In sub-Saharan Africa (SSA), the etiological factors of epilepsy are multiple and phacomatoses, in particular Sturge weber's disease, are rarely reported due to under-medicalization and insufficient multidisciplinary care. We carried out a retrospective study of 216 patients hospitalized for recurrent epileptic seizures between 2015 and 2022 in the neurology and pediatrics department of the University Hospital Center of Conakry, among whom eight (8) patients were identified for Sturge Weber's disease in order to reassess this pathology from a clinical and paraclinical point of view in a tropical environment. Sturge Weber's disease was retained in eight (8) on the presence of symptomatic partial epileptic seizures (age 6 months to 14 years) with frequency of status epilepticus, homonymous lateral hemiparesis linked to occipital involvement, piriform calcifications on imaging and ocular disorders. The delay in consultation and medical care revealed severe mental deterioration in our patients. This study shows a stereotyped clinical picture in a context of aggravation of signs related to a delay in multidisciplinary management. These results are important for the diagnostic, therapeutic and prognostic discussion.

INTRODUCTION: Bone metastasis is frequent in bronchopulmonary cancer. We report a series of 52 patients, and analyze indications and the efficacy of surgery. MATERIALS AND METHODS: We retrospectively studied the records of 52 patients operated on for spinal metastases of bronchopulmonary cancer over a 6-year period from January 2009 to December 2014 in the neurosurgery department of the North Hospital of Marseille, France. RESULTS: Mean age was 63.6years; with a sex ratio of 3:1 (M:F). Spinal pain associated with vertebral fracture and spinal cord compression was the most frequent clinical presentation (59.6%). SINS score was≥7 in 78.9% of cases. Karnofski Performance Status was average in 67.4% of cases. Predicted survival beyond 12months was zero according to the modified Tokuhashi score. The surgical indication was essentially palliative. Evolution showed regression of pain in 84.6% of cases, and stabilization and improvement in motor deficit in 80.6%. Median postoperative survival was 16months. CONCLUSION: Our results highlight the interest of surgery for pain relief, spinal stabilization and improvement in neurological function in patients with spinal metastases of bronchopulmonary cancer, and the unreliability of predictive survival scores.

AIMS: This study aimed to develop an efficient, cost-effective and eco-friendly bacterial consortium to degrade petroleum sludge. METHODS AND RESULTS: Four bacterial strains belonging to genera Acinetobacter and Pseudomonas were selected to constitute three different consortia based on their initial concentration. The highest degradation rate (78%) of 1% (v/v) crude oil after 4 weeks of incubation was recorded when the concentration of biosurfactant (BS) producing isolate was high. Genes, such as alkB, almA, cyp153, pah-rhdGN, nah, phnAC and cat23 were detected using the polymerase chain reaction method and their induction levels were optimal at pH 7·0. A crude oil sludge was artificially constituted, and its bacterial composition was investigated using 16S rRNA gene amplicon sequencing. The results showed that the soil bacterial community was dominated by plant growth-promoting bacteria (PGPB) after crude oil treatment. CONCLUSIONS: Our findings indicate the decontamination of the crude oil contaminated soil was more effective in the presence of both the constituted consortium and PGPB compared to the presence of PGPB alone. SIGNIFICANCE AND IMPACT OF THE STUDY: This study showed that the PGPB (Taibaiella) present in petroleum uncontaminated soil can promote the soil decontamination. The addition of both efficient hydrocarbon-degrading and BS producing bacteria is also necessary to improve the decontamination.

"Blast hand" is a traumatic hand injury related to an explosion. Artisanal gold miners use dynamite to excavate gold pits; such activities expose them to blast hand injuries. This work aims to study blast injuries to gold miners' hands. A 25-month retrospective study was performed. Data on the traumatic event, patients, and injuries were collected and analyzed. Dedicated classifications and scores were used to evaluate the injury topography, injury severity, physical dependence, and aesthetic impact. Data were analyzed statistically. Thirty patients with 46 blast hand injuries among 516 hand injuries were collected. All patients were males and full-time artisanal gold miners. They were seen in the emergency room an average of 10.2hours (1-72) after the explosion. Explosions were caused by a 500g dynamite charge in all cases. The detonation was mainly thermal (n=13), triggered by the patient himself (n=24) and inside the gold pit (n=20). Injuries were bilateral (53%) or left side predominant (59%). Complex injuries were present in 21 hands. The MHISS (Modified Hand Injury Severity Score) was severe (n=7) and major (n=32). Associated injuries were musculoskeletal (n=12), ophthalmologic (n=14) and maxillofacial (n=10). Complexes injuries were correlated to being inside the pit at the time of the explosion. Treatment was conservative more often (n=33) than amputation (n=13). The functional recovery was complete in 22 hands (10 patients). Return to work at the same level was possible for only eight hands (5 patients). The presence of local sequelae or associated injuries negatively impacted the return to work. In Burkina Faso, gold miner's blast hand injuries cause post-traumatic social and professional reintegration issues. Better regulation of artisanal gold mining and expansion of treatment modalities (microsurgery, hand rehabilitation, splinting) may improve the outcome.

