The growth and development of green lettuce plants can be modulated by the prevailing light conditions around them. The aim of this study was to evaluate the effect of ambient light enrichment with different LED light spectra on agronomic characteristics, polyphenol concentration and relative gene expression of enzymes associated with polyphenol formation in 'Levistro' lettuce grown hydroponically in a Nutrient Film Technique (NFT) system for 28 days in a greenhouse. The spectra (blue:green:red:far-red) and red:blue (R:B) ratios obtained by enriching ambient light with Blue (B), White (W), Blue-Red (BR) and Red (R) LED light were B: 47:22:21:10, 0.5:1; W: 30:38:23:9, 0.8:1; BR: 33:15:44:8, 1.3:1 and R: 16:16:60:8, 3.8:1, respectively, and photosynthetically active radiation (PAR) under the different treatments, measured at midday, ranged from 328 to 336 µmoles m-2 s-1. The resulting daily light integral (DLI) was between 9.1 and 9.6 mol m-2 day-1. The photoperiod for all enrichment treatments was 12 h of light. The control was ambient greenhouse light (25:30:30:15; R:B = 1.2:1; PAR = 702 µmoles m-2 s-1; DLI = 16.9 mol m-2 day-1; photoperiod = 14.2 h of light). Fresh weight (FW) and dried weight percentage (DWP) were similar among the enrichment treatments and the control. The leaf number increased significantly under BR and R compared to B lights. The relative index of chlorophyll concentration (RIC) increased as plants grew and was similar among the enrichment treatments and the control. On the other hand, the concentration of chlorogenic acid and chicoric acid increased under BR and B lights, which was consistent with the higher relative expression of the coumarate 3-hydroxylase enzyme gene. In view of the results, it is inferred that half of the PAR or DLI is sufficient to achieve normal growth and development of 'Levistro' lettuce plants, suggesting a more efficient use of light energy under the light enrichment treatments. On the other hand, the blue and combined blue-red lights promoted the accumulation of phenolic compounds in the leaves of 'Levistro' lettuce plants.

WRN helicase is a promising target for treatment of cancers with microsatellite instability (MSI) due to its essential role in resolving deleterious non-canonical DNA structures that accumulate in cells with faulty mismatch repair mechanisms1-5. Currently there are no approved drugs directly targeting human DNA or RNA helicases, in part owing to the challenging nature of developing potent and selective compounds to this class of proteins. Here we describe the chemoproteomics-enabled discovery of a clinical-stage, covalent allosteric inhibitor of WRN, VVD-133214. This compound selectively engages a cysteine (C727) located in a region of the helicase domain subject to interdomain movement during DNA unwinding. VVD-133214 binds WRN protein cooperatively with nucleotide and stabilizes compact conformations lacking the dynamic flexibility necessary for proper helicase function, resulting in widespread double-stranded DNA breaks, nuclear swelling and cell death in MSI-high (MSI-H), but not in microsatellite-stable, cells. The compound was well tolerated in mice and led to robust tumour regression in multiple MSI-H colorectal cancer cell lines and patient-derived xenograft models. Our work shows an allosteric approach for inhibition of WRN function that circumvents competition from an endogenous ATP cofactor in cancer cells, and designates VVD-133214 as a promising drug candidate for patients with MSI-H cancers.

Introduction: Oral transmission of T. cruzi is probably the most frequent transmission mechanism in wild animals. This observation led to the hypothesis that consuming raw or undercooked meat from animals infected with T. cruzi may be responsible for transmitting the infection. Therefore, the general objective of this study was to investigate host-pathogen interactions between the parasite and gastric mucosa and the role of meat consumption from infected animals in the oral transmission of T. cruzi. Methods: Cell infectivity assays were performed on AGS cells in the presence or absence of mucin, and the roles of pepsin and acidic pH were determined. Moreover, groups of five female Balb/c mice were fed with muscle tissue obtained from mice in the acute phase of infection by the clone H510 C8C3hvir of T. cruzi, and the infection of the fed mice was monitored by a parasitemia curve. Similarly, we assessed the infective capacity of T. cruzi trypomastigotes and amastigotes by infecting groups of five mice Balb/c females, which were infected orally using a nasogastric probe, and the infection was monitored by a parasitemia curve. Finally, different trypomastigote and amastigote inoculums were used to determine their infective capacities. Adhesion assays of T. cruzi proteins to AGS stomach cells were performed, and the adhered proteins were detected by western blotting using monoclonal or polyclonal antibodies and by LC-MS/MS and bioinformatics analysis. Results: Trypomastigote migration in the presence of mucin was reduced by approximately 30%, whereas in the presence of mucin and pepsin at pH 3.5, only a small proportion of parasites were able to migrate (∼6%). Similarly, the ability of TCTs to infect AGS cells in the presence of mucin is reduced by approximately 20%. In all cases, 60-100% of the animals were fed meat from mice infected in the acute phase or infected with trypomastigotes or amastigotes developed high parasitemia, and 80% died around day 40 post-infection. The adhesion assay showed that cruzipain is a molecule of trypomastigotes and amastigotes that binds to AGS cells. LC-MS/MS and bioinformatics analysis, also confirmed that transialidase, cysteine proteinases, and gp63 may be involved in TCTs attachment or invasion of human stomach cells because they can potentially interact with different proteins in the human stomach mucosa. In addition, several human gastric mucins have cysteine protease cleavage sites. Discussion: Then, under our experimental conditions, consuming meat from infected animals in the acute phase allows the T. cruzi infection. Similarly, trypomastigotes and amastigotes could infect mice when administered orally, whereas cysteinyl proteinases and trans-sialidase appear to be relevant molecules in this infective process.

