OBJECTIVE: To analyze multiple-causal models, including socioeconomic, obesity, sugar consumption, alcohol smoking, caries, and periodontitis variables in pregnant women with early sugar exposure, obesity, and the Chronic Oral Disease Burden in their offspring around the first 1000 days of life. METHODS: The BRISA cohort study, Brazil, had two assessments: at the 22nd-25th gestational weeks and during the child's second year (n = 1141). We proposed a theoretical model exploring the association between socioeconomic and pregnancy factors (age, smoking, alcohol, sugars, obesity, periodontitis, and caries) and child's variables (sugars and overweight) with the outcome, Chronic Oral Disease Burden (latent variable deduced from visible plaque, gingivitis, and tooth decay), using structural equation modeling. RESULTS: Caries and periodontitis were correlated in pregnant women. Addictive behaviors in the gestational period were correlated. Obesity (Standardized coefficient - SC = 0.081; p = 0.047) and added sugar consumption (SC = 0.142; p = 0.041) were observed intergenerationally in the pregnant woman-child dyads. Sugar consumption by the children (SC = 0.210; p = 0.041) increased the Chronic Oral Disease Burden. CONCLUSIONS: Poor caries and periodontal indicators were correlated in pregnant women and their offspring. Obesity and sugar consumption act intergenerationally. Oral health in early life may change life trajectory since the worst oral conditions predict main NCDs.

OBJECTIVE: To analyze prenatal and perinatal stressors associated with molar incisor hypomineralization (MIH) in adolescents. METHODS: Prospective cohort study collected prenatal (socioeconomic status, maternal age, number of prenatal visits, smoking, obesity during pregnancy, abortion history, gestational hypertension) and perinatal stressors (type of delivery, gestational age, birth weight, intensive care unit-ICU at birth). The outcome was MIH at 18-19 years follow-up (n = 590). MIH was defined according to the Ghanim criteria - Model I. We performed a sensitivity analysis, including opacities demarcated in index tooth, incisive or molars, Model II. Through structural equation modeling, we analyzed direct and mediating pathways between multiple stressors with outcomes. RESULTS: MIH was observed in 15.25% (n = 90), and opacities demarcated in any index tooth were observed in 22.8% of adolescents (n = 135). In Model I, no stressor explained MIH significantly, although we watched high standardized coefficients (SC) for low birth weight (SC = 0.223, p = 0.147), lower gestational age (SC = 0.351; p = 0.254), and ICU admission (SC = 0.447, p = 0.254). In Model II, advanced maternal age (SC = 0.148; p < 0.05) and not undergoing prenatal care (SC = 0.384, p < 0.03) explained opacities demarcated in incisors or molars. CONCLUSION: Advanced maternal age and not undergoing prenatal care were associated with MIH lesion-like in incisors or molars.

BACKGROUND: Mental disorders represent a major public health challenge worldwide, affecting 80% of people living in low- and middle-income countries. Depression, a mental disorder, is a chronic disease of long duration that causes changes in the brain, resulting from a combination of genetic, physiologic, environmental, and behavioral factors. The aim of this study was to investigate possible factors associated with depression in Brazilian adults. METHODS: A population-based, cross-sectional study was carried out using the public domain database of the 2019 National Health Survey, conducted in Brazil. Depression was considered the dependent variable, and through hierarchical analysis, predictor variables were investigated such as, at the distal level-socioeconomic variables, at the intermediate level-variables related to lifestyle behavior, health condition, and history, and at the proximal level-demographic variables. Logistic regression analysis was used to obtain the adjusted Odds Ratio and the respective 95% confidence interval to identify possible factors associated with depression. RESULTS: The study included 88,531 participant records with 10.27% diagnosed with depression. The adjusted association measurements, after selecting the independent variables in the hierarchical analysis, showed the following factors associated with depression with differing magnitudes: age, brown and white race/skin color, female sex, poor, very poor, or regular self-reported health condition, diagnosis of cardiovascular disease, work-related musculoskeletal disorder, history of smoking habit, and macroeconomic region. CONCLUSIONS: An effective strategy for preventing and managing depression in Brazilian adults must include the control of health status and lifestyle behavior factors, with actions and programs to reduce people's exposure to these factors, understanding that socioeconomic-demographic differences of each population can potentially reduce the disease burden.

(1) Background: To investigate the grouping of obesity and insulin resistance with caries and periodontitis from a syndemic perspective through pathways of socioeconomic inequalities, smoking, alcohol, and high sugar consumption in adolescence. (2) Methods: The population-based RPS Cohort study, São Luís, Brazil, in ages 18-19 years (n = 2515) was used. The outcomes were the grouping of pbesity and Insulin Resistance Phenotype (latent variable formed by Triglycerides/HDL ratio, TyG index, and VLDL) and the Chronic Oral Disease Burden (latent variable comprising caries, bleeding on probing, probing depth ≥ 4 mm, clinical attachment level ≥ 3 mm, and visible plaque index ≥ 15%). Socioeconomic Inequalities influencing the Behavioral Risk Factors (latent variable formed by added sugar, smoking, and alcohol) were analyzed using structural equation modeling. (3) Results: Socioeconomic Inequalities were associated with the Chronic Oral Disease Burden [Standardized Coefficient (SC) = 0.222, p < 0.001]. Behavioral Risk Factors were associated with increased Chronic Oral Disease Burden (SC = 0.103; p = 0.013). Obesity was associated with the Insulin Resistance Phenotype (SC = 0.072; p < 0.001) and the Chronic Oral Disease Burden (SC = 0.066; p = 0.005). The Insulin Resistance Phenotype and the Chronic Oral Disease Burden were associated (SC = 0.053; p = 0.032). (4) Conclusion: The grouping of obesity and early events of diabetes with caries and periodontitis call for a syndemic approach in adolescence.

