RESUMEN

OBJECTIVE: To develop updated guidelines for the pharmacological management of rheumatoid arthritis (RA). METHODS: A group of experts representative of different geographical regions and various medical services catering to the Mexican population with RA was formed. Questions based on Population, Intervention, Comparison, and Outcome (PICO) were developed, deemed clinically relevant. These questions were answered based on the results of a recent systematic literature review (SLR), and the evidence's validity was assessed using the GRADE system, considered a standard for these purposes. Subsequently, the expert group reached consensus on the direction and strength of recommendations through a multi-stage voting process. RESULTS: The updated guidelines for RA treatment stratify various therapeutic options, including different classes of DMARDs (conventional, biologicals, and JAK inhibitors), as well as NSAIDs, glucocorticoids, and analgesics. By consensus, it establishes the use of these in different subpopulations of interest among RA patients and addresses aspects related to vaccination, COVID-19, surgery, pregnancy and lactation, and others. CONCLUSIONS: This update of the Mexican guidelines for the pharmacological treatment of RA provides reference points for evidence-based decision-making, recommending patient participation in joint decision-making to achieve the greatest benefit for our patients. It also establishes recommendations for managing a variety of relevant conditions affecting our patients.

BACKGROUND: Fibromyalgia overlaps and/or mimics other rheumatic diseases and may be a confounding factor in the clinimetric assessment of these illnesses. Allodynia is a distinctive fibromyalgia feature that can be elicited during routine blood pressure measurement. For epidemiological purposes fibromyalgia can be diagnosed using the 2016 Wolfe et al. criteria questionnaire. No physical examination is required. OBJECTIVE: To evaluate the role of a straightforward question formulated during routine blood pressure measurement for fibromyalgia detection in a rheumatology outpatient clinic. PATIENTS AND METHODS: All adult patients attending our Rheumatology outpatient clinic were invited to participate. While awaiting their medical consultation, they filled-out the 2016 Wolfe et al. FM diagnostic criteria questionnaire. During the ensuing routine physical examination, the physician advanced the following guideline: "I am going to take your blood pressure; tell me if the cuff's pressure causes pain". Then, blood pressure cuff was inflated to 170 mm/Hg. Sphygmomanometry induced allodynia was defined as any local discomfort caused by blood pressure measurement. If a patient voiced any uneasiness, a follow-up dichotomic question was formulated "did it hurt much or little". Sphygmomanometry-induced allodynia was correlated with the presence of fibromyalgia according to the 2016 Wolfe diagnostic criteria. RESULTS: Four hundred and ninety-one patients were included in the study; most of them (84%) were female. The female cohort displayed the following features: Twenty five percent had fibromyalgia. Twenty seven percent had sphygmomanometry-induced allodynia. In women, sphygmomanometry-evoked allodynia had 63% sensitivity and 84% specificity for fibromyalgia diagnosis. The area under curve was 0.751. Moreover, having "much" local pain elicitation during blood pressure testing had 23% sensitivity and 96% specificity for fibromyalgia diagnosis. Men behaved differently; 15% fulfilled the fibromyalgia diagnostic criteria, but only 2% had sphygmomanometry induced allodynia. CONCLUSIONS: Inquiring female patients about local discomfort during routine blood pressure measurement is a simple and efficient procedure for fibromyalgia detection. This undemanding approach could be implemented in all clinical settings. There is marked sexual dimorphism in the link between sphygmomanometry-induced allodynia and fibromyalgia diagnosis. The presence of fibromyalgia is almost certain in those individuals having substantial pain elicitation during blood pressure measurement.

