The present study evaluated the performance of some enzymatic and non-enzymatic antioxidant systems against oxidative stress for 10 to 30 d of refrigeration (R) and 15 to 50 d in controlled atmosphere (CA) conditions in both exocarp and mesocarp of Hass avocados from early and late harvests and at shelf life (SL) or consumption maturity. The possible relationship of the antioxidant systems with the occurrence of physiological disorders is also evaluated. The results indicate that the enzymatic system-superoxide dismutase (SOD), peroxidase (POD), catalase (CAT), phenylalanine ammonium lyase (PAL) and polyphenoloxidase (PPO)-as well as the non-enzymatic system-such as phenolic compounds (PC)-showed different responses to the stress generated during storage and shelf life. In general, SOD, CAT, PAL and PPO did not significantly vary in storage (R or CA). At consumption maturity, SOD, POD and PAL activities decreased in the mesocarp (RSL and CASL), while CAT increased in the exocarp for CASL15-50d. PC instead decreased in the exocarp as the harvest period progressed while it increased in the mesocarp. Physiological disorders (dark spots) showed only in refrigeration on the exocarp at R30d and in mesocarp at RSL30d coincident with low SOD and low SOD and POD activity values, as well as low PC contents (p-coumaric and its derivatives and caffeic acid derivatives), respectively. The results support the use of CA as a postharvest technology to prevent the development of physiological disorders through the joint action of antioxidative defenses during avocado transport to distant markets until consumption maturity is reached.

BACKGROUND: Inflammasome components NLRP3 and AIM2 contribute to inflammation development by the activation of caspase-1 and IL-1ß. They have not been yet evaluated in samples from patients with active peri-implantitis. Thus, the aim of the present study is to analyze the expression of inflammasomes NLRP3 and AIM2 and subsequent caspase 1 and IL-1ß assessing the microenvironment of leukocyte subsets in samples from patients with active peri-implantitis. METHODS: Biopsies were collected from 33 implants in 21 patients being treated for peri-implantitis. Biopsies from gingival tissues from 15 patients with healthy periodontium were also collected for control. These tissues were evaluated through conventional histological stainings. Then, immunohistochemical detection was performed to analyze NLRP3, AIM2, caspase-1, and IL-1ß and markers of different leukocyte subsets. PCR for inflammasomes and related genes was also done. RESULTS: This manuscript reveals a high immunohistochemical and mRNA expression of NLRP3 and AIM2 inflammasomes, caspase-1, and IL-1ß in biopsies collected from human peri-implantitis. The expression of the tested markers was significantly correlated with the increase in inflammatory infiltrate, probing depth, presence of biofilm, and bleeding on probing. In these peri-implantitis lesions, the area of biopsy tissue occupied by inflammatory infiltrate was intense while the area occupied by collagen was significantly lower. In comparison with periodontal healthy tissues, the inflammatory infiltrate was statistically significantly higher in the peri-implantitis biopsies and was mainly composed of plasma cells, followed by T and B lymphocytes. CONCLUSION: In human peri-implantitis, chronic inflammation can be explained in part by the action of IL-1ß/caspase 1 induced through NLRP3 and AIM2 inflammasome activation.

BACKGROUND: Anorganic bovine bone has been deeply studied for bone regeneration in the oral cavity. Different manufacturing processes can modify the final composition of the biomaterial and the responses that induce. AIM: To evaluate the physico-chemical characteristics of a bovine bone mineral matrix and the clinical, radiographical, histological, and mRNA results after using it for maxillary sinus floor augmentation in humans. MATERIALS AND METHODS: First, the physical-chemical characteristics of the biomaterial were evaluated by X-ray powder diffraction, X-ray fluorescence, and electron microscopy. A frequently used biomaterial with the same animal origin was used as comparator. Then, a clinical study was designed for evaluating clinical, radiographical, histological, and mRNA outcomes. Patients in need of two-stage maxillary sinus floor augmentation were included in the study. Six months after the grafting procedure, a bone biopsy was collected for evaluation. RESULTS: In terms of physico-chemical characteristics, no differences were found between both biomaterials. Clinically, 10 patients were included in the study. After 6 months, clinical and radiographical data showed adequate outcomes for allowing implant placement. Histological, immunohistochemical and mRNA analyses showed that the biomaterial in use provides biological support to induce responses similar to those of other commonly used biomaterials. CONCLUSION: Bovine bone mineral matrix (Creos™ Xenogain) used as a single material for maxillary sinus floor augmentation shows adequate biological, clinical, and radiological outcomes. In fact, the results from this study are similar to those reported in the literature for another bovine bone-derived biomaterial with whom it shares composition and micro- and nanoscale characteristics.

