BACKGROUND: Injury of the bile duct during cholecystectomy (CHE) is a severe complication. The critical view of safety (CVS) can help to reduce the frequency of this complication during laparoscopic CHE. So far, no scoring of CVS images with a grading system is available. METHOD: The CVS images of 534 patients with laparoscopic CHE could be structurally analyzed and assessed with marks from 1 (very good) to 5 (insufficient). The CVS mark was correlated with the perioperative course. Additionally, the perioperative course of patients after laparoscopic CHE with and without a CVS image was investigated. RESULTS: In 534 patients 1 or more CVS images could be analyzed. The average CVS mark was 1.9, whereby 280 patients (52.4%) had a 1, 126 patients (23.6%) a 2, 114 (21.3%) a 3 and 14 patients (2.6%) a 4 or 5. Younger patients with elective laparoscopic CHE had CVS images significantly more frequently (p ≤ 0.04). The statistical examination with Pearson's χ2-test and the F­test (ANOVA) showed a significant correlation between improving CVS marks and reduction of surgery time (p < 0.01) and the hospitalization time (p < 0.01). For senior physicians the quota of CVS images ranged from 71% to 92% and the average marks from 1.5 to 2.2. The marks for the CVS images were significantly better for female than male patients (1.8 vs. 2.1, p < 0.01). DISCUSSION: There was a relatively broad distribution of marks for CVS images. Injuries of the bile duct can be avoided with a high degree of certainty with marks 1­2 for the CVS image. The CVS is not always adequately visualized in laparoscopic CHE.

BACKGROUND: The COVID-19 pandemic made substantial changes in medical care necessary. The aims of this study were to find out what influence the pandemic had on the perioperative course in patients with cholecystectomy (CHE) and to highlight possible residual consequences. METHOD: From 1 July 2018 to 31 December 2021 a total of 735 patients with CHE were analyzed. Up to 21 March 2020 patients were assigned to the regular patient group (Reg, n = 430), patients after this date (first lockdown 22 March 2020) to the Cov19 patient group (Cov19, n = 305) and the 2 groups were compared. RESULTS: The average age of all patients was 59 years and 63% were women. The average length of hospitalization (KrVD, time period between surgery and discharge) was 4.4 days. The patient groups Reg and Cov19 did not differ with respect to age, gender or KrVD. The total number of CHEs carried out was reduced by 21.4% in the Cov19 group. This affected elective and emergency CHE to the same extent. The length of surgery significantly increased in the Cov19 group from 64 min (SD 34 min) to 71 min (SD 38 min). The number of short and long hospital stays (KrVD 2 or >4 days) significantly increased in the Cov19 group from 4 % to 20 % (short stay, p < 0.01) and from 23 % to 27 % (long stay, p < 0.01). This was particularly observed for patients >70 years old with an increase in long stays from 43 % to 56 % in the Cov19 group. CONCLUSION: The COVID-19 pandemic led to a clear reduction in CHE both for elective and emergency interventions. Furthermore, a significant lengthening of the surgery and hospitalization times could be observed for older patients. The residual consequences of the pandemic could be shortened hospitalization times after uncomplicated CHE and more interventional treatment procedures in complex cases.

Woody species were surveyed, and soil and topographic variables measured, in 30 10×40 m plots systematically distributed, with 15 plots in each physiognomy. We found a spatially structured distribution of woody species, and differences of soil traits between cerrado stricto sensu and dry forest areas, mainly related to the aluminum saturation, base saturation, and available phosphorus. Aluminum saturation increased toward the savanna area, while base saturation increased toward the dry forest. Most woody species predominated in one physiognomy, such as Callisthene major in the cerrado stricto sensu and Anadenanthera colubrina in the dry forest. Only 20% of the species were widely distributed across both physiognomies or, not often, restricted to the intermediary values of the soil gradient. General results indicate that contrasting soil traits between cerrado stricto sensu and dry forest produce a strongly spatially organized and sharp transition in terms of species distribution between these physiognomies.

