RESUMO

La diabetes mellitus es una patología metabòlica que altera los niveles de glucosa en el cuerpo, siendo más prevalente la tipo 2, la cual puede llegar a modular enfermedades sistémicas que causan diferentes desórdenes metabòlicos y cambios celulares. En cuanto a las alteraciones oftalmológicas, se pueden resaltar la retinopatía diabética y el ojo seco, siendo esta última una condición inflamatoria crónica que ocaciona daños en la superficie ocular. Por lo tanto, esta revisión brindará información de los factores asociados al desarrollo de ojo seco en la diabetes, resaltando cambios en la glicemia y el rol del metabolismo de glucosa sobre estructuras oculares tales como como la glándula lagrimal, glándulas de Meibomio, y en la microvasculatura, que pueden condicionar a trastornos neuropáticos que conducen a la sintomatologia ocular. De igual modo, se describen eventos biológicos como cambios en la expresión epigenética, estrés oxidativo e inflamación que presumiblemente juegan un papel importante en las diabetes y ojo seco, por lo cual la evaluación y análisis de la lágrima en esta población se hace necesaria, teniendo en cuenta los cambios en las estructuras oculares en la diabetes y las novedosas investigaciones sobre biomarcadores de diabetes a través de la película lagrimal.


Diabetes mellitus is a metabolic pathology that alters glucose levels in the body, being type 2 more prevalent, which can lead to modular systemic diseases that cause different metabolic disorders and cellular changes. Regarding ophthalmological alterations, diabetic retinopathy and dry eye can be highlighted, the latter being a chronic inflammatory condition causing damage to the ocular surface. Therefore, this review will provide information on the factors associated with the development of the dry eye in diabetes, highlighting changes in glycemia and the role of glucose metabolism on ocular structures such as the lacrimal gland, Meibomian glands, and the microvasculature, which can condition neuropathic disorders that lead to ocular symptoms, biological events such as changes in epigenetic expression, oxidative stress, and inflammation are prescribed, which presumably play an important role in diabetes and dry eye, for which the evaluation and analysis of tears in this population is done necessary, taking into account the changes in the ocular structures in diabetes and the new research on biomarkers of diabetes through the tear film.

Se comparó el grado de decoloración de la corona clínica en piezas dentarias anteriores humanas ex-traídas, sometidas a procedimientos de regeneración endodóntica, utilizando MTA blanco o sustituto bioac-tivo de la dentina (Biodentine) como barrera cervical, en presencia de coágulo sanguíneo como andamio. En total se prepararon 24 piezas dentarias anterio-res superiores humanas que fueron divididas en dos grupos control (GC) y dos grupos experimentales (GE). Cada uno incluía 6 piezas dentarias. En los GE se colo-có sangre humana fresca en el interior del conducto, y se confeccionó una barrera de Biodentine (GE3) o MTA (GE4). En los GC se colocó una torunda de algo-dón estéril saturada con solución fisiológica estéril, y se confeccionó una barrera de Biodentine (GC1) o MTA (GC2). El color se evaluó de acuerdo con el espa-cio de color CIE L* a* b* utilizando imágenes fotográfi-cas digitales estandarizadas en dos puntos de tiempo: día 0 (T0) y día 35 (T35). La descripción de los datos in-cluyó mediana (Md), primer cuartil (Q1), tercer cuartil (Q3), media y desviación estándar (DE). Se emplea-ron las pruebas de los rangos con signo de Wilcoxon (RSW) y ANOVA de una vía; p < 0,05 fue considerado significativo. Cuando se comparó ∆E se observaron diferencias significativas entre GC1 y el resto de los materiales (p < 0,05). ∆E fue menor en GC1 (media ± DE; 2,1 ± 1,6) que en los grupos restantes que no pre-sentaron diferencias significativas entre sí. En base a estos resultados, la estabilización del coágulo san-guíneo, como así la limpieza de la cavidad previa a la colocación del Biodentine y el uso de barreras cervi-cales, es imprescindible para evitar la coloración de la corona clínica (AU)