Resumo O objetivo foi analisar os fatores associados ao uso dos serviços odontológicos no Brasil. A estratégia de revisão incluiu as bases PubMed, SciELO, LILACS, BBO, EMBASE, Scopus, WOS e Google Scholar, além de repositórios e bancos de dissertações e teses. Os estudos foram selecionados ​​usando a estratégia PEO (população/exposição/desfechos). O desfecho deste estudo foi o uso dos serviços odontológicos no último ano pela população brasileira, tratado como variáveis dicotômicas para as análises: ≤ 1 ano e > 1 ano. Foi realizada uma metanálise de efeito randômico de Mantel-Haenszel, estimando-se razões de prevalência (RP) e intervalos de confiança de 95% (IC95%). Foram selecionados 94 estudos. A maioria (98%) tinha delineamento transversal, sendo 63% oriundos de dados primários. Para a metanálise, 25 estudos foram incluídos. O uso dos serviços odontológicos no último ano esteve associado com maior escolaridade (≥ 8 anos de estudo) (RP = 0,49, (IC95%: 0,39-0,60)); maior renda familiar (≥ 2 salários-mínimos) (RP = 0,79, (IC95%: 0,74-0,84)); e residir na zona urbana (RP = 0,79, (IC95%: 0,64-0,97)). A oferta de serviços odontológicos no Sistema Único de Saúde precisa ser ampliada entre pessoas com menor renda, menor escolaridade e moradores da zona rural.


Abstract The scope of this study was to analyze the factors associated with the use of dental services in Brazil. The review strategy included PubMed, SciELO, LILACS, BBO, EMBASE, Scopus, WOS and Google Scholar databases, in addition to repositories and databases of dissertations and theses. Studies were selected using the PEO (Population/Exposure/Outcomes) strategy. The outcome of this study was the use of dental services in the last year by the Brazilian population, treated as dichotomous variables for the following analyses: ≤ 1 year and > 1 year. Mantel-Haenszel random effect meta-analysis was performed, estimating Prevalence Ratios (PR) and 95% confidence intervals (95%CI). A total of 94 studies were selected. The majority (98%) had a cross-sectional design, with 63% derived from primary data. For the meta-analysis, 25 studies were included. The use of oral health services in the last year was associated with higher education (≥ 8 years of schooling) (PR = 0.49, (95%CI: 0.39-0.60)); higher family income (≥ 2 minimum wages) (RP = 0.79, (95%CI: 0.74-0.84)); and living in urban areas (RP = 0.79, (95%CI: 0.64-0.97)). The availability of dental services in the Unified Health System needs to be made readily accessible to people with lower income, less education and those living in rural areas.

AIM: To investigate pathways between unhealthy and healthy dietary patterns and periodontitis in adolescents (18-19 years of age). MATERIALS AND METHODS: This population-based study (n = 2515) modelled direct and mediated pathways (via biofilm and obesity) from patterns of healthy diet (fruits, fibre, vegetables, and dairy) and unhealthy diet (sugars, snacks, and salty/fast foods) with initial periodontitis (bleeding on probing [BoP], probing depth [PD] ≥ 4 mm, clinical attachment loss [CAL] ≥ 4 mm), moderate periodontitis (BoP, PD ≥ 5 mm, and CAL ≥ 5 mm), and European Federation of Periodontology and the American Academy of Periodontology (EFP-AAP) periodontitis definitions, adjusting for sex, socio-economic status, smoking, and alcohol, through structural equation modelling (α = 5%). RESULTS: Higher values of healthy diet were associated with lower values of initial periodontitis (standardized coefficient [SC] = -0.160; p < .001), moderate periodontitis (SC = -0.202; p < .001), and EFP-AAP periodontitis (p < .05). A higher value of unhealthy diet was associated with higher values of initial periodontitis (SC = 0.134; p = .005) and moderate periodontitis (SC = 0.180; p < .001). Biofilm mediated the association between higher values of unhealthy diet and all periodontal outcomes (p < .05). CONCLUSIONS: Our findings suggest that both healthy and unhealthy dietary patterns may contribute to reduced or increased extent and severity of periodontitis by local and systemic mechanisms, preceding the effect of other established causes such as smoking and obesity, in younger population.

RESUMO Objetivo. Identificar a associação da estrutura das unidades básicas de saúde (UBS) e do processo de trabalho das equipes de atenção primária com determinantes sociais e taxas de internações por condições sensíveis à atenção primária (ICSAP) em menores de 5 anos no Brasil. Métodos. Estudo longitudinal ecológico, com dados secundários do Sistema de Informações Hospitalares e dos três ciclos do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) (2012, 2014, 2017/2018). Foram analisadas 42 916 UBS. Foi realizada modelagem multinível com angulação fixa e intercepto aleatório. No primeiro nível, foram analisados o desfecho (taxas de ICSAP) e as variáveis explicativas (indicadores estruturais e processuais), agregados por UBS. No segundo nível, foram incluídos determinantes sociais municipais (representados por critério de estratificação que combina o porte do município com indicadores que influenciam a gestão de saúde). Utilizou-se o teste t com correção de Bonferroni para comparar as médias dos indicadores entre as regiões, e a regressão linear multinível para estimar os coeficientes de correlação. Resultados. A taxa de ICSAP em menores de 5 anos foi de 62,78/100 mil habitantes por área estimada de cobertura de UBS. Apresentaram associação direta com o desfecho: participação em um ou mais ciclos do PMAQ-AB; planejamento da equipe; horário especial; dependências para atenção infantil na unidade; e disponibilidade de vacinas. A variável equipamentos, materiais e insumos e a classificação como município de porte pequeno ou médio associaram-se inversamente às ICSAP. Conclusões. As ICSAP em menores de 5 anos podem ser potencialmente reduzidas pela melhoria dos indicadores estruturais e processuais das UBS e dos determinantes sociais municipais.