Abstract Urban agriculture is very common in the cities, as Rio de Janeiro and adjacencies, being important as economic activity. However, this practice can create appropriate conditions for establishment and maintenance of life cycle of many parasites. Lymneid snails can act as intermediate hosts of many trematodes species, including those that infect humans and wild and domestic. In the present study, the snail Lymnaea columella was collected from an urban watercress plantations and the morphology of cercariae released by them was analyzed by light and scanning electron microscopy. The specimens were identified as xiphidiocercariae, belonging to the Cercariae Ornatae group, of the order Plagiorchiiformes. Histological examination of the digestive gland of the infected snails revealed the presence of developing sporocysts, with hemocitary infiltration and metaplasia in the epithelial tissue.


Resumo Agricultura urbana é muito comum nas cidades, como Rio de Janeiro e adjacências, sendo uma importante atividade econômica de subsistência. No entanto, esta prática pode criar condições adequadas para o estabelecimento e manutenção do ciclo de vida de muitos parasitos. Os moluscos limneídeos podem atuar como hospedeiros intermédios de muitas espécies de trematódeos, incluindo aqueles que infectam humanos e animais selvagens e domésticos. No presente estudo, o molusco Lymnaea columella foi coletado de plantações urbanas de agrião e a morfologia das cercárias liberadas por eles foi analisada por microscopia eletrônica de varredura leve e de varredura. As larvas foram identificadas como xifidiocercárias, pertencentes ao grupo das Cercariae Ornatae, Ordem Plagiorchiiformes. Análises histológicas da glândula digestiva dos moluscos infectados evidenciaram esporocistos, com formação de granuloma e metaplasia do tecido epitelial.

Emissions of C2-C5 alkanes from the U.S. oil and gas sector have changed rapidly over the last decade. We use a nested GEOS-Chem simulation driven by updated 2011NEI emissions with aircraft, surface and column observations to 1) examine spatial patterns in the emissions and observed atmospheric abundances of C2-C5 alkanes over the U.S., and 2) estimate the contribution of emissions from the U.S. oil and gas industry to these patterns. The oil and gas sector in the updated 2011NEI contributes over 80% of the total U.S. emissions of ethane (C2H6) and propane (C3H8), and emissions of these species are largest in the central U.S. Observed mixing ratios of C2-C5 alkanes show enhancements over the central U.S. below 2 km. A nested GEOS-Chem simulation underpredicts observed C3H8 mixing ratios in the boundary layer over several U.S. regions and the relative underprediction is not consistent, suggesting C3H8 emissions should receive more attention moving forward. Our decision to consider only C4-C5 alkane emissions as a single lumped species produces a geographic distribution similar to observations. Due to the increasing importance of oil and gas emissions in the U.S., we recommend continued support of existing long-term measurements of C2-C5 alkanes. We suggest additional monitoring of C2-C5 alkanes downwind of northeastern Colorado, Wyoming and western North Dakota to capture changes in these regions. The atmospheric chemistry modeling community should also evaluate whether chemical mechanisms that lump larger alkanes are sufficient to understand air quality issues in regions with large emissions of these species.

3D printers are increasingly run at home. Nanoparticle emissions from those printers have been reported, which raises the question whether adverse health effects from ultrafine particles (UFP) can be elicited by 3D printers. We exposed 26 healthy adults in a single-blinded, randomized, cross-over design to emissions of a desktop 3D printer using fused deposition modeling (FDM) for 1 hour (high UFP-emitting acrylonitrile butadiene styrene [ABS] vs low-emitting polylactic acid [PLA]). Before and after exposures, cytokines (IL-1ß, IL-6, TNF-α, INF-γ) and ECP in nasal secretions, exhaled nitric oxide (FeNO), urinary 8-isoprostaglandin F2α (8-iso PGF2α ), and self-reported symptoms were assessed. The exposures had no significant differential effect on 8-iso PGF2α and nasal biomarkers. However, there was a difference (P < .05) in the time course of FeNO, with higher levels after ABS exposure. Moreover, indisposition and odor nuisance were increased for ABS exposure. These data suggest that 1 hour of exposure to 3D printer emissions had no acute effect on inflammatory markers in nasal secretions and urine. The slight relative increase in FeNO after ABS printing compared to PLA might be due to eosinophilic inflammation from inhaled UFP particles. This possibility should be investigated in further studies using additional biomarkers and longer observation periods.

