Introduction: The effectiveness of the supplementation of prebiotics, probiotics and synbiotics as a therapeutic approach in colorectal cancer (CRC) remains unclear. The aim of this systematic review is to critically examine the current scientific evidence on the impact of modulating the microbiota, through the use of prebiotics, probiotics and synbiotics, in patients diagnosed with CRC undergoing treatment, to determine the potential therapeutic use of this approach. Methods: This systematic review was made according to the PRISMA 2020 guidelines. Inclusion criteria were randomized controlled trials (RCT) comparing the impact of pre-, pro-, or synbiotic supplementation with placebo or standard care in patients with CRC undergoing treatment. Exclusion criteria were non-human studies, non-RCTs, and studies in languages other than English or Portuguese. Six databases were consulted, namely, Cochrane Library, Pubmed, Scopus, Cinahl, MedicLatina and Web of Science until May of 2023. RAYYAN software was used to manage the search results and risk of bias was assessed according to the guidelines of the Cochrane Collaboration using the Rob 2.0 tool. Results: Twenty-four RCTs met the inclusion criteria and were included in this review. Administration of pre-, pro-, or synbiotics improved surgical outcomes such as the incidence of infectious and non-infectious postoperative complications, return to normal gut function, hospital length of stay, and antibiotic usage. The supplementation of these microorganisms also alleviated some symptoms from chemotherapy and radiotherapy, mainly diarrhea. Evidence on the best approach in terms of types of strains, dosage and duration of intervention is still scarce. Conclusions: Pre-, pro-, and synbiotics supplementation appears to be a beneficial therapeutic approach in CRC treatment to improve surgical outcomes and to alleviate side-effects such as treatment toxicity. More RCTs with larger sample sizes and less heterogeneity are needed to confirm these potential benefits and to determine the best strains, dosage, and duration of administration in each situation. INTRODUCTION: Malnutrition is common in patients suffering from malignant diseases and has a major impact on patient outcomes. Prevention and early detection are crucial for effective treatment. This study aimed to investigate current international practice in the assessment and management of malnutrition in surgical oncology departments. MATERIAL AND METHODS: The survey was designed by European Society of Surgical Oncology (ESSO) and ESSO Young Surgeons and Alumni Club (EYSAC) Research Academy as an online questionnaire with 41 questions addressing three main areas: participant demographics, malnutrition assessment, and perioperative nutritional standards. The survey was distributed from October to November 2021 via emails, social media and the ESSO website to surgical networks focussing on surgical oncologists. Results were collected and analysed by an independent team. RESULTS: A total of 156 participants from 39 different countries answered the survey, reflecting a response rate of 1.4%. Surgeons reported treating a mean of 22.4 patients per month. 38% of all patients treated in surgical oncology departments were routinely screened for malnutrition. 52% of patients were perceived as being at risk for malnutrition. The most used screening tool was the "Malnutrition Universal Screening Tool" (MUST). 68% of participants agreed that the surgeon is responsible for assessing preoperative nutritional status. 49% of patients were routinely seen by dieticians. In cases of severe malnutrition, 56% considered postponing the operation. CONCLUSIONS: The reported rate of malnutrition screening by surgical oncologists is lower than expected (38%). This indicates a need for improved awareness of malnutrition in surgical oncology, and nutritional screening.

BACKGROUND & AIMS: The high prevalence of malnourished cancer patients highlights the importance of sensitive and specific tools for nutritional risk and status assessment screening, namely the Patient-Generated Subjective Global Assessment (PG-SGA®). This study aimed to assess whether the short-form version of the PG-SGA® (PG-SGA© SF) would be appropriate to identify the nutritional risk of patients when compared with the final global score of PG-SGA© (long-form version). METHODS: This transversal and observational study comprised a convenience sample of cancer patients undergoing chemotherapy at the Champalimaud Clinical Centre between December 2016 and February 2018. Clinical data and anthropometric parameters were collected in order to apply PG-SGA® and PG-SGA© SF. The data was statistically analysed through SPSS version 22 (SPSS Inc, Chicago, IL, USA). RESULTS: In this study 355 patients were enrolled and PG-SGA© SF results showed that 69.3% (n = 246) of the population presented at least one risk factor for malnourishment (Σ (box A) ≥1). Additionally, PG-SGA® revealed that 50% of patients (n = 177) have a risk of developing malnourishment or are already malnourished (B and C classification). The concordance of results showed to be high (coefficient k = 0.62; p < 0.001), meaning that PG-SGA SF© has a good sensibility (95%) and specificity (67%) for the identification of nutritional risk and assessment of nutritional status when compared with the complete version of PG-SGA©. CONCLUSIONS: According to our results, PG-SGA© SF is a useful and sufficient tool, representing an easier and faster way to identify at-risk or malnourished patients.
