RESUMO

AIM: Epithelial ovarian cancer (EOC) is the most ominous tumor of gynecological cancers due to its poor early detection rate and unfavorable prognosis. To date, there is no reliable screening method for the diagnosis of ovarian cancer at an early stage. MiRNAs are small non-coding RNA molecules, and their main function is to regulate gene expression. The present study compared the serum miR-1181 and miR-4314 levels in patients with EOC and healthy controls to measure the diagnostic and prognostic value as candidate biomarkers. MATERIALS AND METHODS: We collected serum samples from a total of 135 participants (69 patients with EOC and 66 healthy controls). Relative expressions of miR-1181 and miR-4314 were measured by quantitative real-time polymerase chain reaction assay (qPCR). RESULTS: The present study revealed that both serum miR-1181 and miR-4314 levels in patients with EOC were significantly increased compared to healthy controls for each marker. In addition, there was a significant relationship between miR-1181 and miR-4314 overexpressions and the stage and prognosis of the disease. Finally, patients with high expression levels of miR-1181 and miR-4314 had significantly shorter survival rates than those with low expression levels. CONCLUSION: The current study proposed that serum miR-1181 and miR-4314 could discriminate the EOC patients from healthy controls. In addition, both miR-1181 and miR-4314 may be predictive biomarkers for ovarian cancer prognosis. Further studies are needed to confirm the findings of the present study.

Microplastics (MPs) have become prevalent in various environmental compartments, including air, water, and soil, attracting attention as significant pollutant parameters. This study investigated the prevalence of MP pollution in surface sediments along Istanbul's Marmara Sea, encompassing the megacity and the Bosphorus. A comprehensive sampling approach was employed, covering 43 stations across four seasons and depths ranging from 5 to 70 m. The objective was to assess the impact of terrestrial, social, and industrial activities on MPs. The average concentrations varied per season, with fall, winter, spring, and summer values recorded as 2000 ± 4100, 1600 ± 3900, 4300 ± 12,000, and 9500 ± 20,300 particles/kg-DW. The study identified river stations in the Golden Horn and sea discharge locations as hotspots for high concentrations. Notably, the dominant shape shifted from fibers in fall, winter, and spring to fragments during summer, coinciding with mucilage occurrences. The study identified 11 different polymers, with polyethylene (44 %) and polypropylene (31 %) being the most common.

This case report illustrates the follow-up of a 57-year-old female with a type B aortic dissection (AD) under dabigatran treatment. The patient had been operated on 8 years earlier due to type A AD. The aortic valve was repaired and a 26-mm polyester fiber graft was applied to the ascending aorta and the aortic arch. In computerized tomography scans taken after the procedure, a dissection flap extending from the descending aorta to the iliac arteries was seen, but the patient was asymptomatic and no further surgery was performed. The patient was subsequently diagnosed with atrial fibrillation. A CHA2DS2VASc score of 3 was recorded and dabigatran treatment was initiated. The aortic aneurysm and dissection were followed up via computed tomography and echocardiography at regular intervals, and at 6 months no progression was seen. No thromboembolic or hemorrhagic events were observed. To our knowledge, this is the first case report of dabigatran treatment for a patient with a type B AD. Based on this case, the use of dabigatran would appear to be safe in a patient with an uncomplicated type B AD, but the results of this case need to be confirmed with extended follow-up and additional patients.

RESUMO

: Left ventricular thrombus (LVT) is a commonly seen complication of myocardial infarction and it also can be seen because of hypertrophic cardiomyopathies, nonischemic dilated cardiomyopathies, malignancies, and so on. Guidelines suggest the use of warfarin for left ventricular thrombi, but recent case reports show that direct oral anticoagulants (DOACs) are beginning to be used for this complication. DOACs are strong alternatives for warfarin because of their efficacy and safety even though there is no randomized controlled trial that proves the effect of DOACs against LVT. In this article, we gather the case reports of DOACs against left ventricular thrombi in various conditions.