Malignancy is a leading cause of morbidity and mortality in organ transplant recipients who receive immunosuppression. Cancers associated with viruses such as nonmelanotic skin cancer and Kaposi sarcoma occur in allograft recipients at rates that far exceed that in the general population. The increased risk and tumor type may depend not only on degree of immune system modulation but also on the type of organ transplanted. In kidney transplant recipients, the risk of cancers such as prostate and breast does not seem to be increased. However, these cancers tend to be advanced and aggressive. The management of these cancers is similar to the general population with the additional consideration for reduction in immunosuppression and conversion to sirolimus. Given the increased survival of both transplanted organs as well as organ recipients along with the increased number of older recipients, the diagnosis of prostate cancer in the older male organ recipient is increasing. The long-term outcomes using current treatment guidelines for prostate cancer in these individuals are not clear. We report a case of known localized prostate cancer in a renal transplant recipient presenting with metastasis diagnosed as tumor infiltration of the allograft. Our patient, upon initial diagnosis of cancer, opted for radiation with eventual androgen-deprivation therapy. This unusual site of prostate cancer spread heightens the need for awareness among providers as well as the need for further studies of the outcomes in these patients undergoing treatments designed using guidelines developed for those with normally functioning immunity.

Antibody-drug conjugates (ADCs) have demonstrated clinical benefits that have led to the recent FDA approval of KADCYLA and ADCETRIS. Most ADCs that are currently in clinical use or development, including ADCETRIS, are produced by chemical conjugation of a toxin via either lysine or cysteine residues, inevitably leading to heterogeneous products with variable drug-to-antibody ratios (DARs). Here, we describe the in vitro and in vivo characterization of four novel ADCs that are based on the anti-CD30 antibody cAC10, which has the same polypeptide backbone as ADCETRIS, and compare the results with the latter. Bacterial transglutaminase (BTG) was exploited to site-specifically conjugate derivatives of monomethyl auristatin E (all comprising a cleavable linker) to the glutamine at positions 295 and 297 of cAC10, thereby yielding homogeneous ADCs with a DAR of 4. In vitro cell toxicity experiments using two different CD30-positive cell lines (Karpas 299 and Raji-CD30(+)) revealed comparable EC50 values for ADCETRIS (1.8 ± 0.4 and 3.6 ± 0.6 ng/mL, respectively) and the four cAC10-based ADCs (2.0 ± 0.4 to 4.9 ± 1.0 ng/mL). Quantitative time-dependent in vivo biodistribution studies (3-96 h p.i.) in normal and xenografted (Karpas 299 cells) SCID mice were performed with a selected (125)I-radioiodinated cAC10 ADC and compared with that of (125)I-ADCETRIS. The chemo-enzymatically conjugated, radioiodinated ADC showed higher tumor uptake (17.84 ± 2.2% ID/g 24 h p.i.) than (125)I-ADCETRIS (10.5 ± 1.8% ID/g 24 h p.i.). Moreover, (125)I-ADCETRIS exhibited higher nontargeted liver and spleen uptake. In line with these results, the maximum tolerated dose of the BTG-coupled ADC (>60 mg/kg) was significantly higher than that of ADCETRIS (18 mg/kg) in rats. These results suggest that homogeneous ADCs display improved pharmacokinetics and better therapeutic indexes compared to those of chemically modified ADCs with variable DARs.

Primary aldosteronism (PA) is the most frequent cause of secondary arterial hypertension. The aldosterone to renin ratio (ARR) is the gold standard for screening, but variability between biochemical methods used remains of concern. The aim of the study was to analyze center-specific features of biochemical diagnostic strategies prior to the 2008 consensus within the German Conn's Registry. The study was designed as a retrospective study in 5 tertiary care hospitals. Patients analyzed for PA between 1990 and 2006 were studied. Characteristics of the assays used to determine ARR during establishing the diagnosis of PA were analyzed in the retrospective part of the German Conn's Registry. Eighty-six out of 484 documented ARR values had to be excluded from further evaluations because the laboratory or the assays were unknown. In the remaining 398 patients ARR was determined using 10 different assay combinations in the centers (aldosterone plus plasma renin activity or concentration). Considerable differences were seen between the mean concentrations for aldosterone (p<0.0001), renin concentration (p<0.001), and renin activity (p=0.009) for the different assays. The differences between the absolute concentrations measured by the different assays also had significant impact upon the resulting mean ratios. If published cutoff values are applied, the use of different commercial assays to determine the ARR in clinical routine results in major differences in positive screening rates. This heterogeneity affects sensitivity and specificity of screening for PA. Our data emphasize the importance of standardized screening procedures, which must include standardization of biochemical methods.