Resumen Introducción: La musculatura del core involucra a todos los grupos musculares ubicados en la zona media del cuerpo. La metodología clínica de elección para el registro y análisis de su actividad bioeléctrica es la electromiografía superficial (sEMG), metodología clínica para el registro y posterior análisis de las actividades bioeléctricas del músculo esquelético. Objetivo: El objetivo de esta investigación es determinar si existen diferencias de actividad electromiográfica en la musculatura del recto abdominal y oblicuo externo entre practicantes de musculación y practicantes de calistenia. Metodología: El estudio es de tipo comparativo, de diseño no experimental, transversal de enfoque cuantitativo en una muestra de 20 sujetos; 10 practicantes de calistenia y 10 que entrenan con ejercicios de musculación clásicos. Resultados: Los ejercicios que tuvieron mayor respuesta electromiográfica fueron abdominal en suspensión y abdominal inclinado. Conclusiones: La calistenia es una alternativa viable para la activación muscular a nivel del Core.


Abstract Introduction: The Core musculature involves all the muscle groups located in the middle area of the body whose activity can be measured using superficial electromyography (sEMG), a clinical methodology for recording and subsequent analysis of skeletal muscle bioelectric activities. Objectives: The objective of this research is to determine if there are differences in electromyographic activity in the muscles of the rectus abdominis and external oblique between bodybuilding practitioners and calisthenics practitioners. Methodology: A comparative study, non-experimental design, cross-sectional with a quantitative approach in a sample of 20 subjects; 10 calisthenics practitioners and 10 who train with classic bodybuilding exercises. Results: The exercises with the greatest electromyographic response were abdominal in suspension and abdominal incline. Conclusions: Calisthenics is a viable alternative for muscle activation at the Core level.

NiResumen Objetivo: Evaluar los resultados de la retinaculotomía endoscópica para tratar el síndrome del túnel carpiano mediante la técnica de doble portal de Chow, entre enero de 2006 y diciembre de 2015. Materiales y Métodos: Estudio de 179 pacientes (edad promedio 48.2 años [rango 32-68]), con 217 casos de síndrome del túnel carpiano idiopático y un seguimiento promedio de 97.9 meses. Los pacientes eran 145 mujeres (81%) (31 bilaterales) y 34 hombres (19%) (7 bilaterales) y fueron evaluados con la Symptom Severity Scale (SSS) y la Functional Status Scale (FSS) del Boston Carpal Tunnel Questionnaire (BCTQ). Resultados: El puntaje medio de la SSS-BCTQ fue de 3,20 + 0,26 antes de la cirugía, mejoró a 1,30 + 0,12 a los 6 meses y se mantuvo en 1,25 + 0,11 a largo plazo. El puntaje medio de la FSS-BCTQ fue de 2,57 + 0,29 antes de la cirugía, mejoró a 1,28 + 0,18 a los 6 meses y se mantuvo en 1,20 + 0,09 a largo plazo. Hubo 7 casos (3,2%) de neuropraxia posquirúrgica transitoria. No hubo conversiones a técnica abierta. Conclusión: La liberación endoscópica del túnel carpiano con la técnica de Chow es un método quirúrgico eficaz y seguro para tratar el síndrome del túnel carpiano idiopático. Objective: To evaluate the outcomes of endoscopic release of the transverse carpal ligament (TCL) in carpal tunnel syndrome (CTS) using the Chow dual-portal technique between January 2006 and December 2015. Materials and Methods: Study population consisted of 217 cases of idiopathic CTS, in 179 patients, 145 females (81%) (31 bilateral cases) and 34 males (19%) (7 bilateral cases), with an average age of 48.2 years (range, 32-68) and an average follow-up of 97.9 months. The symptom severity and functional evaluations were performed using the Boston Carpal Tunnel Questionnaire Symptoms Severity Scale (BCTQ-SSS) and the Functional Status Scale (BCTQ-FSS). Results: The average BCTQ-SSS was 3.20±0.26 in the preoperative period, which improved to 1.30±0.12 at the 6-month postoperative follow-up and remained at 1.25±0.11 in the long-term. The average BCTQ-FSS was 2.57±0.29 in the preoperative period, which improved to 1.28±0.12 at the 6-month postoperative follow-up and remained at 1.20±0.09 in the long-term. There were 7 cases (3.2%) of transient postoperative neurapraxia. No patient required to be converted to open technique. Conclusion: The endoscopic carpal tunnel release with Chow technique is an effective and safe surgical method for the treatment of idiopathic CTS. Level of Evidence; III

El síndrome de dolor crónico postmastectomía es frecuente en pacientes sometidas a cirugía por cáncer de mamas, llegando a ser invalidante en casos severos. Existen distintos factores de riesgo para que éste se presente, siendo el dolor agudo postoperatorio el único en el cual podemos intervenir. La importancia de implementar estrategias de prevención en el periodo perioperatorio es fundamental. En la mantención de la anestesia general, la utilización de propofol se asocia con menor incidencia de dolor agudo postoperatorio. El uso de anestésicos locales, ya sea por vía tópica, subcutánea y sistémica, ha demostrado su beneficio para disminuir el dolor agudo. No obstante, solo el uso tópico y endovenoso ha sido útil para reducir el dolor hasta 3 meses postcirugía. Técnicas de analgesia regional, como el bloqueo PEC I y II, bloqueo paravertebral y bloqueo del erector de la espina han demostrado ser útiles para reducir el dolor agudo; pero solo el bloqueo paravertebral ha demostrado su utilidad en reducir el dolor hasta 1 año postcirugía. El uso de gabapentina, pregabalina y ketamina desde el periodo preoperatorio disminuyen la incidencia de dolor agudo postoperatorio. En tanto, la venlafaxina y dexmedetomidina demostraron su utilidad para reducir el dolor crónico.


