RESUMO

Improvements in fetal ultrasound have allowed for the diagnosis and treatment of fetal diseases in the uterus, often though surgery. However, little attention has been drawn to the assessment of fetal pain. To address this gap, a fetal pain scoring system, known as the Fetal-7 scale, was developed. The present study is a full validation of the Fetal-7 scale. The validation involved two steps: i. four fetuses with indication of surgery were evaluated in three conditions perioperatively: acute pain, rest, and under loud sound stimulation. Facial expressions were assessed by 30 raters using screenshots from 4D high-definition ultrasound films; ii., assessment of sensitivity and specificity of the Fetal-7 scale in 54 healthy fetuses and two fetuses undergoing acute pain after preoperative anesthetic intramuscular injection. There was high internal consistency with Cronbach's alpha (α) of 0.99. Intra-rater reliability of the Fetal-7 scale (test-retest) calculated by intraclass correlation coefficient (ICC) was 0.95, and inter-rater reliability was 0.99. The scale accurately differentiated between healthy fetuses at rest and those experiencing acute pain (sensitivity of 100% and specificity of 94.4%). The Fetal-7 scale is a valid tool for assessing acute pain-related behavior in third trimester fetuses and may be of value in guiding analgesic procedures efficacy in these patients. Further research is warranted to explore the presence of post-operative pain in fetuses and its effects afterbirth. PERSPECTIVE: Recordings with three-dimension ultrasound of human fetuses undergoing pre-operative anesthetic injections revealed complex facial expressions during acute pain, similar to those collected in newborns. This study presented the validation process and cut-off value of the Fetal-7 scale, paving the way for the study of pain before birth in humans.

Background: The neutrophil-lymphocyte ratio (NLR) is an easily accessible and inexpensive biomarker that has been shown to predict morbidity and mortality in congenital cardiac surgery. However, its regulatory mechanism remains unclear. This study aims to compare and correlate the tumor necrosis factor alpha (TNF-α), interleukin (IL)-1ß, IL-6, and IL-10 messenger RNAs (mRNAs) with the NLR in patients with tetralogy of Fallot (ToF) and ventricular septal defect (VSD). Methods: A prospective translational study was conducted on 10 children with ToF and 10 with VSD, aged between 1 and 24 months. The NLR was calculated from the blood count taken 24 hours before surgery. The expression of these mRNAs was analyzed in the myocardial tissue of the right atrium prior to cardiopulmonary bypass. Results: Patients with ToF exhibited a higher NLR [ToF 0.46 (interquartile range; IQR) 0.90; VSD 0.28 (IQR 0.17); P=0.02], longer mechanical ventilation time [ToF 24 h (IQR 93); VSD 5.5 h (IQR 8); P<0.001], increased use of vasoactive drugs [ToF 2 days (IQR 1.75); VSD 0 (IQR 1); P=0.01], and longer ICU [ToF 5.5 (IQR 1); VSD 2 (IQR 0.75); P=0.02] and hospital length of stays [ToF 18 days (IQR 17.5); VSD 8.5 days (IQR 2.5); P<0.001]. A negative correlation was found between NLR and oxygen saturation (SaO2) (r=-0.44; P=0.002). In terms of mRNA expression, the ToF group showed a lower expression of IL-10 mRNA (P=0.03). A positive correlation was observed between IL-10-mRNA and SaO2 (r=0.40; P=0.07), and a negative correlation with NLR (r=-0.27; P=0.14). Conclusions: Patients with ToF demonstrated a higher preoperative NLR and lower IL-10 mRNA expression by what appears to be a pro-inflammatory phenotype of cyanotic patients.

INTRODUCTION: High-resolution anoscopy (HRA) is the gold standard for detecting anal squamous cell carcinoma (ASCC) precursors. Preliminary studies on the application of artificial intelligence (AI) models to this modality have revealed promising results. However, the impact of staining techniques and anal manipulation on the effectiveness of these algorithms has not been evaluated. We aimed to develop a deep learning system for automatic differentiation of high-grade squamous intraepithelial lesion vs low-grade squamous intraepithelial lesion in HRA images in different subsets of patients (nonstained, acetic acid, lugol, and after manipulation). METHODS: A convolutional neural network was developed to detect and differentiate high-grade and low-grade anal squamous intraepithelial lesions based on 27,770 images from 103 HRA examinations performed in 88 patients. Subanalyses were performed to evaluate the algorithm's performance in subsets of images without staining, acetic acid, lugol, and after manipulation of the anal canal. The sensitivity, specificity, accuracy, positive and negative predictive values, and area under the curve were calculated. RESULTS: The convolutional neural network achieved an overall accuracy of 98.3%. The algorithm had a sensitivity and specificity of 97.4% and 99.2%, respectively. The accuracy of the algorithm for differentiating high-grade squamous intraepithelial lesion vs low-grade squamous intraepithelial lesion varied between 91.5% (postmanipulation) and 100% (lugol) for the categories at subanalysis. The area under the curve ranged between 0.95 and 1.00. DISCUSSION: The introduction of AI to HRA may provide an accurate detection and differentiation of ASCC precursors. Our algorithm showed excellent performance at different staining settings. This is extremely important because real-time AI models during HRA examinations can help guide local treatment or detect relapsing disease.