BACKGROUND: Recently, positive circumferential resection margin has been found to be an indicator of advanced disease with a high risk of distant recurrence rather than local recurrence. OBJECTIVE: The study aimed to analyze the prognostic impact of the circumferential resection margin on long-term oncological outcomes in patients with rectal cancer. DESIGN: This was a multicenter, propensity score-matched (2:1) analysis comparing the positive and negative circumferential resection margins. SETTINGS: The study was conducted at 5 high-volume centers in Spain. PATIENTS: Patients who underwent total mesorectal excision with curative intent for middle-low rectal cancer between 2006 and 2014 were included. MAIN OUTCOME MEASURES: The main outcomes were local recurrence, distant recurrence, overall survival, and disease-free survival. RESULTS: The unmatched initial cohort consisted of 1599 patients, of whom 4.9% had a positive circumferential resection margin. After matching, 234 patients were included (156 with a negative circumferential margin and 78 with a positive circumferential margin). The median follow-up period was 52.5 (22.0-69.5) months. Local recurrence was significantly higher in patients with a positive circumferential margin (33.3% vs 11.5%; p < 0.001). Distant recurrence was similar in both groups (46.2% vs 42.3%; p = 0.651). There were no statistically significant differences in 5-year overall survival (48.6% vs 43.6%; p = 0.14). Disease-free survival was lower in patients with a positive circumferential margin (36.1% vs 52.3%; p = 0.026). LIMITATIONS: This study was limited by its retrospective design. The different neoadjuvant treatment options were not included in the propensity score. CONCLUSIONS: The positive circumferential resection margin was associated with a higher local recurrence rate and worse disease-free survival in comparison with the negative circumferential resection margin. PURPOSE: The purpose of this study was to analyze the efficacy of a social interaction intervention delivered during the early postoperative time period on the coping process in persons with a new ostomy. DESIGN: Randomized controlled trial. SUBJECTS AND SETTING: The sample comprised 52 persons who underwent surgical management of colorectal including a fecal ostomy; 27 were randomly allocated to the intervention group and 25 to the control group. One participant allocated to the control group died before data collection; thus, findings are based on data from 51 participants. The study setting is the Regional University Hospital of Malaga, located on the southern coast of Spain. METHODS: Ostomy visitors (persons living with an ostomy) were taught to listen to the experiences and the concerns of the participant and to serve as an example of an individual who has successfully learned to live with a fecal ostomy. The intervention was a visit with a person with a new ostomy during the early postoperative period. Control group participants were offered standard care that did not include the visit from a person with an ostomy. Outcome measures were taken from the Nursing Outcomes Classification taxonomy. The main outcome measure was coping; secondary outcome measures were health beliefs and acceptance of health status. Bivariate analyses were performed to evaluate differences between groups in terms of the primary and secondary outcomes. A multivariate linear regression analysis was performed to identify predictor variables of the primary outcome, and effect size calculations were used to differentiate statistical significance versus clinical relevance. RESULTS: There were no differences in demographic or pertinent characteristics of participants in the 2 groups. Participants who received the intervention achieved a higher mean coping process scores: 3.90 vs 3.19, P = .002 and Cohen d = 0.97. In addition, intervention group participants achieved a higher mean efficacy for the secondary outcomes: 3.78 versus 2.97 (P = .0004) and Cohen d = 1.11 for health beliefs, and 3.68 versus 2.83 (P = .0001) and Cohen d = 1.24 for acceptance of health status. Linear regression analysis indicated that the social interaction intervention (ß= .799; P = .000) and undergoing urgent versus routine ostomy surgery (ß=-.610; P = .005) were related to coping. CONCLUSIONS: Findings indicate that a visit with a trained person living with an ostomy facilitated coping and improved health beliefs and acceptance of health status.