Postmastectomy chronic pain syndrome is common in patients undergoing surgery for breast cancer, becoming invalidating in severe cases. There are different risk factors for this to occur, with acute post-operative pain being the only one in which we can intervene. The importance of implementing prevention strategies in the perioperative period is essential. In the maintenance of general anesthesia, the use of propofol is associated with a lower incidence of acute post-operative pain. The use of local anesthetics, whether topically, subcutaneously and systemically, has shown its benefit in reducing acute pain. However, only topical and endovenous use has been useful to reduce pain up to 3 months after surgery. Regional analgesia techniques, such as PEC I and II block, paravertebral block, and spinal erector block have been shown to reduce acute pain; but only the paravertebral block has shown its usefulness in reducing pain up to 1 year after surgery. The use of gabapentin, pregabalin and ketamine from the preoperative period reduce the incidence of acute postoperative pain. Meanwhile, venlafaxine and dexmedetomidine proved useful in reducing chronic pain.

El dolor crónico postoperatorio de cirugía de columna lumbar tiene una incidencia entre un 10-50 por ciento de los pacientes postoperados, siendo de difícil manejo. Se ha denominado síndrome de fracaso de cirugía espinal lumbar (FBSS) a aquellos pacientes que han sido beneficiarios de una cirugía del raquis lumbosacro y en quienes no se logra una mejoría de manera satisfactoria, presentando dolor crónico lumbar, siendo su incidencia de alrededor de un 20 por ciento, y debido a una mala evolución del proceso de recuperación que puede ocasionar fibrosis en el canal medular. Existen diversas alternativas terapéuticas para afrontar este problema, donde destacan el tratamiento farmacológico, medicina física y el abordaje intervencionista no quirúrgico y quirúrgico implementados últimamente, con el fin de disminuir de manera efectiva el dolor en este grupo de pacientes. Sin embargo, a pesar de las terapias combinadas, existe un grupo de pacientes que debe ser sometido a una nueva intervención quirúrgica con resultados poco promisorios.


Chronic postoperative pain of lumbar spine surgery has an incidence between 10-50 percent of postoperative patients, being difficult to manage. The Failed Back Surgery Syndrome (FBSS) has been referred to those patients who have been beneficiaries of lumbosacral spinal surgery and who do not achieve a satisfactory improvement, presenting chronic lumbar pain with an incidence around 20 percent and due to a poor evolution of the recovery process that can cause fibrosis in the medullary canal. There are several therapeutic alternatives to address this problem, which highlights the pharmacological treatment, physical medicine and the non-surgical and surgical intervention approach recently implemented, in order to effectively reduce pain in this group of patients. However, despite the combination therapies, there is a group of patients that must undergo a new surgical intervention with little promising results.

BACKGROUND: Propofol and remifentanil are commonly combined during total intravenous anesthesia. The impact of remifentanil in this relationship is poorly quantified in children. Derivation of an integrated pharmacokinetic and pharmacodynamic propofol model, containing remifentanil pharmacodynamic interaction information, enables propofol effect-site target-controlled infusion in children with a better prediction of its hypnotic effect when both drugs are combined. AIMS: We designed this study to derive an integrated propofol pharmacokinetic-pharmacodynamic model in children and to describe the pharmacodynamic interaction between propofol and remifentanil on the electroencephalographic bispectral index effect. METHODS: Thirty children (mean age: 5.45 years, range 1.3-11.9; mean weight: 23.5 kg, range 8.5-61) scheduled for elective surgery with general anesthesia were studied. After sevoflurane induction, maintenance of anesthesia was based on propofol and remifentanil. Blood samples to measure propofol concentration were collected during anesthesia maintenance and up to 6 hours in the postoperative period. Bispectral index data were continuously recorded throughout the study. A pharmacokinetic-pharmacodynamic model was developed using population modeling. The Greco model was used to examine the pharmacokinetic-pharmacodynamic interaction between propofol and remifentanil for BIS response RESULTS: Propofol pharmacokinetic data from a previous study in 53 children were pooled with current data and simultaneously analyzed. Propofol pharmacokinetics were adequately described by a three-compartment distribution model with first-order elimination. Theory-based allometric relationships based on TBW improved the model fit. The Greco model supported an additive interaction between propofol and remifentanil. Remifentanil showed only a minor effect in BIS response. CONCLUSION: We have developed an integrated propofol pharmacokinetic-pharmacodynamic model that can describe the pharmacodynamic interaction between propofol and remifentanil for BIS response. An additive interaction was supported by our modeling analysis.

INTRODUCCIÓN: la anestesia espinal unilateral (AEU) es una alternativa para el desarrollo de la cirugía ortopédica ambulatoria (COA). el OBJETIVO de este ensayo clínico fue el evaluar una dosis baja de bupivacaína hiperbara (BHB), asociada a fentanilo intratecal (FI) para obtención de AEU en COA. MATERIALES Y MÉTODOS: ensayo clínico, prospectivo, controlado en pacientes ASA I a II. la AEU se efectuó con el paciente en decúbito lateral (DL), con el lado operatorio hacia abajo, a nivel de L3-L4. se inyectó 5 mg de BHB 0,75% asociado a 20 µg de FI. se evaluó el bloqueo sensitivo (BS) y motor (BM) cada 3 minutos, los primeros 20 minutos y luego cada 15 minutos. Se midió el nivel máximo alcanzado, el tiempo de regresión a T12 y el tiempo de regresión de dos segmentos. se registró la incidencia de complicaciones. RESULTADOS: 50 pacientes fueron incluidos, 66% de sexo masculino. Se obtuvo anestesia unilateral en el 84% y BM total unilateral en el 86%. La altura del BS osciló entre T9 y T11. la duración del BS fue 116 min. (SD± 23). la regresión a T12 fue de 47 min (SD±19). No se encontró correlación entre talla, dosis y duración de bloqueo. ningún paciente necesitó anestesia general. 1 paciente presentó bradicardia. DISCUSIÓN: la AEU con dosis bajas de BHB asociado a FI es eficiente en COA. se obtuvo adecuados BS y BM en la mayoría de los pacientes. la AEU mostró ser confiable, permitiendo una alta precoz y segura.