ABSTRACT BACKGROUND: The chest pain classifications that are currently in use are based on studies that are several decades old. Various studies have indicated that these classifications are not sufficient for determining the origin of chest pain without additional diagnostic tests or tools. We describe a new chest pain scoring system that examines the relationship between chest pain and ischemic heart disease (IHD). DESIGN AND SETTING: Cross-sectional study conducted in a tertiary-level university hospital and two public hospitals. METHODS: Chest pain scores were assigned to 484 patients. These patients then underwent a treadmill stress test, followed by myocardial perfusion scintigraphy if necessary. Coronary angiography was then carried out on the patients whose tests had been interpreted as positive for ischemia. Afterwards, the relationship between myocardial ischemia and the test score results was investigated. RESULTS: The median chest pain score was 2 (range: 0-7) among the patients without IHD and 6 (1-8) among those with IHD. The median score of patients with IHD was significantly higher than that of patients without IHD (P = 0.001). Receiver operating characteristic analysis showed that the score had sensitivity of 97% and specificity of 87.5% for detecting IHD. CONCLUSION: We developed a pre-test chest pain score that uses a digital scoring system to assess whether or not the pain was caused by IHD. This scoring system can be applied easily and swiftly by healthcare professionals and can prevent the confusion that is caused by other classification and scoring systems.

RESUMO

Surgery is indicated for symptomatic patients with papillary fibroelastomas (PFE) on the aortic valve. The valve is commonly spared during tumor excision. Rarely, aortic valve replacement (AVR) is needed. We present a case requiring AVR for an aortic valve PFE and review the literature to determine the risk factors for failure of aortic valve-sparing techniques in patients with PFE.

AIM: An experimental study was performed to evaluate the protective effects of probiotics on gut mucosa in peritonitis through antioxidant mechanisms. METHODS: Thirty-two male Wistar albino rats were divided equally into four groups. The rats in Group 1 (control group) underwent laparotomy only. In group 2 (peritonitis group), peritonitis was induced in the rats by the cecal ligation and puncture (CLP) model. In group 3, the rats were treated with probiotics for five days after CLP-induced peritonitis. The last group of rats (group 4) were fed probiotics for five days before the CLP procedure and five days after the surgery. On the fifth day after surgery, all rats were killed, and tissue samples from the terminal ileum were obtained to evaluate the activities of myeloperoxidase (MPO), malondialdehyde (MDA), and glutathione (GSH). Histopathologic examinations were also performed to evaluate the grade of intestinal injury. RESULTS: Myeloperoxidase and MDA activities were increased, GSH concentrations were decreased in group 2, compared with group 1. Intestinal MPO activities in group 4 were decreased compared with group 1 and group 2, indicating a reduction in oxidant activity. Malondialdehyde decreased in group 3 and decreased even more in group 4, compared with the peritonitis group (group 2). Glutathione concentrations were increased in group 4 compared with group 2 and group 3 (p < 0.05). The Chiu scores of the probiotics groups, groups 3 and 4, were lower than those in group 2, indicating reduced mucosal damage in the probiotically fed groups. CONCLUSION: Probiotics have protective effects in peritonitis, which may be related to antioxidant mechanisms. This antioxidant effect of probiotics might occur when pre-conditioning with probiotics before peritonitis because there is sufficient time to prepare the tissues for oxidative damage.

Tilapia (Oreochromis niloticus) was exposed to different sizes of zinc oxide nanoparticles (ZnO-NPs) to evaluate their organ pathologies (kidney, liver, gill, and intestine), osmoregulatory responses and immunological parameters. Sub-chronic exposure was conducted in fresh water with 1 and 10 mg/L concentrations of the small (10-30 nm) and large-size ZnO (100 nm) particles for 7 and 14 days. In this study, it is found that small and large forms of ZnO-NPs cause various pathologic findings in the target organs at all concentrations. These findings are increased of melanomacrophage aggregates, tubular deformations, necrosis and cytoplasmic vacuolations in the kidney, oedema, mononuclear cell infiltrations, fatty changes, pyknotic nuclei and hepatocellular vacuolations in the liver, hyperplasia, aneurysms, and epithelial liftings in the gills, and hyperplasia, swelled of goblet cells, villus deformations in the intestine. Results showed that respiratory burst and potential killing activity at the small-size ZnO concentration significantly increased compared to the control group (p < 0.05) but significant reductions of these parameters at the large-size ZnO concentrations compared to control (p < 0.05) were measured. These findings demonstrate the potential of each particle size to cause significant damage to the immune system. Moreover, because ZnO NPs inhibit the Na(+), K(+)-ATPase activity at all concentrations and increase serum Ca(2+) and Cl(-) levels especially in gill, these particles are osmoregulatory and toxicant for tilapia fish. As a summary, both sizes of the particles have led to organ damage, osmoregulatory changes and immune disorder in tilapia fish.