Tuberculosis is an endemic disease in Senegal. Cutaneous forms are rare and are characterized by their clinical polymorphism. They account for 2% of the extrapulmonary locations of tuberculosis [1, 2]. We report an observation of a tuberculous gumma of the buttock that led to the discovery of an active pulmonary localization in an immunocompetent adult. A 47-year-old man was admitted for a painful swelling of the right buttock that had been developing for four years. The physical examination noted an indurated, multinodular, and polyfistulized plaque, with confluent yellowish pus emission, on the inferomedial face of the right buttock, associated with bilateral inguinal lymphadenopathy. Cutaneous tuberculosis was found in the histology, which showed TB granuloma, and the gastric tube fluid was positive for acid-fast bacilli. Thoracoabdominopelvic CT showed multiple basal acinar micronodules in both lungs. Serology was negative for both HIV and HBV. All signs of tuberculosis disappeared after six months of treatment. In endemic zones, cutaneous tuberculosis is characterized by its clinical polymorphism. It must also be looked for in any perineal abscess. Early management greatly improves the prognosis.

INTRODUCTION: The objective of this study was to determine the incidence of the operating site infection, precise the etiologies and identify the risk factors. PATIENTS AND METHOD: This was a prospective study conducted at the department of orthopedics and traumatology at the hospital of Sikasso. The study was conducted from January 2016 to December 2018. The infection criteria were those of CDC from Atlanta. RESULT: This study was conducted on 140 patients including 105 men and 35 women with an average age of 40, 1 years old (3 and 88 years old). We have made 150 interventions including 131 osteosynthesis, 14 arthroplasties and 5 corrective osteotomies. The global incidence of the operating site infection was of 19, 3% with a 13, 4% incidence in surgery class I, 21, 4% in the class II, 26, 5% in the class III and 50% in the class IV. . It was superficial in 18 cases (62, 1%) and deep in 11 cases (37, 9%). The average post-surgery hospitalization duration was 13 days (5 and 70 days in extreme cases). The isolated germs were Staphylococcus aureus in 6 cases (37,5 %), Pseudomonas aeruginosa in 3 cases (18,7 %), Enterobacter cloacae in 3 cases (18,7%), Escherichia Coli in 2 cases (12,5 %,), Klebsiellapneumoniae in 1 case (6,3%) and Proteus mirabilis in 1 case (6,3%). They were 100% sensible to imipramine. 90% of the cases were resisting to ampicillin and penicillin G. CONCLUSION: The incidence of the ISO in our department was high. The risk factors of Operating Sites Infections were: hospitalization duration beyond 2 days, ASA Score over 2 days long, the surgical intervention class and the operation duration higher than 3 hours.


INTRODUCTION: Le but de cette étude était de déterminer l'incidence de l'infection du site opératoire, de préciser les étiologies et d'identifier les facteurs de risque. PATIENTS ET MÉTHODE: Il s'agissait d'une étude prospective réalisée dans le service de chirurgie orthopédique et traumatologique de l'hôpital de Sikasso. L'étude s'est déroulée de Janvier 2016 à décembre 2018. Les critères d'infection ont été ceux du CDC d'Atlanta. RÉSULTATS: Cette étude concernait 140 patients dont 105 hommes et 35 femmes d'âge moyen de 40,1 ans (extrêmes : 3 et 88 ans). Nous avions réalisé 150 interventions dont 131 ostéosynthèses, 14 arthroplasties et 5 ostéotomies de correction. L'incidence globale de l'infection du site opératoire était de 19,3% avec une incidence 13,4% dans la classe de chirurgie I, 21,4% dans la classe II, 26,5% dans la classe III et 50% dans la classe IV. Elle était superficielle dans 18 cas (62,1%) et profonde dans 11 cas (37,9%). La durée moyenne d'hospitalisation post-opératoire était de 13 jours (extrêmes 5 et 70 jours). Les germes isolés étaient le Staphylococcus aureus 6 cas (37,5 %) le Pseudomonas aeruginosa 3 cas (18,7 %), l'Enterobacter cloaca e3 cas (18,7%),l' Escherichia Coli2 cas(12,5 %,), le Klebsiella pneumoniae 1cas (6,3%)et le Protéus mirabilis 1 cas (6,3%). Ils étaient sensibles à l'imipenem dans 100 %. Ils étaient résistants à l'ampicilline, à l'amoxicilline et pénicilline G dans 90% des cas. CONCLUSION: L'incidence des ISO dansnotre service était élevée. Les facteurs de risques des ISO que nous avions identifiés ont été la durée d'hospitalisation supérieure à deux jours, le Score ASA supérieur à 2, la classe de l'intervention chirurgicale et la durée opératoire supérieure à 3 heures.