ABSTRACT Background: The psychosocial effects derived from the COVID-19 pandemic have represented a challenge for the population worldwide. Stress is a frequent problem reported by women, impacting the strategies they use to deal with problems in daily life and their ability to recover from this critical phenomenon. Objective: The objective of this study was to evaluate the relationship between stress levels, knowledge about COVID -19, ways of coping, and resilience in women. Method: A non-experimental, cross-sectional, and correlational study was carried out in a group of 386 Mexican women through a format that was distributed on social networks. Results: The data suggest that the higher the level of knowledge about COVID-19, the greater the perceived stress. High stress is positively and statistically significantly related to coping style, denial (r = .245; q = .20) and cognitive-reflective analysis (r = .303; q= .21). Low levels of stress are negatively and statistically significantly associated with a weak effect size with factors like social competence (r= -.198; q= .11) and family support (r = -. 227; q = .16). Conclusion: Stress affects women considerably due to work overload and interpersonal factors. This study offers the possibility of knowing the emotional state of women in times of COVID-19.


RESUMEN Introducción: Las afectaciones psicosociales derivadas de la pandemia por COVID-19 han representado un reto para la población a nivel mundial. El estrés es uno de los problemas emocionales mayormente reportados en las mujeres, impactando en las estrategias que emplean para enfrentar los problemas de la vida cotidiana y su capacidad de recuperarse ante este fenómeno crítico. Objetivo: El objetivo de este estudio fue evaluar la relación entre los niveles de estrés, el conocimiento sobre la COVID -19, los estilos de afrontamiento y la resiliencia en mujeres. Método: Se realizó un estudio no experimental, transversal y correlacional en el que participaron 386 mujeres mexicanas las cuales respondieron un formato que se diseminó en redes sociales. Resultados: Los datos sugieren que a mayor nivel de conocimientos sobre la COVID-19 mayor estrés percibido. El estrés elevado se relaciona de manera positiva y estadísticamente significativa con el estilo de afrontamiento, negación (r=.245; q=.20) y análisis cognitivo-reflexivo (r=.303; q=.21). Niveles bajos de estrés se asocian de forma negativa, estadísticamente significativa y con un tamaño del efecto débil con las variables competencia social (r=-.198; q=.11) y apoyo familiar (r=-.227; q=.16). Conclusiones: El estrés afecta considerablemente a las mujeres debido a la sobrecarga de trabajo y a factores interpersonales. Este estudio ofrece la posibilidad de conocer el estado emocional de las mujeres en tiempos de la COVID-19.

ABSTRACT Objective: To cross-culturally adapt and validate the INICIARE instrument for use in Brazil. Method: methodological study divided into two phases: cross-cultural adaptation and validation. The first phase took place in six stages: translation, synthesis, back-translation, expert review, pre-test and submission to the author. The second, carried out with 130 patients, took place at a private hospital in Porto Alegre, Rio Grande do Sul, Brazil, between May and July 2019. The study was approved by the Research Ethics Committee. Data were analyzed for stability, equivalence and internal consistency. Results: In the cross-cultural adaptation, the expert committee adjusted the translated version, validating the content. At validation, most patients were women (64.6%) with a mean age of 59 ± 15.3. The reliability index was 0.744. Conclusion: The version of the instrument adapted for Brazil proved to be adequate and reflects the reality of daily nursing practice.


