RESUMEN

OBJECTIVE: This study aims to explore the intervention effects of narrative nursing on the reproductive concerns of cervical cancer patients of childbearing age undergoing surgical treatment. METHODS: Patients undergoing cervical cancer surgery with moderate to severe levels of reproductive concerns, treated between January and December 2023 at a tertiary Grade-A oncology hospital in China, were selected as the research subjects. Patients were randomized into an intervention group and a control group, each consisting of 33 patients. The control group received standard nursing care, while the intervention group received a narrative nursing intervention in addition to standard care. The changes in the levels of reproductive concerns, post-traumatic growth, and quality of life scores before and after the intervention were compared between the two groups. RESULTS: After the intervention, the reproductive concerns scores of the cervical cancer patients in the intervention group (32.53 ± 4.77) were significantly lower than those in the control group (59.29 ± 3.24), with a statistically significant difference (t = 26.143, p < 0.001). The post-traumatic growth scores in the intervention group (86.78 ± 3.52) were significantly higher than those in the control group (68.06 ± 6.24), with a statistically significant difference (t = -14.595, p < 0.001). The quality of life scores in the intervention group (149.00 [IQR = 8.75]) were significantly higher than those in the control group (129.00 [IQR = 13.00]), with a statistically significant difference (z = -5.799, p < 0.001). CONCLUSION: Narrative nursing can effectively alleviate reproductive concerns in cervical cancer patients undergoing surgical treatment, promote positive psychological changes post-trauma, and improve patients' quality of life.

RESUMEN

Narrative care for families suffering from perinatal loss is rarely provided by medical institutions in China Mainland. However, with the advancement of the Chinese narrative medicine theory and practice, the clinical significance of narrative care has been increasingly recognized. Based on the principles of Chinese narrative medicine, this narrative case study described traumatic narrative foreclosures occuring in a family suffering from stillbirth, and highlighted the multidisciplinary collaboration for practising narrative care in the process of supporting the bereaved in our hospital. Meanwhile, we advocate the establishment of a narrative care ecology by training more obsteticians and nurses with good narrative competence in purpose of helping the family experiencing perinatal losses to overcome their tramatic narrative foreclosures, increasing the chances of another successful pregnancy and childbirth as well as enhancing their quality of life.

La terapia narrativa surge como una propuesta de intervención, debido a que este modelo permite que las personas puedan generar un proceso donde se desarrolle de manera integral lo que han padecido, y, de esa manera, poder resignificarlo; consiguientemente, cada paciente puede, así, superar la experiencia traumática (Ihl y Diaz, 2021). Método: Este trabajo de integración final de diseño teórico de revisión bibliográfica se propuso analizar la posible eficacia de la terapia narrativa como intervención clínica con jóvenes que han sido víctimas del abuso sexual infantil. Síntesis y conclusiones: La terapia narrativa propone un espacio respetuoso, flexible y de contención; se establece un entorno en el que el paciente tiene la oportunidad de contar su experiencia traumática, sin estar obligado a relatarla. Es decir, este abordaje no se enfoca en la exposición del trauma, ni exige que la persona relate la situación de abuso en un momento específico; la terapia narrativa propone trabajar con los efectos y las consecuencias del trauma AU


Narrative therapy emerges as an intervention proposal because this model allows people to generate a process where what they have suffered is developed in an integral way, and, in this way, to resignify it; consequently, each patient can thus overcome the traumatic experience (Ihl & Diaz, 2021). ABSTRACT Objective: To know the repercussions of the experience of family violence from the oral history of adolescents. Method: Qualitative study based on the Oral History method, conducted from interviews with adolescents enrolled in a public school in Salvador, State of Bahia, Brazil. The data were systematized according to the thematic analysis and supported by theoretical references on family violence and adolescence. Results: Family violence implies physical illness associated with the damage caused by physical aggression and somatization of violent events, as well as compromising mental health, provoking feelings of deep sadness, self-injury behavior and suicidal ideation. All of these factors impair interpersonal relationships, school performance, as well as making them more vulnerable to alcohol intake. Final Considerations: The study indicates signs suggestive of grievance, from which the professionals should proceed the investigation in order to refute or confirm the experience of the phenomenon, as well as intervene in the cases.