AIM: To determine the frequency of the surgery wound infection, its favoring factors, causative germs and its antibiotic treatment of choice in the surgery "A" department of the University Hospital Point G. METHOD: In a retrospective descriptive study over a 6-month period in the surgery "A" department of the University Hospital Point G, we enrolled all patients who underwent surgery necessitating at least 48 hours of hospital admission during our study period and in conformity with the Atlanta CDC criteria. Surgery patients with less than 48 hours postoperative hospital stay and those admitted to the hospital without surgery were not included.After the surgery, the nature and location of the infection have been clarified. In this work, ethical considerations have been respected and there is no conflict of interest. RESULTS: Two hundred and sixty-five (n=265) patient files were collected including 24 cases of surgery wound infection (a hospital frequency of 9%. The average age was 41.41 years (y.o) old with extremes of 7 y.o and 102 y.o. The sex ratio was 0.9 in favor of women. The commonly studied pathologies were digestive surgery in 52.8%, gynecological surgery in 24.5%, etc… Patients were seen in regular outpatient surgery visits in 75.8% and surgical emergencies in 24.2%. Of the patients urgently admitted, 26.5% presented an ISO; for those received in ordinary consultation it was 3.48%. Fifteen (15) cases were classified ASA III and two (2) ASA II. Based on the Altemeier classification, surgery was clean in 66.8% of our patients, contaminated clean in 12.4%, contaminated in 12.1%, and dirty in 8.7% (including half resulting in surgery wound infection). In total, 79% of patients whose surgeries lasted more than two (2) hours presented a surgery wound infection. Postoperative infection of patients was superficial in 58.3% and deep in 33.3%. In 8.3% of cases, it was a space infection. At the cytobacteriological examination of the pus from the operative site, Escherichia coli (E. coli) was the most common germ with 58.3%. E. coli was sensitive to amoxicillin-clavulanic acid in 57.14%. Surgically, a re-intervention was performed in 20.8% of cases, a secondary suture in 12.5%, and a single dressing in 66.7%. The average postoperative stay was 6.5 days with extremes of 2 days and 69 days. In 3 months postoperatively we recorded 4 cases of death. CONCLUSION: The surgery wound infection constitutes a major complication in a surgical environment starting with surgical act itself.Particular emphasis should be placed on prevention, which will reduce the risks of ISO occurrence.


BUT: déterminer la fréquence des infections du site opératoire (ISO), les facteurs favorisants la survenue des ISO, les germes responsables et les antibiotiques actifssur les infections du site opératoire dans le service chirurgie « A ¼ du CHU du Point G. MÉTHODE: Il s'agissait d'une étude rétrospective et descriptive sur une période de 6 mois réalisée dans le service chirurgie « A ¼ du CHU du Point G. Ont été inclus tous les patients ayant subi une intervention chirurgicale pendant cette période et hospitalisés au moins 48 heures après, et qui ont répondu aux critères du CDC d'Atlanta. N'ont pas été inclus, les patients opérés dont le séjour post opératoire a été inférieur à 48 heures, les patients hospitalisés dans le service mais non opérés, et ceux opérés dans le service et non hospitalisés. En postopératoire la nature et le siège de l'infection ont été précisées. Dans ce travail les considérations éthiques ont été respectées et il n'y a pas de conflit d'intérêt. RÉSULTATS: deux cent soixante-cinq dossiers de patients ont été colligés dont 24 cas d'ISO soit 9% des cas. L'âge moyen a été de 41,41 ans avec des extrêmes de 7ans et 102 ans. Le sex ratio a été de 0,2 en faveur des femmes. Les principales pathologies étudiées ont été la chirurgie digestive dans 52,8%, la chirurgie gynécologique dans 24,5%. La majorité des patients ont été reçus en consultation ordinaire dans 75,8% des cas, et en urgences dans 24,2 %. Parmi les patients reçus en urgence 26,5% ont présenté une ISO ; pour ceux reçus en consultation ordinaire elle a été de 3,48%.Parmi les patients infectés 15 étaient classés ASA III et 2 ASA II. Selon la classification Altemeier, la chirurgie propre a occupé 66,8% des patients, la chirurgie propre contaminée 12,4%, la chirurgie contaminée 12,1%, et la chirurgie sale 8,7%. Parmi les patients opérés de chirurgie sales 50% ont présenté une ISO.L'ISO était présente chez 79% des patients ayant effectué plus de 2 heures d'intervention.En postopératoire l'infection était superficielle chez 58,3% des patients, et profonde chez 33,3% des patients. Dans 8,3% des cas, il s'agissait d'une infection d'espace. A l'examen cytobactériologique du pus Escherichia coli (E coli) avec 58,3% a été le germe le plus fréquent sur le site opératoire. E coli était sensible à l'association amoxicilline - acide clavulanique dans 57,14%. Sur le plan chirurgical une réintervention a été réalisée dans 20,8% des cas, une suture secondaire dans 12,5%, et un pansement seul dans 66,7%. La durée moyenne de séjour post opératoire était de 6,5 jours avec des extrêmes de 2 jours et 69 jours. En 3 mois postopératoire nous avons enregistré 4 cas de décès. CONCLUSION: L'ISO constitue une complication majeure en milieu chirurgical compromettant l'acte chirurgical. Un accent particulier doit être mis sur la prévention qui diminuerai les risques de survenue de l'ISO.