ABSTRACT Objective. To investigate whether structural aspects of primary care units (PCUs) and the work processes of primary care teams are associated with the rate of hospitalizations for primary care-sensitive conditions (HPCSC) in children younger than 5 years of age in Brazil. Method. For this longitudinal ecological study, secondary data were obtained from the Brazilian Hospital Information System and from three cycles of the National Program for Access and Quality Improvement in Primary Care (PMAQ-AB) (2012, 2014, 2017/2018). The analysis included 42 916 PCUs. A multilevel random intercept model with fixed slope was used. In the first level, the outcome (HPCSC rates) and explanatory variables (structure and process indicators) aggregated by PCU were analyzed. Social determinants (represented by a stratification criterion combining municipality population and health care management indicators) were entered in the second level. The t test with Bonferroni correction was used to compare indicator means between regions, and multilevel linear regression was used to estimate the correlation coefficients. Results. The HPCSC rate in children younger than 5 years was 62.78/100 thousand population per estimated PCU coverage area. A direct association with the outcome was observed for: participation in one or more PMAQ-AB cycles; team planning; special hours; dedicated pediatric care area; and availability of vaccines. Equipment, materials, supplies, and being a small or medium-size municipality were inversely associated with HPCSC. Conclusions. HPCSC rates in children below 5 years of age may potentially be reduced through improvements in PCU structure and process indicators and in municipal social determinants.

Determinar la asociación de la estructura de las unidades básicas de salud (UBS) y del proceso de trabajo de los equipos de atención primaria con los determinantes sociales y las tasas de hospitalización por afecciones que podrían tratarse en la atención primaria en menores de 5 años en Brasil. Métodos. Estudio longitudinal ecológico, con datos secundarios del Sistema de Informaciones Hospitalarias y los tres ciclos del Programa Nacional de Mejora del Acceso y de la Calidad de la Atención Básica (PMAQ-AB) (2012, 2014, 2017-2018). Se analizaron 42.916 UBS. Se aplicó un modelo multinivel con pendiente fija e intercepción aleatoria. En el primer nivel, se analizaron el desenlace (tasas de hospitalización por afecciones que podrían tratarse en los servicios de atención primaria) y las variables explicativas (indicadores estructurales y procedimentales) consolidados por UBS. En el segundo nivel, se incluyeron determinantes sociales municipales (representados por un criterio de estratificación que combina el tamaño del municipio con indicadores que influyen en la gestión de salud). Se utilizó la prueba de la t con la corrección de Bonferroni para comparar las medias de los indicadores entre las regiones y la regresión lineal multinivel para estimar los coeficientes de correlación. Resultados. La tasa de hospitalización por afecciones que podrían tratarse en los servicios de atención primaria en menores de 5 años fue de 62,78/100 mil habitantes por área estimada de cobertura de UBS. Los siguientes factores presentaron una asociación directa con el desenlace: participación en uno o más ciclos del PMAQ-AB; planificación del equipo; horario especial; dependencias de atención infantil en la unidad; y disponibilidad de vacunas. La variable relativa a equipos, materiales e insumos y la clasificación como municipio pequeño o mediano se asociaron inversamente con las hospitalizaciones por afecciones que podrían tratarse en los servicios de atención primaria. Conclusiones. Las hospitalizaciones de menores de 5 años por afecciones que podrían tratarse en los servicios de atención primaria pueden reducirse mejorando los indicadores estructurales y procedimentales de las UBS y los determinantes sociales municipales.

Abstract This article aims to assess whether alterations of oral functions (AOF) are associated with malocclusion (MO)'s type and severity. Cross-sectional study of a representative sample of 332 adolescents aged 12 years in São Luís-MA, Northeastern Brazil. MO criteria included Angle's classification, Dental Aesthetic Index, and other morphological problems. The AOF were evaluated by breathing, phonation, chewing, and swallowing. Odds ratios (OR) and 95% confidence intervals (95%CI) were calculated using logistic and multinomial regression analyses (α=5%). Mouth breathing was associated with defined (OR=3.84; 95%CI=1.45-10.12), disabling (OR=4.34; 95%CI=1.99-9.49), and class III (OR=4.15; 95%CI=1.19-14.54) MO. Phonation problems were associated with defined (OR=2.01; 95%CI=1.02-4.39), disabling (OR=3.04; 95%CI=1.55-5.96), and Class II (OR=2.02; 95%CI=1.28-3.18) MO. Chewing disorders were associated with posterior crossbite (PCB) (OR=2.32; 95%CI=1.12-4.82). Swallowing disorders were associated with Class III MO (OR=5.66; 95%CI=1.35-23.71), PCB (OR=6.13; 95%CI=2.76-13.62), and posterior open bite (OR=4.53; 95%CI=1.72-8.92). Breathing and phonation alterations are associated with MO in anterior arch segments, while chewing and swallowing disorders, in the posterior segments.