To characterize CD4+CD28null cells in chronic hyperuricemia and investigate whether allopurinol could restore CD28 expression and the balance of T helper phenotypes. Asymptomatic individuals with chronic hyperuricemia and ultrasonographic findings evocative of urate deposition in the joints. Age- and gender-matched normouricemic individuals were also studied. Oral allopurinol at 150 mg/day for 4 weeks, followed by 300 mg/day through week 12. Color-flow cytometry on peripheral blood mononuclear cells (PBMC) with antibodies against CD4, CD28, T-bet (Th1), GATA-3 (Th2), and RORγt (Th17). Six patients (five men, median age of 53 years) and seven controls were studied. At baseline, hyperuricemic patients had more CD4+CD28null/CD4+ cells than normouricemic subjects (36.8% vs. 6.1%; p = 0.001), with a predominance of T-bet+ cells (98.5% vs. 6.6%; p = 0.001) and few RORγt+ cells (0.7% vs. 89.4%; p = 0.014). In hyperuricemic patients, the number of CD4+ cells/10,000 PBMC was similar before and after allopurinol (3378 vs. 3954; p = 0.843). Conversely, CD4+CD28null cells decreased from 36.8% (23.0-43.7) to 15.8% (4.7-28.1; p = 0.031). CD4+CD28nullT-bet+ cells decreased from 98.5% (95.0-99.4) to 88.3% (75.2-98.9; p = 0.062), CD4+CD28nullGATA-3+ cells increased from 0% (0-4.0) to 2.8% (0.1-15.6; p = 0.156), and CD4+CD28nullRORγt+ cells increased from 0.7% (0.4-7.0) to 4.5% (1.3-28.1; p = 0.031). The CD4+CD28null cell subset is abnormally expanded in chronic hyperuricemia, despite the absence of overt urate-related disease. Allopurinol may partially restore CD28 expression on CD4+ cells while enhancing the homeostatic balance of T helper phenotypes. ClinicalTrials.gov, number NCT04012294.

INTRODUCTION: Anti-Ro52/TRIM21 antibodies are markers for several systemic autoimmune rheumatic diseases (SARD). OBJECTIVE: To assess whether anti-Ro52/TRIM21 antibodies are related to abnormalities in inflammatory circuits. METHODS: Cross-sectional study of consecutive outpatients with SARD. Anti-Ro52/TRIM21 antibodies and serum amyloid A protein were measured by ELISA; panels for 18 cytokines and nine chemokines were analyzed on a Luminex reading platform, while high-sensitivity C-reactive protein (hs-CRP) and complement were measured by nephelometry. RESULTS: Among 167 included patients, 143 had systemic lupus erythematosus (SLE), 16 had primary Sjögren's syndrome and eight had systemic sclerosis; 41 (24%) were positive for anti-Ro52/TRIM21 antibodies. Patients with anti-Ro52/TRIM21 antibodies had higher serum levels of IL-2, IL-4, IL-6, GM-CSF, IL-21, IL-22, hs-CRP and chemokines CCL4, CXCL8, CXCL10 and CXCL12, but lower levels of complement C4. Anti-Ro52/TRIM21 antibody titers were positively correlated with IL-2, IL-4, IL-6, IL-10, IL-21, IL-22, CXCL10, and hs-CRP, and negatively with complements C3 and C4. When only SLE patients were included, no association was identified between anti-Ro52/TRIM21 antibodies and disease activity or organ-specific involvement. CONCLUSIONS: Anti-Ro52/TRIM21 antibodies are associated with aberrant cytokine circuits and elevated levels of angiogenic molecules and neutrophil and monocyte chemoattractants, which suggests an active role for these antibodies in SARD.

Abstract Systemic lupus erythematous (SLE) is an autoimmune disease with clinical manifestations in multiple organs, primarily striking women of reproductive age. Women with SLE can became pregnant such as any other healthy woman and carrier their pregnancy to term due to the improvement of health systems, but their specific inflammatory conditions could affect the microenvironment in which the fetus grows, and influence the development of placenta and the fetal heart. Until now, there is very little evidence of any increased risk of postnatal cardiovascular disease (CVD) in the apparently healthy children from women with SLE, but it is this great variability in the effects of lupus on pregnant products is related to.


