BACKGROUND: Melanoma mortality rates in the US are highest among older men, individuals of lower socioeconomic status (SES), and people of color. To better understand these inequities, a qualitative exploratory study was conducted in Northern and Southern California to generate knowledge about barriers and facilitators of awareness, prevention, and early detection of melanoma in lower SES Latinx and non-Latinx White (NLW) individuals living in urban and semi-rural areas. METHODS: Nineteen focus groups were conducted (N = 176 adult participants), stratified by race/ethnicity (Latinx, low-income NLW), geography (semi-rural, urban), and language (English and Spanish). Inductive and deductive thematic analysis was conducted, and the findings were organized using the socioecological model framework: individual, interpersonal, community, and health system/policy levels. RESULTS: Four socioecological themes describe how key factors affect knowledge, perceived risk, preventive behaviors, and melanoma screening. Individual level findings revealed that many participants were not familiar with melanoma, yet were willing to learn through trusted sources. Having brown or darker skin tone was perceived as being associated with lower risk for skin cancer. Interpersonally, social relationships were important influences for skin cancer prevention practice. However, for several Latinx and semi-rural participants, conversations about melanoma prevention did not occur with family and peers. At the community level, semi-rural participants reported distance or lack of transportation to a clinic as challenges for accessing dermatology care. Healthcare systems barriers included burdens of additional healthcare costs for dermatology visits and obtaining referral. CONCLUSIONS: Varying factors influence the awareness levels, beliefs, and behaviors associated with knowledge, prevention, and early detection of melanoma among low-income Latinx and NLW individuals and in semi-rural areas. Results have implications for health education interventions. Navigation strategies that target individuals, families, and health care settings can promote improved prevention and early detection of melanoma in these communities.

OBJETIVE: To review and discuss the current evidence of the use of metformin as a therapeutic tool in frequent skin diseases. DESIGN: Original article. Qualitative research. Narrative review. LOCATION: Aragon and Murcia, Spain. PARTICIPANTS: Resident Physicians. Dermatology and Primary Health Care. METHOD: A narrative review has been carried out using the PubMed bibliographic database, being the search date the 27th of January of 2022. RESULTS: Metformin has proven to be effective in the treatment of inflammatory skin diseases such as acne, hidradenitis suppurativa, psoriasis and allergic contact dermatitis. It has also shown antitumor properties regarding basal cell carcinoma, squamous cell carcinoma and melanoma. Additionally, beneficial effects of adjuvant treatment with metformin have been described in patients with basal cell carcinoma receiving photodynamic therapy. In patients with endocrinology-related dermatosis such as hirsutism, acanthosis nigricans and eruptive xanthomas, treatment with metformin has demonstrated therapeutic effectiveness. Topical treatment with metformin has also been useful in the treatment of melasma. Finally, it has been proposed as a drug with anti-aging and wound-healing promoting properties. Severe adverse effects have not been observed for any of the previously described indications, being this a well-tolerated treatment. CONCLUSIONS: Metformin is an effective and safe adjuvant in the therapeutic scheme of various inflammatory dermatoses, skin neoplasms, endocrinology-related dermatosis, melasma, skin aging and wound healing processes.

BACKGROUND: Community-based physical activity (PA) programs are appealing to women in Latin America and show potential for improving women's health. This study aimed to engage healthy middle-aged women, breast cancer survivors and local stakeholders participating in two publicly funded community-based PA programs in Bogotá, Colombia (Recreovía and My Body) to assess and visually map the perceived barriers, facilitators, and outcomes to promote programs' improvement, scaling and sustainability. METHODS: We used two participatory action research methods, the 1) Our Voice citizen science method to capture data and drive local change in built and social environmental facilitators and barriers that influence women's engagement in community-based PA; and 2) Ripple Effects Mapping to visually map the intended and unintended outcomes of PA programs. We used thematic analysis to classify the results at the individual, social, and community levels. RESULTS: The stakeholders engaged in the participatory evaluation included cross-sector actors from the programs (N = 6) and program users (total N = 34) from the two programs (Recreovía N = 16; My Body N = 18). Program users were women with a mean age of 55.7 years (SD = 8.03), 65% lived in low-income neighborhoods. They identified infrastructure as the main feature affecting PA, having both positive (e.g., appropriate facilities) and negative (e.g., poorly built areas for PA) effects. Regarding program improvements, stakeholders advocated for parks' cleaning, safety, and appropriate use. The most highlighted outcomes were the expansion and strengthening of social bonds and the engagement in collective wellbeing, which leveraged some participants' leadership skills for PA promotion strategies in their community. The facilitated dialogue among program users and stakeholders fostered the sustainability and expansion of the community-based PA programs, even during the COVID-19 pandemic. CONCLUSIONS: The implementation of both participatory methodologies provided a multidimensional understanding of the programs' impacts and multisectoral dialogues that fostered efforts to sustain the community-based PA programs.