INTRODUCTION: Hyperprolactinemia, which is a supra-physiological secretion of prolactin, is the most common anterior pituitary disorder encountered in clinical practice. Its incidence and prevalence are poorly defined in Africa and the rest of the world. The objectives were to study the clinical, paraclinical, etiological and therapeutic aspects of hyperprolactinemia at the Mali hospital. METHODOLOGY: This was a 5-year cross-sectional study. Data collection was retrospective (July 2011 to October 2015) and prospective (December 2015 to July 2016). RESULTS: We collected 37 cases of hyperprolactinemia. The sex ratio was 0.85. The average age was 37.32 years with extremes ranging from 15 to 74 years. The clinical picture was dominated in women by amenorrhea (80%), galactorrhea (70%), headache (55%), hypofertility (50%), visual disorders (25%) and in men by decreased libido (64.7%), gynecomastia (47.1%), headache (47.1%), visual disorders (41.2%) and erection disorders (29.4%). Basal prolactinemia was greater than 100ng/ml in 45.9% of patients. Cerebral CT had objectified: 11 cases of macroadenomas and 5 cases of pituitary microadenomas. The main causes of hyperprolactinemia were: prolactin pituitary adenoma (43.24%); hypothyroidism (5.40%) and estrogen-progestin use in 5.40%. For treatment, 64.9% of patients were placed on cabergoline; 27% on bromocriptine and 8.10% on simple clinical and biological monitoring. CONCLUSION: Hyperprolactinemia is a condition that exists in our health care facilities. Clinicians should consider this in the face of galactorrhea amenorrhea or decreased libido. It is also necessary to improve the technical platform for better care.


INTRODUCTION: L'hyperprolactinémie, qui est une sécrétion supra physiologique de prolactine est en clinique le désordre hypophysaire antérieur le plus fréquemment rencontré. Son incidence et sa prévalence sont peu définies, en Afrique et dans le reste du Monde. Les objectifs étaient d'étudier les aspects cliniques, paracliniques, étiologiques et thérapeutiques de l'hyperprolactinémie à l'hôpital du Mali. MÉTHODOLOGIE: Il s'agissait d'une étude transversale de 5 ans. La collecte des données a été rétrospective (juillet 2011 à octobre 2015) et prospective (décembre 2015 à juillet 2016). RÉSULTATS: Nous avons colligés 37 cas d'hyperprolactinémie. Le sex ratio était de 0,85. L'âge moyen était 37,32 ans avec des extrêmes allant de 15 à 74 ans. Le tableau clinique était dominé chez les femmes par l'aménorrhée (80%), la galactorrhée (70%), les céphalées (55%), l'hypofertilité (50%), les troubles visuels (25%) et chez les hommes par la baisse de la libido (64,7%), la gynécomastie (47,1%), les céphalées (47,1%), les troubles visuels (41,2%) et les troubles de l'érection (29,4%). La prolactinémie basale était supérieure à 100ng/ml chez 45,9% des patients. La tomodensitométrie cérébrale avait objectivé : 11 cas de macroadénomes et 5 cas de microadénomes hypophysaires. Les principales causes de l'hyperprolactinémie étaient : l'adénome hypophysaire à prolactine (43,24%) ; l'hypothyroïdie (5,40%) et la prise d'oestroprogestatifs chez 5,40%. Pour le traitement, 64,9% des patients étaient mis sous cabergoline ; 27% sous bromocriptine et 8,10% sous simple surveillance clinique et biologique. CONCLUSION: L'hyperprolactinémie est une pathologie qui existe dans nos structures de santé. Les cliniciens doivent y penser devant une aménorrhée galactorrhée ou une baisse de la libido. Il est aussi nécessaire d'améliorer le plateau technique pour une meilleure prise en charge.