RESUMEN Objetivo: Adaptar y validar transculturalmente el instrumento INICIARE para uso en Brasil. Método: estudio metodológico dividido en dos fases: adaptación y validación transcultural. La primera, en seis etapas: traducción, síntesis, retrotraducción, revisión de expertos, pre-test y envío al autor. El segundo, realizado con 130 pacientes, se llevó a cabo en un hospital privado de Porto Alegre, Rio Grande do Sul, Brasil, entre mayo y julio/2019. El estudio fue aprobado por el Comité de Ética en Investigación. Los datos se analizaron mediante estabilidad, equivalencia y consistencia interna. Resultados: En la adaptación transcultural, el comité de expertos ajustó la versión traducida, validando el contenido. En el momento de la validación, la mayoría de los pacientes eran mujeres (64,6%) con una edad media de 59 ± 15,3 años. La fiabilidad fue de 0,744. Conclusión: La versión del instrumento adaptada para Brasil resultó adecuada y refleja la realidad de la práctica diaria de enfermería.

Introducción: Desde el registro de los primeros casos de COVID-19 en México, se han derivado una serie de respuestas emocionales caracterizadas por miedo y estrés. Dicho impacto emocional se debe en gran medida a la inundación de información paralela a las fases de la pandemia y la transición entre ellas y la percepción que los individuos tienen de la enfermedad. El objetivo del presente trabajo fue comparar la percepción del COVID-19 entre la fase 1 y 2 de la pandemia y entre los medios de información usados para informarse en población mexicana. Métodos: Considerando un muestreo en cadena, se realizó un estudio comparativo en el que se diseminó por medio de correo electrónico y redes sociales una batería de evaluación que respondieron 1560 participantes. Resultados: La preocupación por las consecuencias del COVID-19 y su impacto emocional incrementaron al pasar de la fase 1 a la fase 2 de la pandemia. Además, se identificó que el impacto emocional fue mayor en quienes se informaron a través de Facebook® y televisión. Conclusiones: La pandemia tendrá un impacto emocional progresivo en medida en que avancen sus fases y en la importancia de informarse en medios adecuados para prevenir consecuencias emocionales.


Background: Since the first COVID-19 cases in Mexico there have been a variety of emotional responses which have in common fear and stress. The emotional impact of COVID-19 is builded in some way because the information flooding parallel to the pandemic phases, the transition between them and illness perception. The aim of the present work was to compare the perception of COVID-19 between phase 1 and 2 of the pandemic and between the information media used to inform themselves in the Mexican population. Methods: Considering a chain sampling, a comparative study was carried out in which an evaluation battery was disseminated through email and social networks, which was answered by 1560 participants. Results The concern about the consequences of COVID-19 and its emotional impact increased when going from phase 1 to phase 2 of the pandemic. In addition, it was identified that the emotional impact was greater in those who reported through Facebook® and television. Conclusions: The pandemic will have a progressive emotional impact as its phases progress and the importance of informing oneself in adequate means to prevent emotional consequences.

Abstract Introduction The prevention of COVID-19 infections involves the implementation of behaviors to reduce risk and protect health. However, engaging in these behaviors depends on the perception of the threat posed by the illness. Previous research shows the importance of illness perception in the case of communicable and non-communicable diseases, showing that they can change depending on the severity and risk attributed to them. Objective Compare the illness perception and the practice of preventive and exposure behavior based on the severity and the risk attributed to COVID-19 at the end of phase 1 and the beginning of phase 2 of the pandemic in Mexico. Method By means of a chain sampling, a comparative study was conducted in which an evaluation battery was disseminated through e-mail and social networks. Results It was found that evaluating COVID-19 as a serious disease and perceiving oneself as being at risk of contracting it had small and moderate effects on the perception of the consequences of the illness (r = .34; r = .26), emotional impact (r = .32; r = .25), personal control (r = .24) and engagement in preventive (r = .05), and exposure behaviors (r = .07; r = .07). Discussion and conclusion This article shows the relevance of the perceptual variables that impact concern due to the social and emotional consequences of COVID-19, as well as those that encourage preventive behaviors and the minimization of exposure behavior.


