RESUMO

The rail industry in Australia screens workers for probable obstructive sleep apnea (OSA) due to known safety risks. However, existing criteria to trigger screening only identify a small proportion of workers with OSA. The current study sought to examine the relationship between OSA risk and rail incidents in real-world data from Australian train drivers, and conducted a proof of concept analysis to determine whether more conservative screening criteria are justified. Health assessment (2016-2018) and subsequent rail incident data (2016-2020) were collected from two passenger rail service providers. Predictors included OSA status (confirmed no OSA with a sleep study, controlled OSA, unknown OSA [no recorded sleep assessment data] and confirmed OSA with no indication of treatment); OSA risk according to the current Standard, and OSA risk according to more conservative clinical markers (BMI threshold and cardiometabolic burden). Coded rail safety incidents involving the train driver were included. Data were analysed using zero-inflated negative binomial models to account for over-dispersion with high 0 counts, and rail safety incidents are reported using Incidence Risk Ratios (IRRs). A total of 751 train drivers, typically middle-aged, overweight to obese and mostly men, were included in analyses. There were 43 (5.7%) drivers with confirmed OSA, 62 (8.2%) with controlled OSA, 13 (1.7%) with confirmed no OSA and 633 (84.4%) drivers with unknown OSA. Of the 633 train drivers with unknown OSA status, 21 (3.3%) met 'at risk' criteria for OSA according to the Standard, and incidents were 61% greater (IRR: 1.61, 95% Confidence Interval (CI) 1.02-2.56) in the years following their health assessment compared to drivers who did not meet 'at risk' criteria. A more conservative OSA risk status using lower BMI threshold and cardiometabolic burden identified an additional 30 'at risk' train drivers who had 46% greater incidents compared to drivers who did not meet risk criteria (IRR (95% CI) 1.46 (1.00-2.13)). Our more conservative OSA risk criteria identified more workers, with greater prospective incidents. These findings suggest that existing validated tools could be considered in future iterations of the Standard in order to more sensitively screen for OSA.

OBJECTIVES: Up to a quarter of the world's population experience chronic pain, which, in addition to interfering with daily activities and waking function, is often associated with poor sleep. Individuals experiencing poor sleep are often encouraged to implement sleep hygiene strategies. However, current sleep hygiene strategies have not been developed considering the unique challenges faced by individuals with chronic pain and therefore they might not be as effective in this population. The aim of this scoping review is to map the state of the existing literature examining sleep hygiene strategies in individuals with chronic pain. DESIGN: This scoping review included a search of four online databases (Medline, Embase, PsycINFO and CINAHL) to identify articles examining the use of sleep hygiene strategies in populations with chronic pain. RESULTS: Thirty articles investigated at least one sleep hygiene strategy in individuals with chronic pain, with improvements to sleep reported for six sleep hygiene strategies (education, exercise, limiting alcohol use, limiting tobacco use, prebed state and sleep environment). However, the timing of these strategies was often not reported which limits the degree to which these strategies can be generalised for use as a presleep strategy. CONCLUSION: This scoping review examined the existing literature focusing on sleep hygiene strategies for people with chronic pain. There are limitations to the methodology of the existing literature and gaps in our understanding of sleep hygiene strategies in some chronic pain conditions that must be addressed in future research before the effectiveness of these strategies can be understood.

Shift work is known to be associated with poor health outcomes, however our understanding of health behaviours (sleep, physical activity and nutritional intake) longitudinally in shift workers is currently limited. Systematic searches of four data bases were conducted. Using PRISMA-ScR guidelines we report a scoping review of 15 eligible studies. Of the included studies, 11 studies examined sleep outcomes, three examined physical activity and two examined nutritional intake. The number of follow ups conducted in each study varied from one to six, with the majority of studies reporting one follow up. Study length varied from six months to 16 years. Findings suggest that shift workers have stable but largely insufficient sleep longitudinally. Many shift workers, particularly inexperienced shift workers, are additionally experiencing poor sleep quality. There is limited data longitudinally on physical inactivity and nutrition intake, but included studies indicate that shift workers may also be physical inactive and with some poor nutritional intake (i.e., high levels of saturated fat intake). Longitudinal studies of shift work with multiple follow-ups are needed to address the current gaps in literature. The evidence that shift workers may be susceptible to poor health behaviours longitudinally is important as it provides a means by which strategies and interventions can be targeted, at both individual and organisational levels, to assist in better long term health outcomes.

OBJECTIVES: This study investigated the effect restricted sleep has on wildland firefighters' acute cytokine levels during 3 days and 2 nights of simulated physical wildfire suppression work. METHODS: Firefighters completed multiple days of physical firefighting work separated by either an 8-h (Control condition; n = 18) or 4-h (Sleep restriction condition; n = 17) sleep opportunity each night. Blood samples were collected 4 times a day (i.e., 06:15, 11:30, 18:15, 21:30) from which plasma cytokine levels (IL-6, IL-8, IL-1ß, TNF-α, IL-4, IL-10) were measured. RESULTS: The primary findings for cytokine levels revealed a fixed effect for condition that showed higher IL-8 levels among firefighters who received an 8-h sleep each night. An interaction effect demonstrated differing increases in IL-6 over successive days of work for the SR and CON conditions. Fixed effects for time indicated that IL-6 and IL-4 levels increased, while IL-1ß, TNF-α and IL-8 levels decreased. There were no significant effects for IL-10 observed. CONCLUSION: Findings demonstrate increased IL-8 levels among firefighters who received an 8-h sleep when compared to those who had a restricted 4-h sleep. Firefighters' IL-6 levels increased in both conditions which may indicate that a 4-h sleep restriction duration and/or period (i.e., 2 nights) was not a significant enough stressor to affect this cytokine. Considering the immunomodulatory properties of IL-6 and IL-4 that inhibit pro-inflammatory cytokines, the rise in IL-6 and IL-4, independent of increases in IL-1ß and TNF-α, could indicate a non-damaging response to the stress of simulated physical firefighting work. However, given the link between chronically elevated cytokine levels and several diseases, further research is needed to determine if firefighters' IL-8 and IL-6 levels are elevated following repeated firefighting deployments across a fire season and over multiple fire seasons.

There are many factors that may affect the sleep behaviour and subsequent fatigue risk of shift workers. In the Australian rail industry the emphasis is primarily on the impact of working time on sleep. The extent to which factors other than working time might affect the sleep behaviour of employees in the large and diverse Australian rail industry is largely unknown. The present study used sleep, work and fatigue diaries completed for two weeks, in conjunction with actigraphy, to understand the contribution of demographic and health factors to sleep behaviour in 40 rail safety workers. Both shift type and having dependents were significant predictors of sleep duration (P<.05). Sleep duration was greatest prior to night shifts, followed by afternoon shifts and morning shifts. Participants with dependents got significantly less sleep than participants without dependents. Both timing of sleep and smoking were significant predictors of sleep quality (P<.05). Day sleeps were associated with lower subjective sleep quality than night sleeps and smokers reported poorer sleep quality than non-smokers. These findings indicate that factors other than working time have the potential to influence both the sleep duration and subjective sleep quality of rail safety workers.