Autonomous secretion of adrenal hormones can follow a subclinical course or even be masked by other frequent diseases. Patients with incidentally discovered adrenal masses (incidentaloma) represent another diagnostic challenge. Their frequency has increased through the growing number of medical imaging procedures. Although the proportion of malignant or hormonally active lesions is low, patients with adrenal incidentalomas (> 1cm) should undergo an endocrine work-up in order to detect subclinical courses of Cushing's syndrome, pheochromocytoma or primary aldosteronism. Enhanced CT is of help in the assessment of the dignity of discovered lesions. In order to evaluate the hormonal activity, it is recommended to perform low dose dexamethason suppression test and to determine the plasma metanephrines and the aldosterone to renin ratio (in hypertensive patients). The therapy of suclinical Cushing's syndrome (operative vs. medicamentous therapy) remains a single-case decision in the absence of randomised prospective studies. All tumors with criterias for malignancy (> 4cm) and subclinical pheochromocytoma and aldosterone producing adenomas should undergo surgery. In the case of non operated tumors annual bichemical follow-up controls should be performed over a duration of 5 years. In the case of a significant gain of tumor size during follow-up, adrenalectomy should be considered.

Feeding of Pitangus sulphuratus (Tyrannidae) nestlings have been poorly studied. Here we describe the foraging behavior of a P. sulphuratus pair and the searching and offering time of food items to nestlings in the Pantanal, Brazil. Data collection was carried out over 25 days on the outskirts of the Base de Estudos do Pantanal building, inhabited by insectivorous bats. Records were based on direct observations with the help of binocular. The pair required little time for searching for small insects and fruits, but these items comprised a little amount of food per event of capture. Some large prey was more time-costly for searching, but the long period that these food items were offered to nestlings overcompensated the searching time. Considering the time of feeding nestlings (benefit) in relation to the searching time by the parents (cost), bats and snails are the most advantageous items for P. sulphuratus parents feeding nestlings at the study site.


A alimentação de ninhêgos de Pitangus sulphuratus (Tyrannidae) tem sido pouco estudada. Descrevemos, neste estudo, o comportamento de forrageamento de um casal de P. sulphuratus e o tempo para busca e oferta de itens aos ninhêgos, no Pantanal, Brasil. A coleta de dados foi realizada durante 25 dias nas proximidades do prédio da Base de Estudos do Pantanal, habitada por morcegos insetívoros. Os registros foram baseados em observações diretas com auxílio de binóculo. O casal demandou pouco tempo de busca para a captura de pequenos insetos e frutos, porém esses itens representaram pouca quantidade de alimento por evento de captura. Algumas presas grandes demandaram mais tempo para busca, mas o longo período que esses itens foram oferecidos aos ninhêgos sobrecompensou o tempo de busca. Considerando-se o tempo de alimentação dos ninhêgos (benefício) em relação ao tempo de busca pelos pais (custo), morcegos e caramujos são itens mais vantajosos para os pais de P. sulphuratus alimentar ninhêgos no local de estudo.