Resumo O objetivo deste artigo é avaliar se alterações das funções orais (AFO) estão associadas ao tipo e gravidade da maloclusão (MO). Estudo transversal com amostra representativa de 332 adolescentes de 12 anos em São Luís-MA, Nordeste do Brasil. Critérios de MO incluíram Classificação de Angle, Índice de Estética Dental e outros problemas morfológicos. As AFO foram avaliadas por respiração, fonação, mastigação e deglutição. Odds ratios (OR) e intervalos de confiança de 95% (IC95%) foram calculados em análises de regressão logística e multinomial (α=5%). Respiração oral foi associada com MO definida (OR=3,84; IC95%=1,45-10,12), incapacitante (OR=4,34; IC95%=1,99-9,49) e classe III (OR=4,15; IC95%=1,19-14,54). Problemas de fonação foram associados às MO definidas (OR=2,01; IC95%=1,02-4,39), incapacitantes (OR=3,04; IC95%=1,55-5,96) e Classe II (OR=2,02; IC95%=1,28-3,18). Alterações na mastigação foram associadas à mordida cruzada posterior (MCP) (OR=2,32; IC95%=1,12-4,82). Deglutição atípica foi associada à MO Classe III (OR=5,66; IC95%=1,35-23,71), MCP (OR=6,13; IC95%=2,76-13,62) e mordida aberta posterior (OR=4,53; IC95%=1,72-8,92). Alterações de respiração e fonação estão associadas às MO nos segmentos anteriores do arco, enquanto as de mastigação e deglutição, nos segmentos posteriores.

AIM: To evaluate the association between low bone mineral density (BMD) and severe periodontitis at the end of the second decade of life. MATERIALS AND METHODS: This population-based study analysed 2032 youngers (18-19 years old) of the RPS cohort. BMD of lumbar spine (BMD-LS) and of the whole body (BMD-WB) were assessed by dual x-ray emission densitometry. Low BMD-LS (Z-score ≤ -2) and low BMD-WB (Z-score ≤ -1.5) were correlated with severe periodontitis. The extent of periodontal disease was also evaluated as the following outcomes: proportions of teeth affected by clinical attachment loss ≥5 mm and probing depth ≥5 mm. Multivariate models by sex, education, family income, risk of alcohol dependence, smoking, plaque, bleeding index, and body mass index were estimated through logistic regression (binary outcomes) and Poisson regression (continuous outcomes). RESULTS: The prevalence of severe periodontitis was 10.97%. Low BMD-LS (odds ratio [OR] = 2.08, confidence interval [CI] = 1.12-3.85, p = .01) and low BMD-WB (OR = 1.34, CI = 1.001-1.81, p = .04) were associated with severe periodontitis in the final multivariate models. Low BMD-LS and BMD-WB were also associated with a greater extent of periodontitis (p < .05). CONCLUSIONS: Low BMD was found to be associated with the severity and extent of periodontitis in adolescents. Adolescents at peak bone mass age presenting low BMD are more likely to be affected by severe periodontitis.

Resumo O objetivo foi analisar a associação de fatores socioeconômicos com a prevalência de cárie dentária em adolescentes de São Luís, Maranhão, para responder se as iniquidades sociais persistem na distribuição desta doença. Este é um estudo transversal aninhado a uma coorte prospectiva. Incluímos 2.413 adolescentes de 18-19 anos, avaliados em 2016 (2º seguimento). O desfecho foi a ocorrência de dentes com cárie dentária não tratada (sim ou não), avaliada pelo índice CPO-D. Características socioeconômicas e demográficas foram as variáveis independentes. Foram realizadas análises estatísticas descritivas e de regressão de Poisson, calculando-se razões de prevalência (RPs) brutas e ajustadas (alpha=5%). Pertencer às classes econômicas C (RP=1,23; IC95%:1,11-1,37) ou D-E (RP=1,48; IC95%: 1,32-1,65), estar casado/morar com companheiro (RP=1,22; IC95%:1,07-1,39), ter pais separados (RP=1,11; IC95%1,03-1,19) e maior número de pessoas na residência (RP=1,05; IC95%:1,03-1,07) foram associadas a maior prevalência de cárie dentária. Apesar da implementação da Política Nacional de Saúde Bucal, as iniquidades sociais em saúde bucal de adolescentes persistem. É fundamental que o modelo de atenção à saúde vigente busque a reorientação das estratégias de educação em saúde, direcionando-as a populações vulneráveis.


Abstract This study aimed to analyze the association of socioeconomic factors with the prevalence of dental caries in adolescents from São Luís, Maranhão, Brazil, to answer whether social inequalities persist in distributing this disease. This is a cross-sectional study nested in a prospective cohort. We included 2,413 adolescents aged 18-19 years evaluated in the 2016 second follow-up. The outcome was teeth with untreated dental caries (yes or no) assessed by the DMFT index. Socioeconomic and demographic characteristics were the independent variables. Descriptive statistical and Poisson regression analyses were performed, calculating crude and adjusted prevalence ratios (PRs) (alpha=5%). Belonging to economic classes C (PR=1.23; 95% CI: 1.11-1.37) or D-E (PR=1.48; 95% CI: 1.32-1.65), being married/living with a partner (PR=1.22; 95% CI: 1.07-1.39), having separated parents (PR=1.11; 95% CI 1.03-1.19) and a greater number of people in the household (PR=1.05; 95% CI: 1.03-1.07) were associated with a higher prevalence of dental caries. Social inequalities in adolescent oral health persist despite the implementation of the National Oral Health Policy. The current health care model should seek to reorient health education strategies, targeting them at vulnerable populations.

Objetivo: Analisar a estrutura das unidades básicas de saúde (UBS) e o processo de trabalho das equipes de atenção básica em saúde no cuidado à criança no Brasil. Métodos: Estudo ecológico com dados dos três ciclos do Programa de Melhoria do Acesso e Qualidade da Atenção Básica 2012-2018, por Unidade da Federação e suas macrorregiões. Foram analisados sete indicadores estruturais e 13 processuais. Utilizou-se o teste t de Student para comparar as médias dos indicadores entre as macrorregiões nacionais. Resultados: Participaram dos três ciclos do programa 85.845 equipes, agrupadas em 68.320 UBS. No último ciclo avaliativo (2017/2018), apresentaram maiores médias percentuais de adequação entre os indicadores estruturais: funcionamento da unidade (99%), equipamentos/materiais (82%), disponibilidade de vacinas (74%) e dispensação de medicamentos (70%). População descoberta (68%) e agendamento para especialistas (52%) corresponderam aos menores percentuais de adequação dos indicadores processuais. Conclusão: Os indicadores de processo apresentaram melhores adequações que os indicadores estruturais.