Aim: Despite some successful examples of therapeutic nanoparticles reaching clinical stages, there is still a significant need for novel formulations in order to improve the selectivity and efficacy of cancer treatment. Methods: The authors developed two novel dendrimer-gold (Au) complex-based nanoparticles using two different synthesis routes: complexation method (formulation A) and precipitation method (formulation B). Using a biomimetic cancer-on-a-chip model, the authors evaluated the possible cytotoxicity and internalization by colorectal cancer cells of dendrimer-Au complex-based nanoparticles. Results: The results showed promising capabilities of these nanoparticles for selectively targeting cancer cells and delivering drugs, particularly for the formulation A nanoparticles. Conclusion: This work highlights the potential of dendrimer-Au complex-based nanoparticles as a new strategy to improve the targeting of anticancer drugs.

Objetivo: Descrever, segundo a literatura, a relação entre o Transtorno do Espectro Autista e a ecolalia. Método: Realizou-se uma revisão integrativa da literatura científica na BVS, PubMed e Scielo, utilizando os descritores DECS/MESH "Transtorno do Espectro Autista AND ecolalia" e "Autism Spectrum Disorder AND Echolalia" combinados pelo operador booleano "AND". Resultados: Predominaram estudos qualitativos (35,7%) com baixo nível de evidência (92,8%). A maioria das pesquisas (78,6%) foi conduzida nos Estados Unidos da América em 2021, com participantes de 1 a 40 anos, sendo a ecolalia frequentemente observada em crianças de 2 anos (28,6%). A abordagem ao tratamento da ecolalia foi mencionada em apenas 21,4% dos estudos, destacando métodos comportamentais. Conclusão: A ecolalia, fenômeno persistente no TEA, é uma notável peculiaridade na comunicação verbal, apresentando variações imediatas, tardias e mitigadas. Suas implicações continuam a desafiar o desenvolvimento e as intervenções clínicas.(AU)

Method: An integrative review of scientific literature was conducted using BVS, PubMed, and Scielo databases, employing the DECS/MESH descriptors "Autism Spectrum Disorder AND echolalia" and "Autism Spectrum Disorder AND Echolalia" combined with the Boolean operator "AND." Results: Qualitative studies predominated (35.7%) with low levels of evidence (92.8%). The majority of research (78.6%) was conducted in the United States in 2021, involving participants aged 1 to 40, with echolalia frequently observed in 2-year-old children (28.6%). The approach to echolalia treatment was mentioned in only 21.4% of the studies, emphasizing behavioral methods. Conclusion: Echolalia, a persistent phenomenon in Autism Spectrum Disorder, represents a notable peculiarity in verbal communication, exhibiting immediate, delayed, and mitigated variations. Its implications continue to challenge development and clinical interventions.(AU)

Microfluidic organs and organoids-on-a-chip models of human gastrointestinal systems have been established to recreate adequate microenvironments to study physiology and pathophysiology. In the effort to find more emulating systems and less costly models for drugs screening or fundamental studies, gastrointestinal system organoids-on-a-chip have arisen as promising pre-clinicalin vitromodel. This progress has been built on the latest developments of several technologies such as bioprinting, microfluidics, and organoid research. In this review, we will focus on healthy and disease models of: human microbiome-on-a-chip and its rising correlation with gastro pathophysiology; stomach-on-a-chip; liver-on-a-chip; pancreas-on-a-chip; inflammation models, small intestine, colon and colorectal cancer organoids-on-a-chip and multi-organoids-on-a-chip. The current developments related to the design, ability to hold one or more 'organs' and its challenges, microfluidic features, cell sources and whether they are used to test drugs are overviewed herein. Importantly, their contribution in terms of drug development and eminent clinical translation in precision medicine field, Food and Drug Administration approved models, and the impact of organoid-on-chip technology in terms of pharmaceutical research and development costs are also discussed by the authors.

Introduction: The increase in the incidence of diabetes mellitus has led to public health consequences. Several individuals with diabetes lose sensitivity and may present foot deformities, resulting in diabetic foot. Knowing about self-care deficits can allow for planning healthcare in the management of diabetic foot and improve the patient's quality of life. Objective: To identify, from the scientific literature, the main self-care deficits present in elderly people with diabetic foot ulcers, based on Orem's self-care deficit theory. Materials and methods: This is an integrative review study. The search for studies was performed in January 2022, using the descriptors "diabetic foot", "self-care," and "aged." The following virtual library and databases were used: BVS, Lilacs, Medline and BDEnf via PubMed. The adaptation of the Prisma flowchart was used. A synthesis of the articles was performed with the description of the main results and self-care deficits according to Orem's self-care theory. Results: Socioeconomic conditions, skin characteristics, activities, leisure and rest, diet, neurological conditions, and lack of knowledge about the need for therapeutic support and treatment were the main factors related to self-care deficits. Conclusion: The scientific evidence generated provides a foundation for carrying out new studies on the theme, as well as assisting in decision-making by nurses to adopt measures that assist in the self-care of this population group, in addition to guiding this population regarding the necessary care.