Resumen Introducción Si bien la prevención de contagios de COVID-19 supone la implementación de comportamientos para reducir el riesgo y proteger la salud, la práctica de estas conductas está en función de la percepción que se tenga sobre la amenaza de la enfermedad. Investigaciones previas evidencian la importancia de la percepción de enfermedad cuando se trata de padecimientos transmisibles y no transmisibles, señalando que ésta puede modificarse en función de la gravedad y el riesgo atribuido. Objetivo Comparar la percepción de enfermedad y la práctica de conductas de prevención y exposición con base en la gravedad y el riesgo atribuidos al COVID-19 al término de la fase 1 e inicio de la fase 2 de la pandemia en México. Método A partir de un muestreo en cadena, se realizó un estudio comparativo en el que se diseminó una batería de evaluación por medio de correo electrónico y redes sociales. Resultados Se encontró que evaluar el COVID-19 como una enfermedad grave y percibirse en riesgo de contraerla tiene efectos moderados y leves, respectivamente, sobre la percepción de las consecuencias de la enfermedad (r = .34; r = .26), el impacto emocional (r = .32; r = .25), el control personal (r = .24) y la práctica de conductas de prevención (r = .05) y exposición (r = .07; r = .07). Discusión y conclusión El presente estudio muestra la relevancia de las variables perceptuales que impactan en la preocupación por consecuencias sociales y emocionales del COVID-19, así como de aquellas que favorecen las conductas de prevención y la minimización de conducta de exposición.

Sarcomas are mesenchymal cancers with poor prognosis, representing about 20% of all solid malignancies in children, adolescents, and young adults. Radio- and chemoresistance are common features of sarcomas warranting the search for novel prognostic and predictive markers. GARP/LRRC32 is a TGF-ß-activating protein that promotes immune escape and dissemination in various cancers. However, if GARP affects the tumorigenicity and treatment resistance of sarcomas is not known. We show that GARP is expressed by human osteo-, chondro-, and undifferentiated pleomorphic sarcomas and is associated with a significantly worse clinical prognosis. Silencing of GARP in bone sarcoma cell lines blocked their proliferation and induced apoptosis. In contrast, overexpression of GARP promoted their growth in vitro and in vivo and increased their resistance to DNA damage and cell death induced by etoposide, doxorubicin, and irradiation. Our data suggest that GARP could serve as a marker with therapeutic, prognostic, and predictive value in sarcoma. We propose that targeting GARP in bone sarcomas could reduce tumour burden while simultaneously improving the efficacy of chemo- and radiotherapy.

OBJECTIVE: To examine and map the consequences of chronic pain in adulthood. METHOD: Documents addressing the impact of chronic pain on the psychological and social spheres of people suffering from chronic pain, published in Spanish and English between 2013 and 2018, were included. Those who addressed pharmacological treatments, chronic pain resulting from surgical interventions or who did not have access to the full text were excluded. Finally, 28 documents from the 485 reviewed were included. RESULTS: Studies show that pain is related to high rates of limitation in daily activities, sleep disorders and anxiety-depression spectrum disorders. People in pain have more problems to get the workday done and to maintain social relationships. Chronic pain is also associated with worse family functioning. CONCLUSIONS: This review shows that limitations in the ability to perform activities of daily living, sleep, psychological health, social and work resources and family functioning are lines of interest in published articles. However, knowledge gaps are detected in areas such as the influence of having suffered pain in childhood or adolescence, the consequences of non-fulfillment of working hours and gender inequalities.

Multipotent mesenchymal stromal cells (MSCs) have emerged as a promising cell therapy in regenerative medicine and for autoimmune/inflammatory diseases. However, a main hurdle for MSCs-based therapies is the loss of their proliferative potential in vitro. Here we report that glycoprotein A repetitions predominant (GARP) is required for the proliferation and survival of adipose-derived MSCs (ASCs) via its regulation of transforming growth factor-ß (TGF-ß) activation. Silencing of GARP in human ASCs increased their activation of TGF-ß which augmented the levels of mitochondrial reactive oxygen species (mtROS), resulting in DNA damage, a block in proliferation and apoptosis. Inhibition of TGF-ß signaling reduced the levels of mtROS and DNA damage and restored the ability of GARP-/low ASCs to proliferate. In contrast, overexpression of GARP in ASCs increased their proliferative capacity and rendered them more resistant to etoposide-induced DNA damage and apoptosis, in a TGF-ß-dependent manner. In summary, our data show that the presence or absence of GARP on ASCs gives rise to distinct TGF-ß responses with diametrically opposing effects on ASC proliferation and survival.