Objetivo: Analizar la estructura de las unidades básicas de salud (UBS) y el proceso de trabajo de los equipos de atención primaria en el cuidado infantil en Brasil. Métodos: Estudio ecológico con datos de los tres ciclos del Programa de Mejora del Acceso y Calidad de la Atención Básica 2012-2018, por unidad federativa y regiones. Se analizaron siete indicadores estructurales y trece procedimentales. La prueba t de Student se utilizó para comparar medias de los indicadores entre los macrorregiones. Resultados: Participaron 85.845 equipos de los tres ciclos del programa, agrupados en 68.320 UBS. En el último ciclo (2017/2018), mostraron mayores porcentajes de adecuación entre los indicadores estructurales: funcionamiento de la unidad (99%), equipos/materiales (82%), vacunas (74%) y dispensación de medicamentos (70%). Población sin cobertura (68%) y programación para especialistas (52%) correspondieron a los porcentajes más bajos de adecuación de los indicadores procedimentales. Conclusión: Los indicadores del proceso mostraron mejores adaptaciones que los estructurales.


Objective: To assess the structure of primary health care centers (PHCCs) and the work process of primary care teams in child care in Brazil. Methods: This was an ecological study with data from the three cycles of the Program for Primary Health Care Access and Quality Improvement 2012-2018, by states and regions. Seven structural and thirteen procedural indicators were analyzed. Student's t-test was used to compare indicator averages between regions. Results: 85,845 teams participated in the three cycles of the program, grouped into 68,320 PHCCs. In the last evaluation cycle (2017-2018), mean percentage adequacy rates were higher among the structure indicators: health center operation (99%), equipment/materials (82%), vaccine availability (74%) and medication dispensing (70%). Population without coverage (68%) and making appointments with specialists (52%) corresponded to the lowest percentages of adequacy of process indicators. Conclusion: Process indicators had higher levels of adequacy than structure indicators.

Resumen Introducción y objetivo: La incorporación de propóleos en los dentífricos tiene como objetivo ayudar de manera más efectiva al control y la prevención de patologías orales a través de la eliminación de los patógenos presentes en la biopelícula. Sin embargo, se sabe poco sobre la eficacia antimicrobiana de diferentes productos en el mercado de microorganismos para estas patologías. El objetivo de este estudio fue investigar la acción antimicrobiana de tres dentífricos que contienen propóleos sobre los patógenos orales. Material y métodos: Se utilizó el método de difusión en agar para analizar tres dentífricos basados en propóleos, que incluyen: Noplak Max®, Protta® y Forever Bright®. Se utilizó un dentífrico sin propóleos (Malvatrikids®) como control negativo. Los controles positivos fueron 0,2% de clorhexidina diluida adicionalmente al 30% para igualar la concentración de clorhexidina de uno de los dentífricos evaluados, y el extracto de propóleos (Apis Flora®) al 11%. Para la determinación de la actividad antimicrobiana se utilizaron las cepas de Fusobacterium nucleatum y Aggregatibacter actinomycetemcomitans, Enterococcus faecalis, Streptococcus mutans, Lactobacillus acidophilus y Candida albicans. Resultados: De los dentífricos probados, Protta® y Forever Bright® mostraron acción inhibitoria contra S. mutans, E. faecalis y microorganismos de C. albicans. El dentífrico Nopla k® mostró baja actividad antimicrobiana, limitándose a S. mutans y E. faecalis. Cuando hubo un efecto antimicrobiano, los diámetros de los halos de inhibición del crecimiento variaron de 9mm a 28,83mm. Conclusión: El uso de un dentífrico que contiene propóleos para su uso eventual como complemento terapéutico en odontología está justificado, considerando las actividades farmacológicas.


Abstract Introduction and objective: The incorporation of propolis in dentifrices aims to more effectively assist the control and prevention of oral pathologies through the elimination of pathogens present in the biofilm. However, little is known about the antimicrobial efficacy of different products on the market for microorganisms for these conditions. The aim of this study was to investigate the antimicrobial action of three propolis-containing dentifrices on oral pathogens. Material and methods: The agar diffusion method was used to analyze three propolis-based dentifrices, including: Noplak Max®, Protta® and Forever Bright®. A non-propolis dentifrice (Malvatrikids®) was employed as a negative control. Positive controls were 0.2% chlorhexidine, further diluted 30% to match the chlorhexidine concentration of one of the evaluated dentifrices and 11% propolis extract (Apis Flora®). For the determination of antimicrobial activity the strains of Fusobacterium nucleatum and Aggregatibacter actinomycetemcomitans, Enterococcus faecalis, Streptococcus mutans, Lactobacillus acidophilus and Candida albicans were used. Results: Of the tested dentifrices, Protta® and Forever Bright® showed inhibitory action against S. mutans, E. faecalis and C. albicans microorganisms. Noplak® dentifrice showed low antimicrobial activity, being limited to S. mutans and E. faecalis. When there was an antimicrobial effect, the diameter of the growth inhibition halos ranged from 9mm to 28.83mm. Conclusion: The use of a propolis-containing dentifrice for eventual use as a therapeutic adjunct in dentistry is fully justified, considering the pharmacological activities.