RESUMEN Objetivo: Conocer las repercusiones derivadas de la violencia intrafamiliar a partir de historias orales de adolescentes. Método: Se trata de una investigación cualitativa basada en el método de la Historia Oral, realizada mediante entrevistas a alumnos adolescentes de una escuela pública de Salvador, Bahía, Brasil. Se sistematizaron los datos según el análisis temático con respaldo en referenciales teóricos sobre violencia intrafamiliar y adolescencia. Resultados: La violencia intrafamiliar trae como consecuencia enfermedades asociadas a la agresión física padecida, somatizando el penoso evento y comprometiendo la salud mental, ya que suscita sentimientos de profunda tristeza, comportamiento de autolesión e ideas suicidas. Todos estos factores perjudican las relaciones interpersonales, el rendimiento escolar y propician el uso del alcohol. Consideraciones Finales: El estudio señala la existencia del agravio, a partir del cual los profesionales deben realizar investigaciones en el sentido de refutar o confirmar la vivencia del fenómeno e intervenir en los casos.


RESUMO Objetivo: Conhecer as repercussões da vivência de violência intrafamiliar a partir da história oral de adolescentes. Método: Pesquisa qualitativa, fundamentada no método da História Oral, realizada a partir de entrevistas com adolescentes matriculadas(os) em uma escola pública de Salvador, Bahia, Brasil. Os dados foram sistematizados conforme a análise temática e respaldados em referenciais teóricos sobre violência intrafamiliar e adolescência. Resultados: A violência intrafamiliar implica adoecimento físico associado aos danos decorrentes da agressão física e à somatização do evento violento, bem como compromete a saúde mental, suscitando sentimentos de tristeza profunda, comportamento de autolesão e ideação suicida. Todos esses fatores prejudicam as relações interpessoais, o desempenho escolar, bem como vulnerabiliza para o uso de álcool. Considerações Finais: O estudo aponta sinais sugestivos do agravo, a partir dos quais os profissionais deverão proceder a investigação no sentido de refutar ou confirmar a vivência do fenômeno, bem como intervir diante dos casos.

BACKGROUND: People living with life-threatening illness experience unmet existential needs despite the growing research and clinical field of palliative care. Narrative interventions show promise in managing these problems, but more knowledge is needed on the characteristics of narrative interventions and the feasibility of using personal narratives in a hospital. AIM: To review the literature on personal narratives in hospital-based palliative care interventions and to strengthen palliative care practices. DESIGN: We conducted a systematic integrative review with qualitative analysis and narrative synthesis in accordance with PRISMA where applicable (PROSPERO#:CRD42018089202). DATA SOURCES: We conducted a systematic search in PubMed, Embase, Scopus, Cinahl, SocINDEX and PsychInfo for primary research articles published until June 2018. We assessed full-text articles against the eligibility criteria followed by a discussion of quality using the Critical Appraisal Skills Programme. RESULTS: Of 480 articles, we found 24 eligible for this review: 8 qualitative, 14 quantitative and 2 mixed methods. The articles reported on dignity therapy, legacy building, outlook, short-term life review and life review. Data analysis resulted in five themes: core principles, theoretical framework, content of narrative, outcome and, finally, acceptability and feasibility. CONCLUSION: Various types of systematic palliative care interventions use personal narratives. Common to these is a shared psychotherapeutic theoretical understanding and aim. Clinical application in a hospital setting is both feasible and acceptable but requires flexibility regarding the practices of the setting and the needs of the patient.

RESUMEN

This article describes the 'Beads of Life' approach--a five-part methodology informed by narrative therapy to enable children and young people to make sense of their cancer journey in ways that make them stronger. Young people are invited to use beads as prompts to tell preferred stories of their identity to create a safe place to stand from which to story their cancer journey. The approach positions young people as experts in their lives. It aims to change their relationship with cancer to reduce its negative impact on life by lessening isolation. By enabling medical staff to get to know the young person apart from the cancer, this approach aims to create hope for the future and improve quality of care.

OBJECTIVE: The aim of this study was to examine the effect of spiritual interventions on quality of life of cancer patients. METHODS: We conducted our search on June 6, 2014 in Medline, PsycINFO, Embase, and PubMed. All clinical trials were included that compared standard care with a spiritual intervention that addressed existential themes using a narrative approach. Study quality was evaluated by the Cochrane Risk of Bias Tool. RESULTS: A total of 4972 studies were identified, of which 14 clinical trials (2050 patients) met the inclusion criteria, and 12 trials (1878 patients) were included in the meta-analysis. The overall risk of bias was high. When combined, all studies showed a moderate effect (d) 0.50 (95% CI = 0.20-0.79) 0-2 weeks after the intervention on overall quality of life in favor of the spiritual interventions. Meta-analysis at 3-6 months after the intervention showed a small insignificant effect (0.14, 95% CI = -0.08 to 0.35). Subgroup analysis including only the western studies showed a small effect of 0.17 (95% CI = 0.05-0.29). Including only studies that met the allocation concealment criteria showed an insignificant effect of 0.14 (95% CI = -0.05 to 0.33). CONCLUSIONS: Directly after the intervention, spiritual interventions had a moderate beneficial effect in terms of improving quality of life of cancer patients compared with that of a control group. No evidence was found that the interventions maintained this effect up to 3-6 months after the intervention. Further research is needed to understand how spiritual interventions could contribute to a long-term effect of increasing or maintaining quality of life.

