PURPOSE: The aim of the present study is to investigate the prognostic significance of nutritional risk factors and sarcopenia on the outcome of patients with recurrent gynaecological malignancies treated by pelvic exenteration. METHODS: We retrospectively evaluated muscle body composite measurements based on pre-operative CT scans, nutritional risk factors as assessed by a validated pre-operative questionnaire, and clinical-pathological parameters in 65 consecutive patients with recurrent gynaecological malignancies, excluding ovarian cancer, treated by pelvic exenteration at the Royal Marsden Hospital London. Predictive value for postoperative morbidity was investigated by logistic regression analyses. Relevant parameters were included in uni- and multivariate survival analyses. RESULTS: We found only (1) low muscle attenuation (MA)-an established factor for muscle depletion-and (2) moderate risk for malnutrition to be independently associated with shorter overall survival (p = 0.006 and p = 0.008, respectively). MA was significantly lower in overweight and obese patients (p = 0.04). Muscle body composite measurements were not predictive for post-operative morbidity. CONCLUSION: The study suggests that pre-operative low MA and moderate risk for malnutrition are associated with shorter survival in patients with recurrent gynaecological malignancies treated with pelvic exenteration. Further studies are needed to validate these findings in larger cohorts.

The PI3K/AKT/mTOR and PIM kinase pathways contribute to the development of several hallmarks of cancer. Cotargeting of these pathways has exhibited promising synergistic therapeutic effects in liquid and solid tumor types. To identify molecules with combined activities, we cross-screened our collection of PI3K/(±mTOR) macrocycles (MCXs) and identified the MCX thieno[3,2-d]pyrimidine derivative 2 as a moderate dual PI3K/PIM-1 inhibitor. We report the medicinal chemistry exploration and biological characterization of a series of thieno[3,2-d]pyrimidine MCXs, which led to the discovery of IBL-302 (31), a potent, selective, and orally bioavailable triple PI3K/mTOR/PIM inhibitor. IBL-302, currently in late preclinical development (AUM302), has recently demonstrated efficacy in neuroblastoma and breast cancer xenografts. Additionally, during the course of our experiments, we observed that macrocyclization was essential to obtain the desired multitarget profile. As a matter of example, the open precursors 35-37 were inactive against PIM whereas MCX 28 displayed low nanomolar activity.

Activation of the phosphatidylinositol 3-kinase (PI3K)/mammalian target of rapamycin (mTOR) signaling pathway occurs frequently in a wide range of human cancers and is a main driver of cell growth, proliferation, survival, and chemoresistance of cancer cells. Compounds targeting this pathway are under active development as anticancer therapeutics and some of them have reached advanced clinical trials or been approved by the FDA. Dual PI3K/mTOR inhibitors combine multiple therapeutic efficacies in a single molecule by inhibiting the pathway both upstream and downstream of AKT. Herein, we report our efforts on the exploration of novel small molecule macrocycles (MCXs) as dual PI3K/mTOR inhibitors. Macrocyclization is an attractive approach used in drug discovery, as the semi-rigid character of these structures could provide improved potency, selectivity and favorable pharmacokinetic properties. Importantly, this strategy allows access to new chemical space thus obtaining a better intellectual property position. A series of MCXs based on GSK-2126458, a known clinical PI3K/mTOR inhibitor is described. These molecules showed potent biochemical and cellular dual PI3K/mTOR inhibition, demonstrated strong antitumoral effects in human cancer cell lines, and displayed good drug-like properties. Among them, MCX 83 presented remarkable selectivity against a panel of 468 kinases, high in vitro metabolic stability, and favorable pharmacokinetic parameters without significant CYP450 and h-ERG binding inhibition. This profile qualified this compound as a suitable candidate for future in vivo PK-PD and efficacy studies in mouse cancer models.

Ovarian cancer is the third most frequent gynaecological malignancy worldwide and in Mexico, with a high mortality rate, due to that in many cases its diagnosis is made in advanced stages. Prognosis is important for determining the subtype and the degree of evolution. During lasts years, the management of ovarian cancer has undergone an important evolution with the incorporation of new therapeutic options, which in turn represent an increase in the survival of these patients. We present recommendations for the management of ovarian cancer developed by an expert panel Mexican based on available evidence so far and the characteristics of health care in the country.