Mycetoma remains endemic in the tropical and subtropical regions of the "mycetoma belt" including Senegal. It affects more commonly young men in the age group of 20 to 40 years. The foot represents the most commonly affected site. The most common extra-podal localizations are leg, knee, buttocks, hand and arm. We report an exceptional case of cervical fungal mycetoma that occurred in a 13-year-old Senegalese child. He consulted for a cervico-submandibular tumefaction with multiple sinuses and black grains discharge evolving since 6 years, associated to laryngeal dyspnoea. Mycological examination with culture isolated Madurella mycetomatis. Cervical CT Scan showed bone and soft tissue invasion. Terbinafine alone was administered. During the evolution, tracheotomy was performed following the aggravation of the laryngeal disorders. Death from severe sepsis occurred after 8 months of evolution. The particularities of our case are the occurrence of fungal mycetoma in a child, the cervical localization and the difficulties of therapeutic management largely due to the diagnostic delay.

To describe the process of establishing a reference center for gestational trophoblastic diseases (GTD) in Senegal and to report its main results so far. We describe the history and establishment of the center, which is based on the experience of the main international centers. The adaptations made to patient follow-up are detailed, while we follow FIGO and WHO diagnostic and management criteria. Finally, we report our main results. Between 2011 and 2017, 878 files were registered at the Center. More than half of the women had no histological confirmation of GTD (60.8 %). The diagnosis was then based on ultrasound images or macroscopic examination of molar vesicles. Spontaneous remission occurred in 64.5 % of the cases, while gestational trophoblastic neoplasia developed in 23.5 %. The FIGO criteria were slightly adapted for hCG monitoring. Methotrexate was the drug of choice in the low-risk group (97.8 %), while the EMACO protocol was financially difficult for nearly half of the high-risk group. The overall remission rate was 83 % and the specific lethality 11.6 %. Our center has demonstrated the efficiency of centralizing the management of GTDs. Difficulty in access to hCG and antimitotic drugs makes management difficult. However, we have introduced alternative solutions that we are working to improve.

Dermatophytic mycetoma of the scalp is a rarity, especially when not associated with tinea capitis. Here, we report a case of dermatophytic mycetoma of the scalp, that was not associated with tinea capitis, caused by an atypical Microsporum audouinii strain. To our knowledge, this is the second reported case, thirty-eight years after the first that was published in 1980.

Thymic epithelial tumors (TET) are rare lesions. The brain metastases of these tumors are even rarer. We report a case of brain metastases in a known patient with a thymic carcinoma diagnosed in October 2016. She was a 73-year-old woman who presented with headache, nausea, and right hemiplegia. Brain MRI revealed five lesions (1 insular, 1 frontal and 2 left temporal, 1 right parafalcine). These lesions were initially treated using two stereotactic radiosurgery gamma knives. A macroscopically complete excision of the left frontal lesion was subsequently performed without any complications with a good evolution of the neurological symptoms postoperatively. Immunohistochemical examination was compatible with metastatic thymic carcinoma. The patient died 14 months after the initial diagnosis. A review of the literature in English has reported another 45 TET cases with brain metastases.

RESUMO

Radical cystectomy is the standard treatment for bladder tumors infiltrating the bladder muscle, for patients who have tumors without invasion of neighboring organs and without metastasis. After radical cystectomy, the urinary diversion is a challenge for any urologist. Ileal neo bladder is a rare surgery in our country. The ileal neo bladder is recommended in these patients. We report a case of ileal neo bladder in a 42-year-old woman with a bladder tumor. We discuss the clinical, diagnostic and therapeutic aspects.


La cystectomie totale constitue le traitement de référence des tumeurs de vessie infiltrant le muscle vésical, pour les patients qui ont une tumeur sans envahissement des organes voisins et sans métastase. Après la cystectomie, la dérivation des urines est un challenge pour tout urologue. Le remplacement de vessie est un geste rarement réalisé dans notre pays. Un remplacement de vessie par l'intestin est recommandé chez ces patients. Nous rapportons un cas de remplacement de vessie chez une femme de 42 ans ayant une tumeur de vessie. Nous discutons les aspects cliniques, diagnostiques et thérapeutiques.