Introdução: o aumento da incidência do diabetes mellitus vem acarretando consequências à saúde pública. Muitos indivíduos com diabetes perdem a sensibilidade e podem desenvolver deformidades nos pés, ocasionando o pé diabético. Conhecer os déficits de autocuidado pode permitir o planejamento da assistência à saúde no manejo do pé diabético e melhorar a qualidade de vida do paciente. Objetivo: identificar, a partir da literatura científica, os principais déficits de autocuidado presentes em idosos com úlceras de pé diabético, fundamentando na teoria do déficit de autocuidado de Orem. Materiais e método: trata-se de um estudo do tipo revisão integrativa. A busca dos estudos ocorreu em janeiro de 2022, a partir dos descritores "pé diabético", "autocuidado" e "idoso". Utilizaram-se as bases de dados e biblioteca virtual: BVS, Lilacs, Medline e BDEnf por meio da PubMed. A adaptação do fluxograma Prisma foi utilizada. Uma síntese dos artigos foi realizada com a descrição dos principais resultados e déficits de autocuidado conforme a teoria de autocuidado de Orem. Resultados: condições socioeconômicas, características da pele, atividades, lazer e repouso, alimentação, condições neurológicas e falta de conhecimento sobre a necessidade de suporte terapêutico e de tratamento foram as principais situações relacionadas aos déficits de autocuidado. Conclusão: a evidência científica gerada serve como base para realizar novos estudos sobre a temática, assim como auxilia na tomada de decisões por parte do enfermeiro para adotar medidas que auxiliem no autocuidado desse grupo populacional, além de orientar essa população quanto aos cuidados necessários.

Muchos individuos con diabetes pierden sensibilidad y pueden desarrollar deformidades en los pies, dando lugar al pie diabético. Conocer los déficits de autocuidado puede permitir la planificación de la asistencia sanitaria en el manejo del pie diabético y mejorar la calidad de vida del paciente. Objetivo: identificar, desde la literatura, los principales déficits de autocuidado presentes en adultos mayores con úlceras de pie diabético, con base en la teoría del déficit de autocuidado de Orem. Materiales y método: se trata de una revisión integradora. La búsqueda de estudios se realizó en enero de 2022, utilizando los descriptores "pie diabético", "autocuidado" y "anciano". Se utilizaron las siguientes bases de datos y bibliotecas virtuales: BVS, Lilacs, Medline y BDEnf a través de PubMed. Se utilizó la adaptación del diagrama de flujo Prisma. Se realizó una síntesis de los artículos con la descripción de los principales resultados y déficits de autocuidado según la teoría de autocuidado de Orem. Resultados: condiciones socioeconómicas, características de la piel, actividades, ocio y descanso, alimentación, condiciones neurológicas y desconocimiento de la necesidad de apoyo terapéutico y tratamiento fueron las principales situaciones relacionadas a los déficits de autocuidado. Conclusiones: se destaca la importancia de la educación sanitaria, el manejo terapéutico, los cuidados individualizados y el uso de una herramienta de consultas de enfermería para apoyar la prevención del pie diabético y mejorar la calidad de vida de los ancianos.

Inhibition of ABC transporters is a promising approach to overcome multidrug resistance in cancer. Herein, we report the characterization of a potent ABCG2 inhibitor, namely, chromone 4a (C4a). Molecular docking and in vitro assays using ABCG2 and P-glycoprotein (P-gp) expressing membrane vesicles of insect cells revealed that C4a interacts with both transporters, while showing selectivity toward ABCG2 using cell-based transport assays. C4a inhibited the ABCG2-mediated efflux of different substrates and molecular dynamic simulations demonstrated that C4a binds in the Ko143-binding pocket. Liposomes and extracellular vesicles (EVs) of Giardia intestinalis and human blood were used to successfully bypass the poor water solubility and delivery of C4a as assessed by inhibition of the ABCG2 function. Human blood EVs also promoted delivery of the well-known P-gp inhibitor, elacridar. Here, for the first time, we demonstrated the potential use of plasma circulating EVs for drug delivery of hydrophobic drugs targeting membrane proteins.

Basic pre-clinical research based on 2D cultures have been very valuable in colorectal cancer (CRC) research but still have failed to improve patient prognostic outcomes. This is because they simply do not replicate what happensin vivo, i.e.2D cultured cells system cannot replicate the diffusion constraints usually found in the body. Importantly, they also do not mimic the dimensionality of the human body and of a CRC tumour (3D). Moreover, 2D cultures lack the cellular heterogeneity and the tumour microenvironment (TME) such as stromal components, blood vessels, fibroblasts, and cells of the immune system. Cells behave differently whether in 2D and 3D, in particular their different genetic and protein expression panels are very different and therefore we cannot fully rely on drug tests done in 2D. A growing field of research based on microphysiological systems involving organoids/spheroids or patient-derived tumour cells has become a solid base for a better understanding of the TME and as a result is a step towards personalized medicine. Furthermore, microfluidic approaches have also started to open possibilities of research, with tumour-on-chips and body-on-chips being used in order to decipher complex inter-organ signalling and the prevalence of metastasis, as well as CRC early-diagnosis through liquid biopsies. Herein, we focus on the state-of-the-art of CRC research with emphasis on 3D microfluidicin vitrocultures-organoids, spheroids-drug resistance, circulating tumour cells and microbiome-on-a-chip technology.