Resumen El lupus eritematoso sistémico (LES) es una enfermedad autoinmune que presenta diversas manifestaciones clínicas en múltiples órganos, y afecta principalmente a mujeres en edad reproductiva. Las mujeres con LES se pueden embarazar y llevar a término su embarazo, sin embargo, las condiciones inflamatorias específicas de la madre pueden modificar el microambiente en el que el embrión y el feto se desarrollan y afectar la formación y desarrollo de la placenta y el corazón fetal. Hasta ahora hay muy poca evidencia de que haya un mayor riesgo de enfermedad cardiovascular (ECV) en hijos aparentemente sanos de madres con LES, a pesar de que se sabe que hay un mayor riesgo de alteraciones cognitivas y neuronales, así como de desarrollar enfermedades autoinmunes en esos niños. El objetivo de esta revisión fue realizar una búsqueda bibliografía cruzando palabras clave acerca la enfermedad cardiovascular en hijos sanos de mujeres con LES. La evidencia mostró que la autoinmunidad materna puede favorecer la predisposición para el desarrollo de ECV en sus hijos, por medio de la modificación de señales que alteran el microambiente durante la gestación, lo que puede afectar la respuesta inmunitaria y cambios epigenéticos durante la vida posnatal.

ABSTRACT Background: The inflammatory response in gout disease is induced by the activation of NLR family pyrin domain-containing 3 (NLPR3) signaling pathway mediated by IL-1β release. Objective: The objective of the study was to determine the association between single nucleotide polymorphisms (SNPs) within NLRP3 inflammasome genes and gout susceptibility. Methods: Mexican patients with gout from the National Rehabilitation Institute and General Hospital of Mexico were enrolled. A healthy control group was also included. We analyzed the frequency and allelic distribution of eight SNPs from seven different genes within the NLRP3 inflammasome signaling pathway: TLR4 rs2149356, CD14 rs2569190, NLRP3 rs3806268, NLRP3 rs10754558, CARD8 rs2043211, IL-1β rs1143623, P2RX7 rs3751142, and PPARGC1B rs45520937 SNPs. Results: We found that the SNP rs45520937 of PPARGC1B was associated with the risk of developing gout when it was analyzed using the dominant model (Odds ratio [OR] = 2.30; 95% confidence interval [CI]: 1.09-4.86; p = 0.030), and it is proposed that the adaptor molecule CD14 rs2569190 polymorphism could be associated with a lower risk of gout under an additive model (OR= 0.41;95% CI: 0.16-1.05; p = 0.064). No significant associations were identified for the remaining SNPs. Conclusion: Our findings suggest that the PPARGC1B rs45520937 SNP is associated with gout susceptibility.

Abstract Determining the relative risks of extinction of declining taxa is important to delineate conservation priorities and to guide the investments in conservation. Brazil concentrates the greatest number of endangered avian taxa on Earth, yet demographic information is lacking for most of them. Here we present distance-sampling population density estimates for three endangered bird taxa endemic to the Pernambuco Endemism Center (PEC), the most critically disturbed Atlantic Forest region. The analyzed taxa were the White-shouldered Antshrike Thamnophilus aetiops distans (Endangered), the Brown-winged Mourner Schiffornis turdina intermedia (Vulnerable), and the White-bellied Tody-tyrant Hemitriccus griseipectus naumburgae (Vulnerable). The estimated numbers of individuals/ha in an approximately 1,000 ha forest fragment were 0.21, 0.14, and 0.73, respectively. Our findings corroborated the premise that even taxa classified in similar threat categories based on habitat characteristics alone can have different population densities and consequently, divergent risks of extinction. Although population densities can vary among fragments, the extrapolation of our data to the whole PEC confirmed the Vulnerable status of the Brown-winged Mourner, and indicated the Vulnerable and Least Concern categories for the White-shouldered Antshrike and for the White-bellied Tody-tyrant, suggesting that for the two later taxa, the current classifications (Endangered and Vulnerable) based on their Areas of Occupancy must prevail.