INTRODUCTION: The unilateral spinal anesthesia (USA) is an alternative for orthopedic ambulatory surgery (OAS). The AIM of this study was to evaluate a low dose of hyperbaric bupivacaine (HB) associated with intrathecal fentanyl (ITF) to produce USA. MATERIALS AND METHODS: Prospective, controlled clinical trial in ASA I ­II patients. The USA was developed in lateral position with the operating side downwards between L3-L4 space with 5 mg of HB + 20 µg ITF. We evaluated sensitive level block (SB), motor level block (MB), maximum level obtained, discharge time (DT) and collateral effects incidence. RESULTS: 50 patients were included, 66% male. The USA was obtained in 84% and total BM was present in 86% of cases. The level obtained of SB was between T9-T11 and the duration was 116 min (SD± 23). No correlation was obtained between patients height, BH dose and block duration. 1 patient presented bradycardia. No general anesthesia was necessary. DISCUSSION: The USA with a low dose of HB and ITF it's safe to developed OAS. Only 11 min were necessary to obtain a good SB and MB.

BACKGROUND: The aim of this paper is to demonstrate the clinical and radiological results of non-fusion in patients with degenerative disc disease and lumbar by placing static interspinous spacer. METHODS: A retrospective study was made in the period between January 2010 and January 2013, 130 patients were treated by placing static interspinous spacer. Patients were divided into: Group 1 (treated for diagnosis of degenerative lumbar spinal stenosis), and group 2 (those diagnosed with lumbar disc herniation). It is improving pain by visual analog scale (VAS), functional improvement based on the Oswestry scale (ODI) and radiological outcome at 6,12 and 24 months was evaluated. RESULTS: 57 female patients and 73 male were included. The most affected segments were L4 - L5 and L5 - S1. In Group 1, there was improvement in pain (VAS) at 6 months to 2 points which was maintained through 12 months and 24 months was found in 4 points. Functional improvement (ODI) at 6 and 12 months remained 13 points and 24 points and 21 months in group 2: VAS 2.7, 3, 3.5 and ODI 15 respectively at 6 and 12 months; at 24 months was found in 20 points. There were no fatal complications in 1.5% in both groups. Regarding radiological changes increased the diameter foraminal which was held at the 6, 12 and 24 month follow-up. CONCLUSIONS: The interspinous device is an effective alternative treatment for lumbar degenerative disc disease and because it was associated with significantly improved lumbar pain and return to work activity.


Introducción: el objetivo de este trabajo es demostrar los resultados clínicos y radiológicos de la no fusión en los pacientes con enfermedad degenerativa lumbar y discal mediante la colocación de espaciador interespinoso estático. Métodos: estudio retrospectivo, de enero de 2010 a enero de 2013, con 130 pacientes tratados mediante colocación de espaciador interespinoso estático. Los pacientes se dividieron en: grupo 1 (tratados por diagnóstico de canal lumbar estrecho degenerativo) y grupo 2 (pacientes con diagnóstico de hernia discal lumbar). Se evaluó la mejoría de dolor mediante escala visual análoga (VAS), mejoría funcional con base en la escala de Oswestry (ODI) y resultado radiológico a los 6, 12 y 24 meses. Resultados: se incluyeron 57 pacientes del sexo femenino y 73 masculinos. Los segmentos más afectados fueron L4 - L5 y L5 - S1. En el grupo 1 hubo mejoría del dolor a los 6 meses a 2 puntos, la cual se mantuvo hasta los 12 meses; mientras que a los 24 meses se encontró en 4 puntos. La mejoría funcional (ODI) a los 6 y 12 meses se mantuvo el 13 puntos y a los 24 meses en 21 puntos. Mientras que en el grupo 2: VAS de 2.7, 3, 3.5 respectivamente y ODI de 15 a los 6 y 12 meses; a los 24 meses se encontró en 20 puntos. Hubo complicaciones no mortales en 1.5% en ambos grupos. En cuanto a los cambios radiológicos, se demostró un aumento en el diámetro foraminal el cual se mantuvo a los 6, 12 y 24 meses de seguimiento. Conclusiones: el dispositivo interespinoso es una alternativa de tratamiento eficaz para la enfermedad degenerativa lumbar y la enfermedad discal, ya que se asoció con mejora significativa de dolor lumbar y reincorporación a actividad laboral.

ABSTRACT Objective: To present the results of treatment of patients with cervical discopathy by anterior cervical approach, discectomy and placement of a PEEK interbody cage without anterior plate fixation. Methods: Retrospective, cross-sectional study from March 2013 to March 2015. Sixteen patients with radiculopathy or clinical signs of myelopathy were included; all patients underwent cervical surgery through anterior approach, discectomy, and placement of PEEK cages on three levels. Decompression levels were determined according to the correlation between preoperative radiological and clinical findings. Results: Sixteen patients predominantly male were included, with mean age of 50 years at the onset of the condition. Ten patients had involvement of C4-C5, C5-C6, C6-C7 levels, and six patients C3-C4, C4-C5 and C5-C6. Fourteen patients had cervicobrachialgia and two myelopathy. The preoperative visual analog scale average was 8/10 and the average postoperative value at 6 months was 3/10. At 6 months, there was no radiological evidence of recurrence. One patient had non-fatal complications. Conclusions: The treatment of cervical discopathy by anterior approach with interbody fusion with PEEK cage on three levels, with no plate fixation seemed to be safe and effective with better long-term results in terms of pain and myelopathy. The clinical results compare favorably with other similar series and, most importantly, the complications associated with anterior fixation plate are avoided.