PURPOSE: This study aimed to assess the prevalence and progression of lower urinary tract symptoms following laparoscopic surgery for deep-infiltrating endometriosis of the rectosigmoid and identify preoperative factors associated with worse postoperative outcomes. METHODS: Prospective, observational study. SETTINGS: single-center, referral hospital for endometriosis. Patients undergoing laparoscopic surgery for deep-infiltrating endometriosis of the rectosigmoid colon between October 2016 and October 2018. MAIN OUTCOME MEASURES: urinary function was assessed with the validated Portuguese language version of the International Prostate Symptom Score, which is also used in women. The score was collected before and after surgery. The Wilcoxon signed-rank test was used to compare pre and postoperative scores and the chi-square test compared symptoms categorized by severity. RESULTS: Fifty-three patients were assessed and 44 were included. Concerning urinary symptoms after surgery, the irritative symptoms prevailed over the obstructive ones. Additionally, 58.8% and 54.5% of the women reported moderate or severe symptoms at pre and postoperative, respectively. In at least one questionnaire category, the postoperative questionnaire scores increased in ten (22.7%) participants. A statistically significant difference was found comparing the changes from absent/mild to moderate/severe IPSS categories (P = 0.039). No significant changes were identified in any of the International Prostate Symptom Score pre and postoperatively (P = 0.876). CONCLUSIONS: There was a high prevalence of pre and postoperative urinary symptoms. Patients with preoperative moderate/severe International Prostate Symptom Score are at risk of persisting urinary dysfunction after surgery for rectosigmoid deep endometriosis.

BACKGROUND: Arrhythmogenic right ventricular cardiomyopathy (ARVC), a rare inherited disease, causes ventricular tachycardia, sudden cardiac death, and heart failure (HF). We investigated ARVC clinical features, genetic findings, natural history, and the occurrence of life-threatening arrhythmic events (LTAEs), HF death, or heart transplantation (HF-death/HTx) to identify risk factors. METHODS: The clinical course of 111 consecutive patients with definite ARVC, predictors of LTAE, HF-death/HTx, and combined events were analyzed in the entire cohort and in a subgroup of 40 patients without sustained ventricular arrhythmia before diagnosis. RESULTS: The 5-year cumulative probability of LTAE was 30% and HF-death/HTx was 10%. Predictors of HF-death/HTx were reduced right ventricle ejection fraction (HR: 0.93; P=0.010), HF symptoms (HR: 4.37; P=0.010), epsilon wave (HR: 4.99; P=0.015), and number of leads with low QRS voltage (HR: 1.28; P=0.001). Each additional lead with low QRS voltage increased the risk of HF-death/HTx by 28%. Predictors of LTAE were prior syncope (HR: 1.81; P=0.040), number of leads with T wave inversion (HR: 1.17; P=0.039), low QRS voltage (HR: 1.12; P=0.021), younger age (HR: 0.97; P=0.006), and prior ventricular arrhythmia/ventricular fibrillation (HR: 2.45; P=0.012). Each additional lead with low QRS voltage increased the risk of LTAE by 17%. In patients without ventricular arrhythmia before clinical diagnosis of ARVC, the number of leads with low QRS voltage (HR: 1.68; P=0.023) was independently associated with HF-death/HTx. CONCLUSIONS: Our study demonstrated the characteristics of a specific cohort with a high prevalence of arrhythmic burden at presentation, male predominance, younger age and HF severe outcomes. Our main results suggest that the presence and extension of low QRS voltage can be a risk predictor for HF-death/HTx in ARVC patients, regardless of the arrhythmic risk. This study can contribute to the global ARVC risk stratification, adding new insights to the international current scientific knowledge.

A amostra foi constituída por 231 idosos. RESULTADOS: Dos 231 idosos que participaram do estudo 36,4% eram do sexo masculino e 63,6% do sexo feminino. A média de idade observada foi de 73,4 (± 8,7) anos. Houve uma diferença estatisticamente significativa entre as variáveis "sexo", "quantidade de medicamentos" e "possui cuidador". Ademais, observou-se a correlação estatística positiva entre a idade e a quantidade de medicamentos utilizada pelo idoso. A prevalência de polifarmácia identificada foi de 16,0%. A maior parte dos usuários de múltiplos fármacos apresentaram adesão ao tratamento (86,5%). CONCLUSÃO: Presume-se a necessidade de uma maior investigação da relação entre o cuidador e a quantidade medicamentos utilizados pelos idosos, além da capacitação profissional para o manejo da polifarmácia.


OBJECTIVE: To evaluate drug adherence in elderly people who use polypharmacy in the context of primary care. METHOD: Cross-sectional study of quantitative character. The sample consisted of 231 elderly. RESULTS: Of the 231 elderly who participated in the study, 36.4% were male and 63.6% were female. The mean age observed was 73.4 (± 8.7) years. There was a statistically significant difference among the variables "sex", "quantity of drugs" and "has caregiver". In addition, a positive statistical correlation was observed between age and the amount of drugs used by the elderly. The prevalence of polypharmacy identified was 16.0%. Most users of multiple drugs showed adherence to treatment (86.5%). CONCLUSION: It is assumed the need for a greater investigation of the relationship between the caregiver and the amount of medicines used by the elderly, in addition to professional training for the management of polypharmacy.