Resumo A determinação dos riscos relativos de extinção dos táxons ameaçados é importante para o delineamento de ações de recuperação e para o direcionamento dos investimentos em conservação. O Brasil é o país que possui o maior número de táxons ameaçados de aves, no entanto, informações sobre aspectos demográficos são inexistentes para a maioria deles. Neste trabalho são apresentadas estimativas de densidades populacionais, baseadas no método de amostragem por distância, para três táxons ameaçados de extinção endêmicos do Centro de Endemismo Pernambuco (CEP), a região mais degradada de toda a Mata Atlântica. Os táxons analisados foram a choca-lisa Thamnophilus aetiops distans (Ameaçada), o flautim-marrom Schiffornis turdina intermedia (Vulnerável) e a maria-de-barriga-branca Hemitriccus griseipectus naumburgae (Vulnerável). Os números de indivíduos/ha estimados para um fragmento de floresta de aproximadamente 1000 ha foram respectivamente 0,21, 0,14 e 0,73. Com isto, foi possível confirmar a premissa de que mesmo táxons classificados em uma mesma categoria de ameaça com base apenas em informações de hábitats podem possuir densidades populacionais bastante divergentes e portanto diferentes graus de riscos de extinção. Embora as densidades populacionais possam variar entre fragmentos, a extrapolação destes dados para toda a área do CEP confirmou a classificação de Vulnerável para o flautim-marrom e indicou as categorias Vulnerável e Pouco Preocupante para a choca-lisa e para a maria-de-barriga-branca, sugerindo que para as duas últimas, as classificações atuais baseadas nos tamanhos das suas Áreas de Ocupação (Ameaçada e Vulnerável) devem prevalecer.

Abstract Scientific collections constitute a valuable source for contributions to scientific research and the training of human resources in systematics, but also other areas of biological knowledge. In this contribution, we intend to discuss these advancements in collections and the role played by FAPESP in sponsoring them, as well as a general overview of the zoological collections in São Paulo state. We also aim to stress the importance of zoological collections and the need for continuous logistic and financial support from institutions and research agencies to maintain and develop these unique repositories of biodiversity. From 1980 to the present, FAPESP supported 118 research projects focused on several areas of zoology that are directly or indirectly associated with collections. There is a constant growth in the number of projects, and the financial support provided by FAPESP through the Biota Program was paramount for the advancement of our knowledge of biodiversity in Brazil. Parallel to the scientific advances, but not less important, this support allowed curators to increase the number of specimens, and to organize, maintain and digitize them in these valuable and irreplaceable collections. Regarding the lack of new taxonomists, it is essential that FAPESP and universities in São Paulo encourage the formation of new academics in zoological groups where specialists are rare. Considering the investment provided by FAPESP, it is quite important that the institutions that benefited from these resources took greater responsibility to safeguard these collections, and they should consider including resources on their budgets to obtain safety certificates, ensuring their permanence for many generations to come. Zoological collections are a heritage of humanity and are essential not only for the improvement of our knowledge of biodiversity but also with direct applications, among other services provided by these biological resources. It is important that research and teaching institutions in São Paulo that house specimens under their care start to value more this important patrimony and this heritage, as these collections represent the most valuable testimony of our impressive biodiversity, records of our past, and windows to our future, essential to our academic, scientific, cultural and social sovereignty.


