PURPOSE: This study investigated the mediating role of individual resilience in the relationship between caregiver burden and quality of life (QoL) among Chinese adult children providing care to their parents with advanced cancer, with the aim to inform effective coping strategies and positive caregiving outcomes. METHODS: In a cross-sectional design, 614 caregivers from multiple centers, whose parents were undergoing chemotherapy and/or radiotherapy, completed questionnaires encompassing demographics, caregiver burden, symptoms of anxiety and depression, resilience, and QoL. RESULTS: Findings revealed a moderate level of caregiver burden among participants, significantly influenced by factors including education level, family income, single-child status, and participation in social media patient support groups. Caregivers who were only children or involved in patient support groups reported higher burden. Importantly, path analysis showed a significant impact of caregiver burden, anxiety, and depression on QoL, with these relationships being mediated by individual resilience. CONCLUSIONS: Chinese adult child caregivers face a considerable burden, negatively influencing their QoL. Individual resilience, a modifiable factor, was identified as a critical mediator in this relationship, mitigating the negative implications of caregiver burden, anxiety, and depression. These findings underscore the need for caregiver interventions that consider not only demographics but also the socio-psychological dynamics of caregiving to enhance caregiver QoL.

In order to develop appropriate treatment and rehabilitation plans with regard to different subpathological types (PILs and IAs) of lung nodules, it is important to diagnose them through low-dose spiral computed tomography (LDCT) during routine screening before surgery. Based on the characteristics of different subpathological lung nodules expressed from LDCT images, we propose a multi-dimension and multi-feature hybrid learning neural network in this paper. Our network consists of a 2D network part and a 3D network part. The feature vectors extracted from the 2D network and 3D network are further learned by XGBoost. Through this formation, the network can better integrate the feature information from the 2D and 3D networks. The main learning block of the network is a residual block combined with attention mechanism. This learning block enables the network to learn better from multiple features and pay more attention to the key feature map among all the feature maps in different channels. We conduct experiments on our dataset collected from a cooperating hospital. The results show that the accuracy, sensitivity and specificity of our network are 83%, 86%, 80%, respectively It is feasible to use this network to classify the subpathological type of lung nodule through routine screening.

Perylenequinones (PQ), a class of naturally occurring polypeptides, are widely used as a clinical drug for treating skin diseases and as a photodynamic therapy against cancers and viruses. In this study, the effects of different carbon sources on PQ biosynthesis by Shiraia sp. Slf14 were compared, and the underlying molecular mechanism of fructose as the sole carbon to enhance PQ production was investigated by transcriptome analysis. The results indicated that fructose enhanced PQ yield to 1753.64 mg/L, which was 1.73-fold higher than that obtained with glucose. Comparative transcriptome analysis demonstrated that most of the upregulated genes were related to transport systems, energy and central carbon metabolism in Shiraia sp. Slf14 cultured in fructose. The genes involved in glycolysis and pentose phosphate pathways, and encoding citrate synthase, ATP-citrate lyase, and acetyl-CoA carboxylase were substantially upregulated, resulting in increased overall acetyl-CoA and malonyl-CoA production. However, genes involved in gluconeogenesis, glyoxylate cycle pathway, and fatty acid synthesis were significantly downregulated, resulting in higher acetyl-CoA influx for PQ formation. In particular, the putative PQ biosynthetic cluster was upregulated in Shiraia sp. Slf14 cultured in fructose, leading to a significant increase in PQ production. The results of real-time qRT-PCR and related enzyme activities were also consistent with those of transcriptome analysis. These findings provide a remarkable insight into the underlying mechanism of PQ biosynthesis and pave the way for improvements in PQ production by Shiraia sp. Slf14.

OBJECTIVE: To summarize our experiences on surgical saving of the cervical opening wound. METHOD: Retrospectively analyze 43 cases with cervical opening wound in department of otolaryngology, suizhou central hospital from 1992-2010. RESULT: Forty-one among 43 cases healed with no complications, 1 case died of obstruction of air way and hemorrhagic shock, and 1 case occurred secondary laryngostenosis and resulted in fine prognosis af ter treating with laryngotracheoplasty in higher hospital. CONCLUSION: It is very important to give the appropriate treatment on the injuries of large cervical vessels, nerves, laryngeal cartilages, throat mucosa and complications. It is important to repair the wounds primarily. Correct diagnosis and treatment at first time, and well nursing are crucial for good prognosis.

OBJECTIVE: To observe and summarize the clinical effect of nasal septum surgery combined with multi-sinus surgery and to discuss the safety and feasibility. METHOD: Fifty-six patients with nasal sinuses and nasal cavity diseases received nasal septal reconstruction, Caldwell-Lue procedure, polypectomy, ethmoidectomy and partial middle or inferior turbinate resection simultaneously under local anesthesia, and the complications after surgery were observed. RESULT: Operation in fifty-six patients were completed successfully. Thirty cases were cured, the cure rate of 56 case was 53.57Y, 23 cases were effective recovered (41.07%), 3 cases didn't recover (5.36%). The clinical cure rate was 94.64%. No severe complications occurred. CONCLUSION: The nasal septal surgery combined with multi-sinus surgery simultaneously under local anesthesia is effective and safety procedure. It can decrease the recurrence rate of nasal polyp and sinusitis and don't increase the rate of complication occurring.