INTRODUCTION: Bladder and ureteral lesions are the most common urologic complications occurring during pelvic surgery with 1% and 0.5% to 3%, respectively. These lesions are rarely recognized intraoperatively and pose a major problem to urologists, gynecologists, and general surgeons. OBJECTIVE: To study the factors favoring urologic complications following pelvic surgery at the University Hospital Point-G. PATIENTS AND METHOD: We conducted a retrospective study at the University Hospital Point-G between 2006 and 2015. It involved 23 patients with a whole urological lesion following pelvic surgery. These patients underwent a clinical and para-clinical examination (intravenous urography, methylene blue test) to confirm the urological lesion and to determine its management. RESULTS: The average age of our patients was 32.00 years with extremes of 18 and 40 years old. The leakage of urine was found in 82.6% (19/23). Interventions that caused urologic injury were: caesarean section 52.2% (12/23), hysterectomy 30.4% (7/23), as well as a caesarean section and hysterectomy 17.4% (4/23). Methylene blue was performed in 19 patients, it was positive in 52.2% (12/23) and intravenous urography (IVU) in 7 patients. The lesions encountered were: retrotrigonal fistula vesico-vaginal 10 cases, vesico-vaginal fistula under trigonal 2 cases, uretero-vaginal fistula 7 cases, and bilateral ureteral ligation 4 cases. Fistulorraphy was performed in 52.2% followed by direct ureterovesical reimplantation. The postoperative outcome was satisfactory in 100% of cases with obtaining a good bladder tightness. The average duration of hospitalization was 12 days (+/- 4 days). CONCLUSION: Pelvic surgery results in urological injuries. Caesarean section and hysterectomy are contributing factors. Vesico-vaginal fistula or ureteral lesions are common.


INTRODUCTION: Les lésions vésicale et urétérale constituent les complications urologiques les plus fréquentes survenant au décours d'une chirurgie pelvienne soit respectivement 1 à 4% et 0,5 à 3% [6,7]. Ces lésions sont rarement reconnues en peropératoire et posent un problème important auquel sont confrontés les urologues, les gynécologues, les chirurgiens généralistes. OBJECTIF: Etudier les facteurs favorisants les complications urologiques consécutives à la chirurgie pelvienne au CHU du Point-G. PATIENTES ET MÉTHODE: Il s'agissait d'une étude rétrospective réalisée au CHU du Point-G entre 2006 et 2015. Elle a concerné 23 patientes présentant toute une lésion urologique au décours d'une chirurgie pelvienne. Ces patientes ont subi un examen clinique et para cliniques (Urographie intra veineuse; le test au bleu de méthylène) dans le but de confirmer la lésion urologique et d'en déterminer la prise en charge. RÉSULTATS: l'âge moyen de nos patientes était de 32,00 ans avec des extrêmes allant de 18 à 40 ans. La fuite d'urine était retrouvée chez 82,6% (19/23). Les interventions pourvoyeuses de lésions urologique étaient : la césarienne 52, 2% (12/23), l'hystérectomie 30, 4% (7/23), une association césarienne et hystérectomie 17,4 % (4/23). Le test au bleu de méthylène a été réalisé chez 19 patientes, il a été positif dans 12 cas soit 52,2 % et l'UIV chez 7 patientes. Les lésions rencontrées étaient : la fistule vésico-vaginale retro trigonale 10 cas, la fistule vésico-vaginale sous trigonale 2 cas, la fistule urétéro-vaginale 7 cas, et la ligature urétérale bilatérale 4 cas. La fistulorraphie a été effectuée dans 52,2 % suivie de la réimplantation urétéro-vésicale directe. Les suites opératoires étaient satisfaisantes dans 100% des cas avec l'obtention d'une bonne étanchéité vésicale. La durée moyenne d'hospitalisation était de 12 jours (+/− 4 jours). CONCLUSION: La chirurgie pelvienne est pourvoyeuse de lésions urologiques. La césarienne et l'hystérectomie sont des facteurs favorisants. Les fistules vésico-vaginales ou des lésions urétérales sont fréquentes.