Introdução - Em dezembro de 2019, o mundo foi surpreendido pela epidemia de uma síndrome respiratória aguda grave causada pelo novo Coronavírus e a população idosa foi o grupo que apresentou as maiores taxas de internações diárias e de óbitos em comparação às demais faixas etárias. Para evitar a disseminação do vírus e, na inexistência de imunizante aprovado, foram adotadas medidas de isolamento social que impactaram diretamente a segurança alimentar e nutricional (SAN) da população geral e a realização das atividades de vida diária (AVDs) naquele grupo populacional, contribuindo, assim, para um possível processo de fragilização. O auxílio emergencial foi uma política pública adotada com finalidade de suprir as necessidades dos cidadãos que se viram desempregados ou que tiveram sua renda diminuída. Objetivo - Analisar a situação de SAN e de fragilidade em idosos no primeiro semestre de 2020 (antes e durante a pandemia de COVID-19), assim como verificar o impacto do auxílio emergencial sobre a SAN dos idosos que tiveram acesso ao benefício por meio de seus familiares. Métodos - Trata-se de um estudo longitudinal, descritivo, realizado com idosos matriculados em uma Unidade de Referência em Saúde do Idoso (URSI) localizada na zona sul do município de São Paulo. Os dados foram coletados por meio de entrevista presencial antes do início da pandemia (etapa 1) e por meio de ligação telefônica, na vigência da pandemia (etapa 2), nos meses de fevereiro, março e julho de 2020. Para avaliação da SAN e da fragilidade, foram utilizadas a Escala Brasileira de Insegurança Alimentar (EBIA) e o Instrumento de Avaliação de Fragilidade Autorreferida respectivamente, bem como questionários estruturados para avaliação sociodemográfica e nutricional dos sujeitos da pesquisa. Para a análise estatística, foram utilizados modelos de regressão logística simples (univariados) para testar a hipótese de associação entre diferentes características da amostra e a frequência de insegurança alimentar. Resultados - a amostra foi composta por 46 idosos, com predominância de mulheres, pretos e pardos, viúvos ou casados, com escolaridade média de 4,1 anos de estudo e renda familiar per capita inferior a 1 salário-mínimo. A insegurança alimentar variou de 56,5% na Etapa 1 para 50% na Etapa 2. Essa diferença não foi estatisticamente significante, embora tenha se destacado a proporção considerável de idosos que migraram de segurança para a insegurança alimentar no período (17,39%). Ao se comparar as frequências de fragilidade antes e durante pandemia, foi observado que a frequência de fragilidade variou de 78,3% antes para 67,4% durante a pandemia, mas sem significância estatística. Não foi encontrada associação estatisticamente significante entre auxílio emergencial e SAN pelo teste exato de Fischer (p = 1,000), nem deste sobre a variação de níveis de SAN pelo T teste. Conclusão - não houve evidência suficientemente forte para verificar o impacto da pandemia de COVID-19 para a SAN e para o processo de fragilização de idosos, assim como o estudo não teve dados estatísticos suficientes para provar associação entre auxílio emergencial e SAN.


Introduction - In December 2019, the world was surprised by the epidemic of a severe acute respiratory syndrome caused by the new Coronavirus, and the elderly population was the group that had the highest rates of daily hospitalizations and deaths compared to other age groups. To avoid the spread of the virus and, in the absence of a vaccine, social isolation measures were adopted, what have directly impacted in the food and nutrition security (FNS) of the general population and the performance of activities of daily living (ADLs) by that population group, contributing, thus, for a possible frailty process. "Auxílio Emergencial" was a public policy adopted with the purpose of meeting the needs of citizens who found themselves unemployed or who had their income reduced. Objective - To analyze the situation of FNS and frailty in the elderly in the first half of 2020 (before and during the COVID-19 pandemic), as well as to to verify the impact of "Auxílio Emergecial" on the FNS of the elderly who had access to the benefit through their family members. Methods - This is a longitudinal, descriptive study, carried out with elderly people enrolled in a Health Reference Unit for the Elderly located in the south zone of São Paulo city. Data were collected, during February, March and July 2020, through face-to-face interviews before the beginning of the pandemic (stage 1) and through telephone calls, during the pandemic (stage 2). Structured questionnaires for sociodemographic and nutritional assessment scales were used: the Brazilian Food Insecurity Scale and the Self-Reported Frailty Assessment Instrument. For statistical analysis, simple logistic regression models (univariate) were used to test the hypothesis of association between different characteristics of the sample and the frequency of food insecurity. Results - the sample consisted of 46 elderly people, predominantly women, black and brown, widowed or married, with an average of 4.1 years of schooling and per capita family income of less than 1 minimum wage. Food insecurity ranged from 56.5% in Stage 1 to 50% in Stage 2. This difference was not statistically significant, although the considerable proportion of elderly people who migrated from security to food and nutritional insecurity in the period (17.39%). When comparing the frequencies of frailty before and during the pandemic, it was observed that the frequency of frailty reduced from 78.3% before to 67.4% during the pandemic. No statistically significant association was found between "auxílio emergencial" and FNS by Fisher's exact test (p = 1.000), nor on the effect of "auxílio emergencial" on the amendment of FNS levels by the T test. Conclusion - there was no evidence strong enough to verify the impact of the COVID-19 pandemic on FNS and on the process of frailty of the elderly, as well as the study did not have enough statistical data to prove an association between "auxílio emergencial" and FNS.