Discoloration remains an unfavorable complication of otherwise successful regenerative endodontic procedure of immature teeth with necrotic pulp. Objective: Compare the degree of discoloration of extracted human teeth after regenerative endodontic procedures, using MTA or Biodentine as a cervical barrier with a blood clot as a scaffold. In total 24 human upper anterior teeth were prepared and divided into two control groups (CG) and two experimental groups (EG). In the EG, fresh human blood was placed inside the root and a Biodentine (GE3) or MTA (GE4) barrier was made in. A sterile cotton swab saturated with sterile physiological solution was placed in the GCs and a Biodentine (GC1) or MTA (GC2) barrier was made in. The color was evaluated according to the CIE L* a* b* color space using standardized digital photographic images at two time points: day 0 (T0) and day 35 (T35). The description of the data included median (Md), first quartile (Q1), third quartile (Q3), means, and standard deviation (SD). Wilcoxon signed rank tests (RSW) and one-way ANOVA were used. p < 0.05 was considered significant. When ∆E was compared, significant differences were observed between GC1 and the rest of the materials (p < 0.05). ∆E was lower in CG1 (mean ± SD; 2.1 ± 1.6) than in the remaining groups, which did not present significant differences between them. There was no significant difference between tooth discolorations with materials in the presence of blood. However, in the absence of blood, Biodentine exhibited less tooth discoloration than MTA (AU)

BACKGROUND: Effective treatments are still needed to reduce the severity of symptoms, time of hospitalization, and mortality of COVID-19. SARS-CoV-2 specific memory T-lymphocytes obtained from convalescent donors recovered can be used as passive cell immunotherapy. METHODS: Between September and November 2020 a phase 1, dose-escalation, single centre clinical trial was conducted to evaluate the safety and feasibility of the infusion of CD45RA- memory T cells containing SARS-CoV-2 specific T cells as adoptive cell therapy against moderate/severe cases of COVID-19. Nine participants with pneumonia and/or lymphopenia and with at least one human leukocyte antigen (HLA) match with the donor were infused. The first three subjects received the lowest dose (1 × 105 cells/kg), the next three received the intermediate dose (5 × 105 cells/kg) and the last three received the highest dose (1 × 106 cells/kg) of CD45RA- memory T cells. Clinicaltrials.gov registration: NCT04578210. FINDINGS: All participants' clinical status measured by National Early Warning Score (NEWS) and 7-category point ordinal scales showed improvement six days after infusion. No serious adverse events were reported. Inflammatory parameters were stabilised post-infusion and the participants showed lymphocyte recovery two weeks after the procedure. Donor microchimerism was observed at least for three weeks after infusion in all patients. INTERPRETATION: This study provides preliminary evidence supporting the idea that treatment of COVID-19 patients with moderate/severe symptoms using convalescent CD45RA- memory T cells is feasible and safe. OBJECTIVES: Veillonella parvula is an anaerobic Gram-negative coccus rarely involved in bone and joint infections. PATIENTS AND METHOD: We report the case of a Veillonella parvula vertebral osteomyelitis (VO) in a female patient without any risk factor. RESULTS: The 35-year-old patient was immunocompetent and presented with Veillonella parvula VO. She was admitted to hospital for inflammatory lower back pain. The discovertebral sample was positive for Veillonella parvula. Literature data on Veillonella VO is scarce. Reported cases usually occurred in immunocompromised patients. Diagnosis delay can be up to four months. Patients are usually afebrile. Outcome with antimicrobial treatment alone is favorable in half of cases. Other patients must undergo surgery. CONCLUSIONS: Veillonella VO may occur in immunocompetent patients and have a clinical spectrum of mechanical lower back pain.

