.


Background: Rheumatoid Arthritis (RA) is a disease associated with increased cardiovascular risk when compared to the general population. Leflunomide, used for its treatment, has a hypouricemiante effect but it is not known whether this fact influences the reduction of cardiovascular risk. Objective: To study the influence of the use of leflunomide in serum uric acid levels and the intimal middle layer of carotid. Methods: Sixty patients with RA were studied: 30 using leflunomide and 30 without it. Data on cardiovascular risk factors, serum uric acid and thickness of the middle-intimal layer of carotid by ultrasound were obtained. Result: Leflunomide was able to reduce serum uric acid levels (p=0.03) but did not influence the values of the medium-intimal thickness of carotid (p=0.51). Conclusion: It was not possible to demonstrate a reduction in the thickness values of the media-intima of carotid in patients using leflunomide.

This study investigated the kinetics of cytokines that are involved in the development of interstitial fibrosis in mice that were subjected to UUO, the interstitial type I and III collagen deposition, and the effects of Thalido and Dexa treatment on these parameters. Inbred C57BL/6 mice were divided into the groups: Normal (not submitted surgery), Sham (sham surgery), Control (UUO treated with 0.5% carboxymethyl cellulose), Thalido (UUO treated with 5 mg/kg thalidomide), and Dexa (UUO treated with 1 mg/kg dexamethasone). The treatments began the day before surgery and were administered once daily by gavage for 1, 7, or 14 days. At the end of each treatment period, blood samples were collected for the determination of creatinine, urea, cytokines. The Control group exhibited a increase in creatinine concentration compared with the Normal and Sham groups within the first 24 h after UUO, which remained high until days 7 and 14. The urea concentration was higher on days 7 and 14 in the Control group compared with the Sham group. In the Thalido and Dexa groups, a reduction of serum creatinine concentration was seen on day 14. Treatment with Dexa reduced the serum concentration of urea on day 7. The serum concentrations of cytokines (TNF-α, IL-1ß, IL-6, IL-10 and IL-17) and chemokines (KC, MIG, bFGF) increased in UUO mice at all of the sampling times. The Dexa and Thalido groups exhibited alterations in the concentrations of these cytokines, suggesting the involvement of anti-inflammatory and immunomodulatory mechanisms that may have modified the fibrosis framework.

INTRODUCTION: Autosomal dominant polycystic kidney disease is the most common hereditary renal disease in humans. OBJECTIVE: To examine the prevalence, clinical and laboratory characteristics of patients with polycystic kidneys and relate disease manifestations by gender. METHODS: This was an observational and retrospective study. All the medical records of patients with polycystic kidneys who initiated hemodialysis between 1995 and 2012, in four centers that treat patients of the coverage area of the 15th regional health Paraná (Brazil), were analyzed. RESULTS: The study included 48 patients with polycystic kidneys, the primary cause of stage 5 CKD. Disease prevalence was one in 10,912 people. The average age of dialysis initiation was 50.7 years and the follow-up time on dialysis until transplantation (36.5 months) was lower among men. Hypertension was the most frequent diagnosis in 73% of patients, predominantly in women (51.4%). The liver cyst was the most frequent extrarenal manifestations in men (60.0%). The death occurred in 10.4% of patients using hemodialysis, and 60% of men. The class of antihypertensive drug used was that acts on the renin-angiotensin system with higher frequency of use among women (53.3%). The post-dialysis urea was significantly higher in men. CONCLUSION: The prevalence of the disease is low among hemodialysis patients in southern Brazil. The differences observed between genders, with the exception of the post-dialysis urea, were not significant. The findings are different from those reported in North America and Europe.

Introdução: A doença renal policística autossômica dominante é a enfermidade renal hereditária mais comum em seres humanos. Objetivo: Analisar a prevalência, características clínicas e laboratoriais de pacientes com rins policísticos e relacionar as manifestações da doença por gênero. Métodos: Trata-se de um estudo observacional e retrospectivo. Foram revisados todos os prontuários médicos de pacientes com rins policísticos admitidos para hemodiálise entre 1995 e 2012, em quatro centros que atendem a área de abrangência da 15ª regional de saúde do Paraná, Brasil. Resultados: Fizeram parte do estudo 48 pacientes com rins policísticos, causa primária da doença renal crônica (DRC) estágio 5. A prevalência da doença foi de um em 10.912 habitantes. A média de idade de ingresso na hemodiálise (50,7 anos) e o tempo de seguimento em hemodiálise até o transplante (36,5 meses) foi menor nos homens. A hipertensão arterial foi o diagnóstico mais frequente em 73% dos pacientes, com predominância em mulheres (51,4%). O cisto hepático foi a manifestação extrarrenal mais frequente nos homens (60,0%). Foram a óbito 10,4% dos pacientes que faziam uso de hemodiálise, sendo 60% de homens. A classe de droga anti-hipertensiva mais utilizada foi a que atua no sistema renina-angiotensina, com maior frequência de uso nas mulheres (53,3%). A ureia pós-diálise foi significativamente maior em homens. Conclusão: A prevalência da doença é baixa entre pacientes em hemodiálise no sul do Brasil. As diferenças observadas entre os gêneros, com exceção da ureia pós, não foram significantes. Os dados encontrados são diferentes dos reportados na América do Norte e Europa. .

Avaliar a estrutura microscópica e os aspectos microbiológicos de enxertos aórticos após conservação em Glicerol 98% e glicerina 0,2%. Estudo in-vitro de caráter intervencionista e randomizado com 32 enxertos de aorta de cobaias já sacrificadas. Estes enxertos foram divididos aleatoriamente em 02 grupos, GLU (Glutaraldeído 0,2%) e GLI (Glicerol 98%) de 16 segmentos vasculares cada e mantidos em conservação por 18 meses em temperatura ambiente. Para análise microbiológica foi utilizado coloração de Gram e cultura em ágar-sangue e a histológica a coloração de hematoxilina-eosina. Os resultados foram expressos por meio de frequências e percentuais. Tanto o glicerol, quanto o glutaraldeído, penetraram todas as camadas vasculares em 100% dos enxertos, desde o endotélio à túnica adventícia. As características celulares mantiveram-se nos 32 segmentos aórticos, com ausência de citólise ou autólise. Apenas 2 (6,25%) dos enxertos do grupo glutaraldeído apresentaram contaminação bacteriana. A solução de glutaraldeído 0,2% pode ser sugerida como um eficaz conservante para biopróteses, tanto quanto a já consagrada glicerina a 98%


To evaluate the microscopic structure and microbiological aspects of aortic grafts after preservation in 98% glycerol and 0.2% glycerol. This sutudy is In-vitro, interventionist and randomized with 32 grafts of the aorta of guinea pigs sacrificed already. These grafts were randomly divided into 02 groups, Glu (0.2% glutaraldehyde), and GLY (98% Glycerol) with 16 vascular segments each and kept in conservation for 18 months at ambient temperature. For microbiological analysis was used Gram stain and culture on blood agar and histological staining with hematoxylin-eosin. The results were expressed by means of percentage and frequency. Both the glycerol and the glutaraldehyde penetrated all vascular layers in 100% of the grafts from the endothelium to the tunica adventitia. The cell characteristics remained at 32 aortic segments, with the absence of cytolysis or autolysis. Only 2 (6.25%) graft of group glutaraldehyde showed bacterial contamination. The 0.2% glutaraldehyde solution can be suggested as an effective preservative for bioprostheses, as much 98% glycerin
