RESUMEN

Dehiscence is a feared complication after major surgeries. Patient who had undergone coronary artery bypass grafting developed saphenectomy's dehiscence on lower limb with edema and pain on the 15th postoperative day. Conventional treatment had been initially performed without clinical improvement. On the 30th postoperative day only Low Level Laser Therapy (LLLT) was applied punctually around surgical wounds edge. The results revealed granulated tissue, reduction of inflammatory process and analgesic effect since the first application. In this pilot study, LLLT has shown a considerable role as a wound healing agent, through a new proposal for efficient, safe and noninvasive therapy.

RESUMEN

Deiscência é uma complicação temida em cirurgias de grande porte. Paciente submetida a revascularização miocárdica evoluiu com deiscência de safenectomia em membro inferior, edema e dor no 15º dia pós-operatório (PO), tendo sido realizado inicialmente o tratamento convencional no ambulatório sem melhora clínica. No 30º PO, aplicou-se somente Laser de Baixa Intensidade (LBI) ao redor da borda da ferida, pontualmente. A lesão respondeu com tecido de granulação, diminuição do processo inflamatório e analgesia desde a primeira aplicação. Neste estudo piloto, a laserterapia mostrou ter um papel importante como agente facilitador de cicatrização, por meio de uma terapia nãoinvasiva, eficaz e segura.

RESUMEN

OBJECTIVE: To explore the effect of intravascular low level He-Ne laser irradiation on skin flap survival after orthotopic transplantation in avulsion injury. METHODS: Fifty eight cases suffered avulsion injury were treated by debridement and orthotopic transplantation of avulsed flap within 6 hours, 31 of them were received intravascular low level He-Ne laser irradiation and routine treatment, and 27 of them were received routine treatment as control group. RESULTS: The survival area and quality of avulsed flap in the experimental group were superior to that of control group after 15 days of operation, and the hemorheological items were markedly changed at 5 days after operation. CONCLUSION: The better flap survival after orthotopic transplantation in avulsion injury can be improved by intravascular low level He-Ne laser irradiation through changed superoxide dismutase activity and hemorheological items in optimal irradiation intensity.

The authors have studied the results of 120 experiments on 60 guinea-pigs and 110 rabbits who received the treatment of modelled fenestrated musculocutaneous wounds and trophic ulcers respectively on the replanted extremity and on the back with helium and neon laser until they healed completely. The authors have demonstrated that the laser therapy accelerates the process of reparative regeneration. In 50 intact rabbits who received local irradiation with helium and neon laser there was an increase in the activity of lysosome enzymes and in the protein metabolism values in the blood serum. The use of the helium and neon laser with the specific density of the irradiation rate of 2.5 mWt/cm2 and the exposition during 5 minutes in the treatment of trophic ulcers and persistent wounds in 56 patients as well as in 38 patients with consequences of injuries to the main blood vessels, the nerve trunks, the muscles and the tendons of the extremities before and after the reconstructive operations testifies to its high efficacy.