Resumo Introdução e objetivo: A incorporação de própolis em dentifrícios visa ajudar a controlar e prevenir patologias bucais de maneira mais eficaz, através da eliminação de patógenos presentes no biofilme. No entanto, pouco se sabe sobre a eficácia antimicrobiana de diferentes produtos no mercado de microrganismos para essas patologias. O objetivo deste estudo foi investigar a ação antimicrobiana de três dentifrícios contendo própolis em patógenos orais. Material e métodos: O método de difusão em ágar foi utilizado para analisar três dentífricos à base de própolis, incluindo: Noplak Max®, Protta® e Forever Bright®. Um creme dental não própolis (Malvatrikids®) foi usado como controle negativo. Os controles positivos foram 0,2% de clorexidina diluída para 30% para corresponder à concentração de clorexidina de um dos dentifrícios avaliados e 11% de extrato de própolis (Apis Flora®). Para a determinação da atividade antimicrobiana, foram utilizadas as linhagens Fusobacterium nucleatum e Aggregatibacter actinomycetemcomitans, Enterococcus faecalis, Streptococcus mutans, Lactobacillus acidophilus e Candida albicans. Resultados: Dos dentifrícios testados, Protta® e Forever Bright® apresentaram ação inibitória contra S. mutans, E. faecalis e microorganismos de C. albicans. O creme dental Noplak® apresentou baixa atividade antimicrobiana, limitada a S. mutans e E. faecalis. Quando houve efeito antimicrobiano, os diâmetros dos halos de inibição de crescimento variaram de 9 a 28,83 mm. Conclusão: O uso de um dentifrício contendo própolis para eventual uso como complemento terapêutico em odontologia é totalmente justificado, considerando as atividades farmacológicas.

BACKGROUND: Human papillomavirus (HPV) has been associated with certain types of oropharyngeal cancers and yet, the level of knowledge that dental professionals and the lay public have in terms of HPV transmission, oral sexual activities, and oral cancer development needs exploration. The aim of this study was to assess the knowledge held by practicing dental professionals as well as the lay public regarding Human Papillomavirus (HPV) transmission through oral sex and subsequent oropharyngeal cancer development. METHODS: Textual data were collected from a public forum with dental professionals in. Vancouver, who discussed the HPV-oral sex-oral cancer triad, and from survey data gathered from 212 lay public participants (also in Vancouver) who answered a 13-item questionnaire on the perceived risks of oral sex in terms of HPV infection and oropharyngeal cancer development. The data were analyzed statistically by age group, gender, and sexual orientation using descriptive statistics, while an ANOVA test was used to compare variation in the responses to the survey (p-value = 0.05). RESULTS: The forum engaged 46 health care professionals, many of whom were aware of the potential risks for head and neck cancer development due to HPV infection, while also questioning "how to effectively talk about HPV with patients." The survey revealed that 34.5% of the participants believed that oral sex is an activity with no or low risk for the transmission of HPV, while 84% of participants believed the same sexual practices were of low or no-risk for HIV (Human Immunodeficiency Virus) transmission. Most participants (82%) never discussed oral sexual activities with their physicians or dentists/dental hygienists. CONCLUSIONS: The general public remains mostly unaware of the potential links between HPV infection and oropharyngeal cancer. Physicians and dental providers should discuss oral sexual practice with their patients to raise awareness.

Abstract This study aimed to analyze the association of sociodemographic, child health, healthcare service, and access indicators with developmental defects of enamel (DDE) acquired outside the uterus, based on gestational factors. A cohort of births was carried out, and 982 children aged 12 to 30 months were examined. A total of 1,500 women were followed up as of the 5th month of gestation, and the child's gestational age was evaluated at follow-up. The clinical examination was performed as recommended by the World Health Organization, and defects were classified using the modified DDE index. Six models were considered: presence of DDE (Model 1) or opacities (Model 4), number of teeth with DDE (Model 2) or opacities (Model 5), and incidence rate of DDE (Model 3) or opacities (Model 6). Associations were estimated by relative risk (RR) in Poisson regression models. In the adjusted analysis, the mother's lowest education level was associated with the highest occurrence of DDE in Models 1 (RR = 26.43; p = 0.002), 2 (RR = 9.70; p = 0.009), and 3 (RR = 5.63; p = 0.047). Breastfeeding for over 12 months (RR = 0.45; p = 0.030) and recent use of anti-infection drugs (RR = 0.20; p = 0.039) had a protective effect on DDE (Model 1). The factors associated with the highest incidence of opacities were not having health insurance (RR = 2.00; p = 0.043) (Model 5), and belonging to a family of poor social class (RR = 4.67; p = 0.007) (Model 6). Children in a situation of socioeconomic vulnerability have a higher risk of presenting extrauterine DDE. Breastfeeding was a protection factor for DDE development.

Chronic renal patients are more susceptible to hospital complications and infections such as urinary tract infections, peritonitis, surgery infections, and bacteremia, which are often caused by Pseudomonas aeruginosa. A case of a HIV-positive girl with chronic kidney disease and with serious oral lesions due to P. aeruginosa septic shock is presented. The patient showed necrotic lesions in the oral mucosa, pathological tooth mobility, bone loss, and hematogenous osteomyelitis in the maxilla. The patient was submitted to systemic antibiotic therapy based on screening culture and treatment of bone lesions by eliminating the causal agent and restoring health conditions. This case report is extremely important for health professionals, since the oral cavity can be affected by this pathogen or serve as a colonization site.

RESUMO Pacientes renais crônicos são mais suscetíveis a complicações e infecções hospitalares, tais como infecções do trato urinário, peritonite, infecções cirúrgicas e bacteremia, muitas vezes causadas por Pseudomonas aeruginosa. O presente relato apresenta o caso de uma menina HIV-positiva com doença renal crônica e lesões orais graves devido a choque séptico por P. aeruginosa. A paciente apresentava lesões necróticas na mucosa oral, mobilidade dentária patológica, perda óssea e osteomielite hematogênica maxilar. A paciente foi submetida a antibioticoterapia sistêmica com base na cultura de triagem e tratamento das lesões ósseas por meio da eliminação do agente causal e restauração das condições de saúde. Este relato de caso é extremamente importante para os profissionais da saúde, uma vez que a cavidade oral pode ser afetada por este patógeno ou servir como local de colonização.