BACKGROUND AND AIM: Coping can be defined as the cognitive efforts and behavioral practices that people develop in situations which they consider to be stressful. In people with Chronic Non-Cancer Pain (CNCP), coping is influenced by the biological, psychological, and socio-cultural resources available to them. The aim of this systematic review is to evaluate the psychometric properties of European measuring instruments related to coping with CNCP in non-hospitalized adults. DATABASE AND DATA TREATMENT: The review was conducted following the guidelines of the PRISMA Statement and the methodological framework of the Joanna Briggs Institute. The CINAHL, PubMed, Scopus, and Web of Science databases were searched by two reviewers independently. The analysis of psychometric properties was performed using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist, and the risk of bias was analyzed using the Quality Assessment of Diagnostic Accuracy Study-2 (QUADAS-2) tool. RESULTS: Thirty-six studies validated twenty-four different instruments. The Portuguese version of the Pain Beliefs and Perceptions Inventory (PBPI), which assess catastrophizing, and the Spanish version of the Roland-Morris Questionnaire (RMQ), which assess disability, are the instruments with the best methodological quality and bias control. CONCLUSIONS: There are important gaps in the measurement of different aspects of pain coping, such as stress, social and family support, or self-care. Future studies could consider the creation of an instrument to comprehensively assess the resources that influence coping with chronic non-cancer pain.

RESUMEN OBJETIVO Examinar y mapear las consecuencias del dolor crónico en la edad adulta. MÉTODO Se incluyeron documentos que abordaban las repercusiones del dolor crónico en las esferas psicológica y social de las personas que lo padecen, publicados en español e inglés entre los años 2013-2018. Aquellos que abordaban tratamientos farmacológicos, dolor crónico derivado de intervenciones quirúrgicas o que no tenían acceso a texto completo fueron excluidos. Finalmente, se incluyeron 28 documentos de los 485 revisados. RESULTADOS Los estudios muestran que el dolor se relaciona con altas tasas de limitación en las actividades de la vida diaria, alteraciones del sueño y trastornos del espectro ansiedad-depresión. Las personas con dolor experimentan más problemas para rendir en la jornada laboral y mantener relaciones sociales. Con respecto a la familia, el dolor crónico se ha asociado con un peor funcionamiento familiar. CONCLUSIONES Esta revisión pone de manifiesto que las limitaciones en la capacidad para realizar actividades de la vida diaria, el sueño, la salud psicológica, los recursos sociales y laborales y el funcionamiento familiar son líneas de interés en los trabajos publicados. Sin embargo, se detectan lagunas de conocimiento en áreas como la influencia de haber padecido dolor en la infancia o adolescencia, las consecuencias por incumplimiento de la jornada laboral y las desigualdades de género.

Resumo O artigo objetiva identificar as estratégias utilizadas, para participação do paciente na segurança do cuidado de saúde. Revisão sistemática, norteada pelas recomendações do modelo PRISMA, nos bancos de dados: Scopus, WOS e Medline. Limitou-se a busca a estudos realizados entre janeiro de 2001 e julho de 2016, redigidos em português, inglês ou espanhol. Foram incluídos estudos observacionais, descritivos, qualitativos e/ou epidemiológicos, que descrevessem a metodologia de elaboração e/ou aplicação de, pelo menos, uma estratégia de inclusão dos pacientes na melhoria da segurança dos cuidados. A qualidade metodológica dos artigos foi avaliada usando a ferramenta Cochrane. Para analisar os resultados se fez uma análise temática. Após leitura de títulos, resumos e aplicação de critérios de exclusão, 19 artigos foram selecionados. Nestes se identificam estratégias de mobilização dos pacientes para a segurança dos cuidados, estratégias para promover a participação ativa dos pacientes na segurança dos cuidados e estratégias de solicitação de informação ao paciente sobre a segurança dos cuidados. Há na literatura diversas estratégias que promovem a participação do paciente na segurança dos cuidados, que têm formas e métodos concretos de implementação, bem como objetivos distintos para o seu uso.