El cáncer de ovario es la tercera neoplasia maligna ginecológica más frecuente globalmente y también en México, con una elevada tasa de mortalidad debido a que en muchos casos su diagnóstico se realiza en etapas avanzadas. Para establecer su pronóstico es importante la determinación del subtipo y del grado de evolución. En los últimos años, el manejo del cáncer de ovario ha sufrido una importante evolución con la incorporación de nuevas opciones terapéuticas, que a su vez representan un incremento en la supervivencia de estas pacientes. Se presentan las recomendaciones para el manejo del cáncer de ovario elaboradas por un panel de expertos mexicanos basadas en la evidencia disponible hasta el momento y en las características de la atención sanitaria del país.

Very few data exist on when a particular thrombopoietin-receptor agonist (TPO-RA) is favored in clinical practice for the treatment of patients with immune thrombocytopenia (ITP), about novel risk factors for vascular events (VE) with these drugs, nor about predictive factors for therapy free responses (TFR). We conducted an observational, retrospective, long-term follow-up multicenter study from November 2016 to January 2018 of 121 adult ITP patients initiating TPO-RA between January 2012 to December 2014. Data reflected that a platelet count ≤25 × 109/l at the time when the TPO-RA was initiated was associated with a 2.8 higher probability of receiving romiplostim vs. eltrombopag (P = 0.010). VE on TPO-RA was related to previous neoplasia in patients over 65 years (50% vs. 2.2%, P < 0.001), and to previous splenectomy in younger patients (100% vs. 33%, P = 0.001). Receiving romiplostim as first TPO-RA with no subsequent TPO-RA switching was associated with a 50% likelihood of TFR after 2.9 years of therapy (3.3 years in chronic ITP patients). These real-world data help deciphering some areas of uncertainty, and offer insight into some of the most relevant challenges of ITP which may help clinicians make appropriate treatment decisions in the management of adult ITP patients with TPO-RA.

The objective of the study was to validate externally and prospectively the PROFUND index to predict survival of polypathological patients after a year. An observational, prospective and multicenter study was performed. Polypathological patients admitted to an internal medicine or geriatrics department and attended by investigators consecutively between March 1 and June 30, 2011 were included. Data concerning age, gender, comorbidity, Barthel and Lawton-Brody indexes, Pfeiffer questionnaire, socio-familial Gijon scale, delirium, number of drugs and number of admissions during the previous year were gathered for each patient. The PROFUND index was calculated. The follow-up lasted 1 year. A Cox proportional regression model was calculated, and was used to analyze the association of the variables to mortality and C-statistic. 465 polypathological patients, 333 from internal medicine and 132 from geriatrics, were included. One-year mortality is associated with age [hazard ratio (HR) 1.52 95 % CI 1.04-2.12; p = 0.01], presence of neoplasia [HR 2.68 95 % CI 1.71-4.18; p = 0.0001] and dependence for basic activities of daily living [HR 2.34 95 % CI 1.61-3.40; p = 0.0009]. In predicting mortality, the PROFUND index shows good discrimination in patients from internal medicine (C-statistics 0.725 95 % CI 0.670-0.781), but a poor one in those from geriatrics (0.546 95 % CI 0.448-0.644). The PROFUND index is a reliable tool for predicting mortality in internal medicine PP patients.

A new chemical series, triazolo[4,5-b]pyridines, has been identified as an inhibitor of PIM-1 by a chemotype hopping strategy based on a chemically feasible fragment database. In this case, structure-based virtual screening and in silico chemogenomics provide added value to the previously reported strategy of prioritizing among proposed novel scaffolds. Pairwise comparison between compound 3, recently discontinued from Phase I clinical trials, and molecule 8, bearing the selected novel scaffold, shows that the primary activities are similar (IC(50) in the 20 to 150 nM range). At the same time, some ADME properties (for example, an increase of more than 45% in metabolic stability in human liver microsomes) and the off-target selectivity (for example, an increase of more than 2 log units in IC(50)vs. FLT3) are improved, and the intellectual property (IP) position is enhanced. The discovery of a reliable starting point that fulfills critical criteria for a plausible medicinal chemistry project is demonstrated in this prospective study.