This case study presents the use of poetry in psychotherapy with an adolescent girl, Buhle (a pseudonym), who needed surgery to correct a curvature of her spine due to adolescent idiopathic scoliosis. She experienced anxiety which prevented surgeons from doing the procedure. Psychotherapists used narrative therapy to explore issues associated with and contributing to her anxiety and encouraged her to document her experiences through poetry, after learning that she was a keen poet. During psychotherapy Buhle's poems were used to track and narrate her experiences and as an empowering method allowing her to make personal sense of challenging experiences. Buhle's poems are presented within an account of the psychotherapy leading up to the surgery. Her poetry reveals a juxtaposition of regular adolescent identity issues in the face of coping with a demanding medical condition and the prospect of invasive surgery.

PURPOSE: The purpose of this article is to highlight some new findings from a qualitative study that have not been previously considered. METHOD: The research was based on a qualitative phenomenological method. Data were collected by semi-structured individual interviews. A purposeful sample was recruited from West Norway of 14 women with a possible risk of developing breast and ovarian cancer. Data collection took place at least one year after prophylactic removal of the ovaries. Data from the study was analyzed according to the phenomenological method of Amedeo Giorgi. KEY FINDINGS: Two themes were revealed as essential factors of the interviewed women's experiences: "the loss of a mother" and "dealing with genetic cancer risk." For the most part, these two themes also appeared to be interrelated. When a mother died of ovarian cancer while her daughter was a child or a young woman, this often led to her daughter's strong desire to find an explanation for her mother's death, as well as to her efforts to undergo genetic testing and surgery to prevent cancer in her family in the future. CONCLUSION: The study indicates that women's experiences in families at risk of hereditary cancer are closely related to an understanding of their life stories, particularly their "loss of a mother," and how this influences how they deal with genetic cancer risk. Health care workers can thus help patients identify connections and establish coherence through the act of storytelling, by listening to their illness experiences as part of their life stories.

UNLABELLED: Mindfulness-based narrative therapy (MBNT) is a therapeutic intervention for the treatment of depression in cancer patients. In a previous randomized controlled trial, MBNT was found to ameliorate anxiety and depression, improve functional dimensions of quality of life, and enhance treatment adherence. In this review, we describe MBNT and its technical characteristics in the context of other psychotherapeutic interventions for depression in cancer patients. We highlight needed adjustments to other narrative approaches and recommend clinical modifications tailored to the needs of cancer patients that are intended to encompass the client's initial depressive narrative. The narrative construction is supported by emotional regulation and attachment relationships on the one hand and by individual and social linguistic capabilities on the other. Through destabilization of the depressive narrative, MBNT facilitates the emergence of new meanings using both verbal and non-verbal techniques based on mindfulness. The attitude and practice of mindfulness are integrated throughout the therapeutic process. In summary, MBNT makes use of linguistic interventions, promotes mindfulness and emotional regulation, and can be adapted specifically for use with cancer patients. KEY PRACTITIONER MESSAGE: In this review, we describe mindfulness-based narrative therapy (MBNT) for the treatment of depression in cancer patients. In a previous controlled trial, we found significant benefits of MBNT in terms of reducing depressive symptoms and improving treatment adherence and quality of life in depressed, non-metastatic cancer patients. Narrative construction is socially and neurobiologically derived. MBNT makes use of linguistic interventions, promotes mindfulness and emotional regulation, and can be adapted specifically for use with cancer patients. MBNT is proposed as an interesting and promising intervention, particularly for patients with somatic pathologies.

RESUMEN

This article describes a technique for helping recreational tobacco-using Native American clients change their smoking lifestyle. The technique is based in social work practice with a specific focus on the use of narrative therapy. The foundation of narrative therapy can be used across ethnicities for smoking-related issues. Finally, we propose the use of a clinical instrument, called the Success Journal, for practitioner use.