Resumo As coleções científicas constituem uma fonte valiosa para contribuições à pesquisa científica e para a formação de recursos humanos em sistemática, mas também em outras áreas do conhecimento biológico. Nesta contribuição, pretendemos discutir esses avanços nas coleções e o papel desempenhado pela FAPESP no seu patrocínio, bem como um panorama geral das coleções zoológicas do estado de São Paulo. Também pretendemos enfatizar a importância das coleções zoológicas e a necessidade de apoio logístico e financeiro contínuo de instituições e agências de pesquisa para manter e desenvolver esses repositórios únicos de biodiversidade. Ao longo de 1980 até os dias atuais, a FAPESP apoiou 118 projetos de pesquisa focados em diversas áreas da zoologia, direta ou indiretamente associados a coleções. Há um crescimento constante no número de projetos, e o apoio financeiro da FAPESP por meio do Programa Biota foi fundamental para o avanço do nosso conhecimento sobre a biodiversidade no Brasil. Paralelamente aos avanços científicos, mas não menos importante, este apoio permitiu aos curadores aumentar o número de exemplares, e organizá-los, mantê-los e digitalizá-los nestas valiosas e insubstituíveis coleções. Em relação à falta de novos taxonomistas, é fundamental que a FAPESP e as universidades paulistas estimulem a formação de novos acadêmicos em grupos zoológicos onde os especialistas são raros. Considerando o investimento realizado pela FAPESP, é de suma importância que as instituições beneficiadas com esses recursos tenham maior responsabilidade na salvaguarda desses acervos, devendo considerar a inclusão de recursos em seus orçamentos para obtenção de certificados de segurança, garantindo sua permanência por muitas gerações. As coleções zoológicas são patrimônio da humanidade, e são essenciais não apenas para o aprimoramento do nosso conhecimento sobre a biodiversidade, mas também com aplicações diretas, entre outros serviços prestados por esses recursos biológicos. É importante que as instituições de pesquisa e ensino paulistas que abrigam exemplares sob seus cuidados passem a valorizar mais esse importante patrimônio e essa herança, pois essas coleções representam o testemunho mais valioso de nossa impressionante biodiversidade, registros do nosso passado e janelas para o nosso futuro, essenciais à nossa soberania acadêmica, científica, cultural e social.

RESUMEN

O objetivo do trabalho foi compreender o que esteve em jogo na Câmara Municipal de Vereadores de Porto Alegre, no período de 02 de janeiro a 12 de julho de 2017, em que pese à (re) produção de significados do esporte, recreação e lazer como campo de intervenção de interesse público-social e da ação coletiva 'Somos Todos SME' como forma de luta política para manter a Secretaria. Foi uma pesquisa realizada a partir de uma experiência investigativa de participação observante registrada através de anotações, diários de campo, relatórios e documentos produzidos. Também utilizamos documentos oficiais disponíveis no acervo da Câmara Municipal e no Diário Oficial de Porto Alegre. Implicados por questões teórico-conceituais da obra de Pierre Bourdieu desenvolvemos um processo analítico-interpretativo que nos possibilitou concluir que estiveram em jogo menos os significados do esporte, recreação e lazer como campo de intervenção e mais os significados da ação coletiva que se colocava de maneira crescentemente articulada na cena pública para demandar a manutenção da SME.


This work focuses on understanding what was at stake in the Porto Alegre City Council on January 2-July 12, 2017 regarding (re)production of meanings in sport, recreation and leisure as an intervention field of public-social interest and the collective action 'We're All SME' (We all support the Porto Alegre Municipal Education Department) as a form of political struggle to maintain the Department. The study was based on an investigative experience of observant participation recorded through notes, field diaries, reports and documents produced. We also used official documents publicly available at Porto Alegre' City Council and the Municipal Gazette. Driven by theoretical-conceptual issues from Pierre Bourdieu's work, we developed an analytical-interpretative process that led us to conclude that what was at stake was the meanings of collective action increasingly interconnected in the public scene to demand the maintenance of the Municipal Education Department ­ rather than the meanings of sport, recreation and leisure as a field of intervention.

Intraoperative identification of parathyroid glands is a tough task for surgeons performing thyroid or parathyroid surgery, because the small size, color and shape of these glands hinder their discrimination from other cervical tissues. In 2011, Paras described the autofluorescence of parathyroid glands, a property that could facilitate their intraoperative identification. Parathyroid glands submitted to a 785 nm laser beam emit fluorescence in the near infrared range, with a peak at 822 nm. As the intrinsic properties of secretory tissues may be affected by the exposure to the near infrared light, a situation that could preclude their intraoperative utilization, the authors compared the structural and ultra-structural patterns of rat's thyroid and parathyroid glands submitted to irradiation replicating the conditions that allow their intraoperative identification, with those of non irradiated animals. Twenty-four Wistar rats were divided into six groups: animals of Groups 1, 3 and 5 were submitted under general anesthesia to direct irradiation of the cervical area with a 780 nm LED light for 3 minutes through a cervical incision, and animals of Groups 2, 4 and 6 were submitted to cervical dissection without irradiation. Animals of were euthanized immediately (Groups 1 and 2), at Day 30 (Groups 3 and 4) at and at Day 60 (Groups 5 and 6) and thyroid and parathyroid glands were removed: one lobe was prepared for conventional pathological examination and the other lobe for electron microscopy observed by three experienced pathological experts. Twenty-four samples were prepared for conventional histology and there were no alterations reported in any group. Due to technical problems, only 21 samples were observed by electron microscopy and there were no differences in the ultrastructure of parathyroid and thyroid glands, namely the nuclear pattern, mitochondria, endoplasmic reticulum or secretory granules, in any of the groups. These results confirm the innocuity of near infrared irradiation', allowing its intraoperative utilization.