El hipotiroidismo subclínico puede contribuir al desarrollo de patologías cardiovasculares y se ha demostrado que a partir de niveles de TSH >2,5uUI/mL se desarrolla disfunción endotelial. Objetivos: Determinar el grosor de la íntima media carotídea (GIMC), la presencia de placas en carótidas y el perfil metabólico en mujeres de edad media asintomáticas con TSH =2,5 uUI/mL y compararlas con aquellas con niveles <2,5 uUI/mL. Diseño: Estudio transversal y analítico. Lugar: Hospital Nacional Dos de Mayo e Instituto de Investigaciones Clínicas, Universidad Nacional Mayor de San Marcos, Lima, Perú. Participantes: Mujeres sin historia de enfermedad tiroidea, cardiovascular o diabetes. Intervenciones: En 60 mujeres sin historia de enfermedad tiroidea, cardiovascular o diabetes, con edad promedio de 53,8 ± 5,8 años, se determinó la tirotropina (TSH), colesterol total (CT), colesterol de densidad alta (HDL), triglicéridos (Tg), glucosa (G) basal y a los 120 minutos (TTGO), insulina basal (Ins-B); se calculó las fracciones de colesterol de densidad baja (LDL), el nivel de insulinorresistencia (HOMA-IR), la presión arterial, perímetro de cintura abdominal, el índice de masa corporal (IMC) y el GIMC mediante ecoDoppler. Principales medidas de resultados: TSH =2,5 uUI/mL, perfil metabólico y su relación con GIMC. Resultados: El 38,3 por ciento presentó TSH =2,5 y 61,7 por ciento TSH <2,5 uUI/mL. El 56 por ciento de mujeres con TSH =2,5 y 65 por ciento con TSH <2,5 uUI/ml fueron hipertensas, sin diferencia estadística. El perfil lipídico, G basal, Ins-B e índice HOMA-IR fueron semejantes en ambos grupos. Se observó niveles significativamente más altos del IMC, G a los 120 minutos, el GIMC en carótida izquierda y el mayor entre ambas carótidas en las mujeres con TSH =2,5 uUI/mL (p=0,03, p=0,01, p=0,008 y p=0,02, respectivamente). La presencia de placas en la carótida izquierda y en al menos una de las carótidas fue significativamente más frecuente entre aquellas con TSH =2,5 uUI/mL. Conclusiones: Los niveles de TSH =2,5 uUI/mL en mujeres de edad media predispondrían a la alteración del metabolismo de la glucosa y a aterosclerosis subclínica.


Subclinical hypothyroidism may contribute to development of cardiovascular disease and has been shown that TSH >2.5uUI/mL is associated with endothelial dysfunction. Objectives: To determine carotid intima media thickness (CIMT), presence of carotid artery plaque and metabolic profile in asymptomatic middle-aged women with TSH =2.5 uUI/mL, and compare them with those with levels <2.5 uUI/mL. Design: Analytical and cross-sectional study. Setting: Dos de Mayo Hospital and Institute of Clinical Research, Universidad Nacional Mayor de San Marcos, Lima, Peru. Participants: Women with no history of thyroid disease, cardiovascular disease or diabetes. Interventions: Total cholesterol, HDL cholesterol, triglycerides, oral glucose tolerance test (OGTT), fasting insulin (F-Ins), body mass index (BMI), waist circumference and blood pressure were assessed in 60 women with no history of thyroid disease, cardiovascular disease or diabetes with mean age 53,8 ± 5,8 years. HOMA-IR index and LDL cholesterol were also determined. CIMT was measured by Doppler ultrasound. Main outcome measures: TSH = 2.5 uUI/mL and metabolic profile, and association with CIMT. Results: From 60 women 38.3 per cent had TSH =2.5 and 61.7 per cent TSH <2.5 uUI/mL; 56 per cent of women with TSH =2.5 and 65 per cent with TSH <2.5 uUI/mL were hypertensive with no statistical difference. Lipid profile, fasting glucose, F-ins and index HOMA-IR were similar in both groups. Higher levels of BMI, glucose at 120 minutes, left CIMT and maximal CIMT were observed in women with TSH =2.5 uUI/mL (p=0.03, p=0.01, p=0.008 and p=0.02 respectively). The presence of plaques in the left carotid artery and in at least one of the carotid arteries was significantly more frequent in women with TSH =2.5 uUI/mL. Conclusions: TSH levels =2.5 uUI/mL in middle-aged women predispose to altered glucose metabolism and subclinical atherosclerosis.