BACKGROUND: Cabozantinib prolongs overall survival (OS) and progression-free survival (PFS) in patients with metastatic clear cell renal cell carcinoma (RCC) that progressed on first-line vascular endothelial growth factor receptor-tyrosine kinase inhibitor (VEGFR-TKI). The role of cabozantinib has not been established in non-clear cell renal cell carcinoma (nccRCC). METHODS: This is a retrospective study of 30 patients with nccRCC who received cabozantinib from January 2013 to January 2017. Information collected included baseline characteristics, toxicity, dose reductions, PFS and OS. A fellowship trained abdominal radiologist, blinded to patient history and clinical data, assessed radiographic response using RECIST, v1.1. RESULTS: With a median follow-up of 20.6 months (95% confidence interval [CI]: 11.4-28.8), median PFS was 8.6 months (95% CI: 6.1-14.7), and median OS was 25.4 months (95% CI: 15.5-35.4). Of the 28 patients with measurable disease, 4 had partial responses (2 papillary, 1 chromophobe and 1 unclassified RCC), 18 had stable disease (64.2%) and 6 had progressive disease (21.4%), resulting in a 14.3% objective response rate and a 78.6% disease control rate. Two patients with papillary RCC who had experienced disease progression on savolitinib achieved durable partial response and stable disease, respectively, following treatment with cabozantinib. Of the 21 patients who started cabozantinib at 60 mg/d, 12 (57.1%) required dose reduction due to toxicity. CONCLUSION: In this retrospective study, cabozantinib produced a clinically meaningful benefit in patients with metastatic nccRCC, the majority of whom had disease progression on prior VEGFR-TKIs. Prospective trials of cabozantinib in nccRCC are warranted.

BACKGROUND: : Known to cause important metabolic disturbances, weight gain becomes a major health problem after smoking cessation. Visceral adiposity index (VAI) is becoming increasingly popular in the detection of cardiometabolic risks in several disorders and general population. Here, we aimed to investigate the effects of quitting smoking on VAI levels. MATERIALS AND METHODS: : Of 350 participants included into the cigarette cessation program, 70 (20%) completed the study and were enrolled into the analyses. VAI levels were calculated at the baseline and 3rd month after cigarette cessation. RESULTS: : Thirty-eight (54.3%) out of 70 participants were male. While the mean age was found as 42 ± 1.0 years, mean starting age of smoking was found to be 16.87 ± 0.45 years, and mean smoking time was 23.07 ± 1.18 years. While VAI levels were found higher in men at the baseline, VAI levels were found similar in both genders at the end of the study. Higher VAI levels were found in those smoking >20 cigarettes/day, compared to those smoking ≤20 cigarettes/day. Although weight, waist circumference, body mass index (BMI), and high-density lipoprotein cholesterol levels increased, VAI levels were found to decrease significantly at the 3rd month. In subgroup analyses, VAI levels were seen to decrease significantly only in men (P = 0.005). Furthermore, VAI levels were found to decrease (P < 0.001) in those with BMI ≥25 kg/m2, whereas no significant change was observed in those with BMI <25 kg/m2. CONCLUSIONS: Although body weight increases significantly after quitting smoking, VAI levels, an indicator of cardiovascular risks, decrease significantly, especially in men or obese patients.

INTRODUCCIÓN: Desde que se implantó la primera prótesis total de rodilla en nuestro medio, se advierte que la mayor parte de las personas acuden a cirugía, no solo cuando el dolor se torna insoportable, sino cuando también la deformidad se ha instaurado o aumentó considerablemente y la función se ve disminuida. OBJETIVO: Determinar los resultados funcionales y las complicaciones de la artroplastía total de rodilla en el servicio de Traumatología y Ortopedia del Hospital Obrero N° 1 durante el periodo 2010 ­ 2015. MATERIAL Y MÉTODOS: Estudio serie de casos. Se estudiaron 121 pacientes de ambos sexos, con diagnóstico de gonartrosis primaria o secundaria, internados en el servicio de Traumatología y Ortopedia del Hospital Obrero Nº 1, durante el periodo del 1° de enero del 2010 al 31 de diciembre del año 2015. La valoración funcional se calificó mediante el cuestionario WOMAC y las complicaciones mediante registro de expedientes clínicos. RESULTADOS: El sexo más afectado es el femenino (75.79%), la edad más frecuente oscila entre 60 y 69 años (50.41%), la comorbilidad preoperatoria más frecuente en los pacientes operados es la artritis reumatoide (33.88%), los resultados funcionales post-operatorios del reemplazo articular son en su mayoría buenos (75.21%), la complicación post-operatoria más frecuente, fue la infección en un 9.09%, la cantidad de días de internación post-operatorio promedio es de 7.2 días. CONCLUSIÓN: Se observaron resultados funcionales Buenos (escala WOMAC) y la infección como la complicación más frecuente en los sujetos de nuestro estudio.