O objetivo deste estudo foi analisar as contribuições da terapia comunitária integrativa considerando as mudanças de comportamentos de usuários de um Centros de Atenção Psicossocial (CAPS). Trata-se de uma pesquisa compreensivo-interpretativa de abordagem qualitativa, onde se utilizou a história oral temática. O cenário da investigação foi o CAPS Caminhar, localizado na cidade de João Pessoa, Paraíba, Brasil. O material empírico foi produzido a partir de entrevistas realizadas com seis colaboradores, e o produto desse material foi discutido com base na análise temática proposta por Minayo, que subsidiou a construção de dois grandes eixos temáticos: A terapia comunitária integrativa como práxis libertadora e mudanças que fazem a diferença. As histórias dos colaboradores revelaram que ocorreram mudanças significativas nos campos pessoal, profissional e comunitário, a partir da inserção deles nas rodas de terapia comunitária integrativa, pois se verificou que tal estratégia promoveu a recuperação dos processos de socialização natural que constituem a vida humana. Ficou claro que o uso da terapia comunitária integrativa está relacionado a propostas de inclusão e reabilitação psicossocial de seus participantes.


The aim of this study was to analyze contributions by integrative community therapy to behavior changes in users of Psychosocial Care Centers (CAPS). This was a comprehensive-interpretative study with a qualitative approach, based on thematic oral history. The study site was the Caminhar Center in João Pessoa, Paraíba State, Brazil. The study material was produced with interviews conducted with six subjects and was discussed using thematic analysis as proposed by Minayo, providing the basis for two major thematic lines: integrative community therapy as a liberating praxis and changes that make the difference. The subjects' stories revealed significant changes in the personal, professional, and community fields, based on their inclusion in the integrative community therapy circles, a strategy that promoted the recovery of processes of natural socialization that constitute human life. The use of integrative community therapy was clearly related to proposals for the participants' psychosocial integration and rehabilitation.


El objetivo de este estudio fue analizar las contribuciones de la terapia comunitaria en relación con los cambios en el comportamiento de los usuarios de un Centro de Atención Psicosocial (CAPS). Se trata de un enfoque cualitativo de investigación interpretativa global, en el que se utilizó un conjunto de historias orales temáticas. El centro de la investigación fue el CAPS Caminhar, ubicado en la ciudad de João Pessoa, Paraíba, Brasil. Los datos empíricos fueron obtenidos de entrevistas con seis empleados y el producto de este material se discutió sobre la base del análisis temático propuesto por Minayo, que promovió la construcción de dos grandes temas: terapia comunitaria como praxis liberadora y los cambios que marcan la diferencia. Las historias de los empleados revelaron que se produjeron cambios significativos en los ámbitos personales, profesionales y de la comunidad en la que se encuentran, además de en las sesiones de terapia comunitaria, ya que se comprobó que dicha estrategia promueve la recuperación de los procesos de socialización naturales que constituyen la vida humana. Estaba claro que el uso de las terapias comunitarias está relacionado con las propuestas para la inclusión y la rehabilitación.

OBJECTIVE: Although the Short-Term Life Review elevated the spiritual well-being of terminally ill cancer patients in our previous study, we have not examined what patients reviewed for each question item of it. We examined factors in narratives to questions in the Short-Term Life Review interviews of terminally ill cancer patients and utility of the questions. METHOD: Thirty-four terminally ill cancer patients received the Short-Term Life Review interview in which there were two sessions. In the first session patients reviewed their lives, and an interviewer made a simple album of the patient based on patients' narrative. After 1 week, there was a second session. Qualitative analysis was conducted on patients' answers to each question using computational word mining, and factors were identified. RESULTS: Twenty patients' narratives were analyzed. "Human relationships" was identified under "important things in life." "Pleasant memories" were associated with "impressive memories." "Illness" and "marriage and divorce" were related to "turning points in the life." "Raising children and education" and "company or work" were identified as "roles in life." "Achievements at work" were identified with "pride." "Message to my children" was identified with "what I want to say to my family." "To live sincerely" and "consideration for others" were identified as "advice for the next generation." Patients reviewed few for topics such as "pride," "what I want to say to my family," "advice for the next generation," and "summing up my life." SIGNIFICANCE OF RESULTS: Factors such as human relationships, raising children, and education as a role and source of pride, and concerns about children's future, were associated with elevating spiritual well-being. Question to which that patients easily answered were selected.