RESUMO As condições crônicas complexas de saúde são um grupo de condições clínicas permanentes que demandam cuidados contínuos. Os avanços científicos e tecnológicos permitiram que crianças que antes morriam ao nascimento, ou logo após, ganhassem sobrevida à custa de uma complexa rede de cuidados. Os objetivos do estudo foram identificar e analisar a produção científica nacional acerca da temática em pediatria. Realizou-se uma revisão integrativa nas bases de dados Scopus e Web of Science. Todos os artigos originais completos publicados sem restrição temporal e que continham palavras-chave selecionadas foram incluídos. A busca foi realizada no campo tópico das ferramentas descritas na metodologia, e o resultado foi unificado e harmonizado no software Vantage Point. Após tratamento e análise, encontraram-se 64 artigos. Foi utilizado o software VOSviewer versão 1.6.16 para identificação dos temas de pesquisa. Os resultados expostos são a distribuição geográfica e temporal da produção científica, as principais instituições pesquisadoras e redes colaborativas, e cinco núcleos temáticos centrais. Há ainda inúmeros desafios relativos às frequentes hospitalizações e aos processos de desospitalização que essas crianças enfrentam. Conclui-se com a necessidade de aprofundamento e exploração do tema de forma colaborativa e com novas questões de pesquisa para avultar a construção do conhecimento nessa temática.


ABSTRACT Complex chronic conditions are a group of permanent medical conditions that require continuous care. Techno-scientific advances have allowed children who would previously die at birth, or soon after, to gain survival, but at the expense of a complex care. The aims of the study were to identify and analyze the national scientific pediatric production. We performed an integrative review on the Scopus and Web of Science databases. All full original articles published without time restriction and containing selected keywords were included. The search was carried out in the topic field of the tools described in the methodology and the result was unified and harmonized in the Vantage Point software. After treatment and analysis, 64 articles were found. VOSviewer software version 1.6.16 was used to identify the research topics. The results showed are the geographic and temporal distribution of the scientific production, the main research institutions and collaborative networks, and five central thematic nuclei. There are still numerous challenges related to the frequent hospitalizations and clinical care delivery processes. We conclude with the need to explore and search the theme in a collaborative way and with new research issues to increase the construction of knowledge on the theme.

ABSTRACT Objective: to develop a playful-educational technology for learning Nursing Semiology and Semiotics, as well as to carry out the evaluation of its playability before and after implementing adjustments with undergraduate Nursing students. Method: a methodological study that followed the planning, prototyping and production stages corresponding to the first playable version for development. For the playability evaluation, 62 students were divided into two groups: one for to evaluate the preliminary version and the other for after implementing adjustments; and, for this, the Friedman test was used, which allows detecting differences between medians of more than two paired samples, considering p-value below 5% as statistical significance. Results: the game had three sequential phases: the first addressed vital signs; the second, biosafety measures; and the third, the physical examination. Referring to the playability evaluation, it was possible to observe that the "Concentration" and "Feedback" categories and the general median presented better evaluation scores after implementing adjustments. Conclusion: the current study sought to develop a technology that would allow students attending Bachelor's degree courses in Nursing to sharpen their knowledge in Nursing Semiology and Semiotics. In addition to that, it was sought to evaluate the playability of Semio em Jogo ®; thus, it was observed that the academic community considered the game satisfactory after the modifications as much as before implementing them.


RESUMEN Objetivos: desarrollar una tecnología lúdico-educativa para la enseñanza de Semiología y Semiotécnica en Enfermería; y evaluar la facilidad de juego de una tecnología lúdico-educativa antes y después de implementar ajustes con estudiantes de la carrera de grado en Enfermería. Métodos: estudio metodológico que siguió las etapas de planificación, diseño del prototipo y producción de la primera versión apta para jugar para el desarrollo. A fin de evaluar la facilidad de juego se recurrió a 62 estudiantes divididos en dos grupos: uno para evaluar la versión preliminar y otro para luego de implementar los ajustes; y para ello se utilizó la prueba de Friedman, que permite detectar diferencias entre medianas de más de dos muestras emparejadas, considerando valores de p inferiores al 5% como significancia estadística. Resultados: el juego estuvo compuesto por tres fases secuenciales: en la primera se abordaron los signos vitales; en la segunda, las medidas de bioseguridad; y en la tercera, el examen físico. En cuanto a la evaluación de la facilidad de juego, se pudo observar que las categorías "Concentración" y "Feedback" y la mediana general presentaron una mejor evaluación después de implementar ajustes. Conclusión: el presente estudio pretendió desarrollar una tecnología que permitiese que los alumnos de la Licenciatura en Enfermería agudicen sus conocimientos de Semiología y Semiotécnica en Enfermería. También se buscó evaluar la facilidad de juego de Semio em Jogo ®; de esta manera, se observó que la comunidad académica consideró que el juego alcanzó niveles satisfactorios tanto antes como después de las modificaciones.