INTRODUCTION: Since the first full knee prosthetics was implanted in our city, it is noticed that most people attend surgery, not only when the pain becomes unbearable, but when deformity has also established itself or it increased considerably and the function looks diminished. OBJECTIVE: Determining the functional results and the complications of the total arthroplasty of knee at the service of Traumatología y Ortopedia of the Hospital Obrero N° 1 during the period 2010 ­ 2015. MATERIAL AND METHODS: Case series study. 121 patients of both sexes were studied, with diagnosis of gonarthrosis primary or secondary in the service of Traumatología y Ortopedia of the Hospital Obrero N° 1, from January 1, 2010 to December 31, 2015. The functional assessment was graded by means of the questionnaire WOMAC and the complications by means of clinical records. RESULTS: The most affected sex is the feminine (75,79 %), the most frequent age oscillates between 60 and 69 years (50,41 %), the most frequent preoperative comorbidity in the operated patients is rheumatoid arthritis (33,88 %), the functional post-operative results of the joint replacement are mainly good (75,21 %), the most frequent post-operative complication, was the infection in a 9,09 %, The amount of days of post-operative hospitalization average is 7,2 days. CONCLUSION: Good functional results were observed (WOMAC scale) and infection as the most frequent complication is the subject of our study.

Natural radionuclides are released into the environment together with fly ash from the coal-burning power plant and cause an increase in the natural radioactivity in environmental samples. The study concerns to the evaluation the influence of Kangal lignite-burning power plant (LBPP) with a power of 457 MWe, which has been in operation since 1989, on natural radionuclide a concentration in surface soil samples around it. Activity concentrations of natural radionuclides (226Ra, 232Th, 40K and 222Rn) in the soil samples, and emanation coefficient (EC) and mass (ERM) and surface (ERS) exhalation rate of radon were determined by using a gamma-ray spectrometer with an HPGe detector. The average values of 226Ra, 232Th, 40K and 222Rn, EC, ERM and ERS were found as 37±5, 17±3, 222±30Bqkg-1 and 9±1kBqm-3, 12%, 12.1 µBq kg-1 s-1 and 7.1mBqm-2 s-1, respectively. Absorbed gamma dose rate in outdoor air and the corresponding effective dose rate from external exposure and excess lifetime cancer risk were estimated to evaluate radiological hazards for human population. The results revealed that the Kangal LBPP has caused a small increment in 226Ra concentration in the studied area. No influence was observed for 232Th and 40K.