AIM: Our aim was to evaluate the diagnosis and therapeutic aspects of the amoebic liver abscesses in the surgery «A¼ department of the University hospital Point "G". PATIENTS AND METHODS: Were included in this study, patients admitted to the surgery «A¼ department for amoebic liver abscess confirmed by a positive amoebic serology and the chocolate appearance of bacteria-free pus. Bacterial liver abscesses were not included into this study. RESULTS: Over a 10-year period, 52 cases of patients diagnosed with amoebic liver abscess were collected in the department. There were 41 men and 11 women, with a sex ratio of 3.7. The mean age was 37.8 years old with extremes of 15 and 66 years. The most represented professions were farmers (36.5%) followed by salesmen (26.7%) and students (11.5%). The average outpatient visit delay time was 18.5 days with the extremes of 5 and 34 days. The most frequent clinical signs were pain in the right hypochondrium (86.5%), fever (78.8%) and hepatomegaly (61.5%). Abdominal ultrasound showed a single located abscess in 44 patients (84.6%) and these abscesses were localized in the right hepatic lobe in 34 patients(65.4%). The average volume of the abscess was 366.5 cm3 with the extremes of 36 cm3 and 1580 cm3. Amoebic serology was positive in 38 patients (80.9%). Cytobacteriological analysis of pus in 37 patients (71%) was negative. Patients underwent an ultrasound assisted needle aspiration of pus in 65.4% he. Laparotomy and a laparoscopic approach were performed in 7.7% and in 5.8%, respectively. The clinical course was uneventfulin 94.2%. The mean hospital stay duration was 16.5 days with the extremes of 4 and 29 days. No death was recorded during hospitalization. CONCLUSION: Amoebic liver abscess is an uncommon pathology in a surgical setting. Abdominal ultrasound andechography guided liver puncture allowed the diagnosis. Laparoscopic approach minimizes the burden of the laparotomy.


BUT: Le but de cette étude était d'évaluer les aspects diagnostiques et thérapeutiques des abcès amibiens du foie dans le service de chirurgie " A " du CHU du Point G. PATIENTS ET MÉTHODES: Ont été inclus dans cette étude, les patients admis dans le service de chirurgie "A" pour abcès amibien du foie sur la base de la positivité de la sérologie amibienne, de l'aspect chocolat du pus et de l'absence de bactéries dans le pus. Les abcès bactériens ont été exclus de cette étude. RÉSULTATS: En 10 ans, 52 dossiers de patients diagnostiqués pour abcès amibiens du foie ont été colligés dans le service. Il s'est agi de 41 hommes et de 11 femmes soit un sex-ratio de 3,7. L'âge moyen des patients a été de 37,8 ans avec des extrêmes de 15ans et 66 ans. Les professions les plus représentées ont été les cultivateurs (36,5%), les commerçants (26,7%) et les élèves et étudiants (11,5%).Le délai moyen de consultation a été de 18,5 jours avec des extrêmes de 5 jours et 34 jours. Les signes cliniques les plus fréquents ont été la douleur de l'hypochondre droit (86,5%), la fièvre (78,8%) et l'hépatomégalie (61,5%). L'échographie abdominale a objectivé un abcès unique dans 44 cas (84,6%) et l'abcès a été localisé dans le lobe hépatique droit dans 34 cas (65,4%). Le volume moyen de l'abcès était de 366,5cm3 avec des extrêmes de 36 cm3 et 1580cm3. La sérologie amibienne a été positive dans 80,9% (n = 38). La culture a été stérile à l'analyse cytobactériologique du pus dans 37 cas (71%). Une ponction évacuatrice écho-guidée a été réalisée dans 65,4% des cas. La procédure chirurgicale a consisté à réaliser une laparotomie dans 7,7% des cas et un abord coelioscopique dans 5,8%. Les antibiotiques utilisés ont été le métronidazole par voie intraveineuse chez tous les patients, associés à la céftriaxone dans 78,8% (n=41). L'évolution clinique a été favorable dans 94,2%. La durée moyenne d'hospitalisation a été de 16,5 jours avec des extrêmes de 4 jours et 29 jours. Aucun décès n'a été constaté pendant l'hospitalisation. CONCLUSION: L'abcès amibien du foie est une pathologie peu fréquente en milieu chirurgical. L'échographie abdominale et la ponction écho-guidée ont permis de poser le diagnostic. Le traitement par abord coelioscopique minimise les inconvénients de la laparotomie.