BACKGROUND: Spread of complex regional pain syndrome (CRPS) outside the affected limb is a well-recognized phenomenon; nevertheless, the actual evolution from CRPS to fibromyalgia is poorly documented. Similar mechanisms have been recently put forward to explain the development of CRPS and fibromyalgia including dorsal root ganglia (DRG) hyperexcitability and small fiber neuropathy. OBJECTIVES: The aims of this study were to describe 3 cases with typical CRPS evolving to full-blown fibromyalgia and to discuss the potential pathogenetic mechanisms linking these debilitating illnesses. METHODS: This was a review of medical records and PubMed search on the relationship between CRPS-fibromyalgia with DRG and small nerve fiber neuropathy. RESULTS: Our 3 cases displayed over time orderly evolution from CRPS to fibromyalgia. Dorsal root ganglion hyperexcitability and small fiber neuropathy have been recently demonstrated in CRPS and in fibromyalgia. Dorsal root ganglia contain the small nerve fiber cell bodies surrounded by glial cells. After trauma, DRG perineuronal glial cells produce diverse proinflammatory mediators. Macrophages, lymphocytes, and satellite glial cells may drive the immune response to more rostrally and caudally located DRG and other spinal cord sites. Dorsal root ganglion metabolic changes may lead to small nerve fiber degeneration. This mechanism may explain the development of widespread pain and autonomic dysfunction. CONCLUSIONS: Clinicians should be aware that CRPS can evolve to full-blown fibromyalgia. Spreading of neuroinflammation through DRG glial cell activation could theoretically explain the transformation from regional to generalized complex pain syndrome.

BACKGROUND: Incidental excision of parathyroid glands is a common event during thyroid surgery and in spite the divergent results that can be obtained from the literature about its clinical significance, all efforts must be used to their preservation. Due to the autofluorescence emitted by parathyroid glands, authors began to use a custom device to inspect thyroidectomy specimens for incidentally removed parathyroid tissue; the results of using this device are presented in this manuscript. METHODS: Specimens of 40 consecutive thyroid surgeries were inspected. Localization of suspect high-fluorescence spots were recorded for confirmation with a pathological exam. Determinations of calcium and parathyroid hormone (PTH) were completed prior to surgery and at 24 hours and 15 days after the operation. RESULTS: Patient age ranged from 36 to 83 years and were predominantly female (82.5%). Calcium values at 24 hours post-operation varied between 7.1 and 9.5. The PTH values ranged between 3 and 77. Thirteen patients (32.5%) presented with biochemical hypocalcemia at 24 hours. At 15 days after the operation, only one patient presented with a calcium value below 8 (PTH: 10.9) with complete normalization 6 months after the surgery. Pathological examination identified eight parathyroid fragments in seven patients. There was no correlation between the presence of parathyroid tissue in the specimen and post-operative hypocalcemia (P=0.254). Eight suspicious areas of augmented fluorescence where detected; seven were coincident with the pathological examination and one was a false positive. One intrathyroidal gland was not identified, resulting a sensitivity of 87.5% and specificity 96.2%. CONCLUSIONS: Although no correlation between incidental parathyroidectomy and hypocalcemia was demonstrated, autofluorescence may be a useful tool for in-table identification of incidentally-removed glands.