BACKGROUND: Visceral adiposity index (VAI) is a proposed parameter to evaluate visceral obesity instead of waist circumference (WC) in patients with polycystic ovary syndrome (PCOS). We aimed to evaluate VAI levels in overweight and/or obese, and non-obese PCOS patients and investigate the association between metabolic and inflammatory parameters. METHODS: Seventy-six PCOS patients between 18 and 40, and 38 age- and BMI-matched controls were enrolled into the study. Both PCOS groups and controls were classified into two subgroups according to body mass index (BMI) <25 and ≥25 kg/m2. RESULTS: In PCOS patients, waist/hip ratio (WHR) (p = 0.023), diastolic blood pressure (DBP) (p = 0.001), insulin (p = 0.011), homeostasis of model assessment (HOMA-IR) (p = 0.006) and uric acid (UA) (p = 0.002) were higher than controls. In overweight and/or obese PCOS group, DBP (p < 0.001), insulin (p = 0.002), HOMA-IR (p = 0.001), triglyceride (p = 0.015) and VAI (p = 0.031) were higher than overweight and/or obese controls. In non-obese PCOS group, WHR (p = 0.016), WC (p = 0.030), DBP (p = 0.010) and UA (p < 0.001) were higher than non-obese controls. Similar VAI levels were found in all PCOS and non-obese PCOS subgroups than peer controls. Overweight and/or obese PCOS group had higher VAI levels than non-obese PCOS group (p < 0.001). VAI levels were positively correlated with WHR, glucose, HOMA-IR, high-sensitive CRP and UA in PCOS group. In controls, VAI levels were positively correlated with WHR, insulin and HOMA-IR. CONCLUSION: We found that VAI levels were higher in overweight and/or obese PCOS patients compared to peer controls and non-obese PCOS patients, and associated with some metabolic and inflammatory parameters.

A growing body of evidence implicates the noncanonical NF-κB pathway as a key driver of glioma invasiveness and a major factor underlying poor patient prognoses. Here, we show that NF-κB-inducing kinase (NIK/MAP3K14), a critical upstream regulator of the noncanonical NF-κB pathway, is both necessary and sufficient for cell-intrinsic invasion, as well as invasion induced by the cytokine TWEAK, which is strongly associated with tumor pathogenicity. NIK promotes dramatic alterations in glioma cell morphology that are characterized by extensive membrane branching and elongated pseudopodial protrusions. Correspondingly, NIK increases the phosphorylation, enzymatic activity and pseudopodial localization of membrane type-1 matrix metalloproteinase (MT1-MMP/MMP14), which is associated with enhanced tumor cell invasion of three-dimensional collagen matrices. Moreover, NIK regulates MT1-MMP activity in cells lacking the canonical NF-κB p65 and cRel proteins. Finally, increased expression of NIK is associated with elevated MT1-MMP phosphorylation in orthotopic xenografts and co-expression of NIK and MT1-MMP in human tumors is associated with poor glioma patient survival. These data reveal a novel role of NIK to enhance pseudopodia formation, MT1-MMP enzymatic activity and tumor cell invasion independently of p65. Collectively, our findings underscore the therapeutic potential of approaches targeting NIK in highly invasive tumors.

PURPOSE: Mean platelet volume (MPV) and neutrophil to lymphocyte ratio (NLR) are the new markers of the detection of inflammation. Our aim is to investigate MPV and NLR in lean and obese patients with polycystic ovary syndrome (PCOS). METHODS: This study was designed to investigate MPV, NLR, and high-sensitive C-reactive protein (hsCRP) levels in 25 obese patients with PCOS and 16 lean patients with PCOS, and our study group was matched with 16 obese and 14 non-obese controls, respectively. RESULTS: PCOS group had higher MPV, NLR, neutrophil count, neutrophil to total leucocyte ratio, basophil count, waist circumference (WC), insulin, glucose, and HOMA-IR rates than those of controls. hsCRP levels were similar in both groups. Subgroup analyses revealed that obese PCOS group had higher insulin and HOMA-IR levels, compared to those of controls. In this subgroup, total leucocyte counts, MPV, and hsCRP levels were similar. On the other hand, lean PCOS group had higher WC, NLR, MPV, and basophil count than controls. In correlation analysis, hsCRP was positively correlated with body mass index (BMI), WC, total leucocyte count, neutrophil count, while negatively correlated with lymphocyte ratio. Although leucocyte count was positively correlated with BMI, MPV was negatively correlated with BMI, total leucocyte, platelet, and neutrophil counts. NLR was positively correlated with HOMA-IR, hsCRP, BMI, WC, and insulin. CONCLUSION: Our study demonstrated that MPV and NLR levels are increased despite similar hsCRP levels in patients with PCOS. However, we failed to demonstrate these differences in obese PCOS patients. Further studies with larger sample size are required to determine the significance of BMI in the inflammation of PCOS patients.