Physical inactivity is a key risk factor for a range of chronic diseases and conditions, yet, approximately 50% of U.S. adults fall below recommended levels of regular aerobic physical activity (PA). This is particularly true for ethnic minority populations such as Latino adults for whom few culturally adapted programs have been developed and tested. Text messaging (SMS) represents a convenient and accessible communication channel for delivering targeted PA information and support, but has not been rigorously evaluated against standard telehealth advising programs. The objective of the On The Move randomized controlled trial is to test the effectiveness of a linguistically and culturally targeted SMS PA intervention (SMS PA Advisor) versus two comparison conditions: a) a standard, staff-delivered phone PA intervention (Telephone PA Advisor) and b) an attention-control arm consisting of a culturally targeted SMS intervention to promote a healthy diet (SMS Nutrition Advisor). The study sample (N = 350) consists of generally healthy, insufficiently active Latino adults ages 35 years and older living in five northern California counties. Study assessments occur at baseline, 6, and 12 months, with a subset of participants completing 18-month assessments. The primary outcome is 12-month change in walking, and secondary outcomes include other forms of PA, assessed via validated self-report measures and supported by accelerometry, and physical function and well-being variables. Potential mediators and moderators of intervention success will be explored to better determine which subgroups do best with which type of intervention. Here we present the study design and methods, including recruitment strategies and yields. Trial Registration: clinicaltrial.gov Identifier = NCT02385591.

We describe the association of caspase-dependent and caspase-independent mechanisms in macrophage apoptosis induced by LpqH, a 19 kDa Mycobacterium tuberculosis lipoprotein. LpqH triggered TLR2 activation, with upregulation of death receptors and ligands, which was followed by a death receptor signaling cascade with activation of initiator caspase 8 and executioner caspase 3. In this caspase-mediated phase, mitochondrial factors were involved in loss of mitochondrial transmembrane potential (ΔΨm), release of cytochrome c, and caspase 9 activation. Interestingly, a caspase-independent pathway was also identified; by immunoblot, the mitochondrial apoptosis inducing factor (AIF) was demonstrated in nuclei and cytosol of LpqH-treated macrophages. Confocal microscopy revealed translocation of AIF to the nuclei of the majority of apoptotic cells. These findings emphasize the complex and redundant nature of the macrophage death response to mycobacteria.

OBJECTIVE: To construct a scale for assessing the quality of mealtime habits in a sample of urban Mexican adults, computing the contribution of a set of advisable and unadvisable mealtime habits. MATERIAL AND METHODS: We performed an exploratory factor analysis among 7 472 adults participating in the baseline assessment of the Health Workers Cohort Study, to assess the mealtime habits quality. Likelihood ratio test for difference of two probabilities and test for the difference of two means were used to identify differences between low and high categories of the Mealtime Habits Quality Scale (MHQS) across variables of interest. RESULTS: Participants with the top quality of mealtime habits showed lower rates of overweight, obesity, abdominal obesity, and elevated body fat. They were also more adherent to a prudent dietary pattern than a western dietary pattern, and consumed more fruits and vegetables. CONCLUSIONS: Anthropometric and dietary variables differed across MHQS categories. However, further validation of the scale, and assessment of their ability to predict weight gain or related diseases are needed, using prospective and intervention studies.