Tagosodes orizicolus is one of the main plagues of rice in tropical America causing two types of damages, the direct one, feeding and oviposition effect, and an indirect one, by the transmission of the “Rice hoja blanca virus”. During 2006-2007 we carried out research under greenhouse conditions at Fundación Danac, Venezuela, in order to determine the mechanisms of antixenosis, antibiosis and tolerance to T. orizicolus, which could be acting in commercial varieties and advanced lines of the rice genetic breeding programs of INIA and Fundación Danac. The method of free feeding was used for the antixenosis evaluation, whereas the method of forced feeding was used for antibiosis evaluation (effect on survival and oviposition). Additionally, we used the indirect method based on biomass depression to estimate the tolerance. Some of the evaluated traits included: grade of damage, number of insects settling on rice plants, percentage of sogata mortality at the mature state, number of eggs in the leaf midrib and an index of tolerance. The results showed that rice genotypes possess different combinations of resistance mechanisms, as well as different grades of reactions. The susceptible control ‘Bluebonnet 50’ was consistently susceptible across experiments and the resistant control ‘Makalioka’ had high antixenosis and high antibiosis based on survival and oviposition. The rest of the genotypes presented lower or higher degrees of antixenosis and antibiosis for survival and viposition. The genotype ‘FD0241-M-17-6-1-1-1-1’ was identified with possible tolerance to the direct damage of sogata.


Tagosodes orizicolus es una de las principales plagas del cultivo del arroz en América tropical causando dos tipos de daños a la planta, el directo, por efecto de la alimentación y oviposición; el indirecto, por la transmisión del virus de la hoja blanca del arroz. Durante el período 2006-2007 se llevó a cabo una investigación bajo condiciones de umbráculo en la Fundación Danac, Venezuela, con el objeto de determinar los mecanismos de resistencia de antixenosis, antibiosis y tolerancia a T. orizicolus que pudieran estar actuando en líneas avanzadas y cultivares del programa de mejoramiento enético de arroz de las instituciones INIA y Fundación Danac. Para la evaluación de antixenosis se utilizó el método de libre elección, para antibiosis (efecto sobre sobrevivencia y oviposición de los insectos) el de alimentación forzada, adicionalmente se usó un método indirecto para estimar la tolerancia. Dentro de los principales parámetros determinados se encontraban: grado de daño, número de insectos posados sobre plantas de arroz, porcentaje de mortalidad de sogata en estado adulto, número de huevos y se calculó un índice de tolerancia con base en la biomasa. Los resultados mostraron que los genotipos de arroz evaluados poseen diferentes combinaciones en cuanto a los mecanismos de resistencia, así como el grado de su efecto. El testigo ‘Bluebonnet 50’ se mantuvo consistentemente susceptible en las evaluaciones y el testigo resistente ‘Makalioka’ fue el único genotipo que presentó alta antixenosis y alta antibiosis para sobrevivencia y oviposición. El resto de los genotipos presentaron un menor o mayor grado de resistencia de tipo antixenosis y antibiosis para sobrevivencia y oviposición. Se identificó el genotipo ‘FD0241-M-17-6-1-1-1-1’ con posible tolerancia al daño directo de sogata.

Se calcula que la deleción distal del brazo corto del cromosoma1 ocurre en 1 de cada 5000 recién nacidos vivos y condiciona, aproximadamente, el 1,2 por ciento de los casos de retraso mental sin causa aparente. Esta alteración habitualmente no se puede detectar en el cariotipo convencional, por lo que se requieren técnicas de citogenética molecular para evidenciarla. Además de las manifestaciones neurológicas puede ocasionar malformaciones en órganos internos, como cardiopatía congénita, y un fenotipo facial característico. En este informe se describen los hallazgos clínicos y citogenéticos de los primeros dos casos diagnosticados en México, confirmados mediante la técnica de hibridación in situ con fluorescencia, que se comparan con los descritos en la bibliografía. Además, se discute el probable subdiagnóstico de esta entidad, la importancia de su difusión y los datos clínicos útiles para la sospecha diagnóstica.


