Importance: While the incidence of early-onset metastatic colorectal cancer (mCRC) has been increasing, studies on the age-related disparity in this group of patients are limited. Objective: To evaluate the association of age with treatment-related adverse events and survival in patients with mCRC and explore the potential underlying factors. Design, Setting, and Participants: This cohort study included 1959 individuals. Individual data on 1223 patients with mCRC who received first-line fluorouracil and oxaliplatin therapy in 3 clinical trials, and clinical and genomic data of 736 patients with mCRC from Moffitt Cancer Center were used to assess genomic alterations and serve as an external validation cohort. All statistical analyses were conducted from October 1, 2021, through November 12, 2022. Exposures: Metastatic colorectal cancer. Main Outcomes and Measures: Survival outcomes and treatment-related adverse events were compared among patients in 3 age groups: younger than 50 (early onset), 50 to 65, and older than 65 years. Results: In the total population of 1959 individuals, 1145 (58.4%) were men. Among 1223 patients from previous clinical trials, 179 (14.6%) in the younger than 50 years group, 582 (47.6%) in the 50 to 65 years group, and 462 (37.8%) in the older than 65 years group had similar baseline characteristics except for sex and race. The younger than 50 years group had significantly shorter progression-free survival (PFS) (hazard ratio [HR], 1.46; 95% CI, 1.22-1.76; P < .001) and overall survival (OS) (HR, 1.48; 95% CI, 1.19-1.84; P < .001) compared with the 50 to 65 years group after adjustment for sex, race, and performance status. Significantly shorter OS in the younger than 50 years group was confirmed in the Moffitt cohort. The younger than 50 years group had a significantly higher incidence of nausea and vomiting (69.3% vs 57.6% [50-65 years] vs 60.4% [>65 years]; P = .02), severe abdominal pain (8.4% vs 3.4% vs 3.5%; P = .02), severe anemia (6.1% vs 1.0% vs 1.5%; P < .001), and severe rash (2.8% vs 1.2% vs 0.4% P = .047). The younger than 50 years group also had earlier onset of nausea and vomiting (1.0 vs 2.1 vs 2.6 weeks; P = .01), mucositis (3.6 vs 5.1 vs 5.7 weeks; P = .05), and neutropenia (8.0 vs 9.4 vs 8.4 weeks; P = .04), and shorter duration of mucositis (0.6 vs 0.9 vs 1.0 weeks; P = .006). In the younger than 50 years group, severe abdominal pain and severe liver toxic effects were associated with shorter survival. The Moffitt genomic data showed that the younger than 50 years group had a higher prevalence of CTNNB1 mutation (6.6% vs 3.1% vs 2.3%; P = .047), ERBB2 amplification (5.1% vs 0.6% vs 2.3%; P = .005), and CREBBP mutation (3.1% vs 0.9% vs 0.5%; P = .05), but lower prevalence of BRAF mutation (7.7% vs 8.5% vs 16.7%; P = .002). Conclusions and Relevance: In this cohort study of 1959 patients, those with early-onset mCRC showed worse survival outcomes and unique adverse event patterns, which could be partially attributed to distinct genomic profiles. These findings may inform individualized management approaches in patients with early-onset mCRC.

BACKGROUND: In patients with non-alcoholic fatty liver disease (NAFLD), the impact of the severity of steatosis and inflammatory activity on the accuracy of liver stiffness measurement (LSM) by transient elastography (TE) and by two-dimensional shear wave elastography (2D-SWE) in staging liver fibrosis is still debated and scarce. We aimed to focus on this aspect. METHODS: We prospectively studied 104 patients requiring biopsy for the assessment of NAFLD. We used ordinary least squares regression to test for differences in the association between fibrosis and LSM by TE and 2D-SWE when other factors (steatosis and inflammatory activity) are considered. RESULTS: Among 104 patients, 102 had reliable LSM by TE, and 88 had valid LSM by 2D-SWE. The association between fibrosis based on histology and LSM was significantly stronger when 2D-SWE assessed LSM compared to TE (Spearman's correlation coefficient of .71; P < .001 vs .51, P < .001; Z = 2.21, P = .027). Inflammatory activity was an independent predictor of LSM by TE but not of LSM by 2D-SWE. After controlling for fibrosis, age, sex and body mass index, the inflammatory activity and the interaction between inflammatory activity and fibrosis independently explained 11% and 13% of variance in LSM by TE respectively. Steatosis did not affect the association of fibrosis and LSM by either method. CONCLUSION: Inflammatory activity on histology significantly affects LSM by TE, but not LSM by 2D-SWE in NAFLD. LSM by 2D-SWE reflects liver fibrosis more accurately than LSM by TE. Furthermore, the severity of steatosis on histology did not influence the association of LSM and fibrosis by either elastography method.