Método: estudo metodológico, seguindo as etapas de planejamento, prototipagem e produção da primeira versão jogável para o desenvolvimento. Para a avaliação da jogabilidade, contou-se com 62 estudantes divididos em dois grupos, um para a avaliação da versão preliminar e outro para após a implementação de ajustes, e, para tal, utilizou-se o teste de Friedman que permite detectar diferenças entre medianas de mais de duas amostras emparelhadas, considerando como significância estatística valores de p-valor menor que 5%. Resultados: o jogo contou com três fases sequenciais: na primeira foram abordados os sinais vitais; na segunda, as medidas de biossegurança; e na terceira, o exame físico. Referente à avaliação da jogabilidade, foi possível observar que as categorias "concentração", "feedback" e mediana geral apresentaram melhor avaliação após a implementação de ajustes. Conclusão: o presente estudo buscou desenvolver uma tecnologia que possibilitasse ao aluno de cursos de bacharel em enfermagem aguçar seus conhecimentos em semiologia e semiotécnica em enfermagem. Além disso, buscou-se avaliar a jogabilidade do Semio em Jogo®, assim, observou-se que a comunidade acadêmica considerou o jogo como satisfatório após as modificações tanto quanto antes delas.

Resumo Objetivo estimar a ocorrência de ansiedade em pessoas idosas e sua associação com os diferentes padrões de incapacidade funcional em um município de pequeno porte do estado da Paraíba, Brasil. Método estudo transversal e analítico realizado com 233 idosos vinculados à Estratégia Saúde da Família e aleatoriamente selecionados. Os dados foram coletados por meio dos instrumentos World Health Organization Disability Assessment Schedule 2.0 e o Inventário de Ansiedade Geriátrica, cuja análise foi subsidiada pela estatística descritiva e bivariada, considerando significância quando p<0,05. Resultados 48,1% dos idosos apresentaram algum nível de ansiedade autorreferida, com níveis significativamente maiores entre as mulheres (média dos postos = 128,11; p = 0,002). Também se constatou associação entre o padrão de ansiedade grave com o padrão de incapacidade grave (p < 0,001). Conclusão a elevada ocorrência de diferentes graus de ansiedade geriátrica e sua associação com a incapacidade funcional grave sinaliza a coexistência entre alterações psicoemocionais e motoras, sugerindo a necessidade de romper a cadeia do subdiagnóstico e fortalecer a implementação de intervenções especializadas no campo da gerontogeriatria.


Abstract Objective to estimate the prevalence of anxiety in older adults and its association with different levels of functional disability in a small town in the state of Paraiba, Brazil. Method a cross-sectional analytical study was carried out of 233 randomly-selected older users of the Family Health Strategy program. Data were collected using the World Health Organization Disability Assessment Schedule 2.0 and the Geriatric Anxiety Inventory. Results were analyzed using descriptive and bivariate statistics adopting a significance level of p < 0.05. Results Overall, 48.1% of the participants had some degree of selfreported anxiety, with significantly higher levels among women (mean rank = 128.11; p = 0.002). An association was also found between severe anxiety level and severe disability level (p < 0.001). Conclusion the high prevalence of different degrees of geriatric anxiety and its association with severe functional disability indicates the coexistence of psychoemotional and motor alterations. These findings suggest the need to break the chain of underdiagnosis and strengthen the implementation of specialized interventions in the field of gerontology and geriatrics.

Studies reported post-COVID-19 fatigue in the general population, but not among pregnant women. Our objectives were to determine prevalence, duration, and risk factors of post-viral fatigue among pregnant women with SARS-CoV-2. This study involved 588 pregnant women with SARS-CoV-2 during pregnancy or delivery in Brazil. Three groups were investigated: G1 (n = 259, symptomatic infection during pregnancy); G2 (n = 131, positive serology at delivery); G3 (n = 198, negative serology at delivery). We applied questionnaires investigating fatigue at determined timepoints after infection for G1, and after delivery for all groups; fatigue prevalence was then determined. Cox regression was used to estimate hazard ratio (HR) and 95% CI of the risk of remaining with fatigue in G1. Overall fatigue prevalence in G1 at six weeks, three months and six months were 40.6%, 33.6%, and 27.8%, respectively. Cumulative risk of remaining with fatigue increased over time, with HR of 1.69 (95% CI: 0.89-3.20) and 2.43 (95% CI: 1.49-3.95) for women with moderate and severe symptoms, respectively. Multivariate analysis showed cough and myalgia as independent risk factors in G1. Fatigue prevalence was significantly higher in G1 compared to G2 and G3. Post-viral fatigue prevalence is higher in women infected during pregnancy; fatigue's risk and duration increased with the severity of infection.

Abstract Introduction: Inventories are essential for forest management, but, in the Amazon region, the absence of standardization produces information loss, low accuracy, and inconsistent measurements. This prevents valid comparisons and compromises the use of information in networks and software. Sampling unit size is of key importance in the inventory of native forests, particularly regarding accuracy and costs. Objective: To identify a plot size that provides adequate precision for dendrometric parameters in the Amazon. Methods: In Cotriguaçu, Mato Grosso, Brazil, we tested four plot sizes with six repetitions each: 2 500, 5 000, 7 500, and 10 000 m². We measured diameter at breast height, population density, basal area, and biomass. We applied Shannon and Jaccard indexes; Weibull 2P and Gamma functions to fit the diametric distribution; and the Akaike Information Criterion for the best model. Results: There was a directly proportional relationship between plot area and population similarity, but diversity did not indicate significant alterations. Plot size did not affect dendrometric attributes and diametric distribution. Larger plot areas led to lower coefficients of variation and smaller confidence intervals. The Gamma function was the best model to represent the distributions of different plot sizes. Conclusions: For similar forests, we recommend the 2 500 m² plot to evaluate diameter at breast height, population density, basal area, and biomass.