PURPOSE OF REVIEW: Vasculitis can cause heart disease and are associated with premature atherosclerosis, causing increased morbidity and mortality. Consequently, it is important to know how they can affect the cardiovascular system in order to detect and treat the abnormalities in earlier phases. RECENT FINDINGS: A clear increasing trend of inpatient burden of myocardial infarction and thromboembolic events in granulomatosis with polyangiitis has been observed lately. Behçet's disease has been linked to an increased risk of atrial fibrillation. Studies showing increased atherosclerosis and thromboembolic phenomena in vasculitis are continuously published. Improvement in imaging techniques has consistently showed that subclinical cardiovascular involvement is frequent. Vasculitis may affect seriously the cardiovascular system causing an important increase in morbidity and mortality. Subclinical involvement is frequent. Early treatment with immunosuppression and sometimes surgery, is of paramount importance to improve the prognosis.

This study evaluates possible changes in weight and the secretory patterns of the thyroid and parathyroid glands irradiated with a 780-nm light-emitting diode (LED) source under conditions allowing their identification in an animal model. The use of near-infrared light sources to detect the parathyroid glands has been a subject of research due to the benefit provided to patients undergoing thyroid and parathyroid surgery. However, few studies have explored possible changes in weight and the secretory patterns of the glands when subjected to light stimulation. Thirty-two male Wistar rats were divided into two groups and subjected to cervical dissection and irradiation of the thyroid-parathyroid region under general anesthesia. The animals in group 1 (irradiated) were exposed to a 780-nm LED light beam for 180 s (dose 1.37 J/cm2), whereas the animals in group 2 (control) were not irradiated. Blood samples were collected pre-exposure, 7 min after exposure, and 30 and 60 days after exposure to measure calcium, parathyroid hormone (PTH), triiodothyronine (T3), thyroxine (T4), and thyroid-stimulating hormone (TSH) levels in both groups. Weight variations between the evaluation periods were also analyzed. Parametric statistics were used (Student's t test and ANOVA) after assuming normal distribution of continuous variables with the Shapiro-Wilk test. No significant variation was observed in any of the analyzed parameters pre- and postexposure. A significant increase in the calcium level was observed at 30 days in the irradiated group compared with that in the control group (11.156 ± 1.31 mg/dl vs 10.300 ± 0.30 mg/dl; df = 30 p < 0.03) but this change was no longer observed at 60 days (9.925 ± 0.23 mg/dl vs 10.014 ± 0.18 mg/dl; df = 14 p = 0.21). Irradiated rats gained less weight in the first 30 days after surgery (39.647 ± 32.39 vs 55.917 ± 22.61; df = 30 p = 0.146) and at 60 days (84.000 ± 27.36 vs 84.571 ± 5.62; df = 14 p = 0.957), no differences were observed between the two groups. No significant variations in weight development or changes in the secretory patterns of the thyroid and parathyroid glands exposed to near-infrared stimulation were observed, confirming the safety of this light source in the identification of the parathyroid glands.

BACKGROUND: Intra-operative identification of parathyroid glands is often a challenge for surgeons performing parathyroid or thyroid surgery. Parathyroid glands stimulated by near-infrared light emit autofluorescence, which allows their discrimination from all other tissues in the region, and this may be of value during thyroid and parathyroid surgery. In this study, we present the results of the utilization of a low-cost device developed for the identification of parathyroid glands in surgery for primary hyperparathyroidism. CASE PRESENTATION: In 5 patients operated in our hospital with the diagnosis of primary hyperparathyroidism and non-concordant ultrasonography and Sestamibi scan, we used a 780 nm Light Emitting Diode (LED) to stimulate the cervical area. The resulting autofluorescence was visualized with night vision goggles with a 832 nm filter assembled. In all the five patients, an easily distinguishable nodule was identified and excised, and confirmed as parathyroid adenoma by histological exam. Intra-operative PTH assay showed significant decrease compared with basal values, fulfilling the Miami Criteria for surgical success in use in our institution. CONCLUSION: The utilization of autofluorescence for intra-operative identification of parathyroid glands may have a clinical application in surgery for primary hyperparathyroidism, being of special utility when ultrasonography and Sestamibi Scan are non concordant.