BACKGROUND: The prognostic significance of perineural invasion (PNI) in gastric cancer has been previously investigated in a few studies, but had not reached a consensus. The aim of this study was to determine the prognostic value of PNI in patients with gastric cancer who underwent curative resection. MATERIALS AND METHODS: We retrospectively analyzed 238 patients who had undergone curative gastrectomy. Paraffin sections of surgical specimens from all patients were stained with hematoxylin and eosin. PNI was defined when carcinoma cells infiltrated into the perineurium or neural fascicles. PNI and the other prognostic factors were evaluated by univariate and multivariate analysis. RESULTS: PNI was detected as positive in 180 of the 238 patients (75.6%). pT stage, tumor size, lymph node metastasis, clinical stage, tumor differentiation, Borrmann classification, histological type, lymphatic vessel invasion, and blood vessel invasion were closely associated with the presence of PNI. The PNI-positive tumors had significantly larger size and more lymph node metastasis than the PNI-negative tumors (P = .001 and P < .001, respectively). The median survival of the PNI-positive patients was significantly worse than that of the PNI-negative patients (28.1 vs. 64.9 months, P = .001). Multivariate analysis indicated that the positivity of PNI was an independent prognostic factor (P = .02, hazard ratio [HR]: 2.75; 95% confidence interval [95% CI]:1.12-3.13) as were classical clinicopathological features. CONCLUSION: Our results showed that the frequency of PNI was high in patients with gastric cancer who underwent curative gastrectomy and the proportion of PNI positivity increased with progression and clinical stage of disease. PNI may be useful in detecting patients who had poor prognosis after curative resection in gastric cancer.

A 63-year-old woman with upper abdominal pain radiating to her back was admitted to the hospital, and a mass lesion was identified involving pancreatic body and tail, with peripancreatic lymph node enlargement. The patient had normal tumor marker levels, a very high erythrocyte sedimentation rate (104) and negative malignant cells in fine needle aspiration cytology. The patient underwent laparotomy and histological findings of mass were consistent with retroperitoneal fibrosis. She has been in remission after long-term steroid treatment. In the differential diagnosis of cases with pancreatic mass lesion, retroperitoneal fibrosis should also be considered.

A 21-year-old male patient with a 6-year history of Crohn's disease presented with shoulder pain and edema in the neck and bilateral supraclavicular fossa. Color Doppler ultrasonography and computed tomography scan revealed bilateral thrombosis of the external jugular veins, brachiocephalic veins, and superior vena cava. Other etiologies were appropriately ruled out and the thrombosis was initially attributed to Crohn's disease. In the follow-up, the patient developed recurrent, painful orogenital ulcers and acne vulgaris-like skin eruptions. HLA tissue typing was HLA B-51. The diagnosis of Crohn's disease associated with Behçet's disease was made. Although there are several case reports describing such an association, a Medline research revealed that this was the first presentation of such an association with thrombosis. Thrombosis in unusual sites may reflect the uncommon association of Crohn's disease with hypercoagulable conditions such as Behçet's disease.

A 41-year-old man presenting with lower abdominal pain, constipation, abdominal distention, fever (37.5 degrees C) and fatigue was evaluated, and a mass localized to the left lower abdomen was identified. Radiographic and colonoscopic examination revealed a stricture 10 cm in length localized to the sigmoid-descending colon junction. The diagnosis of amebiasis was confirmed by histological examination of a biopsy specimen taken from the stricture and stool examination. One month after the initiation of metronidazole treatment, complete clinical and laboratory improvement was observed. In the differential diagnosis of colonic stricture, amebiasis should also be considered.

RESUMO

Mitochondrial neurogastrointestinal encephalomyopathy is a rare, multisystem disorder characterized by gastrointestinal dysmotility, ptosis, neurologic findings (e.g., peripheral neuropathy), leukoencephalopathy, and thin body habitus. Gastrointestinal motility studies and skeletal muscle biopsy are recommended diagnostic tools. We report two patients that highlight the diagnostic characteristics of this rare entity.