OBJETIVO: Construir una escala para evaluar la calidad de los hábitos al comer, calculando la contribución de un grupo de hábitos recomendables y no recomendables, en población adulta urbana de México. MATERIAL Y MÉTODOS: Realizamos un análisis exploratorio de factores en 7 472 adultos participantes en el Estudio de Cohorte de Trabajadores de la Salud para evaluar la calidad de los hábitos al comer. Para identificar diferencias entre la baja y alta calidad de los hábitos al comer a través de las variables de interés, utilizamos la prueba de razón de probabilidades a fin de evaluar la diferencia entre dos proporciones y la prueba de comparación de medias. RESULTADOS: Los participantes clasificados en la categoría de alta calidad de los hábitos al comer presentaron prevalencias más bajas de sobrepeso, obesidad, obesidad abdominal y porcentaje de grasa corporal elevado. Además, mostraron mayor adherencia al patrón dietario prudente que al patrón dietario occidental, así como mayor consumo de frutas y verduras. CONCLUSIONES: Las variables antropométricas y de dieta muestran diferencias a través de las categorías de la escala de la calidad de hábitos al comer. Sin embargo, será necesario validar la escala y evaluar su capacidad para predecir ganancia de peso o enfermedades relacionadas, mediante estudios prospectivos o de intervención.

Intraspinal hemorrhage is very rare and intramedullary hemorrhage, also called "hematomyelia," is the rarest form of intraspinal hemorrhage. There are few reports in medical literature. We report the case of a woman of 43 years with diagnosis of systemic lupus erythematosus and secondary antiphospholipid syndrome under anticoagulant therapy that presented hematomyelia. We describe the clinical course, the findings in the image studies and surgery, and the available information in the literature.

A procedure for pineapple [Ananas comosus (L.) Merr.] genetic transformation is described, which involves temporary immersion bioreactors (TIB) for selection of transgenic plants. Success in the production of transgenic pineapple plants combines tissue culture factors. Firstly, the use of regenerable pineapple callus as starting material for transformation whose cells shown to be competent for Agrobacterium infection. Secondly, the used of filtered callus, resulting in homogeneously sized clusters, thereby increasing the contact between the cell surfaces and A. tumefaciens and releasing phenolic compounds which induce Agrobacterium virulence. Thirdly, regeneration of primary plants without selection pressure, that allowing a massive production of putative transgenic pineapples. Finally, we support that TIB technology is a powerful system to recover nonchimera transgenic plants by micropropagation with the use of an adequate selection agent.

Para conocer la prevalencia de aterosclerosis en personas mayores de 60 años, se realizó ultrasonografía de alta resolución y Doppler con imagen de flujo codificada a color, de las arterias carótidas a participantes del proyecto CUPA (estudio de vigilancia epidemiológica iniciado en 1989 en el Distrito Federal). En cada participante se evaluó la presencia de las siguientes lesiones ateroscleróticas: 1) engrosamiento de la última y la media; 2) acodamientos y tortuosidades; 3) placas de ateroma no estenosantes y 4) estenosis carotídea significativa (50 por ciento). El análisis de 198 estudios llevados a cabo en 56 hombres y 142 mujeres mostró que 130 personas presentaron alguna de las lesiones investigadas (65.6 por ciento) y que su frecuencia aumenta con la edad, de tal forma que en menores de 65 años fue de 38 por ciento, en los sujetos de 65 a 74 años de 71 por ciento y en mayores de 75 años de 88 por ciento. La frecuencias de lesiones estenóticas avanzadas fue más bien baja (6 por ciento), mientras que la frecuencia global de las placas de ateroma no estenosante y el engrosamiento de la íntima y la media fue mayor del 60 por ciento. Este último fue mas usual en hombres, mientras que las placas no estenosantes y la estenosis de mayor grado, se observaron con más frecuencia en las mujeres. Sin embargo, no hubo diferencias significativas entre hombres y mujeres al analizarse las lesiones por grupos de edad. Es el primer estudio sobre la prevalencia de aterosclerosis carotídea en México basado en evaluación ultrasonográfica. Los hallazgos de la investigación documentan una elevada frecuencia de ateroclerosis entre residentes de 60 años de la ciudad de México