AIMS/HYPOTHESIS: Non-invasive diagnostic tools specific for pancreatic beta cells will have a profound impact on our understanding of the pathophysiology of metabolic diseases such as diabetes. The objective of this study was to use molecular imaging probes specifically targeting beta cells on human samples and animal models using state-of-the-art imaging modalities (fluorescence and PET) with preclinical and clinical perspective. METHODS: We generated a monoclonal antibody, 8/9-mAb, targeting transmembrane protein 27 (TMEM27; a surface N-glycoprotein that is highly expressed on beta cells), compared its expression in human and mouse pancreas, and demonstrated beta cell-specific binding in both. In vivo imaging was performed in mice with subcutaneous insulinomas overexpressing the human TMEM27 gene, or transgenic mice with beta cell-specific hTMEM27 expression under the control of rat insulin promoter (RIP-hTMEM27-tg), using fluorescence and radioactively labelled antibody, followed by tissue ex vivo analysis and fluorescence microscopy. RESULTS: Fluorescently labelled 8/9-mAb showed beta cell-specific staining on human and mouse pancreatic sections. Real-time PCR on islet cDNA indicated about tenfold higher expression of hTMEM27 in RIP-hTMEM27-tg mice than in humans. In vivo fluorescence and PET imaging in nude mice with insulinoma xenografts expressing hTMEM27 showed high 8/9-mAb uptake in tumours after 72 h. Antibody homing was also observed in beta cells of RIP-hTMEM27-tg mice by in vivo fluorescence imaging. Ex vivo analysis of intact pancreas and fluorescence microscopy in beta cells confirmed these findings. CONCLUSIONS/INTERPRETATION: hTMEM27 constitutes an attractive target for in vivo visualisation of pancreatic beta cells. Studies in mouse insulinoma models and mice expressing hTMEM27 demonstrate the feasibility of beta cell-targeted in vivo imaging, which is attractive for preclinical investigations and holds potential in clinical diagnostics.

Primary aldosteronism (PA) is the most frequent cause of secondary arterial hypertension. To date 3 forms of familial hyperaldosteronism (FH) have been described accounting for a small percentage of all PA cases. In Germany, the prevalence of FH is currently unknown. Our aim was to determine the prevalence of familiarity in a large cohort of patients with PA. A total of 166 patients with apparently sporadic PA in Munich were investigated. FH types I, II, and III were identified using established clinical, biochemical, and molecular criteria. Among the 166 patients with PA, 2 patients (1.2%) reported a family history suggestive of FH. None of the 166 patients showed clinical, endocrine, or genetic evidence of FH type I. The 2 families had characteristic features of FH type II. Family A had 3 subjects affected out of 11 evaluated family members. Family B had 3 out of 4. Bilateral adrenal hyperplasia and unilateral adrenal adenoma were found within the same family. FH type I and FH type III are rare in Germany. With a prevalence of 1.2%, FH type II seems to be more common in apparently sporadic PA than had been assumed so far.

Different works have registered the number of mammal species within the natural habitats of the Pantanal based on currently known records, with species richness ranging from 89 to 152 of annotated occurrences. Our present list sums 174 species. However, at least three factors have to be emphasised to deal with recorded numbers: 1) to establish the ecotone limit between the floodplain (which is the Pantanal) and its neighbouring domain like the Cerrado, besides the existence of maps recently produced; 2) the lack of intensive surveys, especially on small mammals, rodents and marsupials; and 3) the constant taxonomic revision on bats, rodents and marsupials. Some species are very abundant - for example the capybara Hydrochoerus hydrochaeris and the crab-eating fox Cerdocyon thous, and some are rare, and others are still intrinsically rare - for example, the bush dog Speothos venaticus. Abundance of species is assumed to reflect ecological resources of the habitat. Local diversity and number of individuals of wild rodents and marsupials also rely on the offering of ecological resources and behavioural specialisation to microhabitat components. A large number of species interact with the type of the vegetation of the habitat, by means of habitat selection through active patterns of ecological behaviour, resulting on dependency on arboreal and forested habitats of the Pantanal. In addition, mammals respond to seasonal shrinking-and-expansion of habitats due to flooding regime of the Pantanal. The highest number of species is observed during the dry season, when there is a considerable expansion of terrestrial habitats, mainly seasonally flooded grassland. Major threats to mammal species are the loss and alteration of habitats due to human intervention, mainly deforestation, unsustainable agricultural and cattle-ranching practices, which convert the natural vegetation into pastures. The Pantanal still harbours about a dozen of species officially listened as in danger.