Abstract The scope of this article was to identify the strategies used for the participation of the patient in healthcare security in hospital and outpatient environments. It involved a systematic review of the literature based on the recommendations of the PRISMA model on the Scopus, WOS and Medline databases. The search was restricted to studies written in Portuguese, English or Spanish conducted between January 2001 and July 2016. Observational, descriptive, qualitative and/or epidemiological studies that described a development/appliance methodology using at least one patient security improvement strategy of inclusion were included. The methodological quality of the studies was assessed using the randomized Cochrane risk-of-bias tool. Thematic analyses were performed in order to analyze the results. After the application of criteria of title, abstract analysis and exclusion, 19 studies were selected. In these studies, patient security strategies that promoted patients' active participation on patient security and information request strategies were identified. In the literature, sundry strategies promoting patient participation on healthcare security, with concrete implementation methods, as well as distinct purposes for their use, were encountered.

BACKGROUND: The aim of this study was to analyze the incidence, patterns and prognostic factors of recurrence in patients with complicated colon cancer who had emergency surgery within 24 h of admission. METHODS: A retrospective observational study was performed on patients with obstructing or perforated colon cancer having resection with curative intent between 1996 and 2014 at a single center. Data were obtained from a prospectively maintained database. Patients who had rectal cancer, iatrogenic endoscopic perforation, stage IV disease, palliative surgery, a colonic stent or decompressive colostomy were excluded. RESULTS: The study included 393 patients. Obstruction was observed in 320 patients (81.4%) and perforation in 73 (18.6%). Hartmann's procedure was more frequently performed by general surgeons (7.5% vs 23.3%; p = 0.023). 30-day postoperative mortality was 13.5% (53/393), including 47 (14.7%) obstructed and 6 (8.2%) perforated patients. Postoperative complications (Clavien-Dindo III-IV) occurred in 87 patients (22.1%), including 68 (21.2%) of obstructed and 19 (26.0%) of perforated patients. Anastomotic dehiscence was diagnosed in 52 of 329 (15.8%) patients with primary anastomosis and was higher in the obstructing group than in the perforated group (17.4% vs 7.6%). There was a significantly higher anastomotic dehiscence rate after procedures performed by general surgeons when compared with those performed by colorectal surgeons (10.3% vs 21.3%; p = 0.005; OR 2.81, 95% CI 1.4-5.9). With a median follow-up of 6 years, the recurrence rate was 30.1% (67.4% distant, 22.8% local, 9.8% both). Overall and cancer-related survivals were 68.7% and 77.8%, respectively. The presence of positive nodes, male gender, anastomotic dehiscence and diffuse peritonitis were independent predictors for local recurrence while type of surgeon (general) was an independent factor for distant recurrence. CONCLUSIONS: Male gender, diffuse peritonitis, positive lymph nodes, type of surgeon and postoperative anastomotic dehiscence significantly influence recurrence of colorectal cancer in this series.

RESUMO

CRISPR-Cas9-based combinatorial perturbation approaches for orthogonal knockout and gene activation have been impeded by complex vector designs and co-delivery of multiple constructs. Here, we demonstrate that catalytically active CRISPR-Cas12a fused to a transcriptional-activator domain enables flexible switching between genome editing and transcriptional activation by altering guide length. By leveraging Cas12a-mediated CRISPR-RNA array processing, we illustrate that Cas12a-VPR enables simplified multiplexed knockout and transcriptional activation in vitro and in vivo.