RESUMO Objetivo: Apresentar os resultados do tratamento de pacientes com discopatia cervical usando acesso cervical anterior, discotomia e colocação de cage intersomático em PEEK sem fixação com placa anterior. Métodos: Estudo retrospectivo, transversal no período de março de 2013 a março de 2015. Foram incluídos 16 pacientes que apresentavam radiculopatia ou sinais clínicos de mielopatia; os pacientes foram submetidos à cirurgia cervical por acesso anterior, discotomia e colocação de cages em PEEK em três níveis. Os níveis de descompressão foram determinados de acordo com a correlação entre os achados radiológicos e clínicos pré-operatórios. Resultados: Foram incluídos 16 pacientes predominantemente do sexo masculino, com média de idade de 50 anos à apresentação da afecção, sendo 10 com envolvimento dos níveis C4-C5, C5-C6 e C6-C7 e 6 pacientes com envolvimento dos níveis C3-C4, C5-C4 e C5-C6. Catorze pacientes tinham cervicobraquialgia e 2 mielopatia. A escala visual analógica pré-operatória média foi 8/10 e o valor médio pós-operatório aos 6 meses foi 3/10. Aos 6 meses, não se encontrou evidência radiológica de recidiva. Um paciente teve complicações não fatais. Conclusões: O tratamento de discopatia cervical por acesso anterior por fusão intersomática com cage em PEEK em três níveis, sem fixação com placa parece ser seguro e eficaz, com melhores resultados a longo prazo em termos de dor e mielopatia. Os resultados clínicos são favoráveis quando comparados com outras séries semelhantes e, o mais importante, o procedimento evitou as complicações associadas à fixação anterior com placa.


RESUMEN Objetivo: Dar a conocer los resultados del manejo del paciente con discopatía cervical utilizando abordaje cervical anterior, discectomía y colocación de caja intersomática en PEEK sin fijación con placa anterior. Método: Se realizó un estudio retrospectivo, transversal, durante el periodo de marzo de 2013 a marzo de 2015. Se incluyeron 16 pacientes que se presentaron con radiculopatía o signos clínicos de mielopatía; fueron sometidos a cirugía cervical por abordaje anterior, discectomía y colocación de cajas en PEEK en tres niveles. Los niveles de descompresión se determinaron de acuerdo a la correlación entre los hallazgos radiológicos y clínicos preoperatorios. Resultados: Dieciséis pacientes con predominio del sexo masculino, con edad media de 50 años al inicio de la enfermedad, con 10 pacientes con afección en los niveles C4-C5, C5-C6, C6-C7 y 6 en los niveles C3-C4, C4-C5 y C5-C6. Catorce tenían cervicobraquialgia y 2 mielopatía. El promedio de la escala visual análoga prequirúrgica fue 8/10 y el valor postoperatorio promedio a los 6 meses fue 3/10. A los 6 meses, no hubo evidencia radiológica de reincidencia. Un paciente tuvo complicaciones no fatales. Conclusiones: El manejo de discopatía cervical con acceso anterior mediante fusión intersomática con caja en PEEK en tres niveles sin fijación con placa parece ser segura y eficaz con una mejoría en los resultados a largo plazo tanto del dolor y la mielopatía. Los resultados clínicos se comparan favorablemente con otras series similares y, lo más importante, esta técnica evitó las complicaciones asociadas con la fijación anterior con placa.

ABSTRACT Objectives: Determine the most prevalent type of curve in our population, to quantify the radiographic parameters such as PT, IP, SS and compare the physical function according to ODI and SRS-22r. Methods: Retrospective, observational, longitudinal, single-center study, carried out from January 2010 to May 2015 at the Centro Médico ISSEMYM Ecatepec, Spine Surgery Service. Results: A total of 60 patients were obtained, 60% female, with curvatures according to SRS-Schwab, type T (28%), TL (46.6%), D (15%), N (10%), with a mean preoperative VAS of 7 for all curves and post-surgical 2 after 6 months. The SRS-22r preoperative test was 2.1 and postoperatively was 3.75, with p<0.001. Conclusions: Deformities of the adult spine are a growing disease in our country. The surgical management of deformities requires proper clinical and radiographic planning. Patients undergoing surgical treatment in our study showed curvature type TL and demonstrated significant improvement in ODI and SRS-22r.

Métodos: Estudo retrospectivo, observacional, longitudinal, em centro único, realizado no período de janeiro de 2010 a maio de 2015 no Centro Médico ISSEMYM Ecatepec, Serviço de Cirurgia da Coluna. Resultados: Reuniu-se um total de 60 pacientes, 60% do sexo feminino, com curvaturas, de acordo com o SRS-Schwab, tipo T (28%), TL (46,6%), D (15%), N (10%), com EVA média pré-operatória de 7 para todas as curvas e pós-cirúrgica de 2 depois de 6 meses. O teste SRS-22r pré-operatório foi de 2,1 e o pós-operatório foi de 3,75, com p < 0,001. Conclusões: As deformidades da coluna vertebral do adulto são uma doença crescente em nosso país. O tratamento cirúrgico das deformidades requer planejamento clínico-radiográfico adequado. Os pacientes submetidos ao tratamento cirúrgico em nosso estudo mostraram curvatura tipo TL e apresentaram melhora significativa de ODI e SRS-22r.