Resumo O objetivo deste estudo foi investigar a associação entre fatores socioeconômicos, comportamentais, saúde geral, saúde bucal e os desfechos de acúmulo de placa e condição de mucosa bucal em idosos. Este estudo transversal, conduzido entre 2004 e 2005, incluiu 785 idosos de Carlos Barbosa, RS. Questionários foram utilizados para coleta de dados socioeconômicos, comportamentais e estado de saúde. O exame físico das estruturas bucais e o Índice de Mucosa e de Placa de Henriksen (MPS) foram utilizados para categorizar os desfechos. Observou-se maior prevalência de acúmulo de placa moderado/grave entre os idosos mais velhos, do sexo masculino e com menor escolaridade. O uso de prótese dental total superior e renda de até 1 salário mínimo foram fatores de proteção para acúmulo de placa moderado/grave. Apenas as variáveis do primeiro bloco da estrutura conceitual, sexo masculino (OR = 2,13; IC95% = 1,26-3,61) e menor escolaridade (OR = 1,37; IC95% = 1,06-1,78) permaneceram associados como risco à presença de escore inaceitável do MPS. Os achados sugerem que idosos do sexo masculino e com menor escolaridade têm maior chance de saúde bucal precária, conforme o MPS.


Abstract This study aimed to investigate the association between socioeconomic and behavioral factors, general health, oral health, and plaque accumulation and oral mucosa condition outcomes in the elderly. This is a cross-sectional study conducted from 2004 to 2005 with 785 elderly dwellers of the city of Carlos Barbosa (RS), Brasil. We used questionnaires to collect socioeconomic, behavior and health status data. The physical examination of the oral structures comprised the Mucosal-Plaque Index proposed by Henriksen (MPS). A higher prevalence of moderate/severe plaque accumulation was observed in the elderly group, in less educated male elderly. The use of full upper dentures and income under one minimum wage were protective factors for moderate/severe plaque accumulation. Only the variables of the first block of the conceptual structure, male gender (OR = 2.13; 95% CI 1.26-3.61) and lower education (OR = 1.37; 95% CI 1.06-1.78) remained associated with an unacceptable MPS score. The findings suggest that, as per MPS, less educated male elderly are more likely to have poor oral health.

Resumo: O objetivo deste trabalho foi estimar a peregrinação de gestantes no momento do parto e identificar os fatores associados a essa peregrinação em duas cidades brasileiras. Estudo seccional, aninhado à coorte de nascimento BRISA, cuja amostra foi composta por 10.475 gestantes admitidas nas maternidades selecionadas por ocasião do parto em São Luís (Maranhão) e Ribeirão Preto (São Paulo). Entrevistas foram realizadas utilizando-se questionários que continham variáveis sociodemográficas e relacionadas ao parto. Utilizou-se modelagem hierarquizada, e calculou-se o risco relativo utilizando regressão de Poisson. A peregrinação foi mais frequente em São Luís (35,8%) que em Ribeirão Preto (5,8%). Em São Luís, foram fatores associados à maior peregrinação: ser primípara (RR = 1,19; IC95%: 1,08-1,31) e ter escolaridade menor que 12 ou mais anos de estudo. Entretanto, ter 35 anos ou mais (RR = 0,65; IC95%: 0,54-0,84) foi fator associado à menor peregrinação. Em Ribeirão Preto, peregrinaram com maior frequência as gestantes cujos partos foram de alto risco (RR = 2,45; IC95%: 1,81-3,32) e com idade gestacional inferior a 37 semanas (RR = 1,93; IC95%: 1,50-2,50). No entanto, partos com idade gestacional igual ou acima de 42 semanas foi um fator associado à menor peregrinação (RR = 0,57; IC95%: 0,33-0,98). Nas duas cidades, gestantes pobres peregrinaram com maior frequência, e sem garantia de que seriam atendidas, mesmo dentre as que realizaram o pré-natal. O estudo evidenciou ausência da garantia de acesso universal e equânime e reafirmou a desigualdade de acesso à assistência ao parto entre as regiões brasileiras.


Abstract: The objectives of this study were to estimate the involuntary pilgrimage by women in labor in search of childbirth care and to identify factors associated with this endeavor in two Brazilian cities. This was a cross-sectional study nested in the BRISA birth cohort, whose sample consisted of 10,475 women admitted to the selected maternity hospitals for delivery in São Luís (Maranhão State) and Ribeirão Preto (São Paulo State). Interviews were held with questionnaires that contained sociodemographic and obstetric variables. Hierarchical modeling was used, and relative risk was calculated with Poisson regression. Involuntary pilgrimage during labor was more frequent in São Luís (35.8%) than in Ribeirão Preto (5.8%). In São Luís, factors associated with pilgrimage were: first pregnancy (RR = 1.19; 95%CI: 1.08-1.31) and schooling less than 12 complete years. However, age 35 years or older (RR = 0.65; 95%CI: 0.54-0.84) was associated with less pilgrimage. In Ribeirão Preto, such trekking for obstetric care was more frequent in women with high-risk pregnancies (RR = 2.45; 95%CI: 1.81-3.32) and those with gestational age less than 37 weeks (RR = 1.93; 95%CI: 1.50-2.50). Meanwhile, delivery with gestational age equal to or greater than 42 weeks was associated with less pilgrimage (RR = 0.57; 95%CI: 0.33-0.98). In both cities, poor women had to trek more in search of childbirth care and had no guarantee of care, even for those who had received prenatal care. The study revealed the lack of guarantee of universal and equitable access and highlighted the unequal access to childbirth care between Brazil's major geographic regions.