BACKGROUND: We report our initial gastric electrical stimulation experience using the abiliti® system for the treatment of obese patients followed for 1 year. METHOD: Between March 2011 and June 2013, 27 obese patients (BMI 30 to 46 kg/m(2)) were enrolled in a prospective open label study and implanted with a gastric stimulator. The patients were provided with nutritional support, and sensor-based behavioral feedback. RESULTS: At 12 months, percent excess weight loss (%EWL) obtained was 49.3 ± 19.2 % with no significant differences between gender or age sub-groups. The %EWL data were segmented into two groups according to BMI 30-40 kg/m(2) patients (obesity grade I and II) and BMI >40 kg/m(2), with the results of weight loss being significantly higher for the lower BMI group (59.1 ± 19.5 vs. 46.7 ± 13.4, respectively, p < 0.01). One subject requested to have his device explanted, and the minor postoperative adverse events were resolved without hospital admission. All patients experienced early satiety and reduced their intake. CONCLUSIONS: After 12 months of follow-up, gastric electrical stimulation treatment appears to be a safe and effective option for weight loss in obese subjects. Long-term follow-up and further studies are warranted.

OBJECTIVE: The aim of this study was to evaluate the usefulness of ROLL technique (Radioguided Occult Lesion Localization) as a verification method of suspicious lesions not related to breast disease found in PET-CT studies. MATERIAL AND METHODS: We retrospectively evaluated 9 patients diagnosed of cancer or with suspected tumor disease who showed hypermetabolic lymph nodes in (18)F-FDG PET-CT. Subjects underwent diagnostic testing for evaluation of treatment response in lymphoma (3), suspected recurrence in other tumors (3) or biopsy guide (3). The study group consisted of 4 women and 5 men, age range 25-72 years. ROLL technique was performed in surgically accessible lesions (supraclavicular region, lateral cervical, axillary and inguinal) with an intralesional injection of (99m)Tc-albumin macroaggregates guided by ultrasound the day before surgery. A scintigraphic study confirmed the focal tracer deposit and absence of skin contamination. During surgery, a gamma probe and portable gammacamera were used to locate lymph nodes. RESULTS: Surgical localization of radiolabeled lymph nodes was achieved in all cases with minimally invasive surgery and few postoperative complications. Histological study resulted in five tumor involvement (3 lymphoma, 1 germ cell tumor and 1 neuroendocrine carcinoma) and confirmed the existence of four false-positives in PET-CT study (1 sarcoidosis and 3 reactive follicular hyperplasia). CONCLUSION: The ROLL technique proved to be a useful and relatively simple method for the study of no breast lesions suspicious of malignancy in PET-CT study.

Intraspecific variability in parasite life cycle complexity (number of hosts and species of hosts in the life cycle) may have an impact how parasite genetic variation is partitioned among individual parasites, host individuals or host species within a given area. Among digenean trematodes, a three-host life cycle is common. However, a few species are precocious and may reach sexual maturity in what is typically regarded as the second intermediate host. The objective of this study was to determine whether a precocious life cycle predisposes digeneans to possible inbreeding or genetic subdivision among host species. As a study system, we used the digenean Proctoeces cf. lintoni whose metacercariae precociously mature (facultative) without a cyst wall in the gonads of multiple sympatric species of keyhole limpets (Fissurella spp.), typically regarded as the second intermediate hosts. Genotyped parasites were collected from four species of limpets and the clingfish Sicyases sanguineus, the third and final host where sexual maturity occurs. We found very high microsatellite diversity, Hardy-Weinberg equilibrium over all genotyped individuals, and little to no genetic structuring among parasites collected from the different host species. The fact that metacercariae do not encyst in the keyhole limpets, coupled with the high mixing potential of an aquatic environment, likely promote panmixia in local populations of P. cf. lintoni.