Métodos: Estudio retrospectivo, observacional, longitudinal y unicéntrico, realizado en el periodo entre Enero de 2010 y Mayo de 2015 en el Centro Médico ISSEMYM Ecatepec, Servicio de Cirugía de Columna. Resultados: Se obtuvieron un total de 60 pacientes, 60% mujeres, con curvaturas, de acuerdo a la clasificación SRS-Schwab, tipo T (28%), tipo TL (46,6%), tipo D (15%), tipo N (10%), con una EVA prequirúrgica promedio de 7 para todo tipo de curvas y posquirúrgica de 2 a los 6 meses. El test de SRS-22r prequirúrgico fue de 2,1 y el posquirúrgico de 3,75 con p < 0,001. Conclusiones: Las deformidades espinales del adulto son una patología creciente en nuestro país. El manejo quirúrgico de las deformidades requiere una adecuada planeación clínico-radiográfica. Los pacientes sometidos a tratamiento quirúrgico en nuestro estudio mostraron una curvatura tipo TL y presentaron mejoría significativa del ODI y del SRS-22r.

La epimastigogénesis de Trypanosoma cruzi es la transformación del tripomastigota sanguícola en epimastigota que ocurre en la porción anterior del tracto intestinal del insecto vector. Se ha reportado la activación de cisteín proteasas durante la epimastigogénesis in vitro en condiciones axénicas, sugiriendo la utilización de aminoácidos como fuente de carbono para la obtención de energía. El objetivo del presente trabajo fue estudiar a través de indicadores los cambios metabólicos producidos durante la diferenciación tripomastigota-epimastigota. Se partió de un 100% de tripomastigotas sobrenadantes de células Vero, incubados en medio LITB a 27ºC en condiciones de alta (epimastigogénesis horizontal) y baja (epimastigogénesis vertical) tensión de oxígeno. Se realizó contaje de parásitos (Cámara de Neubauer) y recuento diferencial (Giemsa) a diferentes tiempos de incubación (0, 1, 2, 4, 6, 8, 11 y 12 días). A cada tiempo se determinó en los sobrenadantes del medio de cultivo las concentraciones de glucosa y amoníaco, así como los valores de pH. Los resultados mostraron que: 1) Independientemente de la tensión de oxígeno se evidenció formas en diferenciación y amastigotas durante el proceso, siendo la aparición de epimastigotas más tardía en alta tensión de oxígeno; 2) El incremento poblacional fue significativamente mayor en condiciones de alta tensión de oxígeno, registrándose alta mortalidad de amastigotas durante los primeros días de incubación en la epimastigogénesis vertical; 3) El consumo de glucosa fue concomitante con la aparición de epimastigotas en ambas condiciones, siendo mayor en alta tensión de oxígeno; 4) No se registró liberación de amoníaco en ninguno de los casos estudiados; 5) Los valores de pH fueron similares hasta el sexto día en ambas condiciones, evidenciándose una acidificación gradual del medio de cultivo en alta tensión de oxígeno. Todo parece sugerir que durante la epimastigogénesis de T. cruzi, los parásitos involucrados en los procesos de diferenciación no muestran activación de las diferentes rutas metabólicas.


The epimastigogenesis of Trypanosoma cruzi is the transformation from bloodstream trypomastigote to epimastigote that occurs in the anterior portion of the intestinal tract of the insect vector. It has been reported cysteine proteases activation during axenic epimastigogenesis in vitro, suggesting the use of amino acids as a carbon source for the production of energy. The aim of this work was to study through indicators metabolic changes produced during the trypomastigote-epimastigote differentiation. We started with a 100% trypomastigotes derived from Vero cell supernatants, incubated in LITB medium at 27°C under high (horizontal epimastigogenesis) and low (vertical epimastigogenesis) oxygen tension. We estimated parasites number (Neubauer chamber) and differential morphology (Giemsa) at different incubation times (0, 1, 2, 4, 6, 8, 11 and 12 days). Each time was determined in the supernatants of the culture medium glucose and ammonia concentrations as well as the pH values. The results showed that: 1) Regardless of the oxygen tension it was evidenced differentiation forms and amastigote during the process, with the later occurrence of epimastigotes in high oxygen tension; 2) Population growth was significantly greater in high oxygen tension, registering high mortality of amastigotes during the firsts incubation days in the vertical epimastigogenesis; 3) Glucose consumption was concomitant with the appearance of epimastigotes in both conditions, being higher in high oxygen tension; 4) There was no ammonia release in any of the cases studied; 5) The pH values were similar to the sixth day in both conditions, showing gradual acidification of the culture medium at high oxygen tension. Everything seems to suggest that during the epimastigogenesis of T. cruzi parasites involved in differentiation processes show no activation of different metabolic pathways.

Se ha reportado cultivo axénico de amastigotas de Trypanosoma cruzi derivados de epimastigotas. Se cuestiona si formas redondas en cultivos axénicos son verdaderos amastigotas. Aquí se compararon los cambios biológicos y moleculares de epimastigotas creciendo en alta y baja tensión de oxígeno. T. cruzi (EPm6) se creció en frascos con diferentes alturas de medio LITB (condición horizontal 3 mm y vertical 83 mm). Se colectaron masas de parásitos de diferentes tiempos de crecimiento para análisis proteico, glicoproteico y antigénico. Se estudiaron los cambios morfológicos, incremento de inóculo y resistencia al complemento por microscopia de fases y tinción con Giemsa. Los cambios proteicos y glicoproteicos se compararon por SDS-PAGE. Los cambios antigénicos se analizaron por Western blot con suero de conejo y anticuerpos IgY contra formas amastigotas. En condición vertical, los epimastigotas mantuvieron su morfología típica a lo largo de la curva de crecimiento y no mostraron cambios moleculares significativos. En condición horizontal al 4to y 6to día predominaron formas redondas con capacidad de resistir la lisis por complemento. A partir del 4to día se observaron cambios en los perfiles proteicos y glicoproteicos, concomitantemente con los cambios morfológicos. El suero anti-amastigotas reveló un antígeno amastigota-específico transitorio de 55 kDa en parásitos de 4 días en la condición horizontal, mientras el anticuerpo IgY anti-amastigotas no reveló diferencias entre parásitos creciendo en ambas condiciones. Estas evidencias sugieren que epimastigotas de T. cruzi condicionados a sobrevivir en alta tensión de oxígeno puede mimetizar propiedades biológicas y antigénicas propias del estadio amastigota.