BACKGROUND & AIMS: Controlled attenuation parameter (CAP) for steatosis assessment has not been validated in compensated advanced chronic liver disease compensated advanced chronic liver disease (cACLD). We primarily aimed at assessing the accuracy of CAP for the diagnosis and quantification of steatosis in cACLD. Secondary aim: to assess the validity of non-invasive criteria for cACLD according to liver stiffness measurement (LSM). METHODS: This is a single-centre retrospective study including patients with cACLD defined as LSM ≥10 kPa, CAP measurement and liver biopsy (reference standard for steatosis and fibrosis) observed in 06/2015-06/2017. Steatosis was graded as S0 (<5%), S1 (5%-32%), S2 (33%-66%) and S3 (>66%). The diagnostic performance of CAP for any grade of steatosis and for high-grade steatosis (≥S2) was studied. RESULTS: Among 461 consecutive patients, 111 with LSM-based diagnosis of cACLD were included (63% male, median age 55 years, median body mass index 28.1 Kg/m2 , aetiology: 32% non-alcoholic fatty liver disease/non-alcoholic steatohepatitis, 32% alcohol or viral + metabolic syndrome, 15% viral, 6% autoimmune, 4% alcohol, 11% others). Median LSM and CAP were 16.1 kPa and 277 dB/m respectively. On liver biopsy, steatosis was found in 88/111 patients (79%); 44 patients (43 with metabolic syndrome) had high-grade steatosis. CAP was accurate in identifying any grade of steatosis (area under the receiving operating characteristic curves 0.847; 95% CI 0.767-0.926, P < .0001), and ≥S2 steatosis (0.860; 95% CI 0.788-0.932, P < .0001). CAP performed similarly in patients with CAP- interquartile range (IQR) ≥ or <40 dB/m. CONCLUSIONS: Steatosis is frequent in patients with cACLD and metabolic syndrome. CAP diagnostic accuracy for any steatosis and high-grade steatosis is good in this population. A CAP-IQR ≥40 dB/m does not impair CAP diagnostic accuracy in cACLD.

De acuerdo a consensos científicos a nivel internacional el objetivo primordial en el tratamiento de la hepatitis crónica B (HCB) es actualmete lograr la supresión de la replicación viral de manera potente y en el menor tiempo posible. A continuación presentamos la experiencia clínica acumulada en Venezuela empleando el análogo nucleosido telbivudina en pacientes con HCB. Se analizaron 29 portadores con HCB, promedio de edad 44±17 años, con una proporción 2/1 sexo masculino/sexo femenino, 23 con HCB antígeno e positivo y 6 con HCB antígeno e negativo. Las variables escogidas de evaluación fueron la viremia (ADN VHB), el valor de ALT y la tolerancia al tratamiento. Durante un período promedio de tratamiento de 7,3 meses cada paciente recibió 600 mg diarios de telbivudina. 86,2% de ellos disminuyó significativamente la carga circulante de ADN VHB de 7,3±1,2 log10 copias/mL a 1,9±1,0 log10 copias/mL (p= 0,0001). Adicionalmente, se demostró disminución significativa de los valores de ALT, de un promedio de 4,3 veces a una media de 1,4 veces el límite superior normal (p=0,01). Exceptuando un paciente con elevación importante de creatin-quinasa y otro que se quejó de sensación de acidez, la tolerancia reportada fue muy buena. Es concluyente que la telbivudina indujo supresión de la carga viral en forma potente y temprana tanto en pacientes con HCB antígeno e positivo como antígeno e negativo, mejoró los valores de ALT y fue bien tolerada la dosis por la mayoría. La reducción de la carga viral a niveles incluso indetectables durante el primer año de tratamiento, debería contribuir a prevenir la emergencia temprana de cepas del VHB resistentes a esta droga antiviral.
According to international scientific consensus, the fundamental goal in the treatment of chronic hepatitis B (CHB) is currently to achieve suppression of the viral replication in a very potent way at the shortest possible time. It follows our clinical experience accomplished in Venezuela by using the nucleoside analog telbivudine in patients with CHB. We studied twenty-nine carriers with CHB, mean age of 44±17 years old, male/female ratio 2/1, 23 of them with e antigen positive CHB and the remaining 6 with e antigen negative CHB. We selected the viral load (HBV DNA), the ALT value and the treatment tolerance as the parameters to be assessed. During an average treatment period of 7,3 months each patient received 600 mg daily of telbivudine. 86.2% of them showed significant decreased of the circulating HBV DNA load, from 7.3±1.2 log10 copies/mL to 1.9±1.0 log10 copies/mL (p= 0.0001). In addition, a significant decrease of ALT values from a mean of 4.3 fold to 1.4 fold (p=0.01) was also demonstrated. The group of patients showed very good tolerance of the doses, except one who presented increased creatine kinase value and another one who complained from peptic symptoms. It is conclusive that Telbivudine induced early and potent viral suppression, either in e antigen positive or e antigen negative CHB, improved the ALT values and was very well-tolerated by the majority. The viral load reduction, even undetectable during the first year of treatment, should contribute to prevent the early emergency of resistant strains to this antiviral drug.