AIM: This study was aimed to evaluate patients' satisfaction with tranvesical prostatectomy in the Urology department of University Hospital Gabriel Touré according to the (International Prostate Symptoms Score (IPSS) and the quality of life scores. PATIENTS AND METHODS: We conducted a prospective descriptive study over a 12-month period including a consecutive cohort of patients who had transvesical prostatectomy. Each patient was evaluated before and six weeks after the surgery. We used the IPSS as an assessment tool, supplemented by the Quality of Life (SQ) questionnaire. RESULTS: One hundred and thirteen (113) patients aged 71 ± 8 years old on average were evaluated. The mean duration of symptoms associated with benign prostatic hyperplasia was 19 months and 17% of patients had a mechanical complication. All patients had either moderate or severe lower urinary tract symptoms preoperatively, while 99% of them had an IPSS score <8 postoperatively. CONCLUSION: Transvesical prostatectomy allows an important improvement of the IPSS score of the patients and their satisfaction about their quality of life.


BUT: Evaluer la satisfaction des patients après adénomectomie transvésicale de la prostate dans le service d'urologie du CHU Gabriel Touré selon l'International Prostate Symptoms Score (IPSS) et le score de qualité de vie. PATIENTS ET MÉTHODES: il s'agissait d'une étude prospective et descriptive d'une cohorte consécutive de patients opérés pour adénomectomie transvésicale de la prostate réalisée sur une période de 12 mois. Les patients étaient tous évalués avant l'intervention chirurgicale et 6 semaines après. Nous avons utilisé l'IPSS comme outil d'évaluation, complété par la question sur la qualité de vie. RÉSULTATS: Cent treize patients d'âge moyen de 71 ± 8ans ont été évalués. La durée moyenne d'évolution des symptômes liés à l'hypertrophie bénigne de la prostate était de 19 mois et 17 % des patients présentaient une complication mécanique. Tous les patients présentaient des symptômes modérés ou sévères du bas appareil urinaire en préopératoire tandis qu'en postopératoire 99% des patients avaient un score IPSS inférieur à 8. CONCLUSION: l'adénomectomie transvésicale de la prostate permet une amélioration du score IPSS et celui de la qualité de vie des patients avec une importante satisfaction des patients.

RESUMO

The authors report the case of a 22-year-old man referred seven months after the onset of papulo-nodular skin lesions, lymphadenopathy, splenomegaly and hepatomegaly. Mycologic and histologic examination of skin lesions enabled the diagnosis of African histoplasmosis, by Histoplasma capsulatum var duboisii. The lymph nodes were caseous. The culture in Lowenstein-Jensen medium was negative.

RESUMO

We report here a case of intraventricular schwannoma. This location is rare. Our patient was a 68-year-old female with a large intraventricular lesion of the body of the lateral ventricle on the right side. Brain magnetic resonance imaging (MRI) revealed this lesion. After a right parietotemporal craniotomy, microsurgical excision using neuronavigation was performed to completely remove the tumor. Histological and immunohistochemical examination confirmed the diagnosis of intraventricular schwannoma devoid of atypical features. Postoperative MRI showed macroscopically complete tumor removal with no recurrence after 12 months of follow-up. A review of the literature identified 32 such cases published to date.

BACKGROUND: The cosmetic use of bleaching products is common among women from sub-Saharan Africa. The most frequently used products are highly potent corticosteroids (clobetasol propionate) and hydroquinone. Herein, we report 8 cases of SCC in women using skin bleaching products for cosmetic purposes. Our aim is to describe the epidemiological, clinical and pathological aspects of the carcinomas observed during the course of skin lightening. METHODS: We conducted a descriptive multicentre study from August 2005 to January 2016 in three dermatology units in Senegal. We included all patients consulting for cutaneous squamous cell carcinoma associated with skin bleaching. Sociodemographic, clinical, paraclinical and therapeutic data were recorded. RESULTS: A total of 8 female patients were included. The mean age was 48.1 years (37-63 years). Topical hydroquinone and highly potent corticosteroids were the main products used over the whole body, for an average duration of 20.3 years. No pre-neoplastic skin disease was found in our patients. The clinical aspects of tumours were as follows: cauliflower-like (n=4), ulcerated (n=3) and nodular (n=1). The average development time before consultation was 6.75 months. All the cutaneous squamous cell carcinomas were localized to lichenoid lesions or exogenous ochronotic lesions on photo-exposed areas: face (n=1), neck (n=3) or upper back (n=4). The most common histopathological type was the infiltrating form and there was one case of in situ carcinoma. The outcome was favourable in six of eight patients after surgical resection. Two deaths occurred: one through tumour recurrence and the other through haemorrhagic shock. CONCLUSIONS: From 2005 to 2016, eight cases of cutaneous squamous cell carcinomas associated with cosmetic use of bleaching products were reported in Senegal. The mechanism was not fully elucidated and further studies are necessary. These observations provide an additional argument for combating this practice and including skin bleaching among known risk factors for squamous cell carcinoma.