Axenic culture of epimastigote-derived amastigotes of Trypanosoma cruzi has been reported. Have been inquired why round forms appear in culture media and if are true amastigotes. Our proposal was study molecular and biological changes in epimastigotes growing in high and low oxygen tension. T. cruzi (EPm6) were grown in flasks with different height of LITB medium (horizontal condition 3 mm and vertical 83 mm). Parasites were collected masses of different times of growth for proteins, glycoproteins and antigenic analysis. Morphological changes, increase of inoculum and resistance to complement lyses were studied in phase’s microscopy and Giemsa-stained smears. Protein and glycoprotein changes were compared by SDS-PAGE. Antigenic changes were analyzed by Western blot with rabbit serum and IgY antibody against amastigote forms. In vertical condition, the epimastigote maintained its typical morphology along the growth curve and not showed significant molecular changes. In horizontal condition round forms prevailed at 4 and 6 days with capacity to resist lyses by complement. From the 4th day changes in proteins and glycoproteins profiles were observed, concomitantly with the morphological changes. Anti-amastigotes serum revealed a 55 kDa transitory amastigote-specific antigen in four day parasites’ from horizontal condition, while IgY antibody anti-amastigotes don’t revealed differences between parasites grown in both conditions. These evidences suggest that epimastigotes of T. cruzi conditioned to survive in high oxygen tension can mimic biological and antigenic properties own the amastigote-stage.

El medio ML15-HA permite simular in vitro los eventos morfogenéticos del ciclo vital de T. cruzi. Se ha evidenciado que los cambios morfológicos de la epimastigogénesis del clon Dm28c de T. cruzi en ML15-HA, ocurren más rápidamente que para otros aislados. Nuestro propósito fue investigar si los cambios macromoleculares ocurren concomitantemente con la rápida transformación morfológica. Tripomastigotas derivados de células infectadas se incubaron por diferentes tiempos en medio ML15-HA a 27ºC. Los cambios morfológicos se determinaron por microscopía de contraste de fases y coloración con Giemsa. Los perfiles peptídico y glicopeptídico se analizaron mediante SDS-PAGE y coloraciones específicas. Los cambios antigénicos se estudiaron mediante Western blot usando sueros anti-Tripomastigotas y anti-Epimastigotas. Los resultados muestran: (1) caída del porcentaje de tripomastigotas e incremento de amastigotas en las primeras 24 horas, con incremento gradual de epimastigotas a partir de 48 horas; (2) tres perfiles peptídicos y glicopeptídicos diferentes: tripomastigotas, epimastigotas a partir del día 4, y un perfil intermedio los días 2 y 3, demostrando la estrecha correlación entre los eventos morfológicos y los cambios proteicos y glicoproteicos; (3) los perfiles antigénicos revelaron estrecha relación entre los cambios morfológicos y la expresión de antígenos estadio-específicos. Los resultados obtenidos evidencian de manera concluyente que los cambios moleculares son más acelerados para el clon Dm28c en medio ML15-HA que en otros medios. Estos hallazgos abren la interrogante, sí la velocidad de la epimastigogénesis está determinada por la constitución genética del aislado o por la riqueza nutricional del medio de diferenciación.


The ML15-HA medium allows simulate in vitro morphogenetic events in the life cycle of T. cruzi. It has been demonstrated that the morphological changes of epimastigogenesis in ML15-HA of the T. cruzi clone Dm28c, occur faster than for other isolates. Our purpose was to investigate whether macromolecular changes occur concomitantly with rapid morphological transformation. Trypomastigotes derived from infected cells were incubated for different times in ML15-HA medium at 27°C. Morphological changes were determined by phase contrast microscopy and staining with Giemsa. Peptide and glycopeptide profiles were analyzed by SDS-PAGE and specific staining. Antigenic changes were studied by Western blot using serum anti-trypomastigotes and anti-epimastigotes. The results show: (1) decline in the percentage of trypomastigotes and amastigotes increased in the first 24 hours, epimastigotes gradually increases from 48 hours, (2) three peptide and glycopeptide profiles different: trypomastigotes, epimastigotes from the day 4 and an intermediate profile on days 2 and 3, showing the close correlation between morphological events and protein and glycoprotein changes, (3) antigenic profiles showed close relationship between the morphological changes and the expression of stage-specific antigens. The results show conclusively that the molecular changes are more rapid for clone Dm28c in ML15-HA medium than in other media. These findings open the question, if the epimastigogenesis speed is determined by the genetic constitution of isolated or nutritional wealth differentiation medium.