Resumen Introducción: Los inventarios son fundamentales para la gestión forestal, pero en la Amazonía la ausencia de estandarización produce pérdida de información, baja precisión y mediciones inconsistentes. Esto impide comparaciones válidas y compromete el uso de información en redes y programas. El tamaño de la unidad de muestreo es de importancia clave en el inventario de bosques nativos, particularmente en lo que respecta a la precisión y los costos. Objetivo: Identificar un tamaño de parcela que proporcione una precisión adecuada para los parámetros dendrométricos en la Amazonía. Métodos: En Cotriguaçu, Mato Grosso, Brasil, probamos cuatro tamaños de parcela con seis repeticiones cada una: 2 500, 5 000, 7 500 y 10 000 m². Medimos diámetro a la altura del pecho, densidad de población, área basal y biomasa. Se aplicaron los índices de Shannon y Jaccard; Funciones Weibull 2P y Gamma para adaptarse a la distribución diametral; y el Criterio de Información de Akaike para el mejor modelo. Resultados: Hubo una relación directamente proporcional entre el área de parcela y la similitud poblacional, pero la diversidad no indicó alteraciones significativas. El tamaño de la parcela no afectó los atributos dendrológicos y la distribución diametral. Las áreas de parcela más grandes dieron lugar a coeficientes de variación más bajos e intervalos de confianza más pequeños. La función Gamma fue el mejor modelo para representar las distribuciones de diferentes tamaños de parcela. Conclusiones: Para bosques similares, recomendamos la parcela de 2 500 m² para evaluar diámetro a la altura del pecho, densidad de población, área basal y biomasa.

Digestive system cancers account for nearly half of all cancers around the world and have a high mortality rate. Cell culture and animal models represent cornerstones of digestive cancer research. However, their ability to enable cancer precision medicine is limited. Cell culture models cannot retain the genetic and phenotypic heterogeneity of tumors and lack tumor microenvironment (TME). Patient-derived xenograft mouse models are not suitable for immune-oncology research. While humanized mouse models are time- and cost-consuming. Suitable preclinical models, which can facilitate the understanding of mechanisms of tumor progression and develop new therapeutic strategies, are in high demand. This review article summarizes the recent progress on the establishment of TME by using tumor organoid models and microfluidic systems. The main challenges regarding the translation of organoid models from bench to bedside are discussed. The integration of organoids and a microfluidic platform is the emerging trend in drug screening and precision medicine. A future prospective on this field is also provided.

A articulação temporomandibular está vunerável a várias condições de anormalidades já bastante conhecidas, dentre elas, o deslocamento do disco articular sem redução, considerado pela literatura o mais comum das patologias desta região. A deterioração do quadro pode ser um indicativo da necessidade cirúrgica. Em específico, no deslocamento de disco sem redução, o reposicionamento definitivo pode ser adquirido através de discopexia e artrocentese. O presente trabalho tem como objetivo relatar um caso clínico de deslocamento de disco articular bilateral sem redução, apresentando a importância da fisioterapia associada à abordagem cirúrgia na recuperação da função mandibular, bem como, reforçar a importância de uma abordagem conjunta entre análise clínica e imaginológica para resolução de casos. Paciente feminino cursando com diversos sintomas em face e pescoço de forma progressiva ao longo de dois anos foi submetida a cirurgia na articulação temporo-mandibular, onde foi feita a discopexia através de ancoragem dos discos articulares com parafusos. A avaliação física, assim como, a imaginológica, ajudaram na confirmação do diagnóstico. A abordagem cirurgica adotada no tratamento deste caso foi descrita na literatura desde o final do século XIX. Conclui-se que, o método utilizado no tratamento do deslocamento de disco sem redução deve ser baseado nos sinais e sintomas do paciente, cuja abordagem deve ser readequada de acordo com as mudanças do quadro apresentado. Nesta análise, a abordagem cirúrgica associada à fisioterapia específica para as estruturas musculoesquelética da face trouxeram resultados positivos(AU)


The temporomandibular joint is responsible for several well-known conditions of abnormalities, among them, the joint disc displacement without reduction, considered by the literature the most common pathology of this region. Deterioration of the condition may be indicative of surgical need. Specifically, in displacement disc without reduction, definitive repositioning can be achieved through discopexy and arthrocentesis. The aim of the present study is to report a case of unilateral articular disc displacement without reduction, as well the importance of physiotherapy associated with the surgical approach in the recovery of mandibular function, as well as reinforcing the importance of a joint approach between clinical and imaging analysis for case resolution. Female patient with several symptoms in the face and neck progressively over two years, underwent surgery in the temporomandibular joint, where discopexy was performed by anchoring the articular discs with screws. The physical assessment, as well as the imaging, helped to confirm the diagnosis. The surgical approach adopted in the treatment of this case has been described in the literature since the end of the 19th century. In conclusion, the method used to treat articular disc displacement without reduction should be based on the patient's signs and symptoms, whose approach should be readjusted according to the changes in the presented picture.