It is calculated that distal deletion of the short arm of chromosome 1 occurs in one out of every 5000 live births and causes approximately 1.2% of cases of mental retardation of unknown origin. This alteration usually cannot be detected in the standard karyotype, requiring molecular cytogenetic techniques for the diagnosis. In addition to the neurological manifestations, itmay cause internal organs malformations, such as congenitalheart disease, and a characteristic facial phenotype. This report describes the clinical and cytogenetic findings from the first two cases diagnosed in Mexico, confirmed by fluorescence in situ hybridization test, and compares them to those described in the literature. The probable subdiagnosis of this entity, theimportance of improves its recognition and the useful data for the clinical suspicion are also discussed.

Nectars are rich in primary metabolites and attract mutualistic animals, which serve as pollinators or as an indirect defense against herbivores. Their chemical composition makes nectars prone to microbial infestation. As protective strategy, floral nectar of ornamental tobacco (Nicotiana langsdorffii x Nicotiana sanderae) contains "nectarins," proteins producing reactive oxygen species such as hydrogen peroxide. By contrast, pathogenesis-related (PR) proteins were detected in Acacia extrafloral nectar (EFN), which is secreted in the context of defensive ant-plant mutualisms. We investigated whether these PR proteins protect EFN from phytopathogens. Five sympatric species (Acacia cornigera, A. hindsii, A. collinsii, A. farnesiana, and Prosopis juliflora) were compared that differ in their ant-plant mutualism. EFN of myrmecophytes, which are obligate ant-plants that secrete EFN constitutively to nourish specialized ant inhabitants, significantly inhibited the growth of four out of six tested phytopathogenic microorganisms. By contrast, EFN of nonmyrmecophytes, which is secreted only transiently in response to herbivory, did not exhibit a detectable inhibitory activity. Combining two-dimensional sodium dodecyl sulfate-polyacrylamide gel electrophoresis with nanoflow liquid chromatography-tandem mass spectrometry analysis confirmed that PR proteins represented over 90% of all proteins in myrmecophyte EFN. The inhibition of microbial growth was exerted by the protein fraction, but not the small metabolites of this EFN, and disappeared when nectar was heated. In-gel assays demonstrated the activity of acidic and basic chitinases in all EFNs, whereas glucanases were detected only in EFN of myrmecophytes. Our results demonstrate that PR proteins causally underlie the protection of Acacia EFN from microorganisms and that acidic and basic glucanases likely represent the most important prerequisite in this defensive function.

The molecular structure and conformational properties of N-pentafluorosulfur(sulfuroxide difluoride imide), SF5N=S(O)F2, have been studied by vibrational spectroscopy (IR (gas) and Raman (liquid)), by gas electron diffraction (GED), and by quantum chemical calculations (MP2 and B3LYP with (6-31G(d) and 6-311+G(2df) basis sets). According to GED, the prevailing conformer possesses a syn structure (N-SF5 bond synperiplanar with respect to the bisector of the SF2 group). Splitting of the symmetric N=S=O stretching vibration in gas and liquid spectra demonstrates the presence of a second conformer (11(5)%) with anticlinal orientation of the N-SF5 bond according to quantum chemical calculations. The geometric structure, conformational properties, and vibrational frequencies are well reproduced by quantum chemical calculations.