En los últimos años se ha reportado en Venezuela una nueva realidad de la enfermedad de Chagas, con múltiples brotes de transmisión oral y vectorial, con participación de triatominos considerados antiguamente como silvestres que han colonizado zonas urbanas próximas a vegetaciones. El Municipio San Diego del Estado Carabobo se ha caracterizado por un gran crecimiento demográfico con expansión habitacional hacia zonas boscosas, y en donde se ha reportado la captura de triatominos domiciliarios y peridomicilarios. En este estudio se evaluó los factores de riesgo y nivel de conocimiento de la enfermedad de Chagas, así como determinación de infección por Trypanosoma cruzi en el Sector A del mencionado Municipio. A través de pruebas serológicas, se obtuvo una seroprevalencia de 5,6% de un total de 90 individuos evaluados. Haciendo uso de un cuestionario, se encontró baja frecuencia de los factores de riesgo clásicos (condiciones de vivienda y presencia de chipos); no obstante, se obtuvo un porcentaje elevado de presencia de animales en el intra o peridomicilio (74%), así como de vegetación abundante próxima al hogar (82%). En cuanto al nivel de conocimiento, aproximadamente la mitad de los encuestados (46%) mostró un nivel intermedio, seguido de una considerable proporción de participantes con nociones bajas (34%). Los conocimientos más elevados correspondieron a la definición sobre la enfermedad de Chagas y sintomatología más frecuente (60%) e insecto vector (72%). Por otro lado, se obtuvo baja frecuencia en cuanto a los mecanismos de transmisión y reservorios del parásito. Los resultados sugieren la necesidad de ampliar los mecanismos de acción en la lucha contra la enfermedad de Chagas, promoviendo estrategias educativas que aumenten el conocimiento en la comunidad.


In recent years it has been Venezuela reported in Venezuela a new reality of Chagas disease, with multiple outbreaks of oral and vectorial transmission, involving formerly considered as wild triatomines that have colonized urban areas near vegetation. The San Diego County Carabobo state was characterized by a high population growth with residential expansion into forested areas, and where it has been reported domiciliary and peridomiciliary triatomine capture. In this study we evaluated the risk factors and level of knowledge of Chagas disease, as well to identify Trypanosoma cruzi infection in Sector A of the said municipality. Through serological tests yielded a seroprevalence of 5.6 % of a total of 90 individuals tested. Using a questionnaire, we found low frequency of traditional risk factors (housing conditions and presence of triatomines), however, we obtained a high percentage of animals in the presence of intra-or peri (74 %) and abundant vegetation close to home (82 %). Regarding the level of knowledge, about half of respondents (46 %) showed an intermediate level, followed by a considerable proportion of participants with low notions (34 %). Knowledge was highest in the definition of Chagas disease and most common symptom (60 %) and insect vector (72 %). On the other hand, it was obtained a low frequency transmission mechanisms and reservoirs of parasite. The results suggest the need to broaden the mechanisms of action in the fight against Chagas disease, promoting educational strategies that increase community awareness.

La metaciclogénesis de Trypanosoma cruzi es la transformación del estadio epimastigota en tripomastigota metacíciclico que ocurre en la ampolla rectal del insecto vector. Durante este proceso se ha implicado una activación del metabolismo proteico, con probable participación de proteasas en la degradación de proteínas del parásito. En el presente trabajo se determinó el efecto de las condiciones de mantenimiento de T. cruzi en el laboratorio sobre la actividad de proteasas y su relación sobre la metaciclogénesis espontánea en cultivos axénicos. Se trabajó con el clon Dm30L de Trypanosoma cruzi mantenido en el laboratorio mediante pases trimestrales chipo-ratón (condición triatomino: CT) o repiques semanales durante 15 años en medio LITB (condición cultivo: CC). En ambas condiciones, se hizo curva de crecimiento durante 30 días en medio LITB suplementado con 10, 24 y 48 mM glucosa, calculándose el porcentaje de metacíclicos espontáneos y determinación de la actividad de proteasas. En 10 mM glucosa, parásitos de la CT presentaron mayor expresión de proteasas y porcentaje de metacíclicos respecto a la CC. En la CT, el crecimiento en 48 mM glucosa redujo las variables antes mencionadas, mientras que no se evidenció actividad proteolítica ni metaciclogénesis espontánea en parásitos de la CC en altas concentraciones de glucosa. En todos los casos se evidenció mayor actividad de cisteín proteasas durante la fase estacionaria del cultivo. No se reportó actividades metaloproteasas en ninguna de las condiciones estudiadas, pero sí en el clon Dm28c utilizado como control de la expresión de este tipo de enzima. Estos resultados sugieren que la expresión de proteasas está influenciada por la constitución genética del parásito y que la CC provoca disminución de la actividad proteolítica que persisten aún en condiciones de agotamiento de carbohidratos del medio, con una concomitante reducción significativa en el porcentaje de metacíclicos espontáneos.


Trypanosoma cruzi metacyclogenesis is the transformation of epimastigote stage to trypomastigote metacyclic that happens in the rectal ampulla of the insect vector. During this process it has been implicated protein metabolism activation, with proteases likely involvement in parasite protein degradation. In this study we determined the effect of T. cruzi keeping conditions in the laboratory on the activity of proteases and their relationship on spontaneous metacyclogenesis in axenic culture. We worked with Trypanosoma cruzi Dm30L clone maintained in the laboratory by quarterly passes triatomine-mouse (triatomine condition: TC) or serial culture media during 15 years in LITB medium (culture condition: CC). In both conditions, growth curve was made for 30 days in LITB medium supplemented with 10, 24 and 48 mM glucose, calculating the percentage of spontaneous metacyclics and determining proteases activity. In 10 mM glucose, TC parasites showed higher expression of proteases and metacyclic percentage relative to the CC. In TC, the growth in 48 mM glucose reduced the above variables, while no evidenced proteolytic activity nor spontaneous metacyclogenesis on CC parasites in high glucose concentrations. In all cases there was greater cysteine proteases activity during the stationary phase of the culture. Metalloprotease activity was not reported in any of the conditions studied, but it was detected in clone Dm28c used as control of expression of this type of enzyme. These results suggest that the expression of proteases is influenced by the genetic makeup of the parasite and the CC causes decrease of proteolytic activity which persists even in carbohydrate depleted conditions of the medium, with a concomitant significant reduction in the percentage of spontaneous metacyclics.