Diversos trabalhos têm registrado o número de espécies de mamíferos nos diferentes hábitats naturais do Pantanal, com base nos registros do conhecimento corrente, com a riqueza de espécies variando de 89 a 152 ocorrências anotadas. Nossa lista atual soma 174 espécies. Contudo pelo menos três fatores devem ser enfatizados quando se lida com os números encontrados: 1) o limite exato do ecótono entre a bacia de inundação (que é o Pantanal) e os domínios vizinhos, como o Cerrado, apesar da existência de mapas recentes; 2) a falta de levantamentos intensivos, particularmente sobre pequenos mamíferos, roedores e marsupiais; e 3) a revisão taxonômica constante de morcegos, roedores e marsupiais. Algumas espécies são abundantes, como a capivara Hydrochoerus hydrochaeris, e o lobinho Cerdocyon thous, outras são raras e outras ainda são intrinsecamente raras, como o cachorro-vinagre Speothos venaticus. Assume-se que a abundância de espécies reflete os recursos ecológicos contidos nos hábitats. A diversidade local e o número de indivíduos de roedores silvestres e marsupiais também dependem da oferta ecológica de recursos e da especialização de comportamento para explorar componentes de micro-hábitats. Grande número de espécies interage com os tipos de vegetação do hábitat, por meio de seleção de hábitat com padrões ativos de comportamento ecológico, que resultam na dependência dos mamíferos de hábitats arbóreos e florestados do Pantanal. Além disso, os mamíferos respondem ao encolhimento e expansão dos hábitats devido à inundação sazonal do Pantanal, com abundância de espécies mais altas na estação seca, quando há considerável expansão de hábitats terrestres, principalmente campos inundáveis. As ameaças mais importantes para os mamíferos são a perda e alteração de hábitats devido à intervenção humana, particularmente o desmatamento, práticas de agricultura e pecuária insustentáveis, com conversão da vegetação natural em pastos. O Pantanal ainda abriga cerca de uma dúzia de espécies oficialmente ameaçadas de extinção.

We studied the bat fauna of the Pantanal floodplain and its surrounding plateaus in Mato Grosso do Sul, Brazil, based on the scientific collection at Universidade Anhanguera - Uniderp and on the Projeto Morcegos do Pantanal data bank at UFMS, comprising 9,037 captures of 56 species recorded from 1994 to 2007. The Pantanal surveys were carried out in the Nhecolândia, Aquidauana, Miranda, and Paraguai sub-regions; the uplands surveys took place in the Maracaju, Bodoquena, and Urucum formations. Bat specimens were mist-netted over 376 nights in 35 sites, predominantly near fruiting trees, bat shelters, and forest patches. In the floodplain 46 species were recorded (n = 6,292 individuals), and 44 species were found in the uplands (n = 2,745 individuals). Six families were recorded: Phyllostomidae (30 species), Molossidae (12 species), Verpertilionidae (nine species) Noctilionidae (two species), Emballorunidae (two species) and Mormoopidae (one species). The bat fauna was predominantly composed of insectivore (32) and frugivore (15) species. The frugivorous Artibeus planirostris (n = 3,101 individuals) was the commonest species in floodplain and uplands. Other common species were Myotis nigricans (n = 762), Molossus molossus (n = 692), Noctilio albiventris (n = 681), Platyrrhinus lineatus (n = 633), Sturnira lilium (n = 461), Carollia perspicillata (n = 451), Glossophaga soricina (n = 436), Artibeus lituratus (n = 320), and Desmodus rotundus (n = 281). In the floodplain there were three insectivores among the most common species, contrasting with the uplands dominated by the frugivores. The diversity for the 35 sites assembled (H' = 2.5) is comparable to that recorded for tropical forests. The bat fauna presented here represents 34% of the Brazilian bat species, and 62% of species reported for the Upper Paraguay River Basin. Additionally, five species are reported for the first time in Mato Grosso do Sul.


Estudamos a fauna de morcegos na planície do Pantanal e nos planaltos de entorno no Mato Grosso do Sul, Brasil, com base na coleção científica da Universidade Anhanguera - Uniderp e no banco de dados do Projeto Morcegos do Pantanal, UFMS, incluindo 9.037 capturas de 56 espécies, entre 1994 e 2007. Amostragens no Pantanal foram feitas nas sub-regiões da Nhecolândia, Aquidauana, Miranda e Paraguai; no planalto as amostragens foram realizadas nas formações de Maracaju, Bodoquena e Urucum. Espécies de morcegos foram registradas ao longo de 376 noites em 35 sítios, predominantemente com o uso de redes de neblina próximas a árvores frutíferas, abrigos e florestas. Na planície, foram registradas 46 espécies (n = 6.292 indivíduos) e no planalto 44 espécies (n = 2.745 indivíduos). Adicionalmente, cinco espécies são reportadas pela primeira vez no Mato Grosso do Sul.