This study evaluated the effects of the sugarcane borer Diatraea saccharalis (Fabricius, 1794) (Lepidoptera: Crambidae) on cultivars of sweet and biomass sorghum for the selection of resistant cultivars. The present work consisted of two trials, with natural pest infestation. In the first one, 10 sweet sorghum cultivars were analyzed for the following variables: plant height, number of healthy and damaged internodes, gallery position and size, stem infestation level and soluble solids content (°Brix). In the second trial, it was analyzed 16 genotypes of high biomass sorghum, with the same variables above mentioned, in addition to the lignin, cellulose and hemicellulose contents. Among sweet sorghum genotypes evaluated, the genotype CMSXS647 stood out due to the traits: plant height, infestation level, gallery size and soluble solids content. Among the sorghum genotypes evaluated, CMSXS7030, CMSXS7012 and CMSXS7028 presented ideal characteristics for infestation level, plant height and number of lignocellulosic compounds. Such information, in addition to supporting the bioenergy sorghum breeding program, will assist in integrated pest management for sorghum cultivation.


Foram estudados os efeitos causados pela broca-do-colmo Diatraea saccharalis (Fabricius, 1794) (Lepidoptera: Crambidae), em cultivares de sorgo sacarino e biomassa visando seleção de cultivares resistentes à praga. O presente trabalho foi constituído de dois ensaios, com infestação natural da praga. No primeiro, 10 cultivares de sorgo sacarino foram analisadas quanto às seguintes variáveis: altura das plantas, quantidade de internódios sadios e com injúrias, posição e tamanho da galeria, intensidade de infestação de colmos e teor de sólidos solúveis (°Brix). No segundo ensaio, foram analisados 16 genótipos de sorgo biomassa, com as mesmas variáveis supracitadas, além dos teores de lignina, celulose e hemicelulose. Entre os genótipos de sorgo sacarino avaliados, o genótipo CMSXS647 foi o que se destacou em função das características: altura de plantas, intensidade de infestação, tamanho de galerias e teor de sólidos solúveis. Entre os genótipos de sorgo biomassa avaliados: CMSXS7030, CMSXS7012 e CMSXS7028 apresentaram características ideais para intensidade de infestação, altura de plantas e quantidade de compostos lignocelulósico. Tais informações, além de prover o programa de melhoramento de sorgo energia podem ajudar o programa de MIP para a cultura do sorgo, uma vez que o produtor conhece a suscetibilidade dos materiais escolhidos.

BACKGROUND: The frequency of depression and anxiety symptoms in Spanish-speaking patients suffering from rheumatic conditions is unknown when using self-administered detection tools. METHODS: A single-center, cross-sectional survey including 413 patients (341 women) with well-defined rheumatic diseases was conducted. The patient health questionnaire-9 (PHQ-9) and generalized anxiety disorder (GAD)-7 questionnaires were used to detect depression and anxiety symptoms, respectively. RESULTS: A total of 193 patients (46.7%) reported depression symptoms, and increased PHQ-9 scores were more frequently observed in women than in men (23% vs. 13%; p = 0.038), particularly in association with osteoarthritis, fibromyalgia, Sjögren's syndrome, and osteoporosis. From 88 patients (21.3%) with PHQ-9 scores ≥ 10 points (moderate-to-severe depression symptoms), 27 (30.6%) were previously diagnosed to have depression and only four were under antidepressant treatment. Anxiety symptoms were observed in 168 patients (40.6%) and classified as moderate-to-severe by elevated GAD-7 scores in 68 subjects (16.4%). Of them, 12 (17.6%) were previously diagnosed with GAD, but only 4 (5.8%) were under therapy. CONCLUSIONS: An unexpected and unusually high frequency of undiagnosed depression and anxiety symptoms was found in rheumatic patients. Self-administered screening tools adapted to the Spanish language are useful and may help clinicians to suspect these conditions.