Resumen: El objetivo de este trabajo fue estimar los desplazamientos largos de gestantes en el momento del parto e identificar los factores asociados a estos desplazamientos en dos ciudades brasileñas. Se trata de una investigación seccional, encajada en la cohorte de nacimiento BRISA, cuya muestra estuvo compuesta por 10.475 gestantes, admitidas en las maternidades seleccionadas para dar a luz en São Luís (Maranhão) y Ribeirão Preto (São Paulo). Las entrevistas se realizaron utilizando cuestionarios que contenían variables sociodemográficas y relacionadas con el parto. Se utilizó un modelado jerarquizado, y se calculó el riesgo relativo utilizando la regresión de Poisson. Los desplazamientos fueron más frecuentes en São Luís (35,8%) que en Ribeirão Preto (5,8%). En São Luís, los factores asociados a mayores desplazamientos fueron: ser primípara (RR = 1,19; IC95%: 1,08-1,31) y contar con una escolaridad menor a 12 o más años de estudio. Sin embargo, tener 35 años o más (RR = 0,65; IC95%: 0,54-0,84) fue un factor asociado a menores desplazamientos. En Ribeirão Preto, se desplazaron con mayor frecuencia las gestantes cuyos partos fueron de alto riesgo (RR = 2,45; IC95%: 1,81-3,32) y con una edad gestacional inferior a 37 semanas (RR = 1,93; IC95%: 1,50-2,50). Sin embargo, los partos con una edad gestacional igual o por encima de las 42 semanas fue un factor asociado a un menor desplazamiento (RR = 0,57; IC95%: 0,33-0,98). En las dos ciudades, las gestantes pobres se desplazaron con mayor frecuencia, y sin garantía de que serían atendidas, incluso entre quienes realizaron seguimiento prenatal. El estudio evidenció la inexistencia de garantías de acceso universal y ecuánime a la salud y reafirmó la desigualdad de acceso en la asistencia al parto entre regiones brasileñas.

Introduction Anemia is a very frequent event among pregnant women. There are evidences of differences in the incidence of dental caries between pregnant and non-pregnant women, but the relationship between salivary iron (Fe) and serum markers of anemia and caries development has not been investigated. Objective To evaluate the correlation between salivary (Fe) and serum iron (Fe, ferritin and hemoglobin) parameters in pregnant women with the development of dental caries. Methods A prospective cohort was conducted with 59 women. The outcome of interest was represented by new dental caries lesions during pregnancy, using the Nyvad criteria. Pregnant women were evaluated at three clinical times: up to the 16th week of gestational age (GA) (T1), in the last trimester of pregnancy (T2), and postpartum (T3), at the Mother and Child Unit of University Hospital of the Universidade Federal do Maranhão. A stimulated saliva sample was collected for biochemical analysis of salivary Fe, and a blood sample was collected early in the morning. The correlation between salivary and serum Fe was evaluated through the Pearson correlation test. Analysis of variance (ANOVA) and Kruskal-Wallis were used to compare the means of anemia parameters at different times. The Student's t and Mann-Whitney tests were used to compare the anemia parameters between the groups of pregnant women (with and without new caries lesions). Results Serum Fe concentrations were higher in the first trimester of pregnancy and lower after delivery (p = 0.036). It was also observed that the ferritin concentrations were higher in the first trimester and lower at the end of gestation (p = 0.011). There was no association between the expositions of salivary iron and anemia, and the development of dental caries. There was a positive correlation between serum Fe in T1 and salivary Fe in T2 (p < 0.05). Conclusion The serum markers of anemia were more prevalent in the last trimester of pregnancy.

Resumo Introdução A anemia é um evento muito frequente entre mulhres grávidas. Existem evidências de diferenças na incidência de cárie dentária entre mulheres grávidas e não grávidas,mas a relação entre o íon ferro (Fe) salivar, osmarcadores séricos de anemia e o desenvolvimento de cárie não foi investigada. Objetivo Avaliar a correlação entre os parâmetros salivares (Fe) e séricos (Fe, ferritina e hemoglobina) em gestantes e o desenvolvimento de cárie dentária. Métodos Uma coorte prospectiva foi conduzida com 59 mulheres. O desfecho de interesse foi representado por novas lesões de cárie durante a gravidez, medido pelo critério Nyvad. Mulheres grávidas foram avaliadas em três tempos clínicos: até a 16ª semana de idade gestacional (IG) (T1), no último trimestre de gravidez (T2), e no puerpério (T3), na Unidade Materno-infantil do Hospital Universitário da Universidade Federal do Maranhão. A amostra de saliva estimulada foi coletada para análise bioquímica de Fe salivar, e a amostra de sangue foi coletada no início da manhã. A correlação entre o Fe salivar e o Fe sérico foi avaliada através do teste de correlação de Pearson. Os testes ANOVA e Kruskal-Wallis foram utilizados para comparar parâmetros de anemia em diferentes momentos. Os testes t de Student e Mann-Whitney foram utilizados para comparar os parâmetros da anemia entre os grupos de gestantes (come sem lesões de cárie). Resultados As concentrações séricas de Fe foram maiores no primeiro trimestre de gestação e menores após o parto (p = 0,036). Observou-se também que as concentrações de ferritina forammaiores no primeiro trimestre emenores no final da gestação (p = 0,011). Não houve associação entre as exposições e o desenvolvimento de cárie dentária. Houve correlação positiva entre o Fe sérico em T1 e o Fe salivar em T2 (p < 0,05). Conclusão Os marcadores séricos de anemia foram mais prevalentes no último trimestre de gestação.