Neither the purchase nor the distribution of pharmaceuticals in hospitals and community pharmacies in Mexico is under the care of pharmacists. Some are under control of physicians. This report presents the results of the implementation of somef pharmaceutical services for the Jalisco Pain Relief, and Palliative Care Institute (Palia Institute), under the direction of the Secretary of Health, Government of Jalisco. The services implemented were drug distribution system, Drug Information Service, Pharmacovigilance Program, and home pharmacotherapy follow-up pilot program for patients with advanced illness, with the ultimate using the appropriate medication. The drug distribution system included dispensing of opioid pain medications, antidepressants, anticonvulsants, NSAIDs, anxiolytic drugs, steroid drugs, laxatives, and anti-emetics. The frequently used drugs were morphine sulfate (62%), amitriptyline (6.4%), and dextropropoxyphene (5.8%). The Drug Information Service answered 114 consultations, mainly asked by a physician (71%) concerned with adverse drug reactions and contraindications (21%). The pharmacovigilance program identified 146 suspected adverse drug reactions and classified them reasonably as possible (27%), probable (69%), and certain (4%). These were attributed mainly to pregabalin and tramadol. The home pharmacotherapy follow-up pilot program cared patients with different cancer diagnoses and drug-related problems (DRP), which were identified and classified (according to second Granada Consensus) for pharmaceutical intervention as DRP 1 (5%), DRP 2 (10%), DRP 3 (14%), DRP 4 (19%), DRP 5 (24%), or DRP 6 (28%). This report provides information concerning the accurate use of medication and, above all, an opportunity for Mexican pharmacists to become an part of health teams seeking to resolve drug-related problems.

A novel series of 4-phenyl-4-[1H-imidazol-2-yl]-piperidine derivatives has been prepared and their synthesis described herein. In vitro affinities for delta-, mu-, and kappa-opioid receptors, as well as the functional activity in the [(35)S]GTPgammaS assay are reported. The most potent and selective delta-opioid agonist 18a exhibited a K(i) of 18 nM, and was >258-fold and 28-fold selective over mu- and kappa-receptors, respectively; the compound is a full agonist with an EC(50) value of 14 nM.

The molecular structure of N-methyl-S,S-bis(trifluoromethyl)sulfimide, CH3N=S(CF3)2, was determined by gas electron diffraction and quantum chemical calculations [B3LYP and MP2 with 6-31+G(2df,p) basis sets]. Furthermore, vibrational spectra, IR (gas) and Raman (liquid), were recorded. These spectra were assigned by comparison with analogous molecules and with calculated frequencies and intensities (HF, B3LYP, and MP2 with 6-311G basis sets). All experimental data and computational methods result in a single conformer with syn orientation of the CH3 group relative to the bisector of the two CF3 groups. The molecule possesses C1 symmetry, slightly distorted from CS symmetry. The N=S bond length in this compound [1.522(10) A] is longer than that in imidosulfur difluorides RN=SF2 [1.476(4) A - 1.487(5) A].

Variations of Na(+)/K(+)-ATPase activity and fatty-acid composition in the gills of the sturgeon Acipenser naccarii subjected to progressive acclimation to full seawater (35 ppt) were determined in relation to the hypo-osmoregulatory capacity of this species in the hyperosmotic medium. Blood samples were taken and gills arches were removed at intermediate salinity levels between 0 and 35 ppt and after 20 days at constant salinity (35 ppt). Plasma osmolality and Na(+)/K(+)-ATPase activity increased significantly with growing environmental salinity. Total saturated fatty acids (SFAs) decreased, while total polyunsaturated fatty acids (PUFAs) increased significantly with increasing salinity due mainly to changes in n-3 PUFAs (20:5n-3 and 22:6n-3). The n-3/n-6 ratio increased significantly during the acclimation process. The results show a direct relationship between salinity, increased gill Na(+)/K(+)-ATPase activity and ultrastructural changes of the gill chloride cells. Changes in the fatty-acid composition in gills of A. naccarii during progressive acclimation to full seawater suggest that variations of gill fatty acids may also have a role in osmoregulatory mechanisms.

RESUMO

In our previous paper we have described the synthesis of a series of 3-piperazinylmethyl-3a,4-dihydro-3H-[1]benzopyrano[4,3-c]isoxazoles, as novel dual 5-HT reuptake inhibitors and alpha2-adrenoceptor antagonists. That investigation led to the identification of the cinnamyl fragment as the most suitable moiety for combined activity. This paper outlines a further optimisation programme, focused on the exploration of the aromatic ring present on the cinnamyl moiety of compounds 1, 2 and 3.