The preoperative study in search of multifocality, multicentricity and bilaterality is essential for the proper therapeutic management of breast cancer. The reference standard to accomplish this is breast Magnetic Resonance Imaging (MRI). Based on the literature, we adapted the thorax CT protocol for the study of the breast. The aim of this paper is to demonstrate the feasibility of CT with emphasis on the breast, as an alternative to preoperative MRI in places where it is not available. El estudio preoperatorio en búsqueda de multifocalidad, multicentricidad y bilateralidad es fundamental para el adecuado manejo terapéutico del cáncer (CA) mamario. El estándar de referencia para realizarlo es la resonancia magnética (RM) mamaria. Basados en la literatura, adaptamos el protocolo de TC de tórax para el estudio de la mama. El objetivo de este trabajo es demostrar la factibilidad de la TC con énfasis en mama como método alternativo a la RM pre-operatoria, en lugares donde ésta no está disponible. La resonancia magnética (RM) mamaria se está convirtiendo en una herramienta de trabajo frecuentemente utilizada en nuestro medio. Existe un grupo de lesiones que sólo pueden ser identificadas por esta técnica, RMI only. Entre 14 a 20 por ciento de ellas serán malignas, según las diferentes series publicadas. Este tipo de lesiones requieren de biopsia guiada bajo RM. Pacientes y Métodos: se realizó revisión retrospectiva descriptiva de las biopsias asistidas por vacío realizadas en nuestra institución (período entre agosto de 2008 y junio de 2013). Resultados: El 0,3 por ciento (11) de las biopsias realizadas en nuestra institución fueron realizadas bajo RM, en 9 mujeres. En el 55 por ciento de los casos la indicación de la resonancia en la que se detectó la lesión biopsiada, fue etapificación de cáncer mamario recientemente diagnosticado; en el 100 porciento de los casos se realizó ultrasonido de segunda mirada, las lesiones tenían un tamaño promedio de 15 mm (4-29), un 63,6 por ciento fueron nódulos y el resto captaciones tipo no masa. La duración de la biopsia varió entre 40 y 130 minutos, un 27,2 por ciento resultaron lesiones malignas; 36,4 por ciento lesiones de alto riesgo; y 36,4 por ciento benignas. En el 45,5 por ciento el resultado de la biopsia bajo resonancia cambia la conducta quirúrgica. De las 7 lesiones operadas hubo subestimación en un caso de cáncer ductal in situ, que resultó cáncer ductal infiltrante. En el resto la histología quirúrgica fue idéntica a la de la biopsia bajo resonancia. Conclusión: Las biopsias bajo resonancia son infrecuentes, consumen un tiempo considerable, su correlación histológica es muy confiable y cambia la conducta quirúrgica casi en la mitad de los casos, lo cual tiene implicancias en el pronóstico de la paciente.


Abstract. Breast MRI is becoming a frequently used working tool in our environment. A group of lesions exist that can only be identified by this technique, "MRI only". Between 14-20 % of these will be malignant, according to various published series. Such lesions require biopsy guided under MRI. Patients and Methods: A descriptive retrospective review of vacuum-assisted biopsies was performed at our institution (period between August 2008 and June 2013). Results: 0.3 % (11) of the biopsies performed at our institution were done so under MRI, in 9 women. In 55% of the cases the resonance indication in which the biopsied lesion was detected, was newly diagnosed staging of breast cancer; in 100% of the cases a second-look ultrasound was performed, the lesions had an average size of 15 mm (4-29), 63.6% were nodules and the remainder other non-mass type deposits. The duration of the biopsy varied between 40 and 130 min, 27.2% were malignant lesions, 36.4 % high-risk lesions, and 36.4 % benign. In 45.5% the result of the biopsy performed under MRI changed the surgical procedure. Of the 7 operated lesions there was an underestimation in one case of ductal carcinoma in situ, which resulted being invasive ductal carcinoma. In the remainder, the surgical histology was identical to that of the biopsy performed under magnetic resonance. Conclusion: Biopsies performed under MRI are infrequent, they take considerable time, their histological correlation is very reliable and it changes the surgical procedure in almost half of the cases, which has implications in the prognosis of the patient.
